Pre-activation strategy for oligodeoxyribonucleotide synthesis using triaryloxydichlorophosphoranes in the phosphotriester method.

Triaryloxydichlorophosphoranes were tested as condensing agents for oligodeoxyribonucleotide synthesis in the phosphotriester method. Tris(2,4,6-tribromophenoxy)dichlorophosphorane (BDCP) was found to be a relatively stable crystalline material which could be used as a chemical reagent. A notable feature of the BDCP-promoted condensation reaction was studied by 31P-NMR. A small amount of BDCP compared to the conventional condensing agent was effective for the generation of active nucleotide intermediates and BDCP itself was quantitatively converted into an inert material, tris(2,4,6-tribromophenyl)phosphate (2). Thus, BDCP enabled us to separate the activation step from the condensation process in the phosphotriester method. This preactivation method was applied to the solid-phase synthesis.

[Clinical significance of bone scintigraphy for early detection of bone metastasis from breast cancer].

Forty four patients, presenting metastasis only to bone from a primary breast cancer previously undergone mastectomy from 1975 to 1985 at Kinki University Hospital, were investigated for the purpose of early detection of bone metastasis. Clinical follow up and bone scintigraphy at regular interval were the most important to detect the early phase of bone metastasis. The patients with early phase of bone metastasis, detected by regular scintigraphy in every 6 months, had the frequent metastatic lesions in rib and sternum as well as lumbar. These metastatic lesions could not be found by X-ray examination but were confirmed by aspiration cytology. Among the tumor markers, Al-p and LDH, rather than CEA and Ca, were more useful for the diagnosis of early phase of bone metastasis.

Immunocytochemical and biochemical analysis of epidermal growth factor receptor expression in human breast cancer tissues: relationship to estrogen receptor and lymphatic invasion.

Expression of epidermal growth factor receptor (EGFR) and estrogen receptor (ER) was examined by an immunocytochemical assay (ICA) using serial cross-sections of human breast cancer tissues. Immunocytochemical results were compared with those obtained by biochemical competitive binding assay and with histological lymphatic invasion. EGFR was evaluated as positive in 17 (34.0%) out of 50 primary tumors by ICA. A significant inverse relationship of the proportion of stained cells between EGFR and ER was demonstrated. In more than one-half of the tumors that were positive for both EGFR and ER, these 2 receptors were inversely stained in relation to the distribution. In ER-negative cells, EGFR expression was more marked than in ER-positive cells. Biochemical data confirmed the immunocytochemical results, pointing to an inverse relationship between EGFR and ER content. EGFR status correlated well with the degree of lymphatic invasion but not with the number of lymph nodes with metastases.

Prostatic carcinoma with melanosis.

Melanosis in which melanin deposition is found in the epithelia and stroma or blue nevus of the prostate gland is an uncommon lesion, and the histogenesis is still unclear. To our knowledge, there have been only two case reports in which prostatic melanosis and prostatic adenocarcinoma were both present. This article reports a third case having this combination and discusses the existence of melanin pigment in the carcinoma cell with melanosis.

Demonstration of in vivo effects of ACE inhibitors by the use of autoregressive modelling.

We investigated, with use of a technique of feedback analysis, the dynamic changes in and around the renin-angiotensin system in vivo induced by ACE inhibition. Prolonged studies were performed over a half year on rabbits, which were treated by daily injections of saline or one of the inhibitors as follows: captopril, foroxymithine, histargin and pepstatin. In spite of the apparently bizarre movements of the raw data, the analysis with autoregressive modelling unveiled the different modes of feedback regulations of the renin-angiotensin system under the influence of the various inhibitors. In order to interpret the in vivo actions of enzyme inhibitors, it seems essential to take feedback regulations into consideration.

The acceptable delay between biopsy and radical mastectomy in breast cancer patients.

The effects on the prognosis of breast cancer, of the delay between biopsy and radical mastectomy were studied in 394 patients. No delay had been experienced by 148 cases (no delay group), no biopsy had been performed in 166 cases (no biopsy group) and 80 cases had experienced a delay of 1 day or longer after having had a biopsy taken at a different institution (delay group). The recurrence rate of the no delay group was 10.8 per cent (16/148), whereas that of the delay group was 18.8 per cent (15/80). The relapse free survival rate of the no delay group was superior to that of the delay group (Kaplan Meier's method: p less than 0.05). The delay group was further divided into two groups according to the duration of delay, namely: a group whose delay was less than 7 days and another whose delay was longer than 8 days. There was no significant difference between the relapse free survival rates of the no delay group and the less than 7 days group, however, a significant difference was observed between the no delay group and the longer than 8 days delay group (p less than 0.05). The acceptable delay between biopsy and radical mastectomy may therefore be concluded as less than 7 days.

