[Supplemental studies on self-curing resins. 2. On polymerizing temperature of reline materials].

The objective of this study was to find the best way to keep down the peak temperature of self-curing reline materials. The experiments were done by using three kinds of self-curing reline materials (Rebaron, REBASE and KOOLiner) and three kinds of the reline materials of different thickness (1.5mm, 2mm and 3mm) under three different intraoral conditions. When these reline materials cured under rinsing the mouth with cold water (18 degrees C), the data analysis of these reline materials showed a considerable reduction in the peak curing temperatures clinical acceptance. The results obtained were as follows: 1. Both extraoral and interoral curing tests showed that the thicker the material is the higher the peak temperature becomes. 2. Highly significant differences in time to the peak temperature were found among the three kinds of reline materials. 3. Dangerously high temperature was found in polymerizing of the thickest material, 3mm.

[Dental treatment for children under general anesthesia].

A clinico-statistical survey was carried out on dental treatment for children under general anesthesia who visited the clinic of Pedodontic Department, Matsumoto Dental College Hospital during the period of 1986 to 1989. The number of cases subjected in this survey was 108 (57 boys 51 girls), aged from one year and 5 months to 8 years and 9 months. All of these patients were admitted to the hospital for 3 days and were treated under general anesthesia, using GOE in 107 cases (in 80 cases by nasal intubation, in 27 cases by oral intubation). One case was administrated by intravenous anesthesia with Ketamine. The results were as follows: 1. The patient ages of the patients ranged from one year 5 months to 8 years 9 months. 2. Thirteen percent of the patients were handicapped. Fifty three percent of the patients had to visit hospitals in remote areas. 3. The number of restored teeth per patient was 15 on the average. The teeth were treated with composite resin filling in 8.5 teeth, composite resin crown in 2.6 teeth, metal crown 3.6 teeth and other material in 0.8 (extraction). 4. Pulpal treatment was carried out on 42 percent of the subjected teeth. 5. The average anesthesia administration time was 3 hours and 28 minutes. No serious trouble was observed although minor complications were seen post-operatively such as high fever or vomiting in some cases.

[A case of silicate urolithiasis].

We report a case of silicate calculi with no history of taking magnesium trisilicate. A 33-year-old woman was sent to our hospital as an emergency case because of severe right lower flank pain. Physical examination was unremarkable except for severe right cost-vertebral angle knock pain. She denied administration of a magnesium trisilicate anti-acid before. She was admitted to the urologic ward since the pain did not relieve in spite of several analgesics. The stone passed spontaneously on the third hospital day. Analysis by infrared spectrophotometry demonstrated the composition to be over 98% of silicate. A review of the literatures discloses only 21 cases of silicate stones.

[Combination chemotherapy including cisplatinum, vindesine and mitomycin C for non-small cell lung cancer].

Thirty-five patients with non-small cell lung cancer (adenocarcinoma: 15, squamous cell: 15, large cell: 5, Stage I: 3, II: 1, IIIA: 8, IIIB: 5, IV: 18) were treated with combination chemotherapy, CDDP 80 mg/m2 day 1, VDS 3 mg/m2 days 1 & 8, and MMC 8 mg/m2 day 1. This regimen was repeated two times at intervals of four weeks. Sixteen partial responses were observed (response rate of 45.7%), and the median duration of survival for responders was 12.6 +/- 7.5 months and for nonresponders 10.1 +/- 4.8 months. Statistically there was no difference between them, but a survival from the onset of therapy of responders was significantly better (p less than 0.05) than that of non-responders. Toxicity was tolerable with myelosuppression, chemotherapy-induced nausea and vomiting.

Three main components in plasma proteases and their relation to the renin-angiotensin system.

