Clinical report of diabetic patients treated by chronic hemodialysis.

Twenty one diabetic patients were treated by chronic hemodialysis (HD) in the seven year period from 1975 to 1982 in our Department. The actual one-year and five-year survival rate for diabetic patients treated by maintenance HD were 76% and 43%, respectively. Blood urea nitrogen and serum creatinine levels at the HD induction were 111.95 +/- 28.51 mg/dl and 10.73 +/- 4.56 mg/dl, respectively. There were complications in the form of blindness in 8 cases (38.1%), peripheral nerve impairment in 17 cases (81.0%), myopathy in 8 cases (38.1%), diabetic gangrene in 3 cases (14.3%) and glaucoma in one case (4.8%).

[Successful radiotherapy in spontaneous carotid-cavernous sinus fistula--a case report].

The authors reported a case of spontaneous carotid-cavernous sinus fistula fed by meningohypophyseal trunk of the internal carotid artery, that completely disappeared following cobalt 60 irradiation to the region of fistula (27 days; total dose, 4,100 rad). A 65-year-old man was admitted to our hospital on May 25, 1982, with a two-week history of progressive prominence of the left eye, diplopia on left lateral gaze, and continuous intracranial bruit. He first noted redness of the left eye without apparent cause such as head trauma. On admission, slight nonpulsative exophthalmos, periorbital edema, chemosis, and venous congestion of the conjunctiva of the left eye were noted. The pupils were reactive and equal in size, and the left abducent nerve palsy was observed. Fundoscopic examination disclosed to be normal. No bruit was heard over the left orbital region. Left carotid angiography on May 28 demonstrated dural internal carotid-cavernous sinus fistula fed by meningohypophyseal trunk, and drained into superior and inferior ophthalmic veins and basilar venous plexus. Though the patient was treated only conservatively, symptoms became worse to decrease visual acuity. On July 14, we started cobalt 60 irradiation to the region of fistula (27 days; total dose 4,100 rad). Symptoms gradually improved about halfway through irradiation. After completion of irradiation, almost complete improvement of visual and ocular symptoms was observed except the left abducent nerve palsy. Repeated angiography on Aug. 20 revealed complete disappearance of fistula. Three months after treatment, no recurrence of symptoms was observed and the abducent nerve palsy persisted without improvement.(ABSTRACT TRUNCATED AT 250 WORDS)

Secondary hyperparathyroidism in patients on maintenance hemodialysis: study of the N and C-terminal parathyroid hormones.

In 34 dialysis patients without secondary hyperparathyroidism and 10 dialysis patients with secondary hyperparathyroidism, the concentration of parathyroid hormone, total calcium, ionized calcium, inorganic phosphate, total alkaline phosphatase and bone alkaline phosphatase in serum were determined. The following results were obtained: serum N and C-terminal parathyroid hormone levels, serum total and ionized calcium levels, and serum total and bone alkaline phosphatase levels in dialysis patients with secondary hyperparathyroidism were higher than the corresponding values in cases of negative secondary hyperparathyroidism. Serum phosphate levels in dialysis patients with and without secondary hyperparathyroidism did not differ. Serum bone alkaline phosphatase activities were well correlated with serum total alkaline phosphatase activities in patients on hemodialysis.

Secondary hyperparathyroidism in patients on maintenance hemodialysis: study of optimal hemopurification and parathyroidectomy.

We have found that hemofiltration and direct hemoperfusion combined with hemodialysis removed significant amounts of parathyroid hormone. Hemofiltration and direct hemoperfusion combined with hemodialysis seemed to have better effects on secondary hyperparathyroidism. Surgical parathyroidectomy is indicated when medical parathyroidectomy has failed to prevent progressive bone disease. We had ten patients with refractory secondary hyperparathyroidism. In these patients, subtotal parathyroidectomy was attempted. The results obtained were: 1) Subtotal parathyroidectomy was successful in all patients. 2) Preoperative serum parathyroid hormone values were useful for diagnosis of secondary hyperparathyroidism. 3) The average serum N and C-terminal parathyroid hormone values measured preoperatively were 4,491.00 +/- 892.13 pg/ml and 4,580.00 +/- 474.48 pg/ml, respectively. 4) The average N and C-terminal parathyroid hormone values measured postoperatively were 298.60 +/- 128.68 pg/ml and 2,631.00 +/- 719.51 pg/ml, respectively.

[A case of paraganglioma of the urinary bladder].

A very rare case of paraganglioma of the bladder is reported. The patient was a 51-year-old man. He was admitted to our hospital on June 18th, 1981, with the complaint of gross hematuria. He had no attack of hypertension and we could not find in him any parasympathetic symptom except hematuria. Upon urological examination, he was suspected to have stage B cancer of bladder. He received partial cystectomy with uneventful course during the operation on June 30th, 1981 and was discharged on the 15th postoperative day. The postoperative histological diagnosis was paraganglioma of the urinary bladder. He was confirmed to have normal levels of catecholamine and its metabolites in the urine after 4 months postoperatively. At present, he is asymptomatic.

Changes in the concentration of enolase isozymes and S-100 protein in degenerating and regenerating rat sciatic nerve.

