Replantation of untidily amputated finger, hand, and arm: experience of 99 replantations in 66 cases.

Three problems the authors think important in replantation of untidy amputations are discussed based on our 99 replantations with the success rate of 92.6% over a 4-year period. To restore circulation in this type of amputation, such techniques as transfer of blood vessels, use of a neurovascular island flap with neurovascular anastomoses at its distal margin, vein graft, and free split-skin graft directly on the anastomosed blood vessels are recommended. Recovery of tendon gliding when replanted proximally to the MP joint was reasonably good but not when replanted distally to it. Recovery of intrinsic muscles was generally poor. Protective sensation was usually regained, although occasionally accompanied by paresthesia. Amputation of single digit was found not to be an absolute indication for replantation except for the thumb. In multiple digital amputation, more important digits should be restored by amputated digits in better condition. Replantation for cosmetic improvement may be justified in such cases as unmarried young females. In infants, replantation is especially worthwhile because good functional recovery and good further growth can be expected.

Varied pulmonary lesions with intraalveolar large lamellar bodies in an autopsy case with busulfan therapy.

Characteristic pulmonary findings in an autopsy case of a 46-year-old female who presented heavy dyspnea as her chief complaint after 3 months of busulfan therapy for chronic myeloid leukemia were reported. The pulmonary findings were classified into four types: I. alveolar proteinosis type, II. intra alveolar fibrosis type, III. interstitial fibrosis type, and IV. lipid pneumonia type with cholesterol granuloma. No other case with various findings like this case has been previously reported. It was considered that type I is the basic type, type II is a type that developed from type I, type III is a type with interstitial cell infiltration and fibrosis and type IV is a lipidrich variant of type I. A large lamellar body was first found in the granular material of type I. It is supposed that such a body consists of osmiophilic body which originated from type B alveolar epithelial cells and blood plasma.

Intra-His bundle block following ligation of anterior septal artery in dog.

Intra-His bundle block was produced by ligation of the anterior septal artery in dogs. The duration of the His bundle potential was gradually increased following the ligation, and an obvious splitting of the His bundle deflection was observed thereafter. Finally, spontaneous second and/or third degree intra-His bundle block appeared. Before the development of spontaneous block, a pacing induced intra-His bundle block was observed when the duration of the His bundle deflection was prolonged approximately to twice the control value. Careful measurements of the intervals between two split His bundle deflections (H1-H2) in spontaneous as well as in pacing induced block revealed atypical Wenckebach periodicity in all analyzed periods except in one. Repeating and/or transient decreases of H1-H2 intervals were frequently observed as atypical findings.

[Ototoxic effect of BB-K8 administered to pregnant guinea pigs on development of inner ear of intrauterine litters (author's transl)].

UNLABELLED: Transplacental ototoxic effects of BB-K8 on development of inner ear in intrauterine guinea pigs were evaluated by differential frequency pinna reflex test in range of 20 kHZ to 500 HZ and histopathological examination of spiral and vestibular organs in new born guinea pigs from pregnant ones given BB-K8 i.m. during gestation. MATERIALS AND METHODS: Twenty-five pregnant guinea pigs of the Hartley strain and 63 new-born guinea pigs from the pregnant ones were used in the study. Following antibiotics were given i.m. daily to the 20 pregnant guinea pigs from 7th day of gestation for 56 days. BB-K8 was given at the dose level of 100 mg/kg and 200 mg/kg, respectively. Kanamycin (KM) was administered at the dose level of 100 mg/kg and 200 mg/kg, respectively. Physiological saline solution (1.0 ml/kg) was given i.m. to the pregnant guinea pigs for 56 days. The differential frequency pinna reflex test was performed in the frequency range from 20 kHZ to 500 HZ to determine an extent of frequency of pinna reflex loss. The test was carried out before the start of administration of the antibiotics and once a week during the administration. After the last injection of the antibiotics, intravital perfusion fixation was carried out in the new born animals under anesthesia with nembutal. Histopathological examination was made in celloidin serial sections of the cochlea and vestibulum of the new born animals to observe an extent or degree of hair cell damage. RESULTS: (1) The differential frequency pinna reflex test revealed that new born guinea pigs from the pregnant ones given BB-K8 and KM respectively at the dose level of 100 mg/kg did not show occurrence of pinna reflex loss at any frequency tested. At the dose level of 200 mg/kg, BB-K8 did not result in transplacental hearing impairment in new born guinea pigs, but KM caused transplacental pinna reflex loss in the highest frequencies from 20 k to 15 kHZ in 3 of 10 animals. (2) Histopathological examination detected that unilateral incomplete loss of outer hair cells of the spiral organ confined to the basal end of cochlea was found in 3 of 13 new born guinea pigs from the pregnant ones treated with BB-K8 (100 mg/kg). In the animals, slight damage of hair cells in the vestibular organs occurred in 100%. These hair cell damages were enhanced slightly in the new born guinea pigs from the pregnant ones treated with BB-K8 (200 mg/kg) and much more in the new born ones from the pregnant ones treated with KM (100 mg/kg, 200 mg/kg). (3) These results suggest that BB-K8 has a mild potency to cause transplacental ototoxic damage in the inner ear of intrauterine guinea pigs within the pregnant ones treated for 56 days (normal gestation period approximately 70 days).

HLA antigens and Ia antibody in the gastric cancer patient.

