Effect of dried, powdered Chlorella vulgaris on experimental atherosclerosis and alimentary hypercholesterolemia in cholesterol-fed rabbits.

The anti-lipidemic action and anti-atherosclerotic action of dried, powdered Chlorella vulgaris (CVP) were investigated using male Japanese White rabbits. A ten-week load of high-cholesterol diet remarkably increased serum total cholesterol and the beta-lipoprotein cholesterol levels in serum, causing aortic atheromatous lesion. In the Chlorella group which was administered a high-cholesterol diet containing 1% powdered Chlorella vulgaris, increase of total and beta-lipoprotein cholesterol level was suppressed. Further, the development of aortic atheromatous lesions was significantly inhibited. Clofibrate used as positive control in this experiment, did not show any inhibitory effect, either on the increase in serum lipid level or on the development of aortic atheromatous lesion.

Problems in the modified Fontan operation for univentricular heart of the right ventricular type.

Thirteen patients (ages 4 to 16 years) with univentricular heart of right ventricular type, nine with double-inlet right ventricle (DIRV), and four with mitral atresia who underwent a modified Fontan operation were reviewed. Among those with DIRV, right isomerism with a common atrioventricular (AV) valve was found in eight and situs inversus in one; among those with mitral atresia, AV discordance was found in two and concordance in two. Intra-atrial routing using a baffle with atriopulmonary anastomosis was the main procedure (11 patients). There were two operative deaths (one DIRV, one mitral atresia) and three hospital deaths, with an early mortality of 38.5% overall (DIRV 44%, mitral atresia 25%). Low cardiac output was the main cause of death, with relatively high right atrial pressure and with tachyarrhythmias in most of the patients. Mortality was 75% for patients with DIRV who had total anomalous pulmonary venous drainage (n = 4) and 20% for those without (n = 5). Preoperative ventricular volume and ejection fraction were not different between those with severely low cardiac output (n = 4, three deaths) and the others, whereas ventricular mass/volume ratio was significantly lower in the former group. Two late deaths (one DIRV, one mitral atresia) related to the AV valve regurgitation. These results may indicate a relatively poor outcome after the modified Fontan operation for patients with univentricular heart of right ventricular type as a result of basic anatomic and hemodynamic problems.

Production and secretion of immunoreactive growth hormone-releasing factor by pheochromocytomas.

The production and secretion of immunoreactive growth hormone-releasing factor (IR-GRF) by pheochromocytomas were examined immunohistochemically and immunochemically. GRF-immunoreactive (GRF-IR) cells were found, although sparsely, in 2 of 13 tumors (Cases 1 and 2), while somatostatin (SRIF)-IR cells and vasoactive intestinal peptide (VIP)-IR cells were found in nine and five tumors, respectively. Concentrations of tissue IR-GRF of 29.8 and 17.2 ng/g wet weight tissue, respectively, were found in two (Cases 1 and 2) of three tumors examined. These three tumors also contained IR-SRIF at 19.5-105.5 ng/g wet weight tissue and IR-VIP at 13.6-24.8 ng/g wet weight tissue. An increased plasma IR-GRF concentration (30.0 pg/ml) was found in a blood sample taken from the inferior vena cava near the adrenal tumor in Case 1. This is the first report that some pheochromocytomas produce GRF and secrete it into the blood circulation.

Immunoreactive somatostatin and calcitonin in pulmonary neuroendocrine tumor.

A well-differentiated neuroendocrine carcinoma of the lung that secreted immunoreactive somatostatin (IR-SRIF) and IR-calcitonin (CT) in a 72-year-old women is described. The plasma concentrations of IR-SRIF (57.5 pg/ml) and IR-CT (340 pg/ml) before operation were significantly higher than the respective normal ranges. After resection of the tumor, the plasma CT level (105 pg/ml) decreased to within the normal range, and the SRIF level (32.7 pg/ml) also decreased, but was still abnormally higher, which suggested the presence of an unidentified remnant of the tumor. Abnormal accumulation of technetium 99m (99mTc) in the lumbar vertebrae was found 6 months after the operation, which indicated a metastatic tumor. The tissue concentrations of IR-SRIF and IR-CT were 103 and 94 ng/g wet weight, respectively, and SRIF-IR tumor cells and CT-IR tumor cells were demonstrated immunohistochemically. On gel-filtration chromatography of the tumor tissue, two peaks of SRIF immunoreactivity were eluted in the positions of synthetic SRIF-28 and SRIF-14, respectively. Conversion of SRIF-28 to SRIF-14 was suggested from results on changes in the two IR-SRIF components during incubation with a crude enzyme preparation extracted from the tumor tissue.

