Cerebral hemodynamics in patients with moyamoya disease. A study of the epicerebral microcirculation by fluorescein angiography.

Fluorescein angiography was performed in 12 hemispheres of eight young patients with moyamoya disease to study the epicerebral microcirculation. Regional circulation time and the time interval between carotid injection of the dye and first filling of the cortical arteries were various and prolonged in the most parts. There were nonfilling areas by carotid injection of the dye in six hemispheres. By venous injection, the dye appeared in all vessels of the exposed areas. Extravasation of the dye was seen in four hemispheres. The degree of these abnormalities of the epicerebral microcirculation was well correlated with the findings in conventional cerebral angiography.

Dural arteriovenous malformations of the base of the anterior cranial fossa.

Two cases of dural arteriovenous malformation at the base of the anterior fossa are reported. The angiographic appearance in both cases is quite uniform and resembles the findings in 12 cases previously reported. Feeders are anterior and/or posterior ethmoidal arteries and the external carotid system also contributes. A drainage vein always shows aneurysmal dilatation of its proximal portion. CT findings are also mentioned and radiological literature reviewed.

Rupture of anterior communicating artery aneurysm during transsphenoidal surgery for pituitary adenoma.

The case of a 55-year-old man with a nonfunctioning pituitary adenoma associated with an anterior communicating artery aneurysm that ruptured during transsphenoidal surgery is presented. This small aneurysm was not detected on bilateral angiography of the carotid artery before surgery. Computed tomography (CT) scans showed the pituitary adenoma extending to the suprasellar region and the sphenoid sinus. After intracapsular removal of the tumor, cerebrospinal fluid from an intraspinal catheter became bloody and CT scans disclosed massive subarachnoid hemorrhage. Repeated angiography revealed a tiny anterior communicating artery aneurysm, which was clipped on the 37th day after the operation. Risk factors in the procedure of transsphenoidal surgery, especially of a pituitary adenoma with suprasellar extension, are discussed.

Treatment of moyamoya disease by temporal muscle graft 'encephalo-myo-synangiosis'.

10 young patients from 7 to 16 years of age with moyamoya disease were treated by temporal muscle graft (encephalo-myo-synangiosis: EMS) for the purpose of increasing extracranial and intracranial collateral circulation. Ischemic cerebrovascular symptoms and signs were encountered in all of them without intracranial hemorrhage. The patients were observed for 6-26 months after the first operation. Transient ischemic attacks disappeared in 4 out of 7 patients, and improved in frequency and duration in the remaining 3. Paresis of the limbs disappeared in 1 patient and improved in 2. In 2 patients, facial palsy disappeared and dysarthria improved as well. Ataxia disappeared in 2 patients, and involuntary movement of the extremities disappeared in 2. There were no ineffective cases clinically. In postoperative carotid angiography, the middle cerebral arteries were visualized clearly to the peripheral portion mainly via the thickened deep temporal arteries, and the abnormal vascular networks in the region of the basal ganglia reduced in size in most of patients. Improvements in intelligence quotient, electroencephalogram and cerebral blood flow were also obtained in several patients, respectively. EMS seemed to be an effective surgical treatment in young patients with moyamoya disease who suffered from cerebral ischemic symptoms.

[Analysis of 223 cases of intracranial aneurysms--with special reference to re-rupture of the intracranial aneurysms].

Reviewing 223 cases of intracranial ruptured aneurysms including 46 cases of re-ruptured aneurysms, the following results were obtained. 1) The outcome of the re-rupture aneurysms was more miserable than that of the first rupture. Even low grade cases (Hunt and Hess 0-II) of re-rupture showed no better outcome than high grade cases (Hunt and Hess IV) of first rupture. 2) Re-rupture most often occurred on the 12th (11.6 +/- 6.7) day after the first attack of the subarachnoid hemorrhage. Re-rupture more often occurred in the vertebrobasilar system. In the internal carotid artery and its branches, aneurysms with daughter or bleb tended to rupture again. Aneurysms with daughter, regardless of these location, showed greater incidence of re-rupture than those without daughter. Also rupture often occurred in the cases of lower grade with less vasospasm and with the acute hydrocephalus. 3) It was suggested that the sequential changes of some factors of both the coagulating system and fibrinolytic system might forecast the vasospasm, infarction, re-rupture and prognosis of the patient of intracranial ruptured aneurysms. And therefore, in the cases in which re-rupture is likely, operation should be performed as early as possible, except in those showing diffuse vasospasm or clinical down hill course or in grade V.

Multi-enzyme membrane electrodes for determination of creatinine and creatine in serum.

An enzyme electrode system for the determination of creatinine and creatine was developed by utilizing three enzymes: creatinine amidohydrolase (CA), creatine amidinohydrolase (Cl), and sarcosine oxidase (SO). These enzymes were co-immobilized onto the porous side of a cellulose acetate membrane with asymmetric structure, which has selective permeability to hydrogen peroxide. Two kinds of multi-enzyme electrodes were constructed by combining a polarographic electrode for sensing hydrogen peroxide and an immobilized CA/Cl/SO membrane or Cl/SO membrane for creatinine plus creatine or creatine, respectively. The multi-enzyme electrodes responded linearly up to 100 mg of creatinine and creatine per liter in human serum. Response time was 20 s in the rate method and the detection limit was 1 mg/L. Only 25 microL of serum sample is required. Analytical recoveries, precisions, and correlations with the Jaffé method were excellent, and the multi-enzyme electrodes were sufficiently stable to perform more than 500 assays. No loss of activity of immobilized enzymes was observed after nine months of storage at 4 degrees C in air.

