Multiple angiitis of mesenterial arteries in guinea pigs induced by intraperitoneal administration high doses of Na2EDTA. I. Light microscopic study.

In this experiment multiple angiitis in various phases was demonstrated after intraperitoneal administration of high doses of Na2EDTA. These vascular manifestations induced by hyperexudation may probably be provoked by the loosening of the vascular wall which was caused by the releasing of calcium ions from its structure. In addition to its functional importance, such as muscular contraction, nerve transmission and the cell junction, the calcium ion is more important for the alteration of the molecular structure of acid mucopolysaccharides. Intraperitoneal administration of high doses of Na2 EDTA resulted in dissociation of hte intermuscular spaces with endothelial cell reaction. The metamorphosis of the vascular muscle cells into spindle shaped one could also be demonstrated. The hyperexudation caused by the above procedures induced angiitis-like lesions with round cell infiltration.

Multiple angiitis of mesenterial arteries in guinea pigs induced by intraperitoneal administration of high doses of Na2 EDTA. II. Electron microscopic study.

By application of high doses of EDTA we could induce angiitis-like lesions in the mesenterial arteries. In this paper we report the findings of electronmicroscopic investigations of these lesions. They consist of dissociation of the smooth muscle cells by scanty intermuscular spaces, probably by loosening of the basal laminae and matrix. Both endothelial cells and smooth muscle cells show minimal changes. The elastic lamellae were partially interrupted.

The increase of glycosaminoglycan synthesis and accumulation on the cell surface of cultured skin fibroblasts in Werner's syndrome.

Glycosaminoglycans (GAGS) synthesis and accumulation on the cell surface of skin fibroblasts in two patients with Werner's syndrome were measured by the incorporation of 3H-glucosamine and 35S-sulfate. The strains of fibroblasts from Werner's syndrome (WF) had a much higher production of GAGS and accumulation on the cell surface than did the controls. Uronic acid was measured by means of the carbazole sulfuric acid method and orcinol method and it was revealed that WF fibroblasts had a large amount of total uronic acid in the cell layer, the largest portion being the iduronic acid fraction. Electrophoretic analysis of the GAGS of WF showed an increase of production. Increased release into the medium and accumulation of pericellular glycosaminoglycans was observed in hyaluronic acid (HA), heparan sulfate + dermatan sulfate (HS + DS), and chondroitin sulfate (CHS) fractions. No specific accumulation of GAGS in the intracellular pools was observed.

Pulmonary fibrosis of low calcium diet fed guinea pigs induced by the administration of a soluble immune complex.

Feeding animals a low calcium diet leads to lymphangiectasis of peribronchial lymphatic vessels, sometimes followed by overflow of lymphatic fluid into the surrounding tissue. Adding a soluble immune complex (SIC) to the diet, the peculiar diffuse interstitial pneumonia and finally pulmonary fibrosis were noted. Small honey-combed structures and sometimes histologic accommodation of epithelial lining cells were observed. Sometimes accumulation of leucocytes within honey-combed structures was seen.

The influence of calcium ions on the synthesis of collagen and glycosaminoglycans in human diploid cells in culture.

Synthesis of collagen and glycosaminoglycans of skin fibroblasts derived from an adult normal male in culture under different Ca2+ concentrations were investigated. Collagen synthesis was increased in hypocalcemic culture and decreased in hypercalcemic culture. Ca ionophore A23187 added to the medium caused reduction of collagen synthesis. From these results, it was suggested that intracellular Ca2+ controlled the collagen synthesis of fibroblasts. Uronic acid measurements of cell surface glycosaminoglycans (csGAGS) showed the reduction of uronic acid contents along with an increase of Ca2+ concentration in the medium. 3H-glucosamine and 35S-SO42-incorporation into csGAGS showed that glycosaminoglycans synthesis was reduced along with the increase of Ca2+ concentration in the medium. The results from the uronic acid measurement and the radio-isotope incorporation suggested that sulfated glycosaminoglycans, which contained isuronic acid in high proportions such as dermatan sulfate and heparan sulfate were increased in csGAGS in hypocalcemic culture.

