Autogenous, allogenic, and beta-TCP grafts: comparative effectiveness in experimental bone furcation defects in dogs.

The healing responses of three implant materials--fresh autogenous bone, allogenic decalcified freeze-dried bone, and alloplastic implant, beta-tricalcium phosphate (beta-TCP)--were evaluated and compared. Through-and-through furcational defects were surgically created in the third and fourth mandibular premolars of three dogs. Implant material was then placed individually in the surgical defects. All defects without grafting served as controls. Following grafting (ten weeks into the healing period), individual dogs were killed and prepared for light microscopy. The following results were recorded: (1) All materials were well-tolerated. No post-surgical complications occurred. (2) Graft sites showed more pronounced regeneration and the possibility of higher periodontal attachment than did control sites. (3) The defects filled with autogenous bone showed slightly more pronounced regeneration than in allogenic freeze-dried bone and beta-TCP.

[Combination therapy with medroxyprogesterone acetate and tegafur in tamoxifen- and adriamycin-resistant advanced breast cancers].

Medroxyprogesterone acetate (MPA) plus Tegafur (TGF) therapy was performed to evaluate the efficacy in a treatment for Tamoxifen- and Adriamycin-resistant advanced breast cancers. The patients were medicated in different doses on 800 mg or 1,200 mg daily po for MPA and TGF. Sixteen patients were evaluable in this trial. According to criteria of the Japan Mammary Cancer Society, one was regarded as CR and five as PR. The response rate was 37.5%. There was no difference in response rates between premenopausal and postmenopausal patients or ER positive and negative patients. No difference was noted either in response rates and incidence of side effects between two different doses of MPA. This means that an 800 mg daily dose of MPA lower than the recommended 1,200 mg might be better in the treatment for Japanese patients with advanced breast cancers. Gastrointestinal disorders, bone marrow suppression and liver dysfunction which could be found in high-dose TGF therapy, were not frequently observed in this trial. Patients could expect good quality of life during the treatment because of fewer side effects. This combination therapy with MPA and TGF was regarded as a good modality for the treatment of Tamoxifen- and Adriamycin-resistant advanced breast cancers.

[Electrocardiograms during acute occlusion of the left circumflex artery as compared with that of the right coronary artery].

To assess the characteristic electrocardiographic (ECG) ST changes during acute occlusion of the left circumflex artery (LCX), we observed ECG changes during percutaneous transluminal coronary angioplasty (PTCA) of the LCX and compared the results with those obtained during right coronary angioplasty. Results were as follows: 1. In the 30 patients who had LCX angioplasty (group LCX), ST-segment elevation occurred most frequently in lead V6 (67.7%) and in lead III (46.7%), but rarely in leads and aVL. ST depression occurred most frequently in lead V3 (80.0%) and in lead V2 (73.3%), but rarely in other leads except for leads I and aVL (23.3%, 33.3%). 2. Four types of ST change in lead combinations were observed. These included: (1) ST elevation in the inferior leads (II, III and aVF), (2) ST elevation in the lateral leads (V5 and V6), (3) ST depression in the anterior leads (V2-V4) and (4) ST depression in the high lateral leads (I and aVL). In group LCX, nine cases (30.0%) manifested all four changes, and six cases (20.0%) revealed only ST depression in the anterior leads without ST changes in other leads. 3. ST depression in the anterior leads and ST elevation in the inferior and lateral leads were observed in nearly equal frequency in patients who received PTCA at the proximal site (Seg. 11) and at the distal site (Seg. 13) in the group LCX. However, ST depression in the high-lateral leads was more frequently observed in the distal than in the proximal cases (66.7% vs 33.3%).(ABSTRACT TRUNCATED AT 400 WORDS)

Chronic effects of nicorandil on exercise tolerance in patients with stable effort angina pectoris.

To examine the chronic effects of nicorandil, a new coronary vasodilator, on exercise tolerance, we gave this drug for ten days at 15 mg/day orally in three divided doses to 10 patients with stable effort angina. An increase in exercise capacity by more than 1 stage of modified Bruce's protocol was observed in 6 of 10 patients on the 5th and 10th day after the administration. The exercise duration was significantly prolonged from 294 +/- 86 sec (mean +/- SD) before the administration to 363 +/- 106 sec and 405 +/- 139 sec on the 5th and 10th day after the administration, respectively. With nicorandil administration, the time to 0.1 mV of ST-segment depression was also significantly delayed from 3.1 +/- 1.9 minutes to 4.5 +/- 2.6 minutes on the 10th day. On the other hand, systolic blood pressure, heart rate and pressure rate product did not demonstrate any significant change before and after the administration. These results indicate that oral nicorandil of 5 mg 3 times a day can chronically increase exercise tolerance without decreasing myocardial oxygen consumption in patients with stable effort angina.

