Metastatic processes and a potential indication of treatment for metastatic lesions of renal adenocarcinoma.

Renal adenocarcinoma findings from 307 autopsied cases with metastasis to 1 and 2 organs were investigated concerning the mode of metastasis. The lung was the most frequent site of metastasis to 1 organ, although the frequency was rather low (30 per cent), followed by bones (over-all), lymph nodes (over-all) and brain, and involving frequently the thoracic spine and retroperitoneal lymph nodes. In patients with metastases to 2 organs a significant correlation was found between the pulmonary-tracheal lymph nodes and those to the lungs. Potential indications for treatment of metastatic lesions were 1) lymphadenectomy and/or radiation therapy for the retroperitoneal and para-aortic lymph nodes and 2) resection of metastatic lesions in the lungs combined with lymphadenectomy and/or radiation therapy for the pulmonary-tracheal lymph nodes. In patients without lung metastasis lymphadenectomy and/or radiation therapy for the pulmonary-tracheal lymph nodes is not always necessary.

Distant metastasis of renal adenocarcinoma in nephrectomized cases.

In an analysis of the autopsy records of 1,828 patients with renal adenocarcinoma 424 nephrectomized and 1,404 nonnephrectomized cases were compared to explore the effect of nephrectomy on metastasis. Over-all, no significant difference was observed except for a high incidence of metastases to the brain and femur, and a low incidence of metastases to the ipsilateral adrenal and renal hilar lymph nodes in nephrectomized cases. No significant difference was seen between the nephrectomized and nonnephrectomized cases with metastatic involvement of 1 organ, with respect to the frequency with which any given organ was involved. There were 5 patients with metastasis in whom survival was 10 years or longer after nephrectomy. Lung metastasis was present in 4 cases and lymph node metastasis in 1. Of these 5 patients 3 had undergone nephrectomy only. The results failed to show any evidence of the effectiveness of nephrectomy or adjuvant therapy. It was inferred that the mode of metastasis and the clinical progress of patients after nephrectomy for renal adenocarcinoma are influenced more profoundly by characteristics of the tumor itself than by nephrectomy.

Minimum dose contrast bolus in computed angiotomography of the brain.

We have carried out basic investigations of an intravenous minimum dose bolus (MinDB) injection method for cerebral computed angiotomography by comparing blood iodine concentrations with high resolution computed tomography (CT) images of cerebral blood vessels. With the GECT/T 8800 scanner, a MinDB injection of 1 ml/kg of 60% Conray at a rate of 2 ml/s made it possible to increase the blood iodine concentration in the carotid artery to more than 15 mg/ml for about 20 s. Computed tomography performed during this period enabled us to obtain clear images of the circle of Willis, including other main cerebral arteries, deep veins, and cortical and lenticulostriate arteries. Cerebral computed angiotomography using this method should be useful for noninvasive screening or detection of cerebrovascular lesions themselves, such as cerebral aneurysms, cerebrovascular obstructions, arteriovenous malformations, and moyamoya disease, and should also be useful for determining the anatomical relationship between the cerebral blood vessels and other parenchymal or space occupying lesions.

Normal anatomy of cerebral vessels by computed angiotomography in the coronal, Towne, and semisagittal planes.

This report presents a detailed analysis of the normal cerebrovascular anatomy on the modified coronal, Towne (half-axial), and semisagittal computed tomographic (CT) images. Direct multiplane CT scans were obtained from fresh cadavers injected with gelatinous iodine solution. The modified coronal plane is optimal for delineation of the internal carotid bifurcations and the anterior and the middle cerebral arteries, and for identification of the lenticulostriate arteries and the angiographic sylvian point. The Towne plane is optimal for delineation of the vertebrobasilar arterial system. Thorough knowledge of the normal appearance of cerebral blood vessels in biplane or multiplane computed angiotomography is a prerequisite for understanding the spatial relationships of the abnormal cerebrovascular anatomy.

Disulfiram-like effect of diethyl maleate on barbiturate-induced hypnosis and 5-hydroxytryptamine metabolism.

Diethyl maleate (DEM, 600 mg kg-1 i.p.) significantly potentiated hexobarbitone hypnosis and lowered plasma hexobarbitone level on awakening. Sleeping time following intracerebroventricular (i.c.v.) injection of phenobarbitone was also prolonged by DEM treatment. When administered to DEM-treated rats, L-tryptophan (50 mg kg-1 i.p.) significantly potentiated hexobarbitone hypnosis, although alone it had no effect in control rats. DEM markedly diminished the activity of brain low-Km aldehyde dehydrogenase (A1DH) and the formation of 5-hydroxyindoleacetic acid from 5-hydroxytryptamine (5-HT) without affecting MAO activity in various areas of the brain. Conversely, the protein-bound radioactivity derived from i.c.v. [14C]-5-HT was increased by DEM treatment. These results showed that DEM is comparable with disulfiram, a brain A1DH inhibitor, in terms of its effect on 5-HT metabolism and barbiturate hypnosis.

Glycine cleavage system in ketotic hyperglycinemia: a reduction of H-protein activity.

