[Randomized study on the long-term adjuvant chemotherapy with ftorafur and mitomycin C for stomach cancer: second report].

In view of the usefulness of long-term adjuvant chemotherapy, the Cooperative Study Group of Surgical Adjuvant Chemotherapy for Gastric Cancer adopted as the second cooperative study a prospective randomized trial on three groups; MMC alone, Futraful alone and a combination of MMC and Futraful. In the group of Futraful alone the survival and disease-free rate were improved in three years after operation, especially marked in patients with the advanced stage. Moreover, the survival and disease-free rate in the group of the combination therapy were higher than those in the group of Futraful alone, and the relapse rate was lower in the former. Therefore, a long-term combined adjuvant chemotherapy with MMC and Futraful seems to be one of the most effective chemotherapy regimens for resectable advanced gastric cancer.

[Clinical study of recurrent stomach cancer].

There are various patterns of recurrence of gastric cancer after radical resection, such as hepatic metastasis, carcinomatous peritonitis, residual stomach recurrence, local lymph node metastasis and establishment of distant metastasis. In cases of residual stomach recurrence, resection is sometimes feasible. Kruckenberg's tumor resulting from metastasis to the ovary can frequently be removed. With such resectable metastasis, surgical procedure is actively employed, with subsequent chemotherapy. Chemotherapy in such a case consists of combined chemotherapy by arterial infusion for induction of remission and administration of oral preparation and/or suppositories for maintenance. In the treatment of recurrent gastric cancer by arterial infusion, we made it a rule to administer drugs through a catheter inserted subselectively into the aorta. In the treatment by arterial infusion, the daily administration of 5-FU serves as the basic regimen. Dissolve 250 mg 5-FU in about 20 cc physiologic saline or 5% dextrose solution, and infuse the solution over 2 hrs with the use of a continuous arterial infusion pump. Administer of 5-FU daily, and fortify this treatment by one-shot injection of MMC 10mg/body each time, MMC is usually given 3-4 times, with intervals between its administrations adjusted according to WBC and platelet counts. ADM is given at dosage of 40 mg/body each time. We found it advisable to continue the administrations of 5-FU until its total dose reached about 20 g, while giving sufficient doses of ADM or MMC for induction of remission. The results obtained from 108 cases of the recurrent gastric cancer were shown as follows. The median survival period was 5 months. The twenty-one cases out of 108 cases in recurrent gastric cancer survived more than one year, because they received the intensive chemotherapy such as arterial infusion chemotherapy and oral or rectal administration of FT. The most patients with liver metastasis were treated with selective arterial infusion chemotherapy consisting of 5-FU plus MMC or ACNU. And the efficacy of arterial infusion chemotherapy was remarkable. Our efforts must be made to continue any treatment as long as possible and change drugs as necessary. Also we must keep general condition of the patients as good as possible using support therapy such as IVH, prevention of infection, immunotherapy, drainage so on.

Spontaneous gastric erosions in NOD and KK-A gamma.

Histopathological examinations of stomachs of insulin-dependent (NOD) and noninsulin-dependent (KK-A gamma) mice were carried out. Spontaneous erosions in the fundic area were observed in 31% of NOD and 35% of KK-A gamma mice. The morphological features of the erosions were similar to those of erosions produced by stress. Although no erosions were detected in nondiabetic C3H/Slc mice raised and maintained in the same animal room, the presence of hyperglycemia and glycosuria in NOD and KK-A gamma mice did not appear to have a direct correlation with development of erosions. NOD and KK-A gamma mice may be useful animal models for human stress ulcers.

Pharmacokinetics of propylthiouracil in children and adolescents with Graves disease after a single oral dose.

Pharmacokinetics of propylthiouracil after a single oral dose were studied in six pediatric patients with Graves disease, with respect to influence of food intake. Propylthiouracil administration in the fasting state induced a rapid rise of plasma level reaching a peak of 30 to 60 min. Peak values ranged from 7.2 to 18 micrograms/ml with administered dose (100 to 280 mg/m2 BSA) and plasma half-life was 1.3 +/- 0.41 hr (mean +/- SD). Single compartment model with first order absorption showed excellent fit to the data obtained in the fasting state, but not in the fed state. Most individuals showed marked difference in the pattern of propylthiouracil concentration-time curves between the fasting and the fed state. Food intake prior to the drug ingestion was associated with lower and delayed peak and variable AUC values. These results indicate that propylthiouracil administration in the fasting state is more advisable for obtaining a consistent bioavailability.