Linkage of tyrosine kinase activity with transforming ability of the p185neu oncoprotein.

The neu oncogene encodes a 185 kd glycoprotein (p185neu) with intrinsic tyrosine kinase activity. Sequencing data has demonstrated that oncogenic p185neu differs from c-neu by a single point mutation within the transmembrane region of the glycoprotein. This mutation results in the substitution of a glutamic acid residue for a nonpolar valine residue at amino acid position 664 of the rat neu gene product. Recent studies have demonstrated that this mutation results in specific aggregation of the p185neu oncoprotein mimicking ligand induced dimerization events. The cellular consequences of the aggregated phenotype may include the enzymatic activation of p185neu. We demonstrate that the oncoprotein p185neu possesses higher intrinsic tyrosine kinase activity and that this increase in enzymatic activity is apparent within the plasma membrane, manifesting itself through the increased tyrosine phosphorylation of substrates. Furthermore, the neu oncoprotein itself is also phosphorylated on tyrosine to a higher extent than the proto-oncoprotein. These results strongly link enzymatic activation of p185neu to cellular transformation events. To test directly the effect of p185neu tyrosine kinase activity on cellular transformation we constructed mutant p185neu devoid of ATP binding ability. This mutant protein is expressed at high levels, but is unable to induce the transforming phenotype. The point mutation within the transmembrane region of p185neu mimics aspects of ligand induced activation events including increases in the specific tyrosine kinase activity of the molecule leading to cellular transformation.

[Influence factors on coronary collateral development].

In order to evaluate the influence factors on coronary collateral development, we compared various factors between two groups. Group I consisted of the 69 patients performed coronary angiography within 6 hours after the onset of acute myocardial infarction (MI) and Group II consisted of the 56 patients without MI because having good collaterals nevertheless whose one coronary artery was totally occluded or had a 99% stenosis. Group I (24/69, 35%) had a higher frequency of Diabetes mellitus than Group II (11/56, 20%) and had a lower frequency (30/69, 43%) of multivessel disease than Group II (39/56, 70%). The patients having angina duration more than 6 months were more in Group II (37/56, 60%) than in Group I (18/69, 26%). No significant differences existed between these two groups in age, smoke, hyperlipidemia and hypertension. In conclusions, the patients with good collateral have high frequency of multivessel disease and long history of angina pectoris. Diabetes mellitus is a inhibit factor on coronary collateral development.

[Pancreatic secretory trypsin inhibitor in urothelial cancer].

Pancreatic secretory trypsin inhibitor (PSTI) levels are known to increase in some kinds of cancer. We determined the PSTI levels in patients with urothelial cancer to evaluate its usefulness as a tumor marker. The subjects were 101 patients who had bladder cancer (n = 81) or renal pelvis and ureter cancer (n = 20). Twenty-six healthy volunteers were used as controls. PSTI was measured by an RI kit using the double antibody method, and levels above 20 ng/ml were taken as positive. Serum tissue polypeptide antigen (TPA) levels were also measured in 32 cases. None of the controls, 65% of the patients with renal pelvis and ureter cancer, and 30.9% of bladder cancer had positive levels of PSTI. The positive rate for each stage was 22.3% for Tis, Taand T1, 38.5% for T2, 73.3% for T3 and 100% for T4. The rate tended to increase with the progress of tumor infiltration. All subjects with positive readings above 30 ng/ml had progressive cancer. The usefulness of PSTI as a tumor marker was similar to that of TPA. PSTI is not a specific marker of urothelial cancer, but it showed an association with the progression of cancer. Therefore, it is considered to be a useful marker to determine postoperative metastasis, recurrence, and disease progression.

[The effect of open heart surgery and an operation on thoracic aneurysm on organs and the prevention of an organ system failure: comparison between the group using ulinastatin and not using group].

We studied the effect of the open heart surgery and the thoracic aortic aneurysm surgery on renal, liver and respiratory function and the coagulofibrinolytic system. We also investigated as to whether ulinastatin was effective or not with regard to preventing an organ system failure. The renal function, liver function and the coagulofibrinolytic system were preserved, although there was a greater number of severe cases in the group which used ulinastatin (US group) than in the group that did not use ulinastatin (non-US group). In the US group, PaO2 did not decrease postoperatively. However, in the non-US group, PaO2 decreased significantly after the operation. The variables in relation to using ulinastatin, examined by a stepwise method, included kind of disease, emergency operation, PaO2, BUN and serum plasminogen. The multiple coefficient for these five variables was 0.623 (p less than 0.01) and the contribution was 38.8%. It was suggested that ulinastatin could prevent an organ system failure, especially respiratory failure, after open heart surgery and the thoracic aortic aneurysm surgery.

Suppression of the activities of T-lymphocyte-related enzymes in spleen by administration of an immunosuppressant, 15-deoxyspergualin.

