Electrophysiologic effects of intravenous adenosine triphosphate disodium on the paroxysmal supraventricular tachycardias.

We studied the electrophysiologic effects of intravenous adenosine triphosphate disodium (ATP-2Na) on 15 patients with paroxysmal supraventricular tachycardias (PSVTs). One patient had sinus node (SN) reentry and 2 patients had intraatrial (IA) reentry. Five patients had AV nodal reentry and 7 patients had atrioventricular reentrant tachycardias (AVRTs) with accessory pathways (APs). ATP-2Na was injected during ventricular pacing (VP) in patients with AVRTs with APs. A bolus injection of ATP-2Na terminated all the PSVTs within 40 sec except in one case of IA reentry. The sites of block at the termination were the atrium in SN reentry and IA reentry, between A and H (AH) block or between H and A (HA) block in AV nodal reentry and AH block in all the AVRTs with APs. ATP-2Na during VP in patients with AVRTs with APs produced the changes of atrial activation sequences in 3 patients, induction of PSVT in 2 patients and a Mobitz type II VA block in 2 patients. The former two phenomena suggested a retrograde AV nodal block and raised the possibility of a simple test for retrograde atrial fusion during VP in patients with WPW syndrome. Chest discomfort of short duration was most commonly noted after ATP-2Na. Inosine pretreatment potentiated the effects of ATP-2Na. This combination may further alleviate the side effects of ATP-2Na, while preserving the effective action of ATP-2Na for rapid termination of PSVTs.

Histamine induced or enhanced delayed afterdepolarization and triggered activity in guinea-pig papillary muscles. A preliminary study.

We used standard microelectrode techniques to study the histamine induced or enhanced delayed afterdepolarization (DAD) and triggered activity (TA) of guinea-pig papillary muscle superfused with low-potassium Tyrode's solution. Before histamine, a series of driven action potentials did not induce DAD and TA. Immediately after histamine (10(-5)M), DAD was induced and, finally, TA was induced after high rate pacing (150/min to 300/min). The effect of histamine was antagonized by cimetidine (5 X 10(-6) to 5 X 10(-5)M) but not by diphenhydramine. Also, the amplitude of DAD decreased after verapamil (10(-7) to 3 X 10(-6)M) and lidocaine (4 X 10(-5) to 8 X 10(-5)M). To investigate indirect evidence of increased cyclic AMP mediation in this histamine induced DAD, we studied the effects of a phosphodiesterase inhibitor (papaverine 10(-5)M) or activator (N-methylimidazole 20 mM) on the histamine induced or enhanced DAD. The former enhanced and the latter depressed the histamine-induced (or enhanced) DAD. Thus, histamine may induce or enhance the DAD and TA by increasing the slow inward current. This mechanism may be mediated by histamine H2-receptors and the adenylate cyclase system in the cardiac ventricular muscle.

Classification of thumb hypoplasia.

The author describes eight types of hypoplastic thumb. This classification is important in planning reconstructive surgery, as the selection of an appropriate procedure depends largely upon the classification of the deformity.

Remote diagnosis via a telecommunication satellite--ultrasonic tomographic image transmission experiments.

An experiment to transmit ultrasonic tomographic section images required for remote medical diagnosis and care was conducted using the mobile telecommunication satellite OSCAR-10. The images received showed the intestinal condition of a patient incapable of verbal communication, however the image screen had a fairly coarse particle structure. On the basis of these experiments, were considered as the transmission of ultrasonic tomographic images extremely effective in remote diagnosis.

[Clinical evaluation of cefminox, a new cephamycin antibiotic, in the pediatric infections].

Cefminox (CMNX, MT-141) was evaluated for its safety and efficacy in children. Fifteen cases of bacterial infections were treated with intravenous bolus injections of 30 to 100 mg/kg/day of CMNX. Each 5 cases of acute respiratory tract, urinary tract, and gastrointestinal infections were included. All the cases were cured after the CMNX therapy. No adverse reactions were encountered with the therapy. The serum half-life was approximately 1.5 to 2 hours after intravenous bolus injection in children. The data suggest that CMNX is a safe and effective antibiotic when used in children with susceptible bacterial infections.

[Clinical evaluation of a new parenteral penicillin, aspoxicillin, in children].

