Coronal computerized angiotomography for the diagnosis of isodense chronic subdural hematoma.

The authors demonstrate the value of coronal computerized angiotomography for the diagnosis of and screening for isodense chronic subdural hematoma. Specific features of coronal computerized angiotomograms include 1) thick lines of high density with slight convexity; 2) lines of high density parallel with the inner table of the skull; or 3) high-density dotted lines away from the inner table. These high-density lines or dots represent the superficial cerebral veins and cortical branches of the middle cerebral artery, which have been displaced by the hematoma. When these characteristic features are revealed on noninvasive coronal computerized angiotomography, cerebral angiography may be unnecessary.

B-factor, an essential regulatory substance inducing the production of rifamycin in a Nocardia sp.

"Curing" treatment of a rifamycin-producing Nocardia sp. resulted in a mutant deficient in the synthesis of antibiotics. This deficiency was reversed in a medium containing yeast extract. The active substance, named B-factor, which induced rifamycin production in the mutant was purified from yeast extract, and its structure, 3'-(1-butylphosphoryl) adenosine, was determined by structural analysis and chemical synthesis. An extremely low concentration of B-factor (10 ng/ml) caused recovery of rifamycin B synthesis in the mutant and stimulated synthesis of the antibiotic in the parental strain.

[Experimental and clinical evaluation on ceftriaxone in the field of obstetrics and gynecology].

Ceftriaxone (CTRX), a new cephalosporin antibiotic, was evaluated in the field of obstetrics and gynecology and the following results were obtained. The concentration of CTRX after an intravenous injection with 1 g was determined in the uterine artery, cubital vein and in the intrapelvic genital tissues such as the oviduct, ovary, endometrium, myometrium, cervix uteri and portio vaginalis. The peak level was 160 micrograms/ml at 26 minutes after injection both in the uterine arterial serum and cubital venous serum, 48 and 38 micrograms/g at 1 hour and 18 minutes in the tissues of oviduct and ovary, respectively, 54 and 50 micrograms/g at 48 minutes in the myometrium and cervix uteri, respectively, while 46 micrograms/g at 39 minutes in the portio vaginalis. The mean level 18 to 24 hours after administration was 19 micrograms/ml in the uterine arterial serum and cubital venous serum and 6.3 micrograms/g in the intrapelvic genital tissues. A case of intrapelvic infection clinically showed an excellent response without any side effects by intravenous drip infusion with 2 g divided as twice a day for 7 days. The above results show that CTRX is useful in the field of obstetrics. and gynecology.

[Clinical evaluation of cefmenoxime in internal medicine, with special reference to infections associated with hematological disorders].

Clinical evaluation of cefmenoxime (CMX, Bestcall) was examined in the infection associated with hematological disorders, respiratory tract and other disorders. Clinical effectiveness for severe infection of hematological disorders was 47.4% in good and 84.2% in fair response, however, in the respiratory tract infections, 89.7% in good response was obtained. Opportunistic infection due to Gram-negative bacilli are often experienced in patients with hematological disorders. It was discussed that CMX would be a good therapeutic agent against infectious diseases associated with hematological disorders because it's antibacterial spectrum would be parallel to pathogens of such disorders.

Effect of steroidal sex hormones on the sex-related differences in the hepatic activities of gamma-glutamyltranspeptidase, glutathione S-transferase and glutathione peroxidase in rats.

The activities of gamma-glutamyltranspeptidase (gamma-GTP) and glutathione peroxidase (GSH-Px) were higher in female than in male rat liver. Glutathione S-transferase (GST) activities were conversely higher in males than females. Estradiol administration to castrated rats enhanced the gamma-GTP activity. Castration produced a significant increase in GSH-Px activities which was reduced to the control level by subsequent testosterone treatment. Ovariectomized rats had markedly increased the GST activity. These results suggest that GSH-related enzymes studied here may be subject to modulation by androgenic and estrogenic influences.

Effects of a single administration of 6-aminonicotinamide on hepatic microsomal drug metabolism in rats.

Pretreatment of rats with 6-aminonicotinamide (6-AN) induced a significant increase in aminopyrine (AM) N-demethylation and hexobarbital (HX) hydroxylation in rats. In this study, the concentrations of AM, 4-monomethylaminoantipyrine (MAA), aminoantipyrine (AA) and further metabolites were measured by using the GC/MS method. The increase in AM N-demethylation induced by 6-AN treatment was confirmed by measuring the area under the plasma concentration-time curve (AUC) for AM, MAA and AA in rats after AM administration. Of the first two steps of AM metabolism mediated by cytochrome P-450-coupled monooxygenase, the second step (from MAA to MA) was found to be more affected by 6-AN than the first one (from AM to MAA).

[clinical studies on primary renal pelvic tumors].

