[A case of isolated tricuspid regurgitation associated with protein-losing gastroenteropathy].

A 43 year old woman was admitted to our hospital in April 1987 due to shortness of breath and pedal edema. She had a history of sepsis associated with the crisis of hyperthyroidism 15 years prior to the admission. Physical examination revealed a badly nourished with ascites: weight was 56 kg and height 156 cm. The heart sounds were distant with mild holosystoric murmur (grade I/VI) at xiphoisternum. The chest X-ray showed cardiomegaly (CTR: 72.3%) with pleural effusion. The electrocardiogram showed atrial fibrillation, low voltage and right ventriculer hypertrophy. The echocardiogram showed marked dilatation of right atrium and ventricle with very short septal leaflet of tricuspid valve. The anterior and posterior leaflets were undetected. The tricuspid regurgitant doppler signal was recorded up to hepatic vein. No other abnormalities were noted in other valves. The white cell count was 4900 with lymphocytopenia (26%; T-cell 82%, B-cell 13%). Serum total protein was reduced to 3.4 g/dl with albumin 1.64 g/dl. Immunoelectrophoresis showed normal IgG, IgA and IgM. Proteinuria was not recognized. Fecal excretion of polyvinylpyrrolidone-131I (PVP) was elevated to 2.8%, The systolic pressure in pulmonary artery, right ventricle, right atrium, superior and inferior vena cave were almost equal as 26 mmHg. The pulmonary arterial scintigraphy disclosed multiple peripheral defects in both lungs. Two weeks after the operation of tricuspid valve replacement based on the diagnosis of protein-losing enteropathy due to isolated tricuspid regurgitation, serum total protein and albumin were normalized to 6.8 g/dl and 3.6 g/dl respectively, but the lymphocytopenia was persistent. She become very well, with free of ascites and edema.(ABSTRACT TRUNCATED AT 250 WORDS)

Changes in relationship between blood glucose level and plasma 1,5-anhydroglucitol level in KK mice.

We compared the relationship of the blood glucose level to the plasma 1,5-anhydroglucitol (1,5AG) level between KK mice with abnormal glucose metabolism and ICR mice as controls. Although the plasma 1,5AG level did not show any significant correlation with the blood glucose level in the controls, it tended to logarithmically decrease with the rise in the blood glucose level in KK mice. Thus it is possible that the plasma 1,5AG level is specifically related to the abnormal glucose metabolism in this model of diabetes mellitus and that its routine examination in diabetic patients may help delineate the metabolic derangement in the disease.

[Antagonistic effect of 4-aminopyridine derivatives on dTc induced block].

A new muscle relaxant antagonist, 4-aminopyridine (4-AMP), has various problems related to its side effects. In order to obtain 4-AMP derivatives with less side effect and the same antagonizing effect on dTc block as that of 4-AMP, three types of derivatives were synthesized. They were 4-methylaminopyridine (4-MAMP), 4-dimethylaminopyridine (4-DAMP), and 4-trimethylaminopyridine (4-TAMP). For the purpose of studying their effects on neuromuscular junction, the antagonistic effects of the three derivatives on the dTc block were compared with those of 4-AMP, by using rat diaphragm preparation in vitro, and rat sciatic nerve tibialis anterior muscle preparation in vivo. These three derivatives exhibited earlier onset time compared with 4-AMP, but their efficacy was lower in terms of ED50 in vivo and in vitro. Also their duration of effect was shorter than 4-AMP. In particular, the ED50 of 4-DAMP was much larger (9 times that of 4-AMP), and its duration of effect was shorter (11 minutes), indicating that it is unsuitable as a practical antagonistic agent. ED50 of 4-TAMP was the closest to that of 4-AMP (three times that of 4-AMP), and it exhibited effects earlier than 4-AMP (one fourth that of 4-AMP), but was inferior in duration of effect (13 minutes). With respect to antagonistic effects, these three agents cannot be described as perfect, and development of new derivatives will be required.

[The results of an investigation of the various factors influencing the late patency ratio of coronary bypass grafting].

The purpose of this communication was to find out the factors influencing the late patency ratio of coronary artery bypass grafts of 42 patients in our institution. The following results were obtained: 1. The size of the measured diameters of coronary arteries and the graft flow rates were correlated well to the late patency ratio (p less than 0.05). 2. The regular physical exercise trainings were significantly important to keep patent grafts (p less than 0.01). 3. In blood serum studies, serum uric acid levels were significantly lower (p less than 0.05), the average mean HDL cholesterol levels were much higher (p less than 0.001) and the ratio of total cholesterol to HDL cholesterol were significantly lower in the group with late patent grafts than with grafts occluded. Therefore, we found out the variables affecting graft patency ratio which were examined by a stepwise method were the diameter of the coronary artery, regular physical exercise training, the average of lower serum uric acid levels and higher HDL cholesterol levels. The multiple coefficient for these four variables was 0.562 (p less than 0.001) and the contribution was 31.6%. According to the base of this studies, we have concluded that postoperative regular physical exercise, low fat and restricted calorie intake were very important to keep the late patent grafts in long-term follow up period after coronary bypass surgery.