Aspoxicillin (ASPC) was evaluated for its efficacy and safety in 30 infants and children with acute bacterial infections. The disease categories included acute respiratory tract (22), soft tissue (3), urinary tract (3) infections, sepsis with pyothorax (1) and purulent meningitis (1). ASPC was effective in all but 1 case of pneumonia due to beta-lactamase-positive H. influenzae (effective rate; 96.7%). Adverse reactions and abnormalities of the laboratory tests were not associated with the ASPC therapy in any of the cases. The serum half-life of ASPC after an intravenous bolus injection was 0.883 +/- 0.194 hour and excretion into urine was rapid. From the present results, ASPC is a safe and effective antibiotic when used in patients with susceptible bacterial infections.

Co-existing tachycardia and bradycardia dependent incomplete and complete right bundle branch block: evidence for diffuse conduction system involvement.

A case of co-existing tachycardia and bradycardia dependent incomplete and complete right bundle branch block (BBB) is reported. The degree of BBB was related to the duration of the preceding cycle length. In addition, more diffuse conduction system disease was suggested by evidence of sino-atrial and His-Purkinje conduction abnormalities.

An experimental model of calcification in the vessel wall.

Multiple angiolytic changes of the mesenteric arteries due to administration of vast amounts of Na2EDTA into the peritoneal cavity of guinea pigs have already been demonstrated by Yamaguchi et al. (1981 a, b). In this experiment, one week after Na2EDTA administration calcium lactate was administered in the same manner. As a result calcium deposition was observed in the intimal elastic fibers, in and at the collagenous fibers and intermuscular spaces of lytic and ensuing dilatated areas of the vascular wall. This suggests that dystrophic calcification might be induced by conjugation of calcium ions to the free negative charges of acid mucopolysaccharides (aMPS) which are the main component of the above structural elements.

Biochemical studies on the collagen of the palmar aponeurosis affected with Dupuytren's disease.

The palmar aponeurosis of patients affected with Dupuytren's disease was examined for collagen characteristics with regard to extractability, polymorphism, and posttranslational modifications, and the results were compared with those from normal subjects. The increased proportion of type III collagen relative to type I collagen in the affected tissue confirmed the previous findings in this disease. A slight but significant increase in a ratio of glucosylgalactosylhydroxylysine to galactosylhydroxylysine in the Dupuytren's tissue may be interpreted by the increase in the content of type III collagen. The affected tissue contained increased amounts of dihydroxylysinonorleucine as the reducible cross-link of collagen. These data support the view that Dupuytren's tissue contains collagen resembling that in granulation and embryonic tissues. Pyridinoline was shown to occur in normal and affected aponeurosis. No change in its content suggests that this cross-link is not involved in the pathogenesis of contracture in this disease.

[Clinical evaluation of sulbactam/cefoperazone in the pediatric infections].

Sulbactam/cefoperazone (SBT/CPZ), a fifty-fifty combination of a beta-lactamase inhibitor, SBT, and an already marketed broad spectrum cephalosporin, CPZ, was evaluated for its efficacy and safety in 25 children. The diagnoses included purulent lymphadenitis, pneumonia, acute UTI, bacteremia and purulent meningitis. SBT/CPZ was effective in all the 20 cases with bacterial infections, but strains highly resistant to CPZ were not isolated in this study. The serum and cerebrospinal-fluid levels of SBT were grossly parallel with those of CPZ, and the half-life of the serum SBT was 0.754 hour. Although severe adverse reactions were not encountered with SBT/CPZ therapy, loose stools in 20% and diarrhea in 16% of the cases were observed.

[Bacteriological, pharmacokinetic and clinical studies on sulbactam/cefoperazone in the pediatric field].

