[Clinical analysis of renal trauma].

Since the start of the urological service in our hospital in 1986, we have experienced six male and one female patients with renal trauma. These cases were clinically analyzed and compared with hepatic trauma treated during the same period in our hospital. There were two cases of renal contusion, laceration and injury to the pedicle, and one of hemorrhage into the cyst in polycystic kidney. The causes of the trauma were mainly traffic accidents. Among them, 4 patients had injuries to other organs such as liver and brain and to the thorax. Two cases were able to retain both their kidneys, but the rest were treated with single nephrectomies. Six of the seven patients are alive. This proved that computerized tomography was very useful to decide the severity of injury, presence of lesions to other organs, and the treatment method. No close relation was observed between the time from injury to operation and the amount of blood transfusion. Moreover, hematological and biochemical analysis did not express the patient's status.

[Repair of pure mitral insufficiency--experience in 50 patients].

Between February 1982 and March 1987, 50 adults underwent mitral valve repair for pure mitral insufficiency representing 54% of all adults having mitral valve surgery for pure mitral insufficiency. Ages ranged from 20 to 73 years (50.0 +/- 11.6). Anterior leaflet prolapse was present in 11, posterior leaflet prolapse in 26 and annulus dilation in 13. The mitral valve was repaired by quadrangular resection in 21, triangular resection in 2, plication without resection in 4, chordal shortening in 8 and annuloplasty alone in 15. Commissural annuloplasty was performed in addition to leaflet repair or chordal shortening in other 14 patients. The competence of the mitral valve was usually evaluated under beating conditions. 79% of the attempted repairs was considered successful in the posterior leaflet, contrasting to only 37% in the anterior leaflet. Prolapse of the anterior leaflet remains a surgical challenge. There were two early deaths (4%). During the follow up period (Mean 1.9 years, range 0.2-5.2 years), 2 late deaths (4%) were observed, one of them from thromboembolism, before adopting the policy of routinely anti-coagulating these patients during the first few months. 4 patients (9%), early in our experience, required reoperation within 4 months of surgery: two for severe mitral insufficiency and two for severe mechanical hemolysis. At reoperation, residual insufficiency was present in one, valve suture tear due to technical imperfection in two and complete disruption of the valve tissue at the suture line in one. Four additional patients had transient mechanical hemolysis requiring no specific therapy.(ABSTRACT TRUNCATED AT 250 WORDS)

[A successful surgical case of aortic arch aneurysm associated with aortic regurgitation].

A 57-year-old male patient with aortic regurgitation and aneurysm of the ascending aorta and the aortic arch underwent aortic valve replacement and graft replacement from the ascending aorta to the aortic arch. The operation was done using cardio-pulmonary bypass and selective cerebral perfusion with deep hypothermia. Postoperative DSA revealed no dilatation of the sinus Valsalva and a good configuration of the anastomosis. It seems that selective cerebral perfusion with deep hypothermia is a safe method to prevent cerebral damage in a case of arch aneurysm.

[Transcatheter arterial embolization for cystic hepatic metastases from leiomyosarcoma of gastrointestinal tract].

Transcatheter arterial embolization was performed in three patients with cystic hepatic metastases from leiomyosarcoma of gastrointestinal tract. In one patient who had been alive and examined by CT scanning for more than 3 years, it was considered as valid for evaluating efficacy of TAE that the size reduction of cystic lesions followed the reduction of mural nodules and smoothing of marginal irregularity.

[Intra-arterial chemotherapy of relapsed and hormone-resistant prostate cancer using reservoir system].

Ten patients with relapsed and hormone-resistant prostate cancer were given intra-arterial infusion with, mainly, cisplatin using the reservoir system. The tip of the indwelling infusion catheter was inserted from the femoral artery into the internal iliac artery or common iliac artery. The opposite end of the infusion catheter was connected to a reservoir implanted subcutaneously at the thigh portion. Combination chemotherapy using methotrexate, adriamycin and cisplatin (MAC therapy) was mainly performed. According to criteria of the Jpn. Assoc. for Cancer Ther., the response rate was 23%, including 3 or PR cases. Regarding the survival rate, the 1-year survival rate was 66.7% and the 2-year rate was 33.3%. Concerning adverse reactions, nausea, vomiting and anorexia were noted in all cases. Stomatitis, leukopenia and thrombocytopenia were also found in 38%. We consider that the IA-MAC therapy is one of the most useful regimen for the treatment of the relapsed and/or hormone-resistant prostate cancer.