Levels of enolase isozymes (alpha alpha, alpha gamma, and gamma gamma forms) and S-100 protein in rat sciatic nerves were determined during their degeneration and regeneration processes. The sciatic nerves were unilaterally crushed or severed. The rats were killed 1, 2, 6, and 8-9 weeks later, and both the proximal and distal portions of the damaged nerves were dissected. Control samples were obtained from the untreated contralateral hindlimbs. Enolase isozymes and S-100 protein in the nerve segments were determined with the enzyme immunoassay method. The control nerves contained about 40, 90, and 30 pmol/mg protein of alpha alpha, alpha gamma, and gamma gamma enolases, respectively, and 0.85 microgram/mg protein of S-100 protein. These levels were not affected by repetitive electrical stimulation of the nerve fibers in vivo. The levels of the nervous system-specific forms of enolase (alpha gamma and gamma gamma) and S-100 protein decreased markedly within a week in the distal portion of the crushed nerve (alpha gamma, 27 pmol/mg; gamma gamma, 5.5 pmol/mg; S-100 protein, 0.36 microgram/mg) with apparently no change in the concentration of alpha alpha enolase. These levels in the proximal portion of the crushed nerve remained unaltered. The sensory and motor functions impaired by the sciatic nerve crush showed a recovery more or less after 4-9 weeks. This recovery was accompanied by a gradual regaining of the specific proteins in the distal portion of injured nerves (alpha gamma, 64 pmol/mg; gamma gamma, 13 pmol/mg; S-100 protein, 0.63 microgram/mg at the 8-9th week).

Toxicity and biotransformation of acetaminophen in rat hepatocytes (I). Uptake and release.

Uptake and conjugation of acetaminophen were studied in isolated rat hepatocytes. Acetaminophen was taken up by the cells and the conjugates formed in the cells were rapidly released from the cells at a constant rate. The kinetic analysis demonstrated that uptake was due to a saturable process as well as a simple diffusion. Decrease in uptake of acetaminophen by metabolic inhibitors suggested that uptake of acetaminophen by hepatocytes might be, at least in part, due to an energy dependent process.

Perturbation of hepatic glutathione level and glutathione-related enzyme activities by repeated administration of aminopyrine in rats.

The influence of repeated administration of aminopyrine on the tissue glutathione level and related enzyme activities was investigated in rats. Reduced glutathione level in the liver was not changed after 5 days of treatment but a significant increase was seen after 15 days of aminopyrine treatment. Oxidized glutathione level was unaltered throughout the experiment. Repeated administration of aminopyrine for 5 days caused a marked increase in gamma-glutamyl transpeptidase activities in liver whole homogenates as well as in the nuclear fraction, but not in liver microsomes. These results suggest that gamma-glutamyl transpeptidase located in plasma membrane may be induced by repeated administration of aminopyrine for 5 days. The activities of cytosolic glutathione peroxidase, which modulates glutathione level, were also significantly increased by aminopyrine treatment. Under the same conditions, glutathione peroxidase activity with H2O2 as a substrate was unaltered, while a time-dependent increase in the activity was found when cumene hydroperoxide was used as a substrate, even after a single administration of aminopyrine. The intracellular cysteine level was increased accompanying the increased gamma-glutamyl transpeptidase activities. Therefore, induced gamma-glutamyl transpeptidase may play a role in the reclamation of extracellular oxidized glutathione.

Cross-correlation analysis of midbrain reticular neuron pairs during sleep-waking cycle of the cat.

By simultaneously inserting three extracellular microelectrodes, separated by 1.0 mm from each other, into the cat's midbrain reticular formation (MRF), temporal cross-correlation of firing of neuron pairs was measured to investigate the mode of interaction among the MRF neurons during different states of sleep and wakefulness. None of 97 neurons pairs studied gave clear-cut cross-correlograms suggesting cascade connection between two neurons. However, 29 neuron pairs showed weakly synchronized firing which occurred periodically with a mean interval of 1.23 s. This interval was different from the respiratory cycle. The rhythmic synchronization appeared most obviously during slow wave sleep. The synchronized firing was encountered more often in the pairs of adjacent neurons picked up with a single electrode and in the pairs picked up with separate electrodes positioned along the mediolateral axis, when compared with the neuron pairs located along the rostrocaudal axis. About 80% of the neurons (37 our of 46) which showed periodically synchronized discharge had no corresponding periodicity in their auto-correlograms. Simple synaptic linkage conceivable for the observed periodic synchronization of a neuron pair would be shared inhibition with the common inhibitory source activated periodically. Some neurons in the dorsal raphe nucleus and in the solitary tract nucleus were also examined for the cross-correlation with the MRF neurons. However, no positive relationship was found.

D-lactic acidosis in two patients with short bowel syndrome: bacteriological analyses of the fecal flora.

Two cases of D-lactic acidosis associated with short bowel syndrome are described. The administration of kanamycin to the patients showed a decrease in D-lactate in blood and urine in parallel with disappearance of metabolic acidosis. Bacteriological analyses of the fecal flora showed an increase in Lactobacillus buchneri in the first patient and Lactobacillus fermenti IVa in the second; both bacteria were sensitive to kanamycin. Quantification of in vitro production of D-lactate by each species of bacteria isolated from the feces revealed that Lactobacillus produced more D-lactate than other species of bacteria. These observations indicate that Lactobacillus may play an important role in the induction of D-lactic acidosis in patients with short bowel syndrome.

Effect of disulfiram in combination with L-tryptophan and lithium on pentylenetetrazol-induced seizure.

Disulfiram prolonged the latency to clonic seizure caused by pentylenetetrazol (PTZ, 100 mg/kg SC). The effect of disulfiram was augmented by combination with tryptophan plus lithium, although neither tryptophan or lithium prolonged the latency to clonic seizure. The latency to tonic seizure was also prolonged in disulfiram-treated animals in parallel with the prolongation of the latency to clonic seizure. Lithium treatment completely prevented the incidence of tonic seizure, while this effect was cancelled in disulfiram-treated animals. Disulfiram acts on the clonic and tonic seizures in different ways.
