Experimentally induced aneurysms of mesenterial arteries. Studies on the histogenesis of acquired aneurysm.

In previous studies (YAMAGUCHI et al. 1981 a, b) we administered vast amounts of Na2EDTA into the peritoneal cavity of guinea pigs and demonstrated multiple angiolytic changes of mesenterial arteries. In this experiment, with the same procedures, aneurysm formation of these arteries was encountered based on the above angiolysis. The vascular changes described may be induced by the same mechanisms which provoked angiolysis followed by loosening of elasticity, raised blood pressure and lack of surrounding support.

Free nail bed graft for treatment of nail bed injuries of the hand.

Free full-thickness grafts of nail bed of the lesser toes or an amputated fingertip were successfully performed on 11 fingers of 10 patients since 1979. In nine patients in whom the nail beds had been severely crushed or lost, but the nail matrix was intact, the end results of this technique were excellent. In one patient in whom both the nail bed and matrix had been lost, free grafting of the toenail bed and matrix was performed, with a good result. The procedure can be used when restoring the length of the tip in fingertip amputation if used in combination with local skin flaps such as V-Y advancement or local rotation flaps.

The effect of disopyramide phosphate on ventricular fibrillation threshold of normal and ischemic ventricles.

The effect of disopyramide phosphate (DP) infusion (1 mg/Kg and 2 mg/Kg BW) on ventricular fibrillation threshold (VFT) was studied in anesthetized dogs. In the 1 mg/Kg group, the VFT increased 47% above the control level immediately after infusion. In the 2 mg/Kg group, a delayed increase in VFT was observed after the initial period and VFT was not correlated with the plasma concentration of DP. This phenomenon was abolished by pretreatment with methacholine. These results suggested an anticholinergic action of DP on the VFT. In the ischemic dogs produced by acute coronary occlusion, DP prevented a decrease in VFT.

[Clinical evaluation of T-1982 (cefbuperazone) in the pediatric infections].

T-1982 (cefbuperazone) was evaluated in 25 children with a suspicion of bacterial infections, of the 21 confirmed bacterial infections, 18 were shown to be effective (efficacy rate, 85.7%). The diagnosis included pneumonia (4), bronchopneumonia (3), acute bronchitis (4), acute pharyngitis (1), acute laryngitis (1), acute epiglottitis (1), acute enterocolitis (3), cervical lymphadenitis (1), acute pyelonephritis (1) and suspected septicemia (2). The etiologic pathogens recovered were Haemophilus influenzae (4), Staphylococcus aureus (2), Salmonella typhimurium (1), Salmonella subgenus (1), and enteropathogenic Escherichia coli (2). Among these strains, 7 strains were eradicated after treatment. A case of suspected septicemia and 2 cases of acute enterocolitis with Salmonella infection were not effectively treated with T-1982. The serum half-life of T-1982 was 1.2 hours after an intravenous bolus injection. No severe adverse reaction was encountered with the T-1982 therapy. The data suggest that T-1982 is an effective and safe parenteral antibiotic in the treatment of susceptible pediatric bacterial infections.

[High-dose combination chemotherapy with autologous bone marrow transplantation in breast carcinoma].

Twelve patients with breast carcinoma were treated with high-dose combination chemotherapy supported by autologous bone marrow transplantation (ABMT). Seven of them had advanced metastatic disease and received 12 cours of A treatment, response rate (CR +PR) was 71.5% (CR: 1, PR: 4, NC: 2 and PD: 0) and it should be noted that all 4 patients who had received adriamycin 150 mg+5-fluorouracil 1,500 mg divided in two consecutive days or equivalent high-dose of other chemotherapeutic regimen responded (CR: 1 and PR: 3). To the therapy Five other patients received ABMT-supported adjuvant chemotherapy because of high risk of recurrence. One of them died of unrelated cause and other 4 are free of disease at 11, 14, 17 and 17 months following this treatment. A major toxicity was of myelosuppression and nadirs of neutrophils were 100-1,800 on days 9-19 (median: 400 on day 12), neutrophils recovered rather rapidly to levels of greater than or equal to 1,000, greater than or equal to 1,500 and greater than or equal to 2,000 by day 15, 16 and 17, respectively. High-dose combination chemotherapy with ABMT seemed quite effective in advanced breast carcinoma and a similar approach in adjuvant setting has also been suggested.