The relation of plasma renin activity (PRA) and plasma levels of angiotensin I (AI) and II (AII) to those of various proteases, including eight endopeptidases and four aminopeptidases, was investigated in 51 normal control subjects. The multivariate study using factor analysis showed that the plasma proteases can be classified into three main components: the aminopeptidase, the plasmin, and the kinin-kallikrein. PRA and AI were related almost exclusively to the aminopeptidase component, while the AII level was related not only to the same component but also to the kallikrein-kinin component. This kind of multivariate study may help in the elucidation of the role of proteases and bioactive peptides, such as angiotensin derivatives, in essential hypertension through a comparison of multivariate relationships in controls and patients.

The relationship between the thiamin pyrophosphate effect and the saturation status of the transketolase with its coenzyme in human erythrocytes.

The thiamin pyrophosphate effect has been used as a reliable index to evaluate the nutritional status of thiamin. But there has not been any report concerning whether or not the thiamin pyrophosphate effect really reflects the saturation status of transketolase with thiamin pyrophosphate. In this report we studied the relationship between the thiamin pyrophosphate effect and the saturation status of transketolase. First, we determined the thiamin pyrophosphate concentrations, transketolase activities, thiamin pyrophosphate effects, and transketolase concentrations in human hemolysates from 16 apparently healthy subjects. The molar ratio of thiamin pyrophosphate to transketolase was in inverse proportion to the thiamin pyrophosphate effect. Second, we prepared apotransketolase preparations and reconstituted it with various concentrations of thiamin pyrophosphate. The thiamin pyrophosphate effects in these preparations were in good correspondence with the ratios of apotransketolase. These results indicate that the thiamin pyrophosphate effect really reflects the saturation status of transketolase with coenzyme.

[A case of bilateral adrenal myelolipoma].

We reported a case of bilateral adrenal myelolipoma. Low height, obesity and and elevation of 17-KS were found in this case. Tumors were hypovascular adrenal masses with low attenuation numbers on CT. MRI was useful in demonstrating the relation of tumors to adjacent organs. It should be considered that, although very rare, bilateral myelolipoma is possible.

[A study on endoscopic hemostasis from the viewpoint of morphologic traits of Dieulafoy ulcer--an attempt at endoscopic hemostasis by local injection of aethoxysklerol and ethanol].

Characteristics of the vascular course of ruptured vessels in the submucosal layer of Dieulafoy ulcer were studied, and endoscopic hemostatic treatment was performed using Aethoxysklerol and ethanol. 1) Observations of vessel courses in serial sections revealed that ruptured vessels in Dieulafoy ulcer exhibit rhexis in the lateral portions of the vessel and long courses over the ulcer margin in the submucous layer. 2) More dependable hemostasis seemed to be provided by thrombosis not only at the source of bleeding. i.e., at the site of vessel rupture, but also in the region of the whole ruptured vessel in the submucous layer. 3) Hemostatic treatment by local injection of Aethoxysklerol and ethanol was performed in 4 patients with Dieulafoy ulcer, and complete hemostasis was achieved in all 4 patients.

[A case report of psittacosis and chlamydial isolation from a dead pet bird].

A 33-year-old previously healthy woman was seen at this hospital after a week of fever and nonproductive cough. A roentgenogram of the chest showed consolidation in the left upper and middle lung fields. Transbronchial lung biopsy from left S1 + 2 revealed an increase of mononuclear leukocytes within the interstitial spaces and slight cell infiltration with in the alveoli. Bronchoalveolar lavage cellular constituents showed 47% alveolar macrophage, 38% lymphocytes and 15% neutrophils. She had kept a parakeet, which died just after her illness. We could isolated organisms from the liver, spleen, kidney, lung, heart and bowel of her pet bird revealing C. psittaci by Giemsa stain, three days after it was buried in the ground. She was given three hundred mg of ofloxacin per day orally for fourteen days and the clinical effect was good. The in vitro activity of ofloxacin was 0.75 microgram/ml.

[Chronological change of EEG findings in a case of pyridoxine dependency seizures].