Clinical significance of a serum DNA binding protein (64-DP) in evaluating malignant diseases.

A DNA-binding protein with a molecular weight of 64,000 (64-DP) has been found in elevated concentrations in the serum of patients with malignant diseases, and a quantitative assay system (column-SDS disc electrophoresis method) for it has been developed. The average serum 64-DP level in 49 normal controls was 48 +/- 26 microgram/ml, whereas in 96 patients with untreated malignant disease it was 167 +/- 67 microgram/ml. Sera from patients with non-neoplastic diseases showed normal or only slightly elevated 64-DP concentrations. No organ or tissue specificity seems to be required for malignant growths to result in elevated serum 64-DP concentration. Of particular interest was the finding that all 11 patients with early gastric carcinoma and two patients with early esophageal carcinoma, all having histologically proven pT1 lesions, showed elevated levels of serum 64-DP. Following successful surgical resection and/or chemotherapy, 64-DP tends to fall toward the normal value. Although confirmation must await the results of larger clinical trials, our preliminary results strongly suggest that 64-DP evaluated by the column-SDS disc electrophoresis method may prove to be a useful tumor marker in patients with a wide variety of malignancies.

An epidermoid cyst of the tendon.

An epidermoid cyst in the flexor superficialis tendon of the right middle finger of a child limited extension of the finger by preventing the distal gliding of the tendon at the inlet of the flexor tendon sheath. Full extension was regained by excising the tendon.

Rate related block in the canine His-Purkinje system after ligation of the anterior septal artery--observation by His bundle electrocardiography.

In order to investigate the pathophysiology of tachycardia and bradycardia dependent block in the His-Purkinje system, anterior septal artery was ligated and the effects of vagal stimulation and atrial pacing were serially checked in 26 dogs. Tachycardia dependent intra-His bundle block or H-V blocks developed in all cases following ligation of the anterior septal artery and bradycardia dependent block ensued in 3 cases. The latter appeared only in the form of split His bundle or lengthening of the H-V intervals. Spontaneous second or third degree AV block appeared thereafter. Bundle branch block appeared in 19 cases after ligation of the artery. Rate dependent bundle branch block was observed before the development of stable bundle branch block. Frequently, a very long pause was necessary to produce bradycardia dependent block. On the other hand, bradycardia dependent H-V blocks by vagal stimulation were not necessarily dependent upon cardiac cycle length alone. Tachycardia dependent paroxysmal A-V blocks were induced in 7 cases by atrial pacing, soon before or after the onset of spontaneous second degree A-V block. In addition, this block was induced in the very early phase after ligation of the artery in one case. This repetitive intra-His bundle block or H-V block was induced not only by progressive increase in pacing rate but also by gradual decrease in pacing rate after a Wenckebach type of second degree A-H block.

[Cryptococcal granulomatous arachnitis of the spinal cord--a case report (author's transl)].

A case of cryptococcal granulomatous arachnitis of the spinal cord was reported. A 12-year-old boy suffered from sudden occipitalgia and left hemiparesis. The symptoms disappeared spontaneously in about a half year. The next year, he consulted an orthopedist because of lumbago and gait disturbance. Myelography through the cisternal route showed complete block at the level of L1. Exploratory laminectomy of D12 and L1 revealed adhesive arachnitis. Symptoms were improved immediately after the operation. At the age of 15, he was admitted to our clinic, because of sudden onset of headache and vomiting. Computed tomography showed marked hydrocephalus. He recovered by ventriculoperitoneal shunt, and was discharged. The next year, sudden back pain occurred. Gait disturbance, sensory disturbance of the legs and trunks below the mamilla, and dysuria appeared gradually. He was readmitted and laminectomy of D2-4 was performed. The arachnoid membrane was white and 2 mm in thickness diffusely. Thickened arachnoid membrane was removed at the level of D2-4. Histological diagnosis was granulomatous arachnitis. Cryptococcus was seen in the removed tissue. Symptoms were improved after operation. One thousand milligrams of amphoterisin B was injected intermittently. He was discharged on food. Spinal symptoms in cryptococcosis are rare. Operative procedures were effective before the administration of amphoterisin B.