[Studies on the periodicity of fetal and neonatal heart rate variability by fast Fourier transform analysis].

Heart rate changes were analyzed by fast Fourier transform technique in 68 normal fetuses of 23 to 41 weeks, 8 normal deliveries, 11 normal neonates, cases of fetal heart rate late deceleration, sinusoidal pattern, and anencephalic fetus. High spectral densities were noted at 0.02 to 0.12 Hz in inactive phases of normal fetuses, whereas they were 0.2 to 0.4 Hz in active phases. The densities were particularly high at 0.64 to 0.8 Hz in normal neonates. An anencephalic fetus and a case of late deceleration showed high spectral densities at 0.02 to 0.12 Hz. These results suggest that fetal heart rate is controlled basically by 8 sec or greater periodicity. Neonatal 1.3 to 1.6 sec periodicity suggests a relationship between heart rate and respiration. No change in periodicity was found in fetal heart rate after the administration of 0.5 mg of atropine sulfate to the mother in 12 cases.

Ultrasonographic diagnosis of uriniferous perirenal pseudocyst.

A child with a traumatic uriniferous perirenal pseudocyst was presented. The diagnosis was not made by routine radiological studies, but ultrasound examination demonstrated a perirenal fluid accumulation clearly. The usefulness of the ultrasound examination in the diagnosis of this condition was emphasized. The characteristic finding in ultrasonogram consists of a hydronephrotic kidney and its invagination into the echolucent mass. When perirenal cystic lesions are demonstrated by ultrasound, the junctional zone between the cyst and kidney should be carefully checked for signs of invagination of the kidney into the cyst.

Isolation and properties of cadmium-binding protein induced in rat small intestine by oral administration of cadmium.

The chromatographic properties and amino acid composition of Cd-binding protein induced in the rat small intestine by oral administration of Cd were examined. When the intestinal mucosal cytosol from rats treated orally with Cd was chromatographed on Sephadex G-75, Cd in the intestinal mucosa was bound 95.5% to protein of molecular weight about 10,000. A DEAE Sephadex A-25 chromatogram of the Cd-binding protein from Sephadex G-75 showed two major and one minor Cd-containing peaks as hepatic and renal metallothionein. The amino acid composition of the two major Cd peaks resembled that of metallothionein from other materials examined. These results indicate that the low-molecular-weight Cd-binding proteins in the small intestinal mucosa meet the criteria for classification as metallothionein, and suggest that induced intestinal Cd-binding protein may be capable of preventing the transport of orally administered Cd from the intestinal mucosa into the other organs.

[Effect of levamisole hydrochloride on advanced and recurrent breast cancer--LMS randomized controlled study of a group responding to CAF (cyclophosphamide, adriamycin, and 5-FU) therapy].

The effect of levamisole HCL on advanced and recurrent breast cancer was investigated cooperatively by a randomized controlled study using envelope method. Patients with advanced and recurrent breast cancer who had showed either complete response or partial response to the previous CAF therapy (cyclophosphamide 100 mg/body/day p.o. from day 1 to 14, adriamycin 30 mg/body/day i.v. on day 1 and 8, 5-FU 500 mg/body/day i.v. on day 1 and 8) were entered in this study and divided randomly into a control group and a levamisole group. No further treatment was given to patients of the control group until they fell into progressive disease. Patients of the levamisole group were given the drug at a daily dose of 150 mg for 3 consecutive days every fortnight. Fifty-nine cases were entered in this protocol, but 9 cases of them were ruled out as exclusions or dropouts. Therefore, the number of eligible cases was 31 in the levamisole group and 19 in the control group. Both the duration of remission and the survival time were significantly (p less than 0.05) prolonged in the levamisole group compared with the control group. There was no difference in side effects between these two groups.

[Multihospital randomized study on the adjuvant chemotherapy with mitomycin-C and Futraful for gastric cancer V. Estimation of 5-year survival rate].