Multiple enzymes related to differentiation of lymphocyte subpopulations in man.

We studied the relation of various enzymes to subpopulations of lymphocytes in man. T cell-rich fractions were separated with a nylon column from mononuclear cells in the buffy coat. Comparing the enzymatic profiles of the two fractions, we found that the difference between the two groups came from the dominancy of B cells and/or macrophages in the former fraction, and from that of T cells in the latter. The enzymes characterizing T cells included N-Ac-beta-D-glucosaminidase (GlcNAc-ase), prolyl endopeptidase (Post-Pro-Enz), and dipeptidyl aminopeptidase IV (DAP-IV), whereas those characterizing B cells and/or macrophages include poly(ADP-ribose) synthetase, leucine aminopeptidase (Leu-AP), AP-B, cathepsin B, sialidase, and AP-A. Inhibitors of these enzymes may lead to modification of the function of T and B cells.

[Correlation of arteriosclerosis obliterans of the lower extremity with coronary artery disease].

To ascertain the correlation between arteriosclerosis obliterans (ASO) of the lower extremities and coronary artery disease (CAD), 69 patients with lower extremity ASO were observed. Based on clinical symptoms, the patients were categorized as group A: 52 with intermittent claudication and group B: 17 with angina pectoris. All patients underwent angiography of both lower extremities and the heart. Results were as follows: 1. In group A, 33 patients had significant coronary stenosis with single (16 cases), double (12 cases) and triple (five cases) vessel disease. Sixteen patients had histories of myocardial infarction and two had vasospastic angina. In group B, 15 patients had significant coronary stenosis with single (five cases), double (six cases) and triple (four cases) vessel disease including two cases involving the left main trunk. Six patients had histories of myocardial infarction and two had vasospastic angina. 2. Electrocardiography revealed that 37 patients in group A and 14 patients in group B had abnormal ECGs as abnormal Q waves and ST-T changes. However, 10 patients in group A and three patients in group B had significant coronary stenosis despite their normal ECGs. 3. According to the sites of stenotic lesions, all 69 patients with ASO were classified in four types; as diffuse, pelvic, ilio-femoral and femoral. The incidence of CAD did not differ among these four types. 4. In group A, 38 patients experienced revascularization of ASO; 36 cases by bypass grafting, and two by percutaneous transluminal angioplasty (PTA). Ten patients received cardiac revascularization including three cases with coronary artery bypass graft (CABG) and seven cases with percutaneous transluminal coronary angioplasty (PTCA). In group B, 13 patients received revascularization of ASO with bypass grafting. Eleven patients had coronary revascularization including seven cases with CABG and four cases with PTCA. 5. Fourteen of the 69 patients received revascularization for both ASO and CAD. In conclusion, there is a highly significant coincidence of CAD in patients with ASO, and CAD is often silent. These data indicate that routine coronary angiography is necessary for detecting and preventing CAD in all patients with ASO. Coronary artery revascularization, especially PTCA, can be indicated prior to lower extremity revascularization.

Monoclonal antibodies that detect different antigenic determinants of the same human osteosarcoma-associated antigen.

Monoclonal antibodies (MoAbs) against human osteosarcoma cells were obtained by the fusion of NS/1 mouse myeloma cells with spleen cells from the human osteosarcoma cell line-immunized BALB/c mice. Two hybrid clones were established and designated as 2H10 and 2D3. Both MoAbs reacted strongly with all osteosarcoma tissues but not with other bone and soft tissue tumors such as chondrosarcoma, malignant fibrous histiocytoma, liposarcoma, leiomyosarcoma, and rhabdomyosarcoma. In addition, neither MoAb reacted with tumor cell lines and tissues obtained from other cancers. Immunohistochemical analysis demonstrated that 2H10 and 2D3 reacted with endothelial cells in sarcoma tissues, but not with those of other tumors and normal tissues. 2H10 also reacted with cells on the basal layer of epidermis of the skin. 2H10- and 2D3-defined antigen has an approximate molecular weight of 75,000 under nonreducing and reducing conditions, indicating that the antigen has a single chain structure and there is no intramolecular disulfide bond. 2H10- and 2D3-defined antigen has a pI value between 5.5 and 6.2. Sequential immunoprecipitation analysis clearly demonstrated that 2H10 and 2D3 recognized the same antigen molecule. However, further analysis suggested the possibility that 2H10 and 2D3 MoAbs recognized the different antigenic determinants on the same antigen molecule.

