Growth hormone-releasing hormone (GHRH)-secreting pancreatic tumor in a patient with multiple endocrine neoplasia type I.

A growth hormone-releasing hormone (GHRH)-secreting pancreatic tumor in a 36-year-old man, who had typical, familial, multiple endocrine neoplasia (MEN) type I with hyperparathyroidism and acromegaly, is described. The resected tumor, weighing 30 g, showed unusual histological features characterized by a meningioma-like arrangement of crescent-shaped cells and contained many cells that reacted with C-terminal specific antibody to GHRH-44 and a few somatostatin-immunoreactive (IR) and calcitonin-IR cells, but no GH-IR cells. A high concentration of IR-GHRH (9.8-13.2 micrograms/g wet weight tissue) with the full molecular size of GHRH-44 was detected in a tumor extract. Electron immunocytochemical study by the protein A-gold method revealed GHRH-IR granules with a mean diameter of 147 nm. After removal of the tumor, the plasma IR-GHRH level became normal (decreasing from 299 to 16.1 pg/ml) and the plasma IR-GH level also decreased, but still remained slightly high (decreasing from 42.4 to 9.6 ng/ml), suggesting the presence of an adenomatous lesion in the hypophysis.

Spindle cell carcinoma of the uterus.

A case of spindle cell carcinoma of the uterus in a 56-year-old woman is reported. Microscopically, it showed an intimate admixture of epithelial and sarcomatous elements. The epithelial nature, especially the squamous cell nature, in sarcomatous areas was indicated by immunoreactivity for keratin and ultrastructural characters, such as bundles of tonofilaments and some cell junctions, while the tumor cells in these areas were also vimentin-positive. Furthermore, ultrastructural study and immunostaining for actin demonstrated myofilaments in tumor cells in both tumor nests and sarcomatous areas. This may impart the high degree of invasiveness of spindle cell carcinoma.

Differential structural responses of small resistance vessels to antihypertensive therapy.

Regression of left ventricular hypertrophy after control of blood pressure has been documented with some antihypertensive agents but not with others. To determine whether similar differences in regression of wall thickening also occur in resistance vessels during treatment, matched groups of spontaneously hypertensive rats (SHR) were treated for 12 weeks with either hydralazine (H) or captopril and hydrochlorothiazide (C-D) and they were compared with untreated SHR and Wistar-Kyoto rats (WKY). Perfusion pressure was then determined in the hindlimbs of pithed rats under conditions of constant blood flow (4.0 ml/min) and maximal vasodilation (hemodilution to 22% hematocrit combined with continuous nitroprusside and papaverine infusion). This perfusion pressure, which has been validated as an index of thickening (hypertrophy) of resistance vessels walls, averaged 26.8 +/- 0.4(SE) mm Hg in untreated WKY (n = 12) and 37.6 +/- 0.4 mm Hg in untreated SHR (n = 11) (p less than .01). Treatment with H or C-D controlled blood pressure equally in SHR, but the two drugs had significantly different effects on both left ventricular hypertrophy and resistance vessels. Perfusion pressure was reduced from 37.6 +/- 0.4 mm Hg to 34.0 +/- 0.5 mm Hg (p less than .01) with C-D but only to 36.5 +/- 0.5 mm Hg with H (NS). Left ventricular weight was significantly reduced by C-D (2.02 +/- 0.02 vs 2.63 +/- 0.05 mg/g, p less than .01) but only to 2.44 +/- 0.05 mg/g by H.(ABSTRACT TRUNCATED AT 250 WORDS)

[Effects of buflomedil on survival rate, local cerebral blood flow and glucose utilization after cerebral ischemia in spontaneously hypertensive rats].

