The contribution of opioid receptor to lappaconitine (LA) induced analgesia was investigated by in vivo and in vitro methods. The antagonistic effects of naloxone (NLX) on the analgesic actions of LA (s.c.) and morphine (MOR, s.c.) were determined by the tail pressure method, the hot plate method and the acetic acid-induced writhing method in mice. The ED50 values of MOR in the absence of NLX were 3.82, 0.50 and 0.33 mg/kg, respectively, but in the presence of NLX, the analgesic effect of MOR was decreased and the ED50 values were changed to 41.6, 4.40 and 5.97 mg/kg, respectively. However, NLX did not affect the analgesic activity of LA. The ED50 values of LA in the three methods were 9.12, 3.34 and 1.70 mg/kg in the absence of NLX and 12.1, 4.19 and 2.88 mg/kg in the presence of NLX, respectively. The inhibitory effect of the LA on the electrically evoked contractions of isolated guinea-pig ileum was weaker than that of MOR, and differently from MOR, its effect was not antagonized by NLX. Additionally, LA did not abolish the electrically evoked contractions of isolated rabbit and mouse vas deferens. In conclusion, the results suggest that the opioid receptor was not involved in the analgesic activity of LA.
Sialyl Lewis(x)-i (SLX) as a new tumor-associated antigen was assayed in sera of 133 gastric cancer and 62 colorectal cancer patients with preoperative state in order to elucidate the clinico-pathological features. The percent positives in sera of patients with gastric and colorectal cancer were 12.7% (17/133) and 16.1% (10/62), respectively, and there were few positive cases in the early stages. Therefore, it is suggested that the early diagnosis for gastric and colorectal cancer using SLX alone may be difficult. However, it is revealed that SLX has a high tumor-specificity in consideration of the clinico-pathological properties such as H, P, n, ly, v factors, depth of cancer invasion and operative radicality. These data suggested that SLX could be applicable as a useful tumor-marker which shows the metastasis or extension of carcinoma.
UNLABELLED: Paroxysmal nocturnal hemoglobinuria (PNH) is a rare, acquired hemolytic disorder characterized by a membrane abnormality of red cells, and characterized by two major clinical features of gross hemoglobinuria and diffuse venous thrombosis. In Japan, the present report records the first case of acute renal failure complicating PNH with treated by hemodialysis and was almost completely reversible. CASE: A 41 year-old woman was admitted for high fever (39.8 degrees C), dyspnea and clinical signs of a respiratory infection. She was started on Cefotax 1,000 mg 3 times daily. She subsequently developed acute renal failure and which treated by hemodialysis and was almost completely reversible. Following treatment of her renal failure, respiratory infection and anemia, she initially made good progress and was discharged.
A 57-year-old male was admitted to our hospital with bloody sputum, whose right lung had been resected for squamous cell carcinoma of the lung three and a half years ago. Tomographic examinations and chest CT films showed the presence of tracheal tumor. Histological examinations of the biopsy specimen obtained from tracheal tumor, using bronchofiberscopy, identified squamous cell carcinoma. Trachea was resected in the length of six cartilages and end to end anastomosis was performed. Though lung cancer and tracheal cancer were same histological type, clinical findings confirmed that this case was double primary cancer. Postoperative course was uneventful except for minor leakage of anastomosis site.
An autopsy case of a 57-year-old man who had received cisplatin, total dose of 3,250 mg (1,920 mg/m2) after proctectomy for advanced rectal cancer with hepatic metastasis is reported. Platinum concentrations in autopsy tissue samples are also reported. Cisplatin at a dose of 20-40 mg was administered intravenously once a week for 82 weeks, but renal failure did not occur. The patient died 529 days after his last cisplatin therapy, due to acute suppurative cholangitis. On autopsy, the hepatic hilar metastasis involved the bile duct, but the other metastasis of the liver became regeneration and necrosis, and the kidneys showed few findings but congestion and arteriosclerotic change. The spleen evidenced mild hemosiderosis. Tissue samples obtained at the time of autopsy were assayed for platinum using flameless atomic absorption spectrometry. The spleen had the highest concentration, about 2.7 times as high as that of the liver. The liver, the heart and the kidneys had high platinum concentrations (greater than 3.04 micrograms/g), but those of the jejunum and the ileum were low (less than 0.95 micrograms/g). Although cisplatin, FT 207 and krestin were effective for the rectal cancer, platinum was undetectable (less than 0.13 micrograms/g) in the ascending and the transverse colons.