ST-segment depression in inferior ECG leads during percutaneous transluminal coronary angioplasty for left anterior descending artery.

The mechanism of inferior ST depression during percutaneous transluminal coronary angioplasty (PTCA) for the left anterior descending artery (LAD) was studied in 108 patients with isolated LAD lesion. In 49 patients (Group I) ST depression in inferior leads was observed, and 59 patients (Group II) showed no inferior ST depression. In the lateral lead (I or a VL), the incidence (43 cases; 88% vs. 5 cases; 9%) and degree (1.5 +/- 0.8 mm vs. 0.2 +/- 0.4) of ST elevation were significantly greater in Group I than in Group II. There was a significant inverse correlation (r = -0.57, p less than 0.01) between ST depression in the inferior lead and ST elevation in the lateral lead, but no correlation was found between anterior leads (V2-4) and the inferior leads. Elevation of diastolic pressure of the pulmonary artery (8.7 +/- 4.8 vs. 5.7 +/- 2.8 mmHg) and lowering of systolic pressure of the femoral artery (-25.7 +/- 24.3 vs. -11.8 +/- 8.3 mmHg) were significantly greater in Group I. These findings suggest that ST depression in the inferior leads during LAD angioplasty was due to a purely electrical phenomenon and not to the concomitant ischemia of the inferior wall. However, this change was more frequently seen in patients with larger myocardial ischemia and, presumably, was more often related to ST elevation in the lateral lead, which is reciprocal in position.

Cell-mediated cytotoxic activity of spleen cells from patients with gastric carcinoma.

The cell-mediated cytotoxic activities of cells from the spleens (SP cells) of patients with gastric carcinoma were assayed in comparison with the activities of peripheral blood mononuclear cells (PBM cells) from the same patients, and from patients with benign lesions. The natural killer cell (NK) activity of the SP cells and their capacity to generate allogeneic cytotoxicity in mixed lymphocyte culture (MLC) were very similar to those of the PBM cells. The cytotoxic activity of SP cells induced by alloactivation in MLC, however, was significantly higher than that of the PBM cells from the same patient as well as from patients with benign lesions. The production of interleukin 2 (IL 2) and the ability to induce cytotoxic cells after activation with IL 2 (LAK) were therefore examined. Both the ability to produce IL 2 and to generate LAK cells were shown to be significantly increased in SP cells when compared to PBM cells. These results indicate that the spleen may be a potential reservoir for the precursors of these activated killer cells in patients with gastric carcinoma. Furthermore, it may play an important role in the defence against tumors in these patients.

A pulsatile pump for cardiopulmonary bypass and its clinical use.

A pulsatile pump driven by a coil spring, which was designed and constructed by us, is described in this report. It consists of two main parts, a disposable blood chamber and a driving section. The blood chamber has two leaflet valves and a piston, which is covered with two bellofram rolling diaphragms and moves into the housing to draw in and eject the blood. The driving section consists of three cams, an electric motor and a coil spring. The ejection force is wholly produced by the compressed coil spring and is transmitted to the piston in the blood chamber by a rod. This pump allows the ejection pressure, the beat rates, and the stroke volume all to be changed independently. The performance of the pump was tested by using a circulation model where the beat rate was adjusted from 30 to 250 bpm. The output subsequently increased from 0.8 l/min to 5.7 l/min and the stroke volume, from 20.4 ml to 36.7 ml. This new pump has been used for clinical cardiopulmonary bypasses in 24 patients of open heart surgery and the pressure traces during perfusion resembled those of the patients' own hearts.

A trial of postoperative adjuvant combination chemo-immunotherapy for stage IV gastric carcinoma.

A chemo-immunotherapy program was designed on the basis of our clinical findings, which revealed that the ability to generate cell-mediated cytotoxicity was remarkably augmented after adriamycin (AM) administration in cancer patients. Twenty patients with stage IV gastric carcinoma who had undergone noncurative resections were treated with the above regimen that consisted of active immunotherapy with autologous tumor cells, in combination with 30 mg of AM, given 7 days before the immunization, followed by long-term tegafur (FT) and immunomodulators. The survival of these patients was compared to that of 3 other groups of patients, namely; 30 patients treated with another chemo-immunotherapy regimen which comprised autologous tumor cells in combination with several anticancer drugs followed by long-term FT and immunomodulators (CCI1), 17 patients treated with mitomycin C followed by long-term FT and immunomodulators (CI), and 24 patients with comparable histories (HC). The first treatment group had significantly improved survival, as compared to the HC group (p less than 0.01) and the survival tended also to be more favorable, when compared to the CCI1 group (p less than 0.2) or the CI group (p less than 0.2). Moreover, the survival rate at 9 months was significantly higher than that of either the CCI1 group (p less than 0.01) or the CI group (p less than 0.01).