The possible association of 18 HLA specificities in HLA-A locus and 23 specificities in HLA-B locus with susceptibility to gastric cancer was investigated in a total of 87 patients and 355 controls in Japan. Significantly decreased antigen, HLA-B5 (17.2%) was found in gastric cancer patients compared with 40.0% in controls. X2 for this antigen difference is 15.8 (corrected P less than 0.005). The most frequent HLA haplotype was A9-B5 in controls and A2-Blank B locus antigen in gastric cancer patients. No reaction was found of HLA antiserum, Bou-Pri with cells of gastric cancer patients, compared with 29.4% in control subjects. No linkage was observed between HLA-A and -B locus antigens and gastric cancer in family study with double the frequency of HLA haplotype A2-BW40. Ia-like B cell specific antibody was found 10% in 2 out of 20 gastric cancer patients in a preliminary study. There was no clear association between B cell specific antibody and clinical implications.

A half-century of experience in the management of colon injuries: changing concepts.

A total of 773 patients who sustained colon injuries during the period 1927-1974 at Charity Hospital of Louisiana in New Orleans was reviewed. For purposes of comparison the patients were grouped into three 15-year periods. The first period (1927-1942) represented patients treated in the pre-antibiotic era exclusively with exteriorization. The second period (1943-1958) included patients in whom a program of selective surgical treatment of colon injuries was instituted, and included the advent of antibiotics and improved resuscitation. The final period (1959-1974) represented the increased use of primary repair of colon injuries. In comparing the three consecutive periods there was a marked increase (106%) in the frequency of colon injuries, primarily due to gunshot wounds in young males. The average age of our patients during this last 15-year period was 27 years. The transverse colon was the most frequent site of the injury (37%) followed by the left colon (31%) and the right colon (20%). From 60 to 75% of the patients also had associated intra-abdominal or other injuries. The mortality from colon injuries has declined steadily from 67.5% in the initial period to a low of 9.2% during the past 15 years. This drop in mortality occurred despite the fact that a larger number of patients sustained associated injuries. When comparing the three methods of therapy, primary repair, repair with proximal colostomy, and exteriorization was used less frequently during the past 15 years. It was the exclusive form of therapy in all patients treated before 1942. With the advent of selective management and antibiotics we have seen a transition from exteriorization to primary repair without colostomy. During the past 15 years primary repair was used in 44% of the patients; repair with a proximal decompressive procedure was utilized in 22%; exteriorization was carried out in 33% of the patients. The selective use of primary repair has been associated with lower mortality, fewer complications, and a shorter hospital stay, despite the increasing incidence of multiple associated injuries in this latter group of patients. Based on these studies, a plea is made for the selective use of primary repair of colon injuries of patients in good general condition when the local injury to the colon appears to warrant such an approach=

Statistical analysis of diverticulosis of the colon.

Forty-four cases of diverticulosis of the colon were detected among 2,662 cases examined by barium enema in Tohoku University. This corresponds to 1.7% in frequency. On the other hand, 21 cases were found among 1,511 cases examined in Hirosaki University, giving 1.4% in frequency. It was more frequent in the male. The frequency, as calculated on the basis of the numbers of cases examined by barium enema in separate age groups, showed the gradual increase with age except for above the eighth decade. The location where the diverticulum was most frequently seen was the right colon; namely, cases in which the diverticulum occurred from the cecum to the ascending colon and those in which it was found only in the ascending colon occupied 61.4 and 71.4% of the total cases, respectively. Diverticulitis as a complication was rare, while coincidence of polyp or carcinoma was sometimes observed. Furthermore, the value of fiberscopy for the diagnosis of diverticulosis of the colon was discussed, and its usefulness for the detection of complication was emphasized.

[Evaluation of ototoxicity of amikacin (BB-K8) by animal test (author's transl)].

Seventy Hartley strain guinea pigs (350 g body weight at start of the experiment) were used. BB-K8, gentamicin (GM) and kanamycin (KM) were given to the animals intramuscularly for 28 days at following various doses: (see article) These guinea pigs underwent a differential frequency pinna reflex test in wide frequency range from 20KHz to 0.5 KHz before the administration, during the injection and after the last one. For histopathological examination of the inner ears the guinea pigs were subjected to intravital fixation with Wittmaack's fixative under nembutal general anesthesia. Blocks including the bilateral temporal bones were removed from the skulls and then fixed in the same fixative for 1 similar to 2 weeks. After routine procedure for decalcification, dehydration and celloidin embedding, horizontal serial sections of the inner ears were made and stained with hematoxylin-eosin. The differential frequency pinna relfex test in the 70 guinea pigs indicated positive pinna reflex in 90% at 20KHz and in 100% at 15, 12, 10, 8, 6, 4, 3, 2, 1 and 0.5 KHz before start of the administration. Administration of the antibiotics for 28 days occasionally resulted in the pinna reflex loss which always involved the highest frequency, 20 KHz and then was followed in relatively regular succession by 15, 12, 10, 8, 6, 4, 3, 2, 1 and 0.5 KHz (Tables 1 similar to 8). On the other hand, histopathological examination of the inner ears disclosed that the loss of the outer hair cells in the spiral organ which are the most sensitive to the ototoxic antibiotics, occurred always at the basal end of the spiral organ and then spread from there to upper portion of the spiral organ (Tables 9 similar to 16). Based on the differential frequency pinna reflex test in the wide frequency range and extensive histopathological examination of the inner ears, ototoxicity of BB-K8 is considered to be more mild than GM. However, there was not so remarkable difference in ototoxicity between BB-K8 and KM in the present experiment. The result or examination of the guinea pigs received BB-K8 at 40 mg/kg and 100 mg/kg respectively for 28 days suggests that BB-K8 at expecting clinical dose, 500 mg per day (for 28 days) may be safe from ototoxicosis in the inner ears. In the vestibular organs of the animals received BB-K8 there was no decrease in number of the hair cells.