The discrepancy between immunocytochemical and biochemical assay of estrogen receptor in breast cancer patients treated by endocrine therapy.

Estrogen receptor (ER) expression was investigated by ER-immunocytochemical assay (ICA) and the dextran coated charcoal (DCC) method in 10 recurrent or primary-advanced breast cancer patients treated with endocrine or chemo-endocrine therapy. In 6 of these 10 patients, ER was examined both before and after treatments by the 2 methods. ER contents measured by the DCC method were found to be decreased after treatments, however, no change in the immunoreactivities of ER-ICA was observed. In the remaining 4 patients, the ER of new lesions refractory to endocrine or chemo-endocrine therapy was examined. ER status was determined as negative in 3 of the 4 patients by the DCC method, whereas by ER-ICA, the proportion of ER stained cells was about 70 per cent, those cells being diffusely distributed in the section. A discrepancy between ER-ICA and the DCC method was thus demonstrated in breast cancer patients treated by endocrine therapy.

A filter for use in lateral cephalography.

A new filter was devised for density compensation and absorbed-dose reduction in cephalometric radiographs. It is composed of a rare earth screen that plays a role in depicting soft tissue shadows and cephalometric points. The greatest reduction of the absorbed dose was about one seventh at the tube side of the thyroid gland when it was included in the direct x-ray beam. Smaller collimation was the most important factor in reducing the absorbed dose.

Ultrastructural and histochemical study of beta-tricalcium phosphate resorbing cells in periodontium of dogs.

beta-TCP was implanted in surgically prepared alveolar bone defects on the mesial side of the upper canine. The dogs that we used were sacrificed after 5 weeks, fixed by perfusion, and the beta-TCP resorbing cells were examined ultrastructurally and histochemically, with the following results: (1) beta-TCP was resorbed by macrophages and multinucleated giant cells. (2) Mitochondria, vacuoles and Golgi apparatus were abundant in beta-TCP-resorbing multinucleated giant cells that possessed neither ruffled borders nor clear zones. (3) The addition of tartric acid inhibited acid phosphatase activity in the cytoplasm of the multinucleated giant cells and macrophages.

Pharmacologic characterization of a novel non-benzodiazepine selective anxiolytic, DN-2327.

DN-2327, 2-(7-chloro-1,8-naphthyridin-2-yl)-3-[(1,4-dioxa-8- azaspiro[4.5]dec-8-yl)carbonylmethyl]isoindolin-1-one, produced anxiolytic, taming and anti-convulsive effects when administered orally to several species of animals. DN-2327 produced few of the sedative-hypnotic and muscle-relaxant effects observed with diazepam. The durations of the anxiolytic and anti-convulsive activities of DN-2327 were much longer than those of diazepam. Tolerance to DN-2327 did not develop when it was administered daily for 14 days in an anti-conflict test (Vogel conflict test). DN-2327 showed potent displacement activity against [3H]diazepam binding. The binding affinity of DN-2327 for benzodiazepine receptors was about twenty times that of diazepam. Furthermore, the affinity of DN-2327 for benzodiazepine receptors was not enhanced by the presence of GABA. There is a wide margin between the doses of DN-2327 that cause the anxiolytic effects and its sedative-hypnotic/muscle-relaxant effects. These results suggest that DN-2327 has more marked anxioselective properties compared with the benzodiazepines.

[Effects of nicotinate ethyl ester treatment on gingival blood circulation of experimental gingivitis in monkeys].