Glycine cleavage activity was compared in the livers from three cases of ketotic hyperglycinemia (two cases of propionic acidemia and one case of methylmalonic acidemia) and three controls. In one case of propionic acidemia, glycine cleavage activity (5.2 nmole/mg protein/hr) was normal in the liver obtained at biopsy when the patient was well controlled by the treatment with low protein diet (0.8 g/kg/day) and the level of serum glycine was lowered to normal. In the two other cases of ketotic hyperglycinemia, glycine cleavage activity was significantly reduced in the liver obtained at autopsy when the patients died in the state of metabolic acidosis. Its activity in the liver of one case of propionic acidemia (0.7 nmole/mg protein/hr) was 6-26% of that in controls (2.7-10.8 nmole/mg protein/hr), and 2-7% in the case of methylmalonic acidemia (0.2 nmole/mg protein/hr). Analysing of the individual components of the glycine cleavage system, a marked decrease in the activity of H-protein was revealed in the livers of the both patients; it (0.2 nmole/mg protein/hr) was only 3-4% of that in controls (4.9-6.3 nmole/mg Protein/hr). These findings suggest that the reduction of the glycine cleavage system in the liver of ketotic hyperglycinemia occurs secondarily as speculated previously and is caused mainly by a decrease of H-protein activity.

Inhibitory effects of aryl trans-4-(aminomethyl)cyclohexanecarboxylate and aryl trans-4-(guanidinomethyl)cyclohexanecarboxylate on serine proteases, and their antiallergic effects.

Since serine protease in involved in histamine release from mast cells, we attempted to prepare new protease inhibitors, trans-4-(guanidinomethyl)cyclohexanecarboxylic acid (GmcHX-CO2H) esters, and examined their inhibitory effects on typical serine proteases and on histamine release induced by compound 48/80. We compared their effects with those of trans-4-(aminomethyl)cyclohexanecarboxylic acid (AmcHx-CO2H) esters. AmcHxCO2H and GmcHxCO2H esters inhibited the esterolytic activity of trypsin, but GmcHx-CO2H esters had little or no inhibitory effect on caseinolytic activity whereas AmcHxCO2H esters strongly inhibited the latter. AmcHCO2H esters strongly inhibited plasmin but had no effect on chymotrypsin. GmcHxCO2H esters strongly inhibited the esterolytic activity of chymotrypsin, but had no effect on chymotrypsin-induced caseinolysis. Both GmcHxCO2H an AmcHxCO2H esters inhibited urokinase. Of the esters of AmcHxCO2H and GmcHxCO2H tested, only GmcHxCO2H p-tert-butylphenyl ester (GmcHxCOOPhBut) at low concentration (27 microM) strongly inhibited histamine release from rat mast cells induced by compound 48/80. GmcHxCOOPhBut was effective in preventing active systemic anaphylaxis and passively sensitized guinea pigs. Its effectiveness in preventing anaphylactic phenomena might be due to its strong inhibitory effects on histamine release from mast cells.

Comparison of cytosolic and mitochondrial enzyme alterations in the livers of propionic or methylmalonic acidemia: a reduction of cytochrome oxidase activity.

The activities of mitochondrial, cytosolic and microsomal enzymes in liver specimens obtained from three patients with propionic or methylmalonic acidemia were compared with those of control patients who had died from unrelated causes. Only the activity of cytochrome oxidase (mitochondrial enzyme) was significantly reduced in the patients of propionic acidemia and methylmalonic acidemia who were in the state of metabolic acidosis; in two patients the activity was less than 30% of that in controls, but in the other patient of propionic acidemia, who was under the treatment with a low protein diet (0.8 g/kg/day), the activity was 50% of that in controls. The metabolites of branched chain amino acids (tiglic acid, propionic acid, methylmalonic acid, succinic acid, tiglyl-CoA and propionyl CoA) exhibited no inhibitory effect on the cytochrome oxidase activity of the sonicated rat liver mitochondria. The reduction of cytochrome oxidase activity found in these organic acidemias may be caused secondarily by some unknown mechanism.

Delineation of unruptured cerebral aneurysms by computerized angiotomography.

Unruptured aneurysms were diagnosed in 15 of 86 patients with cerebral aneurysms during 2 years beginning in April, 1979. One patient with severe head injury was excluded from the series. Fifteen aneurysms in the other 14 patients were first detected by computerized angiotomography. Six aneurysms were located in the middle cerebral artery, three in the upper half of the basilar artery, two in the anterior communicating artery, two in the posterior cerebral artery, and two at the internal carotid-posterior communicating artery junction (both in the same patient). Three were smaller than 5 mm, eight were between 6 and 10 mm, and four were larger than 10 mm. The noncontrast computerized tomography (CT) findings associated with 15 aneurysms were as follows: five showed defects in the basal cistern of Sylvian fissure, four were calcified or high-density masses, and in the six instances there was no evidence of an aneurysm. Unruptured aneurysms may be suggested by a well demarcated, round, isodense mass which forms a defect in the basal cistern of Sylvian fissure on a plain CT image, and are highly and homogeneously enhanced by computerized angiotomography. A carotid artery blood iodine level of 15 mg/ml is required to obtain clear images. The authors conclude that computerized angiotomography is useful in the delineation of unruptured aneurysms.