The 15 cases of the primary renal pelvic tumors treated at our Hospital between 1974 and 1983, were reviewed retrospectively. The patients ranged in age from 41 to 74 years old (average: 58.3 years old). There were 11 males and 4 females, the ratio being 2.8:1.0. The affected side was left in 9 cases and right in 6 cases. The most frequent symptom was macrohematuria, which was seen in 12 cases (80%). The major finding of IVP was non-functioning kidney, which was seen in 8 cases (53.3%). Positive urinary cytology was obtained in 8 cases (53.3%). As the surgical method, total nephroureterectomy with bladder cuff was performed in 8 cases, nephroureterectomy in one case and nephrectomy in 6 cases. Histologically, 14 cases were transitional cell carcinoma and one case was squamous cell carcinoma. Simultaneous urothelial tumors were seen in the bladder of 2 patients. A subsequent ureteral tumor was found in one of the 7 cases in which ureters were resected incompletely, and subsequent bladder tumors were found in 8 of the 15 cases receiving surgical treatment in the follow-up period. All of tumors were found within 2 years after operation. Over-all actual survival rates at 1, 3 and 5 years were 87%, 67%, 48%, respectively. Three and 5 year actual survival rates were 100%, 100% respectively for the low stage group and 59%, 29% respectively for the high stage group. Three and 5 year actual survival rates were 100%, 78%, respectively for the low grade group and 44%, 27% respectively for the high grade group. Among several factors, stage and grade of the tumor were the most influencing factors for prognosis.

[Clinical studies on primary ureteral tumors].

The 15 cases of the primary ureteral tumors treated at our Hospital between 1974 and 1983, were reviewed retrospectively. The incidence of primary ureteral tumors among the outpatients in our urologic clinic was 0.15%. The patients ranged in age from 50 to 75 years old (average: 65.5 years old). There were 11 males and 4 females, the ratio being 2.8:1.0. The right ureter and the lower third of the ureter were involved more frequently than other areas. The most frequent symptom was macrohematuria which was seen in 12 cases (80%). The major finding of IVP was non-functioning kidney, which was seen in 11 cases (73.3). Positive urinary cytology was obtained in 9 cases (60%). Twelve patients underwent nephroureterectomy with bladder cuff or total cystectomy. Histologically, all cases were transitional cell carcinoma. Simultaneous urothelial tumors were seen in the bladder in 4 cases (26.7%). The over-all actual survival rates at 1, 3 and 5 years were 59%, 42%, 42%, respectively. The 5-year actual survival rate was 63% for the low grade group and 0% for the high grade group. The 5-year actual survival rate was 82% for the low stage group and 0% for the high stage group. Among several factors, grade and stage of the tumor were the most influencing factors for prognosis.

Teratogenicity of aspirin and its metabolite, salicylic acid, in cultured rat embryos.

Rat embryos were exposed to aspirin or its metabolite, salicylic acid in culture. In these embryos acute reduction of heart beat was observed during 4 hours of administration compared to that in non-treated one. Protein contents and crown-rump length of cultured embryos were significantly decreased in aspirin-treated group, but were not so decreased in salicylic acid-treated one. The predominant defects of the embryos exposed to aspirin were edematous facial malformations and abnormality of tail. On the other hand, facial anomalies such as cleft lip and curly tail were observed in the embryos cultured with salicylic acid. Anomalies induced by aspirin were systemic, while salicylic acid induced localized malformations. These results might be due to the differences between aspirin and its metabolite, salicylic acid in their teratogenicity.

Gonadal hormone-induced changes in hepatic microsomal carboxylesterase in rats.

Hepatic microsomal carboxylesterase (E.C. 3.1.1.1) from rat liver microsomes showed a different capacity for the hydrolysis of various substrates. In castrated male rats, the enzyme activities towards p-nitrophenylacetate and malathion were decreased. When testosterone propionate was administered to castrated male rats, the activities of p-nitrophenylacetate and malathion hydrolases were reversely increased. However, in ovariectomized female rats, the carboxylesterase activities showed substrate-dependent changes, i.e., increase in p-nitrophenylacetate and malathion hydrolases and decrease in acetanilide and isocarboxazid hydrolases. When estradiol benzoate was administered to ovariectomized female rats, the activities of p-nitrophenyl-acetate and malathion hydrolases were decreased; and acetanilide and isocarboxazid hydrolases were increased. These results suggest that hepatic microsomal carboxylesterases may be, at least in part, regulated by gonadal hormones which exert different effects on the several isozymes of carboxylesterases.

A simple method for the measurement of pineal melatonin with a high performance liquid chromatography--UV detection.

A new technique for determination of pineal melatonin by a high-performance liquid chromatography (HPLC) with UV detection has been developed. The melatonin level in the pineal glands of normal and orchidectomized rats was measured by this method. The pineal melatonin level gradually decreased after orchidectomy. This method provides new technique for the study of melatonin in endocrinological pharmacology.