The effects of nicotinate ethyl ester treatment on the gingival blood circulation of plaque induced inflammation in monkeys were determined. The blood flow, amount of hemoglobin and oxyhemoglobin rate (oxyhemoglobin/total hemoglobin) in gingival blood circulation in addition to bacterial colony forming units (CFU), gingival index (GI), probing depth (PD), loss of attachment (LA) and amount of gingival crevicular fluid (GCF) were used as parameters. Besides 1% nicotinate ethyl ester paste (nicotinate paste), 0.5% chlorhexidine (CHX) paste as an antibacterial agent, a mixture of the two pastes and the base alone as a control were used. 1. The blood flow, amount of hemoglobin, CFU, GCF, PD and GI increased as inflammation developed, and these showed significant correlations. 2. The oxyhemoglobin rate showed no corresponding change with inflammation. 3. Nicotinate paste, CHX paste and the mixed paste seemed to reduce the inflammation of the gingiva as assessed by the blood flow, hemoglobin amount, CFU and GCF amount compared with the control paste.

[Clinical evaluation of hydroxyapatite implants in human periodontal osseous defects. Clinical observations over a one-year period following surgery].

The purpose of this study was to evaluate the effect of implants of granular hydroxyapatite (HAP). HAP was implanted into twenty-five vertical bone defects of twenty-one patients as bone graft material. Various clinical and radiographic examinations were performed postoperatively over a twelve month period. Redness and swelling of the gingiva, gingival bleeding, postoperative pain and increased tooth mobility developed transiently, but they all recovered in time. Open wounds and out-flow of HAP disappeared within the first month. After twelve months, mean probing depth decrease was 3.7 mm and clinical attachment gain was 2.5 mm. In all cases there was radiographic evidence of alveolar bone repair. These results suggest that HAP is clinically effective as a bone graft in periodontal therapy.

[Frequency of three-rooted mandibular first molars. Survey by x-ray photographs].

The frequency of three rooted mandibular first molars was surveyed. Subjects were of 1,353 male and female students trained in radiology at Fukuoka Dental College whose average age was 24.3 years old. Dental (complete radiographic survey using 14 films) and panoramic radiographs of their mandibular first molars were taken during a period of 9 years from 1978 to 1986. The results were as follows: 1) The number of the three-rooted mandibular first molars was 440 (18.8%) of 2,331 teeth, including 240 (20.6%) of 1,163 in the right side and 200 (17.1%) of 1,168 in the left. 2) 274 (11.8%) of the three-rooted mandibular first molars were identified by radiographs of the premolar region, 42 (1.8%) by radiographs of the molar region, 124 (5.3%) by radiographs of both the premolar and the molar region. 3) Panoramic radiography identified 70 (15.9%) out of 440 three-rooted mandibular first molars. 4) 136 (12.7%) of 1,070 subjects who had bilateral mandibular first molars had three-roots on both sides 127 (11.9%) of 1,070 had them on one side.

Possible relationship between poly(ADP-ribose)synthetase and formation of antibody against acetylcholine receptors in patients with myasthenia gravis.

We examined the relationship of the serum levels of antibody against acetylcholine receptors to the serum levels of 13 enzymes, including various hydrolytic enzymes, poly(ADP-ribose)synthetase (Poly(ADP-ribose)Syn), and sialyltransferase (NANA-trans), in patients with myasthenia gravis. The patients were divided into two groups, depending on the presence or absence of thymoma. In spite of the absence of significant difference in the absolute levels of individual enzymatic activities between the two groups, the network relationships of such enzymes were quite different between the two groups. Of the 13 enzymes examined, only Poly(ADP-ribose)Syn showed a weak but significant correlation with the level of the antibody in the patients without thymoma. A multivariate study more clearly suggested the relationship between the antibody formation and Poly(ADP-ribose)Syn in the patients without thymoma. Such observations were not found in the patients with thymoma.

[Three cases of effective hepatic arterial infusion with OK-432, mitomycin C and 5-fluorouracil in liver metastasis from gastric cancer].

A combined therapy with OK-432. mitomycin C (MMC) and 5-fluorouracil (5-FU) by administration through hepatic arterial infusion was performed in three patients with liver metastasis from gastric cancers. A catheter was introduced into hepatic artery by laparotomy. The dosage was 250 mg/day for 5-FU continuously, 10 mg/week for MMC and 5 KE/week for OK-432, respectively. Two cases with advanced gastric cancer and synchronous hepatic metastasis were treated by hepatic arterial infusion after surgical removal of primary lesion. Metastatic lesion in liver was completely eliminated. Another case with metastatic liver tumor from gastric cancer was also treated by the same protocol. The reduction rate of this case was 70% by CT scan. These results suggested that combined therapy with OK-432, MMC and 5-FU by hepatic arterial infusion is a good modality for liver metastases from gastric cancers.