High-dose chemotherapy supported by autologous bone marrow transplantation in solid tumors.

High-dose chemotherapy supported by autologous bone marrow transplantation (ABMT) was given 34 times on twenty six patients with solid tumor. Excluding 6 patients with adjuvant use and 2 patients with non-evaluable lesion, the response rate (CR + PR) of 44.4% was observed among 18 evaluable patients with advanced solid tumor. The results seem to be quite significant since many of the patients had become refractory to conventional treatments and intensity of chemotherapy was still moderate in some of the patients. ABMT following a course of chemotherapy is a promising approach of overcoming the toxic limitations of intensive chemotherapy since a dose-response relationship exists in many situations of cancer chemotherapy and ABMT appears to prevent fatal myelosuppression.

[Effect of PSK on cell-mediated immune status in the patients of stomach cancer].

Among 56 patients with stomach cancer preoperative cell-mediated immunity in relation to clinical staging was studied: eight cases were subjected to a control study in order to evaluate effects of PSK on cell-mediated immune status, preoperatively and postoperatively at 1, 3, 6, 9 and 12 months. They were given MMC and FT-207, and 4 of them (the study group) received PSK in addition. Preoperative study showed that WBC count, peripheral lymphocyte and T-lymphocyte tended to show a decrease as the disease progressed, whereas there was no major change in PHA response of lymphocytes. Postoperative study on 4 control patients showed that peripheral lymphocyte and T lymphocyte decreased, and that IgG X FcR+ T cells significantly increased during postoperative period. Four patients in the study group who received PSK, however, did not show these characteristics. It thus appears that PSK is effective in activating cell, while immunity during postoperative period, possibly by counterworking suppressor cells including IgG X FcR+T cells.

Secondary hyperparathyroidism in patients on maintenance hemodialysis: study of optimal hemopurification and parathyroidectomy.

We have found that hemofiltration and direct hemoperfusion combined with hemodialysis removed significant amounts of parathyroid hormone. Hemofiltration and direct hemoperfusion combined with hemodialysis seemed to have better effects on secondary hyperparathyroidism. Surgical parathyroidectomy is indicated when medical parathyroidectomy has failed to prevent progressive bone disease. We had ten patients with refractory secondary hyperparathyroidism. In these patients, subtotal parathyroidectomy was attempted. The results obtained were: 1) Subtotal parathyroidectomy was successful in all patients. 2) Preoperative serum parathyroid hormone values were useful for diagnosis of secondary hyperparathyroidism. 3) The average serum N and C-terminal parathyroid hormone values measured preoperatively were 4,491.00 +/- 892.13 pg/ml and 4,580.00 +/- 474.48 pg/ml, respectively. 4) The average N and C-terminal parathyroid hormone values measured postoperatively were 298.60 +/- 128.68 pg/ml and 2,631.00 +/- 719.51 pg/ml, respectively.

[Immunosuppressive acidic glycoprotein (IAP) and immunosuppressive substance].

Serum IAP and IS levels were studied in patients with stomach cancer. We found that the serum levels of IAP and IS increased as the disease progressed; their true positive rates as tumor markers were 49.3% and 39.8%, respectively. There was some delicate difference between these glycoproteins and T-cell subpopulation. IAP and IS correlated well with a correlation coefficient of 0.847, their value in clinical oncology are thought to be equivalent. Some difference was found between their substances and ASP, this might indicate the heterogeneous nature of the circulating glycoproteins in varying cancer-bearing status.

[Epidemiologic study of breast fluid in Japanese women].

Breast fluid was collected from 805 Japanese women using a negative-pressure breast-pump procedure. All subjects were non-lactating; there were 275 breast cancer patients and 530 healthy women. Women who secreted 20 microliters or more were classified as "secretors" (290/805, 36%); those who secreted less than 20 microliters to as "non-secretors". Breast cancer patients showed a significantly higher incidence of secretion, particularly in the diseased breast, than healthy women. Among secretors, the risk for breast cancer was high in those with earlier menarche, unmarried women, those with a smaller number of pregnancies and children and those with older age at their first delivery.

High performance liquid chromatography (HPLC) determination of endogenous serotonin released from aggregating platelets.