A patient of pyridoxine dependent seizures was reported. He was born at 34 weeks' gestation and weighted 2,760 g. Apgar scores were 6 and 9 at 1 and 5 minutes, respectively. He showed the first seizure 2 hours after his birth. Phenobarbital, phenytoin, sodium valproate, diazepam and clonazepam were not effective. Pyridoxal phosphate (50 mg) was given intravenously, resulting in suppression of convulsions. However, muscle tonus was severely depressed. In EEG, a discontinuous pattern was found in quiet and indeterminate sleep on the 2nd day of life. At 5th week multifocal spikes were found, and the discontinuous pattern persisted. Ictal discharges at 13th week showed generalized, continuous, irregular and high voltage slow waves with multifocal spikes. At 27th week of life, high voltage slow waves disappeared and multifocal spike discharges decreased. At 2 years and 10 months of age, the patient was suffering from athetotic cerebral palsy and severe mental retardation. Pyridoxal phosphate at the doses of 35-40 mg/kg/day had been administered. Irritability sometimes occurred and additional 50 mg of pyridoxal phosphate controlled this irritability effectively.

[Surgical treatment of hypoplastic left heart syndrome: a successful case report].

A successful case of Norwood operation for a 5-day-old infant with hypoplastic left heart syndrome is reported. Norwood procedure and central shunting with a 4 mm PTFE tube was performed. Cerebral and coronary artery were perfused independently during reconstruction of aorta in order to shorten the time of circulatory arrest. Post operative UCG documented un-obstructive systemic output from the right ventricle, well regulated pulmonary arterial blood flow and widely patent interatrial communication. This baby was discharged the hospital 66 days after surgery. He has been clinically well for six month after the operation.

[The penetration of ofloxacin and enoxacin into the alveolar spaces in rats].

Ofloxacin (OFLX) or Enoxacin (ENX) was intramuscularly administered at a dose of 100 mg/kg to rats (normal and pneumonia model). Bronchoalveolar lavage (BAL) was performed at 30 min after administration and concentrations of OFLX or ENX in the recovery fluid were determined. Total recovery amounts of OFLX from 10 BAL procedures were 64.7 +/- 18.7 micrograms in the normal group and 126.5 +/- 16.1 micrograms in the pneumonia group, and the amounts of ENX were 15.6 +/- 4.6 micrograms in the normal group and 32.8 +/- 6.1 micrograms in the pneumonia group. Regarding the ratio of total recovery amounts in BAL to serum concentrations of OFLX or ENX, the pneumonia group was higher than the normal group and the ratio of OFLX was higher than that of ENX. Regarding the ratio of total recovery amount in BAL to the amount in lung parenchyma, the ratio of OFLX was higher than that of ENX, but there was no significant difference between the normal group and the pneumonia group in the ratio of OFLX or ENX. With regard to the ratio of the amount of OFLX or ENX in lung parenchyma to their serum concentrations, the ratio was higher in the pneumonia group than in the normal group, but there was no significant difference between OFLX and ENX. Based on the above results, it was concluded that the permeability of OFLX into epithelium of blood capillary is the same as that of ENX, but the permeability of OFLX into alveolus epithelium is superior to that of ENX.(ABSTRACT TRUNCATED AT 250 WORDS)

[Visual evoked potential in guinea pigs].

The present study was designed to establish visual evoked potential (VEP) as one of clinical tests for veterinary medicine. Experiments were carried out on eight adult male guinea pigs weighed 350 to 750 g. We investigated influences of click sound, luminous intensity and habituation on VEP patterns. The VEP of the guinea pig was composed of primary (P 10, N 20, P 30, N 40) and secondary (P 55, N 75, P 100, N 140) components, followed by a rhythmic after-discharge. Click sound with flash produced some unclear peaks in VEP, while click sound without flash elicited clear six peaks. These different components of the response to stimulation suggested that the acoustically evoked potential induced some peaks in VEP. With the intensity used in the present study, changes in luminous intensity resulted in unrecognizable difference among the VEPs. Early components of VEP were not clearly influenced by the habituation to stimulation. As the stimulation was repeated, rhythmic after-discharge seemed to be suppressed in the half of experiments.