It was demonstrated that the inbred strain EHBR had the C phenotype of esterase-3 judging from the absence of liver microsomal beta-glucuronidase and the pattern of esterase activities of liver homogenates after analytical isoelectric focusing. In addition, in the strain EHBR, liver microsomal hydrolase activities of acetanilide and isocarboxazid which are hydrolyzed well by esterase-3 were lower than in outbred Sprague-Dawley rat and inbred LEW rat having the D phenotype of esterase-3. These results suggest that the phenotype difference of esterase-3 is possible to cause the strain differences of liver microsomal carboxylesterase activities.
Cardiopulmonary bypass (C-P bypass) was performed on two patients who had not responded to conventional cardiopulmonary resuscitation (CPR). The first patient, a 56-y-o male, with bilateral pulmonary thromboembolism repeatedly underwent cardiac massage and electric defibrillation for recurrent ventricular fibrillation. A veno-arterial bypass route was prepared during cardiac massage, and bypass circulation was started 3 hours after the onset of the first ventricular fibrillation. Soon after the initiation of C-P bypass, the physical status and EEG of the patient improved. The patient regained consciousness within a few hours and later underwent open chest pulmonary embolectomy. The second patient, a 44-y-o male, developed refractory cardiogenic shock near the end of aortocoronary bypass graft operation. Under closed chest massage, a femoro-femoral cardiopulmonary bypass operation was started. Soon after the initiation of the bypass circulation and IABP, peripheral circulation improved markedly, and consciousness returned within several hours. Though the first patient finally died from far advanced pulmonary embolism, he was conscious as long as the C-P bypass was continued for two days. In the second patient, the cardiac function gradually improved after the 3rd day. C-P bypass was tapered and discontinued on the 5th day. Emergency veno-arterial bypass for CPR is effective means to maintain life until the cardiopulmonary and cerebral functions are restored. Recent advances in emergency C-P bypass are introduced and a new acronym extracorporeal lung and heart assist, ECLHA, is proposed. Emergency ECLHA with veno-arterial cannulations through percutaneous puncture will become a promising adjunct of cardiopulmonary-cerebral resuscitation in the near future.
Reactive systemic amyloidosis associated with rheumatoid arthritis (RA) was studied clinically in 28 patients (2 men and 26 women). The diagnosis of amyloidosis was established by histological examination of biopsy materials. Upper gastrointestinal tract biopsy was performed in 14 patients, and renal and rectal biopsy in 8 and 4 respectively. The mean age and duration of RA at diagnosis of amyloidosis were 58.6 (range 35-72) years and 15.5 (range 4-44) years respectively. Almost all patients had intractable and progressive courses of RA. Serological activities determined by C-reactive protein (CRP) and erythrocyte sedimentation rates were moderate to high in over 80% of the cases. Renal abnormalities were noticed in 19 cases, and gastrointestinal disorders in 10. Eight patients died from 1 to 54 (mean 15.3) months after the diagnosis of amyloidosis; 5 died of renal failure and 2 of gastrointestinal involvements. Renal impairments progressed frequently and serum creatinine elevated over 1.5 mg/dl in another 8 cases. Five patients progressing to renal failure were treated with hemodialysis. Three died within several weeks after the induction of hemodialysis, although 2 were treated for more than 2 years. Intractable hypotension and pulmonary congestion were frequently observed in these cases. A close relationship was found between serum amyloid A protein (SAA) and CRP concentration, so that the measurement of SAA seemed to be valuable in assessing disease activity. Concerning the treatment of amyloidosis, cyclophosphamide and corticosteroids seemed to be effective in several cases, although it had been unsatisfactory in most cases.