The addition of Futraful to MMC therapy of gastric carcinoma was evaluated. Nationwide 297 institutions participated in a randomized controlled study and cumulative five-year survival rate was calculated. A total of 1,673 cases was studied. These were divided in two groups: Group A, without Futraful, and Group B, with Futraful. Futraful was administered postoperatively for 3 months. The MMC treatment was done by moderate intermittent administration (Method I) or by large dose administration (Method II). Futraful improved survival rates regardless of the method of MMC treatment. Especially, Group B' (more than 60 g of Futraful) showed significantly better results in patients with metastases and positive serosal invasion in both methods, and beta type infiltration in Method I. Furthermore the Group B' patients had higher survival than Group A patients in Method II for patients in stage III and gamma type infiltration. Method II, a large dose MMC, plus Futraful was suggestive of particular effectiveness in preventing peritoneal metastases in curatively resected cases.

[Effect of levamisole in postoperative adjuvant immunochemotherapy of stomach cancer--randomized controlled study of MMC-5-FU combination therapy with or without levamisole. 1].

A randomized controlled study by envelope method was carried out with the purpose of evaluating effects and side effects of levamisole in patients with resectable stomach cancer. The patients were randomly allocated to the treatment either with control or levamisole according to the indication of the envelope opened at least 3 days prior to surgery. The control group was treated with Mitomycin C (day 0, 20 mg day 1, 10 mg, one shot i.v.) and 5-FU(150 mg/day, p.o.). The levamisole group was treated with Mitomycin C, 5-FU and levamisole. Levamisole was administered at a daily dose of 150 mg for 3 consecutive days before surgery, and the 3 consecutive days administration schedule was repeated every fortnight for one year after surgery. Four hundred and forty-six patients were entered in this trial. However, with the exclusion of 104 patients as exceptions and dropouts, the total eligible patients were 342, consisting of 167 in the control group and 175 in the levamisole group. The effects were evaluated by comparing the disease-free interval or the survival time of both groups. There was no significant difference in the disease-free interval and survival. In this study, we have not yet reached the conclusion that levamisole is effective in prolonging disease-free interval and survival time, because high survival rates are still maintained in both groups for 2 years after surgery. The final conclusion would be drawn with the follow-up results in the future.

[Effect of postoperative long-term chemotherapy of stomach cancer using mitomycin C and futraful--the secondary study (1)].

The Cooperative Study Group of Surgical Adjuvant Chemotherapy for Gastric Cancer in Japan with the participation of 334 institutes nation-wide had conducted the secondary study in order to investigate the usefulness of futraful in long term treatment, based on the results obtained by the primary study. The following method was taken for the study and patients were randomly divided into three groups: A group-given a high dose of MMC immediately after operation; B group-given futraful for one year after MMC administration; C group-given futraful alone for long-term. The number of eligible cases for data analysis was 3,o30. Side effects observed during the long term treatment with futraful were considered to be minor. Although the occurrence of hematocytopenia was slightly frequent because of possible increase of influence by the concomitant use of MMC, no tendency of hepatic disorders and any subjective side effects to be strengthened by MMC combination was observed at all. As for 2-year survival, an elevated survival was found in the group of concomitant use of MMC and futraful; Especially, a significantly high survival was found in the cases of stage III and n (+) . ps (+). Furthermore, the survival was elevated in proportion to the increase of total dosage of futraful. This suggested the usefulness of futraful for long term treatment.

Hanganutziu-Deicher type-heterophile antigen-positive cells in human cancer tissues demonstrated by membrane immunofluorescence.

The possible expression of Hanganutziu-Deicher (H-D)-type heterophile antigen on the cell surface of several types of human cancer tissues was investigated by the membrane immunofluorescence test with anti-H-D serum. Anti-H-D serum of high titer was obtained from chickens immunized with H-D antigen-active glycosphingolipid, N-glycolylneuraminyl-lactosylceramide (equine hematoside). H-D antigen was demonstrated in 3 to 6 gastric cancer tissues, 2 of 3 lung cancer tissues, and 2 of 4 breast tissues examined, but not in 5 colorectal cancer tissues examined.