High performance liquid chromatography with electrochemical detection was developed for the determination of serotonin (5-hydroxytryptamine) in rabbit platelet and plasma. A peak from tissue extract (peak a) was identified as serotonin based on retention time on the chromatogram, using a potential that gave limiting current on both a current-potential curve and gas chromatography-mass spectrum, as compared with that of authentic serotonin. For the assay of serotonin, 5-hydroxy-N-methyltryptamine was used as an internal standard and the extraction recovery of the serotonin added to the platelet-rich plasma was almost total. Furthermore, using this highly sensitive method, it has been demonstrated that the release of endogenous serotonin from rabbit platelets occurs during collagen-induced platelet aggregation but not during adenosine diphosphate-induced platelet aggregation. The procedures described should prove to be simple and useful for pharmacological and physiological investigations on the relationship between platelet aggregation and release reaction.

Electrophysiological effects of intravenous disopyramide phosphate on the Wolff-Parkinson-White syndrome.

The effects of a single intravenous infusion of 2mg/kg body weight disopyramide phosphate (DP) on the mode of initiation of reentrant supraventricular tachycardia were assessed in seven patients with Wolff-Parkinson-White (WPW) syndrome using bundle of His electrograms and the ventricular extrastimulus method. The delta wave disappeared in three patients after DP. However, retrograde conduction via the accessory pathway persisted even after DP administration in all patients. These effects contributed to the induction of reciprocating tachycardia after DP. The retrograde functional refractory period of the His-Purkinje system (HPS) and the effective refractory period of the accessory pathway were increased in all cases and contributed to the development of reentry HPS. After DP, the zone of reentry HPS widened in four cases (including a newly developed case) and remained unchanged in three cases. Reentrant supraventricular tachycardia zones widened in three cases; these widened reentrant supraventricular tachycardia zones were induced by the widened reentry HPS, that is, reentry HPS was followed by the reentrant supraventricular tachycardia. This study demonstrates that persistence of retrograde accessory pathway conduction and widened reentry HPS which might be dose-related after DP could be the retrograde determinants affecting the reentrant supraventricular tachycardia zone.

The role of a serum DNA-binding protein (64DP) in the diagnosis of malignant neoplasms.

A DNA-binding protein with a molecular weight of 64,000 (64DP) has been found in elevated concentrations in the serum of patients with malignant diseases. 64DP has been found to be different from any previously described tumor markers, such as AFP, CEA, and C3DP. The results obtained with column SDS - polyacrylamide gel electrophoresis are as follows. 64DP is normally present in the serum at the concentration of 48 +/- 26 micrograms/ml (n = 49). The mean value of 64DP in 96 patients with untreated malignant disease was 167 +/- 67 micrograms/ml, whereas in 41 patients with nonneoplastic disease it was 61 +/- 23 micrograms/ml. No organ or tissue specificity seems to be required for malignant neoplasms to result in elevated serum 64DP concentrations. The clinical stage of the disease does no have much effect on the levels of 64DP and, even with histologically proven early lesions, the patients showed elevated levels of serum 64DP. Following successful surgical resection, 64DP tends to fall toward the normal value. Purification of 64DP was performed and its physicochemical properties have been defined. 64DP is a glycoprotein present in the alpha1-globulin fraction and it is rich in leucine, glutamic acid, and aspartic acid in its amino acid composition. More recently, using purified 64DP preparation, a single radial immunodiffusion assay system has been developed. According to the results of this method, the diagnostic accuracy of 64DP appears to be somewhat low and even marginal for certain types of commonly encountered surgical malignancies. Studies concerning the discrepancy between the results obtained by the two assay methods are now in progress.

Reconstruction of sternal defects with autologous bone grafts and myocutaneous flap of the latissimus dorsi muscle.

In one case of sternal metastasis associated with necrotic osteomyelitis from breast carcinoma we performed subtotal sternotomy including adjacent structures. The resulting defect over the pericardium was reconstructed by the fibula, ribs and a myocutaneous flap of the latissimus dorsi muscle for protection and stability of the anterior chest wall after the operation. The usefulness of the myocutaneous flap of latissimus dorsi muscle for skin coverage over a wide defect in the anterior chest wall is discussed.