Induction of hepatic gamma-glutamyl transpeptidase in rats by repeated administration of aminopyrine.

The effect of a 20-day administration of aminopyrine (600 mg kg-1) as well as two metabolites of aminopyrine, 4-aminoantipyrine (525 mg kg-1) and 4-acetamidoantipyrine (635 mg kg-1), on several hepatic, kidney and serum enzyme activities were investigated. In the aminopyrine-treated group, a pronounced induction of gamma-glutamyl transpeptidase was shown in the whole homogenate as compared to that in the hepatic microsomes. Serum gamma-glutamyl transpeptidase activity was also increased by administration of aminopyrine or 4-aminoantipyrine. No change in gamma-glutamyl transpeptidase activity was observed in kidney and hepatic cytosolic fractions. In all cases, 4-acetamidoantipyrine treatment showed no significant change in the enzyme activities tested. Under the same experimental condition, the amounts of cytochrome P-450 and b5, the activities of aminopyrine N-demethylase, aniline hydroxylase and carboxylesterase of liver microsomes were all induced in the aminopyrine- and 4-aminoantipyrine-treated rats. The activities of NADPH cytochrome c reductase and NADH ferricyanide reductase were also increased. The results in this paper support the view that repeated administration of aminopyrine induces hepatic membrane-bound enzyme, particularly, gamma-glutamyl transpeptidase activity.

Influence of age and nephrectomy on metastatic patterns in Wilms' tumors.

Data from 194 autopsy cases of Wilms' tumor were analyzed statistically to investigate the mode of dissemination by classifying them according to the number of organs involved in metastasis, age and history of nephrectomy. Metastasis-free cases were more frequent in cases under two years of age, in bilateral cases, and in non-nephrectomized cases. The frequencies of metastasis to various organs were essentially similar among the various age groups, except those to the contralateral kidney and intestines in cases under two years of age, which were significantly more frequent. Nephrectomized cases showed a significantly higher incidence of metastasis to the peritoneum than non-nephrectomized cases. In cases with metastasis to one organ, metastasis to the lungs, lymph nodes and liver showed low frequencies (17%, 13% and 17%, respectively), but that to the peritoneum showed a rather high frequency (21%). Radiation therapy for the peritoneum after nephrectomy is recommended.

Red blood cell types and living related renal transplantation.

We studied the effects of red blood cell types on 34 cases of living related renal transplantation. Red blood cell types were examined retrospectively concerning the ABO, Lewis, P, Rh-Hr, Kell, Duffy and MNSs systems in both donors and recipients. The correlation between red blood cell type and acute rejection in HLA identical pairs and the effects of the Lewis, P and other systems on graft outcome in HLA semi-identical pairs were considered. In the HLA identical group, the Rh-Hr and MNSs systems participated in acute rejection. In addition to the ABO and Rh-Hr systems, the Lewis and P systems influenced the graft rejection in the HLA semi-identical group.

Complication of renal neoplasms with cystic disease: an indication for nephrectomy in potential transplant recipients.

Thirty-five (1.1%) out of a total of 3,133 autopsied cases of renal tumors were complicated by renal malignant tumors and cystic renal disease. They accounted for 1.4% of 2,417 cases of renal malignant tumors and for 4.7% of 751 cases of cystic renal disease. Among the 35 complicated cases, 10 cases or 28.6% were free of metastasis. The incidence of complicated cases and those without metastasis aged over 60 years or over was 12.2% and 29.2% respectively, while the 10-59 year-old age group displayed a lower incidence of 3.6% and the same incidence of 27.3% respectively. In 26 out of 35 cases the malignant tumors was renal adenocarcinoma with no evidence of metastasis in nine cases (34.6%). Although it is unknown whether cystic renal disease undergoes malignant changes, not all of the patients undergoing chronic hemodialysis should be indicated for nephrectomies since such patients with renal malignancies are mostly elderly and are frequently free from metastases.

Report on blood access for hemodialysis in the Department of Transplantation, Tokai University School of Medicine and a satellite hospital.

There were 316 patients being treated by maintenance hemodialysis in the Department of Transplantation, Tokai University School of Medicine and its satellite hospital (Bohsei Clinic, Medical Treatment Cooperation, Showakai) as of December 31, 1981. Forty eight male (23.1%) and 39 female (35.7%) patients were operated on for external shunts. One case of external shunt was functioning well 48 months later. No patient was dialysed through external shunts on December 31, 1981 in our Department or the satellite. The majority of patients, who were maintained on hemodialysis with an arteriovenous (native vein) fistula available, experienced no difficulties. Nearly 50 percent of cases with the first fistula were functioning on December 31, 1981. There were some complications with fistulas such as pseudoaneurysms, infections, the sore thumb syndrome or the steal syndrome. We experienced 14 hemodialysis cases using expanded polytetrafluoroethylene (E-PTFE) grafts and one case using femoral artery repositioning as blood access.