We have experienced epithelial dysplasia and invasive carcinoma of oral cavity, showing punctation and mosaic like atypical vessels confirmed by usual photographing. Case 1: 42-year-old man. After resection of tongue leukoplakia, the punctation appeared in the distal region of resected wound. Pathological findings were epithelial dysplasia, and expansion of capillaries in the stromal papillae were close to the surface. Case 2: 55-year-old woman. Punctation and mosaic like atypical vessels were recognized in the buccal mucosa, extending from the gingival carcinoma. Pathological findings were epithelial dysplasia and carcinoma in situ in punctation, and early invasive carcinoma in mosaic. There were irregularly expanded capillaries in the stromal papillae being close to the surface at both mosaic and punctation area. Although punctation, atypical vessels and mosaic are the colposcopic findings of uterine cervical cancer, these findings may become diagnostic point of early oral cancer.

The clinical usefulness of physiological components obtained by factor analysis was assessed in 99mTc-DTPA renography. Using definite physiological components, another dynamic data could be analyzed. In this paper, the dynamic renal function after ESWL (Extracorporeal Shock Wave Lithotripsy) treatment was examined using physiological components in the kidney before ESWL and/or a normal kidney. We could easily evaluate the change of renal functions by this method. The usefulness of a new analysis using physiological components was summarized as follows: 1) The change of a dynamic function could be assessed in quantity as that of the contribution ratio. 2) The change of a sick condition could be morphologically evaluated as that of the functional image.

In order to clarify physiological role of carbonyl reductase in rat ovary and effect of antiestrogen on its activity, we investigated effects of five antiestrogens on changes in ovarian enzyme activities towards three carbonyl compounds. All of five antiestrogens tested, significantly decreased 13,14-dihydro-15-ketoprostaglandin F2 alpha 4-benzoylpyridine and menadione reducing activities in ovarian cytosol by both consecutive treatments and single treatment. This inhibition of ovarian enzyme activities by antiestrogen was dramatically antagonized by human chorionic gonadotropin treatment at 3:00 p.m. on the day of proestrus. These results suggest that antiestrogen inhibits LH surge at afternoon of proestrus in rats and that the ovarian enzyme catalyzing reduction of carbonyl compounds is involved in ovulatory process.
Effector lymphocytes cytotoxic to autologous tumor cells consist of heterogeneous populations. In this communication, we have presented the results of experiments, in which these heterogeneous effector cells were classified based on their surface markers and modes of differentiation depending upon culture conditions. NK cells with a typical phenotype of CD2+ 3-16+ were one of major populations, which could respond to IL-2 and be differentiated into lymphokine activated killer (LAK) cells. LAK cells were also induced from a CD3+4-8- T cell subset by IL-2 stimulation. Effector cells with a phenotype of CD2+ 3-4-8-16-, which may belong to the T cells at early stages of differentiation, could also be induced by stimulation of immunoadjuvants. Finally, it was shown that the effector cells with a typical phenotype of cytotoxic T lymphocytes, CD3+4-8+16-, and a non-MHC-restricted cytotoxicity were induced by IL-2 in the tumor infiltrating lymphocytes.
This disease was reported 46-cases in the world literature. We stressed that the diagnosis of a cervicovaginal atresia by MRI offer this advantage of diagnosis in a noninvasive manner prior to surgical intervention.
An immunohistochemical method (peroxidase antiperoxidase method) was carried out to examine the distribution of endogenous estradiol (E2) and testosterone (Te). One hundred and twenty four (48.2%) of 257 colo-rectal cancer cases, and 36.7% of male cases and 58.4% of female cases revealed E2-immunostain. This immunohistochemical reaction for E2 was found more frequently in female patients than in male patients, while Te-immunostain was found in 15 (17.6%) of 85 colo-rectal cancer cases. E2 and Te were demonstrated mainly in the cytoplasms and occasionally in the nuclei of cancer cells. Since E2 or Te-positive cancer cells were mainly distributed in the deeper or invasive portion of tissue sections, it might deserve further investigation to elucidate a certain relationship between cancer growth and endogenous sex-hormones.