We investigated the effects of both (-) and (+)-enantiomers of 15-deoxyspergualin (DSG), an immunosuppressant, on enzyme networks in spleens of ICR mice. Of the 14 hydrolytic enzymes tested, the activity of dipeptidyl aminopeptidase IV (DAP-IV) was found to be significantly suppressed by the administration of (-)-DSG but not by that of (+)-DSG. In contrast, the activity of N-acetyl-beta-D-glucosaminidase (GlcNAc-ase) was suppressed by the administration of both enantiomers of DSG. Judging from the previous reports of ours and others which suggested specific relations of these enzymes to T-lymphocytes, the above findings may warrant further studies of these enzymes in relation to immunologic functions of the body.

[Two fatal cases of hepatic failure due to a malignant lymphoma presenting a liver injury].

Reported are two cases of a malignant lymphoma that presented a liver injury in which death was brought on by hepatic failure. In both instances on admission, liver injury and hepatomegaly were noticed. Abdominal ultrasonography and computerized tomography, however, showed no SOL in the liver or any clear lymph node mass, though a bright echo and a diffuse low density was observed in one of the patients (Case 1). In spite of treatment that included prednisolone, the liver injury in both patients progressed towards hepatic failure. Severe jaundice increased rapidly in Case 1, and a disseminated intravascular coagulopathy complicated by jaundice was observed in the other patient (Case 2). Both autopsies revealed severe lymphoma cell infiltration into the liver, mainly in Glisson's capsule, and diffuse hepatic cell necrosis. Diagnostic imaging of the diffuse lymphoma infiltration into the liver proved very difficult, so that it was necessary to rely on a differential diagnosis, employing laboratory tests and a liver biopsy.

[Clinical evaluation of second-look operations in colorectal cancer].

The second look operation (SLO) for early detection of recurrent tumor is indicated rarely because of the advance in diagnostic techniques such as imaging radiology, tumor marker serology and colonofiberscopy. The SLO was performed in 21 out of 362 curative cases of colorectal cancer resected in our department; 13 for re-elevation of plasma CEA level (Group 1), 6 for unclear symptoms or signs (Group 2) and 2 for postchemotherapy or postirradiation (Group 3). Liver metastases were found in four cases in group 1 and two in group 2. Nothing was found in three cases in group 1 and two in group 2. In group 3 the radical operation was carried out in all cases following SLO with good survival results. In group 1 and 2 the excisional rate of liver metastases in 57%, its salvage rate 75%, and the excisional rate of the lymph nodes is 50%, with a salvage rate of 100%. The SLO today is indicated for cases of asymptomatic re-elevation of CEA titer, for cases of undeterminable signs or for cases of postchemotherapeutic or postirradiation improvement.

[Adenosquamous carcinoma and squamous cell carcinoma of the liver: a resected case and three autopsied cases].

A resected case and an autopsied case of an adenosquamous carcinoma and two autopsied cases of a squamous cell carcinoma of the liver are presented. With regard to the first case, a 58-year-old male underwent an operation, based on the diagnosis of a hypovascular hepatoma or a cholangioma. The resected material showed a nodular whitish gray tumor at the S4 region which histologically proved to be an adenosquamous carcinoma. In the two autopsied cases, found was a huge, grayish necrotic mass at the right lobe of the liver and a defect of gall-bladder. The tumor, in both cases, proved to be squamous cell carcinoma originating in the liver.

[A case of neonatal medulloblastoma].

A nine-day-old female with cerebellar medulloblastoma treated by surgical and radiation therapy is reported. She suffered from vomiting within a few days after birth. Subsequently, her head became enlarged and she was admitted to our hospital. CT scan on admission showed a large mass lesion at the cerebellum expanding to the pineal portion, and marked hydrocephalus. At the age of 14 days, a ventriculo-peritoneal shunt was carried out, and at the age of 67 days, radical resection of the tumor was performed. The histopathological diagnosis was medulloblastoma. Postoperatively, radiation therapy of the brain was carried out. Irradiated dose was 30 Gy on the whole brain and 20 Gy at the location of the lesion. She was discharged at the age of seven months. She remained in good condition with no neurological deficit except for nystagmus. One week later, it was found that she was unable to move her legs and she was readmitted to our hospital. CT scan showed no remarkable change in the intracranial region, but spinal cord swelling was revealed at the Th12-L5 level. Myelography showed a filling defect at the L3-5 level. Radiation therapy of the spinal cord was performed, and the paraparesis gradually improved. However, her general condition was getting worse, and the following CT scan showed intracranial recurrence of the tumor. She deteriorated day by day and at the age of 9.3 months she expired. Brain tumors during the neonatal period are very rare, especially neonatal medulloblastoma. Only 24 cases can be found in the literature.(ABSTRACT TRUNCATED AT 250 WORDS)