Cerebral protective effects of oral Buflomedil administration for 7 days were studied in spontaneously hypertensive rats (SHR). After permanent bilateral carotid artery occlusion (BCAO), the survival rates at 24 hr were 38% and 44% in groups treated with Buflomedil at the doses of 30 mg/kg (n = 16) and 90 mg/kg (n = 15), respectively, which were significantly higher than that in the control group (orally given saline for 7 days), 6%. In rats that survived and given Buflomedil, carbon filling of the brain after BCAO was extended to the territories of the internal carotid artery, while it was restricted to the brainstem and cerebellum in rats died within 3 hr and given either saline or Buflomedil. Local cerebral blood flow (LCBF), monitored by [14C]-Iodoantipyrine autoradiography, was determined at 3 hr after the start of BCAO or at 2 hr after reperfusion in rats orally given either saline or Buflomedil 30 mg/kg for 7 days. Local cerebral glucose utilization (LCGU), monitored by [14C]-Deoxyglucose autoradiography, was determined at 2 hr after reperfusion. At 3 hr after the start of BCAO, the LCBF in the Buflomedil group was higher by 71-128% in the cerebral cortex, 61-150% in the amygdala, caudate-putamen, nucleous accumbens and globus pallidus, and 82% in the internal capsule. At 2 hr after reperfusion, LCBF did not differ between groups, but LCGU in the Buflomedil group was significantly higher by 26-49% in the cerebral cortex, cochlear nuclei and vestibular nuclei than that in the control group. From these results, it is concluded that Buflomedil improved survival rate after permanent BCAO and have beneficial effect on local cerebral blood flow distribution after BCAO in SHR.

Lysine-mediated tissue osmication in combination with a tannin-osmium conductive staining method for non-coated scanning electron microscopy of biological specimens.

Glutaraldehyde fixed rat kidney blocks showed no charging effect when treated with lysine and osmic acid and viewed in a scanning electron microscope using an acceleration voltage of 5-30 kV and a specimen current of 1 X 10(-10) A. The podocyte processes and endothelial micropores of the glomerulus were visible without metal coating. Glutaraldehyde fixed, tannin-osmium impregnated and lysine-osmium treated specimens also showed no charging effect in the scanning electron microscope, yielding instead much clearer scanning images which were comparable to those obtained from gold-coated specimens or from tannin-osmium-thiocarbohydrazide-osmium impregnated specimens (Murakami and Jones, 1980).

Periosteal epithelioid hemangioendothelioma with leptomeric fibrils.

A case of recurrent periosteal epithelioid hemangioendothelioma of the right femur in a 66-year-old woman is reported. Microscopic examination showed epithelioid tumor cells with frequent intracytoplasmic vacuoles arranged in small nests or cords in hyalinized stroma. Immuno- and lectin-histochemical studies for factor VIII-related antigen and ulex europaeus I lectin indicated the endothelial nature of the tumor cells. Ultrastructurally, a few tumor cells contained leptomeric fibrils and crystalline filamentous aggregates in addition to showing certain features of endothelial cells. There seems to be no previous report of a vascular tumor containing leptomeric fibrils, which are often noted in normal striated muscle cells and their tumors. The bland appearance of the epithelioid tumor cells, poor vasoformative nature and prolonged clinical course differentiated this tumor from conventional angiosarcoma and so-called hemangioendothelioma of bone.

Analysis of the effects of the Blalock-Taussig shunt on ventricular function and the prognosis in patients with single ventricle.

Twenty-four patients with single ventricle, six with single left (SLV) and 18 with single right (SRV) ventricle, who received a Blalock-Taussig (BT) shunt at an average age of 3.2 years were studied. Ventricular function was assessed angiographically by end-diastolic volume index (EDVI) and ejection fraction (EF), and attempts were made to measure ventricular mass index (VMI) and VM/EDV. In 14 patients, the preoperative and postoperative results (average 2.4 years after placement of BT shunt) were compared in SLV (n = 5) and SRV (n = 9) groups. The SLV group showed significant increases in EDVI, VMI, and VM/EDV without a significant change in EF. The SRV group showed significant increases in EDVI and VMI, while EF decreased and VM/EDV was unchanged. Late death from congestive heart failure occurred in five patients with SRV. Three patients with atrioventricular valve regurgitation suffered late death. Among the patients with SRV, the late death group had significantly lower preoperative EF and VM/EDV compared with the survivors (n = 13). All of those with a preoperative EF of less than 0.50 and a VM/EDV of less than 0.35 g/ml suffered late death. In summary, patients with SRV appear to fail to develop adaptational hypertrophy to volume loading after the BT shunt procedure, with concomitant depression in ventricular pump function. Also, late cardiac failure seems likely to develop when low EF and VM/EDV are present preoperatively.

