A total of 24 patients with urothelial cancer were treated with a combination chemotherapy consisting of vincristine (1 mg, i.v., day 1), bleomycin (15 mg, or peplomycin, 10 mg, i.m. day 2), mitomycin-C (4 mg, i.v. day 3), tegafur (600-750 mg, p.o., every day) and OK-432 (2-4 KE, i.m., day 1, 3, 5). This one-week course was repeated for an average of 4.6 courses. Of these cases, 13 patients with early stage treated as adjuvant chemotherapy after radical operation showed strong suppressive effects against tumor recurrence or metastasis except 3 cases. However, in advanced cancer this regimen was less effective. Therefore, we modified this regimen: VCR 2mg and PEP, 10 mg on days 2, 4 and 6. This modified regimen was given to two patients and one showed partial response and I-A according to Karnofsky's criteria. Mild side effects such as slight leukopenia, fever up, gastrointestinal symptoms or alopecia were observed. These results suggest that this regimen could be used as an adjuvant chemotherapy after radical operation of urothelial malignancy for the purpose of protecting tumor recurrence or metastasis.

Occurrence of tumor of pineal region in three brothers was presented and previously reported cases of familial occurrence of primary intracranial tumor were reviewed. Two cases were diagnosed as germinoma clinically. The other case was diagnosed as embryonal carcinoma by pathohistological examination. All cases were of germ cell tumor. Reports of pineal tumors occurring in siblings are extremely rare. There have been three reports in the literature but no case of familial occurrence of histologically verified germ cell tumor in the pineal region in three siblings has been reported. Genetic aspects of primary intracranial tumors have been discussed but there has not been established a close relationship between heredity and oncogenecity. This case is a very interesting case because it is not only a rare case of familial occurrence of pineal tumor but it suggests the propriety of the theorum dealing with germ cell origin tumor.

A comparative clinical study of cefaclor (CCL) with cephalexin (CEX) was carried out by randomized double-blind techniques in order to contemplate the clinical efficacy, side effects and usefulness in treatment of 243 patients with dental infections. In the CEX group, CEX was orally administered 4 times a day at a daily dosage of 1,000 mg for 3 to 5 days. In the CCL group, CCL was orally administered 3 times a day at a daily dosage of 750 mg for 3 to 5 days and 1 capsule of placebo was also given every evening in order to keep the blindness of the administration. Evaluable cases for efficacy of athe drugs were 213 consisting of 108 for CCL and 105 for CEX. There were no significant differences in background of the patients and severity of the disease between 2 treatment groups. Clinical effectiveness on the 3rd day was 89.7% in CCL group and 78.6% in CEX group, showing significant difference between 2 treatment groups. Clinical effectiveness on the final day of administration was 94.4% in CCL group and 92.4% in CEX group, showing no significant difference between 2 treatment groups. Side effects were found in 10.5% of 114 patients receiving CCL and in 4.5% of 112 patients with CEX, and there was no significant difference between 2 treatment groups. The side effects were mostly gastrointestinal origin. According to the judgement by physicians in charge, no significant difference was seen in clinical usefulness between the 2 drugs.

A case of asynchronous bilateral non-specific segmental ureteritis is presented. The preoperative evaluations for this lesion were considered extremely important, because the radiological and operative findings of this disease are similar to those of ureteral cancer. This lesion has the possibility of multiplicity, especially bilateral occurrence. Conservative management of the kidney should be performed.