[Development of visual evoked potentials in guinea pigs].

The development of visual evoked potential (VEP) was studied in guinea pigs as a function of age. Ten newborn guinea pigs from 3 litters were used, and experiments were carried out with the animals over an age span ranging from 1 to 90 days. Periodic recordings were obtained at 5 day intervals from the first up to the twentieth day after birth and at 30 day intervals from the thirtieth to ninetieth day after birth. The results were as follows: 1) The VEP comprised 7 peaks at the first day after birth, while after the fifth day it was composed of 9 peaks. 2) During the development of the guinea pigs, the latencies of VEP decreased. The reduction of the latencies of peak N2 amounted to about 25% from the first to the thirtieth day. 3) The amplitude of the initial negative spike increased from the first to the fifth day after birth, and decreased after the fifth day.

Metastatic renal tumor from the lung with regional lymph node involvement: a case report.

We report a case of metastatic renal tumor from lung cancer. A 53-year-old man presented with gross hematuria 2 years after treatment of the primary lesion. Investigations suggested a metastatic tumor in the right kidney from the lung which was accompanied with regional lymph node metastasis. Surgical treatment was not performed because of his poor condition and the lymph node involvement. Therefore, the combination chemotherapy of 5-fluorouracil, vincristine and doxorubicin, OK-432 was applied. Although this conservative management was effective, he died of progression 6 months later and an autopsy confirmed the diagnosis of metastatic renal tumor.

[A case of granulomatous orchitis in a hemodialysis patient].

A case of granulomatous orchitis in a 47-year-old patient is described. He had a history of recurrent pyelonephritis and subsequent chronic renal failure, and was under treatment with hemodialysis therapy. He presented with painful swelling of the right scrotal contents accompanied by fever. After the unsuccessful treatment with antibiotics, an exploration of the right scrotal contents revealed the testis replaced by a hard yellow mass. An orchiectomy was performed and granulomatous orchitis was diagnosed pathologically. The clinical and etiological aspects of this rare inflammatory disease of the testis are reviewed. It is important to recognize this disease, because this is often initially thought to be neoplastic. Although the cause of this disease is still controversial, it is supposed to be associated with sperm extravasation caused by urinary tract infection or trauma.

Characteristic sequences in the upstream region of the human tyrosinase gene.

A human tyrosinase gene containing a 5'-flanking region and the first exon was isolated from a normal human liver library using mouse tyrosinase cDNA as a probe for screening. In the 5'-upstream region of transcriptional start sites, there is a putative TATA box and CAAT box and some palindromic sequences. A characteristic sequence of (GA:TCn, which can assume a hinged-DNA structure, was present from -869 nt to -633 nt in this 5'-flanking region.

An enzyme immunoassay of slow moving protease (SMP) in human gastric mucosa.

A two-site enzyme immunoassay for slow moving protease (SMP) of human gastric mucosa has been developed. The detection limit of SMP was 50 pg/tube and the optimal assay range was 0.5 to 50 ng/tube. This assay system made it possible to measure a trace amount of gastric mucosal SMP in endoscopic biopsy-specimens. Mean gastric mucosal SMP contents of the antrum, the angles, and the distal and proximal corpuses were 4.5, 3.7, 2.6, and 2.5 micrograms/mg protein, respectively. The antral mucosa contained a larger amount of SMP than the gastric mucosa of other area. The present enzyme immunoassay system is sufficiently sensitive for the clinical study of SMP in gastrointestinal tract.

Effect of thyroid hormone on somatomedin-C release from perfused rat liver.

The effect of thyroid hormone on plasma somatomedin-C (SmC) level and on SmC release from perfused rat liver was investigated. Plasma SmC levels and liver tissue SmC were significantly increased in thyroxine-treated rats. Physiological doses of triiodothyronine increased SmC release and SmC concentration in the perfused rat liver. These results indicate that thyroid hormone directly enhances the synthesis and release of SmC in the rat.

Induction by the guanosine analogue oxanosine of reversion toward the normal phenotype of K-ras-transformed rat kidney cells.