[Metallurgical study on Heifetz clip failure (author's transl)].

A metallurgical study on a fractured aneurysm clip (Heifetz) was described. The conclusion can be summarized by the following: 1) Metallurgical data clearly indicate that the mode of failure is intergranular creacking--probably form a stress-corrosion mechanism. 2) Surgically used aneurysm clip is always in corrosive environments form the metallurgical points of view. Apart from inadequate sterilization was well as mishandling, therefore, corrosion of a metal clip is unavoidable. 3) A clip with multiple parts and made from different alloys may have a greater tendency to corrode than a clip of the one-piece variety. In this meaning Yasargil's clips without strut may be most useful.

Computed tomography in moyamoya disease.

Eighteen patients with moyamoya disease were examined by computed tomography (CT). At the first examination, CT findings were normal in one patient and abnormal in 17 patients. Significant abnormalities on precontrast CT were characterized by low density lesions in 14, ventricular dilatation in 9, and cerebral atrophy in 11 patients. Postcontrast CT showed the presence of contrast enhanced lesions in three patients. Follow-up examinations were performed in 13 patients, and the following changes on CT were found in 10 of them: increase or development of low density lesions in 10, ventricular dilatation in 4, cerebral atrophy in 5, appearance of a contrast enhanced lesion in 1, and disappearance of the enhanced lesion in 3. The patient with normal findings at the first examination showed a low density lesion on the second examination. The abnormal vascular networks in the region of the basal ganglia were visualized successfully in six examinations of five patients by higher concentration of contrast medium in blood than usual. Abnormal CT findings consisted of those seen in multiple cerebral infarctions and the visualization of the abnormal vascular networks. Computed tomographic findings on these patients showed evolution as seen in clinical and angiographic findings.

H2Kk can influence whether cytotoxic T lymphocytes recognize trinitrophenyl in association with H2Dk unique or H-2Kk and H-2Dk shared self determinants.

The present study investigates the effect of trinitrophenyl- (TNP) modified H-2Kk (TNP-Kk) antigens on the generation of anti-TNP-Dk restricted cytotoxic T lymphocyte (CTL) responses. C3H.OH mice were primed to TNP-self by skin-painting with trinitrochlorobenzene, and spleen cells from these primed mice were subsequently stimulated in vitro with TNP-self. The effector cells generated exhibited appreciable lysis of TNP-modified C3H.OH blast target cells. Cold target inhibition studies demonstrated the generation of two effector cell populations: one that recognizes TNP in association with unique Dk self determinants, and one that recognizes TNP in association with self determinants shared between TNP-Kk and TNP-Dk. This was in contrast to primed C3H/He spleen cells, which did not generate CTL that recognized TNP in association with unique Dk self determinants. When spleen cells from (C3H/He x C3H.OH)F1 mice primed to TNP were stimulated in vitro with TNP-C3H.OH cells, unique Dk self determinants were recognized in association with TNP. However, in vitro stimulation of the same F1 responding cells with TNP-C3H/He or TNP-F1 cells failed to elicit CTL that utilized these Dk-unique self determinants. The findings of this study demonstrate that unique or shared H-2Dk determinants can be differentially utilized by CTL populations, depending on the H-2 alleles expressed by the stimulator cells.

Effect of helper T cells on prevention of tolerance induction in effector T cells responsible for syngeneic tumor immunity.

The present study investigates the role of helper T cells in preventing tolerance in effector T cells responsible for syngeneic tumor immunity. Tolerance in T cell-mediated immune responses against X5563 plasmacytoma was induced in syngeneic C3H/He mice by intravenous (iv) administration of 7,000 R X-irradiated X5563 tumor cells. Since the anti-X5563 tumor tolerance was observed irrespective of whether the tumor cells had been modified with trinitrophenyl (TNP) hapten prior to iv inoculation, the ability of TNP-reactive helper T cells to prevent the tolerance induced by iv inoculation of TNP-X5563 tumor cells was examined. TNP-reactive helper T cell activity was generated in C3H/He mice by immunization with TNP-isologous mouse gamma-globulin after inoculation with the TNP-conjugate of a non-immunogenic co-polymer of D-glutamic acid and D-lysine. Intravenous inoculation with TNP-modified X5563 cells induced tolerance in mice not primed with TNP. However, such tolerance was prevented in mice in which TNP-reactive helper T cell activity had been generated. These results indicate that helper T cells play a crucial role in preventing effector T cell tolerance. The mechanisms by which tolerance induction in effector T cells is circumvented are discussed in relation to the generation and delivery of signals required for T cell activation.

Brain tumors in tuberous sclerosis. Report of 4 cases.

We report on 4 cases of brain tumors associated with tuberous sclerosis. In each case the tumor was in the region of Monro's foramen and was associated with ventricular dilatation. Computer tomography scans, which were performed in 3 cases, revealed well-circumscribed high-density masses which were markedly enhanced by contrast medium. 3 patients underwent craniotomy. Total extirpation was performed in 2 patients. Both of them are doing well postoperatively. However, 1 patients whose tumor had been partially resected died of recurrence 6 years after surgery. The pathology of three tumors was different in each case: plump cell astrocytoma, hemangioma and astroblastoma.