Bacteriological and clinical effect of a newly developed SBT/CPZ in the treatment for pediatric patients was assessed by a study group consisting of 15 institutions. The results were as follows. Antibacterial effect Susceptibility studies were performed with 93 clinical isolates. The MIC of SBT/CPZ was one-tube inferior or almost similar to that of CPZ in susceptible organisms. In CPZ-resistant organisms at the inoculum of 10(8) cells/ml, however, SBT/CPZ was much superior to CPZ on the basis of the MIC. When the MIC of SBT/CPZ was compared to that of CPZ in 27 strains which have high beta-lactamase-producing activity, it was found that many of CPZ-resistant organisms were susceptible to SBT/CPZ. Serum concentration and urinary excretion The serum concentrations of SBT and CPZ were 33.2 micrograms/ml, respectively at 15 minutes after 20 mg/kg SBT/CPZ was administered by intravenous bolus injection, and those of SBT and CPZ, 51.0 micrograms/ml and 108.3 micrograms/ml, respectively following 40 mg/kg SBT/CPZ therapy. The serum concentrations of CPZ were 2.1-2.4 times as high as those of SBT. The concentrations were dose-related. The half-lives of SBT and CPZ following 20 mg/kg SBT/CPZ administration were 0.94 hour and 1.50 hours, respectively, and those following 40 mg/kg SBT/CPZ were 0.95 hour and 1.53 hours, respectively. There was no significant difference between 20 mg/kg and 40 mg/kg administrations. When compared between SBT and CPZ, CPZ had slightly longer half-lives. At the termination of 1 hour drip infusion of 20 mg/kg SBT/CPZ, the serum concentrations of SBT and CPZ were 16.7 micrograms/ml and 40.1 micrograms/ml, respectively. In the case of 40 mg/kg, the levels of SBT and CPZ were 38.6 micrograms/ml and 94.9 micrograms/ml, respectively. The concentrations were found to be dose-related as were following intravenous bolus injections. The SBT half-lives obtained after 20 mg/kg and 40 mg/kg SBT/CPZ administrations were 1.39 hours and 0.89 hour, respectively; those of CPZ, 2.00 hours and 1.44 hours, respectively. The highest urinary concentration occurred 0-2 hours after intravenous bolus injections of 20 mg/kg or 40 mg/kg SBT/CPZ. Urinary excretion of SBT over 6 hours was 60.0% and 67.7%, and that of CPZ, 21.2% and 25.0%, indicating higher urinary excretion for SBT. When 20 mg/kg SBT/CPZ or 40 mg/kg was administered over 1 hour by drip infusion, urinary excretion became the highest at 1-3 hours after administration. Urinary excretion of SBT over 7 hours following 20 mg/kg and 40 mg/kg SBT/CPZ was 68.8% and 80.3%, respectively, and that of CPZ, 24.4% and 27.3%. The results were similar to those observed following intravenous bolus injections.

[Clinical evaluation of tissue polypeptide antigen in patients with esophageal, stomach and colon cancer].

Serum tissue polypeptide antigen (TPA) levels were measured in 33 patients with esophageal cancer, 39 with stomach cancer and 50 with colon cancer. At the same time five glycoproteins, namely immunosuppressive acidic glycoprotein (IAP), alpha 1-antichymotripsin (alpha 1-ACT), acid soluble glycoproteins (ASP), sialic acid and carcinoembryonic antigen (CEA), were measured for comparison. The mean TPA values were 59.0 +/- 15.4 U/l in 61 normal subjects, 103.6 +/- 104.2 U/l (positive rate, 24.2%) in esophageal cancer patients, 111.9 +/- 49.8 U/l (71.8%) in stomach cancer patients and 124.8 +/- 195.5 U/l (40%) in colon cancer patients. The serum TPA levels in patients with stomach cancer rose with an increased number of involved lymph nodes and with a higher degree of infiltrative growth and increased with the advancement of tumor growth postoperatively. Serum TPA levels correlated well with those of alpha 1-ACT, IAP and ASP in stomach cancer patients and with those of CEA, ASP and sialic acid in colon cancer, but not in esophageal cancer patients. It is suggested that the serum TPA might represent one of the reactant proteins and/or tumor-associated antigens that appear to be dependent upon the cancer status.

Influence of calcium concentration in the medium on the cell proliferation and glycosaminoglycan synthesis of human diploid skin fibroblasts in culture.

Changes of the glycosaminoglycan (GAGs) fractions in the medium, the cell surface of human diploid skin fibroblasts and the relation between GAGs synthesis and cell growth in cultured media containing various Ca++ concentrations were investigated by means of electrophoretic fractionation. The results demonstrated that the synthesis of the hyaluronic acid (HA) fraction was increased with increase of the Ca++ concentration in the medium, and this change appeared to be linked to the condition of cell growth. The synthesis of the heparan sulfate and dermatan sulfate (HS + DS) fraction was decreased with increase of the Ca++ concentration in the medium, and this change depended largely on the Ca++ concentration in the medium rather than the cell growth. The synthesis of the chondroitin sulfate (Ch-s) fraction was also decreased with increase of the Ca++ concentration in the medium. However, this change was neither controlled especially by the Ca++ concentration in the medium nor by the condition of cell growth, but reflected the decrease of total GAGs synthesis by the increase of the Ca++ concentration in the medium.