Oxanosine, a guanosine analogue antibiotic, altered the "transformed" morphology of rat kidney cells integrating a temperature-sensitive K-ras gene into "normal" morphology at 34 degrees C, a permissive temperature. Oxanosine also increased the cellular content of fibronectin to the normal level. The cells under these conditions had lower levels of guanine nucleotides and unstable and less palmitylated Mr 21,000 protein (K-ras gene product, referred to as p21). These changes in p21 were the same as those which occurred to p21 in the cells cultured in the absence of oxanosine at 39.5 degrees C, a nonpermissive temperature.

Functional characterization of the CD45R (2H4) molecule on CD8 (T8) cells in the autologous mixed lymphocyte reaction system.

In the present study, we have investigated the molecular basis for the immunoregulatory function of CD8 cells after autologous mixed lymphocyte reaction (AMLR) activation. We demonstrated that the CD8+CD45R+, but not the CD8+CD45R- subset of cells effected suppression following AMLR activation. In contrast, cytotoxic activity against alloantigens resided in both the CD8+CD45R+ and CD8+CD45R- subsets of cells. Biochemical analysis showed that on CD8 cells, the 220-kDa isoform of the LCA/T200 antigen family was better represented than the 200-kDa isoform, when compared to CD4 cells. The density of the CD45R antigen increased on CD8 cells following activation in AMLR and treatment of AMLR-activated CD8 cells with either anti-CD45R antibody or anti-CD3 antibody abolished the suppressor function of these cells. In contrast, treatment of AMLR-activated CD4 cells with anti-CD45R, but not anti-CD3 antibody, abolished the suppressor/inducer function of these cells. The results suggest that the CD45R antigen as well as CD3 T cell receptor complex have an important role in the suppressor function of AMLR-activated CD8 cells.

Retrovesical squamous cell carcinoma associated with toxoplasmosis: diagnostic images, serum detection of SCC-antigen, and successful treatment by M-VAC regimen.

A case of retrovesical squamous cell carcinoma associated with acquired toxoplasmosis is presented. To date only six cases with retrovesical carcinoma have been reported in the literature. Although it is difficult to make a preoperative diagnosis of this rare neoplasm, transrectal longitudinal ultrasonography was diagnostically valuable, and serial measurements of serum squamous cell antigen (SCC-Ag) were useful for monitoring the clinical course.

Central vasomotor control of the rabbit portal vein.

Contractile responses of the portal vein of the anesthetized rabbit were measured quantitatively by plethysmography during transmural electrical field stimulation (TES, 0.8 ms and 15 V) and during the cerebral ischemic pressor response at various volemic states. To evoke the cerebral ischemic response, the route of blood supply to the brain was surgically restricted to the right internal carotid artery, the artery was then compressed in a stepwise fashion by a micrometer device. The maximum contractile response of the portal vein segment that could be evoked by cerebral ischemia corresponded in magnitude to that produced by TES of 10-11 Hz. The contractile response began when the internal carotid blood flow was reduced to 4 ml/min from its normal value of 13.8 +/- 1.2 (mean +/- SE) ml/min and reached a maximum at 0 ml/min. The maximum contractile response was an increase of 26% from control value under normovolemic condition, 20% after hemorrhage and 31% after volume loading. It was estimated that the contractile response in normovolemia was 95% neurogenic, the rest was thought to be of humoral origin.

CCA [N-(2-carboxyphenyl)-4-chloroanthranilic acid disodium salt], a newly developed immunomodulating drug, suppresses T-cell activation by acting on macrophages.

The cellular mechanism of action of a newly developed drug, CCA, N-(2-carboxyphenyl)-4-chloroanthranilic acid disodium salt, on PHA-, autologous mixed lymphocyte reaction (AMLR)-, and phorbol myristate acetate (PMA)-stimulated T-cell proliferation was investigated. Addition of 50 micrograms of CCA per milliliter suppressed PHA- and AMLR-stimulated T-cell proliferation. In contrast, CCA failed to suppress PMA-stimulated macrophage-depleted T-cell proliferation. After treatment of T cells or macrophages with CCA for 12 h, recombined T cells and macrophages were stimulated with phytohemagglutinin. [3H]Thymidine incorporation by T cells was suppressed when macrophages but not T cells were treated with CCA. These results indicate that CCA suppresses T-cell proliferation by acting on macrophages. The mechanism involved in this suppression of CCA was due to the loss of Ia antigen on macrophages and the loss of interleukin-1 (IL-1) secretion from macrophages.