[An immunohistochemical study with monoclonal antibodies on lymphocytes infiltrating in cervical cancer].

We investigated lymphocytes infiltrating cervical cancer by an immunohistochemical method. Frozen sections (20 with frank invasive cancer, 4 with MIC, 5 with CIS, 6 with dysplasia, 3 normal) were stained by the ABC (avidin-biotin-peroxidase complex) method using monoclonal antibodies to identify functional subsets of lymphocytes. The monoclonal antibodies used were anti Leu 1 (T cell), anti Leu 2a (cytotoxic/suppressor T cell), anti Leu 3a (helper/inducer T cell), anti Leu 10 (B cell) and anti Leu 7 (NK cell). The results were as follows: Many lymphocytes infiltrated and surrounded the cancer nests, and there, T cells predominated over B cells. The intensity of infiltration of T cells was not correlated with the grade and prognosis of cervical cancer. The ratio of Leu 2a+ cells to Leu 3a+ cells tended to change with the advance of cancer. Leu 2a+ cells were relatively predominant in early cases and cases with a good prognosis. There were very few B cells close to cancer nests, and they were not correlated with the grade and prognosis of cervical cancer. NK cells were identified in many cases, but they were scattered and were not correlated with grade and prognosis of cervical cancer.

[Inverted papilloma of the ureter].

Two cases of ureteral inverted papilloma are reported. Case 1: A 48-year-old male had asymptomatic gross hematuria and filling defect of left middle ureter on intravenous pyelography. Segmental resection of ureter was performed. The specimen was a 1 cm polypoid lesion with histologic features resembling "multifocal bud-like proliferation", which was reported as the initial stage of the inverted urothelial tumor by Kunze et al. Case 2: A 64-year-old female with asymptomatic gross hematuria and complete obstruction of left middle ureter on ante- and retrograde pyelograms. Total nephroureterectomy was performed. A 4 cm lobulated and pedunculated lesion with histologic features of typical inverted urothelial papilloma was resected. Twenty-one cases of ureteral inverted papilloma in the literature, including our cases, are analyzed.

[Phase I-II study of recombinant interleukin-2].

A phase I-II study of recombinant interleukin 2 (rIL-2) (Shionogi Pharmaceutical Co.) was conducted against carcinoma of the lung (20 cases) and stomach (1 case) as well as metastatic pulmonary tumor (17 cases). The cytotoxicity of lymphocytes against K562, PC-9, PC-14 and Daudi cells was examined before and 1, 2, 3, 5, 7, 14, 21 and 28 days after rIL-2 administration. The subsets of lymphocytes and pharmacokinetics of rIL-2 were also analyzed. Four administration methods were used. 2-h drip infusion of 6.7 X 10(5) U/m2/day (A1: 6 cases) or 2.2 X 10(6) U/m2/day (A2: 8 cases) for 5 consecutive days, subcutaneous injection of 6.7 X 10(5) U/m2/day (B: 3 cases) for 28 consecutive days, 4-h continuous drip infusion of 3.3 X 10(5) U/m2/day (C1: B cases), 6.7 X 10(5) U/m2/day (C2: 7 cases) or 1.1 X 10(6) U/m2/day (C3: 5 cases) for 28 consecutive days, and (D) 24-h continuous drip infusion of 6.7 X 10(5) U/m2/day (D: 6 cases) for 5 consecutive days every week, repeating four cycles of the course. We discussed whether or not the problems of the phase I study with BRMs could be solved.