Ultrastructural localization of S-100 protein in neurofibroma.

The nature of the cells in neurofibromas was studied by electron microscopy and immunoelectron-microscopic examination of S-100 protein. Ultrastructurally, all five neurofibromas studied were found to be composed of Schwann cells, perineurial cells, and intermediate cells, which had features of both perineurial cells and fibroblasts. The Schwann cells had complex, branched cytoplasmic processes and a continuous basal lamina. The perineurial cells were distinguishable from Schwann cells by the presence of numerous pinocytotic vesicles, unbranched slender cytoplasmic processes and a discontinuous basal lamina. The intermediate cells had no basal lamina, but were topographically related to Schwann cells and had a similar fine structure to that of perineurial cells. Thus, they seemed to be modified neoplastic perineurial cells. Immunoelectron-microscopic studies showed the presence of cells with and without S-100 protein in the neurofibromas: cells with S-100 protein resembled Schwann cells ultrastructurally, and those without S-100 protein were perineurial and intermediate cells. Some Schwann cells with S-100 protein in one neurofibroma had numerous pinocytotic vesicles characteristic of perineurial cells, suggesting that Schwann cells and perineurial cells are functional variants of the same cell type. Thus this study showed that neurofibromas were composed of Schwann cells with S-100 protein and perineurial and intermediate cells, including so-called endoneurial fibroblasts, without S-100 protein. Morphological and functional transition seems to occur between Schwann cells and perineurial cells, and between perineurial cells and intermediate cells.

A new method for extracting circular and supercoiled genome segments from cytoplasmic polyhedrosis virus.

The genome of a cytoplasmic polyhedrosis virus (CPV) consisting of 10 segments was extracted by a new two-step extraction method. The results from polyacrylamide gel electrophoresis, phenol and diepoxybutane treatment and electron microscopic examination indicated the extracted genome segments to have circular and supercoiled structures as genome-protein(s) complexes. Multiple cycles of the transcription of each segmented genome of CPV may take place on the circular structure of the segment.

On the mechanism of the chemical and enzymic oxygenations of alpha-oxyprotohemin IX to Fe.biliverdin IX alpha.

alpha-Oxyprotohemin IX, an early intermediate in heme catabolism, was synthesized and its autoxidation to biliverdin IX alpha was studied. In anaerobic aqueous pyridine, alpha-oxyprotohemin (hexacoordinated) underwent autoreduction to yield an Fe(II) alpha-oxyprotoporphyrin pi-neutral radical bis(pyridine) complex, which reacted with an equimolar amount of dioxygen to give pyridine.verdohemochrome IX alpha and CO in 75-80% yield via an intermediate with an absorption maximum at 893 nm. Verdohemochrome IX alpha did not react with further dioxygen. Reconstituted apomyoglobin.alpha-oxyprotohemin IX complex (pentacoordinated) reacted with an equimolar amount of dioxygen to form an Fe(II) oxyporphyrin pi-neutral radical intermediate, which rearranged to a green compound (lambda max 660 and 704 nm) with elision of CO. The green product, which is probably an apomyoglobin.verdoheme pi-radical complex, reacted with another equimolar amount of dioxygen to give Fe(III).biliverdin IX alpha. Demetallation of this gave biliverdin IX alpha in overall yield of 70-75%. These results indicate that the sequence of oxyheme autoxidation in the presence of apomyoglobin is alpha-oxyprotoheme IX O2----CO----verdohemochrome IX alpha pi-radical O2----Fe(III).biliverdin IX alpha. A similar mechanism may prevail in vivo. The hexa- and pentacoordinated Fe(II) pi-radical form of the oxyporphyrin is crucial in triggering the autoxidation of the complex to verdohemochrome IX alpha. Further oxygenation of verdohemochrome IX alpha to Fe(III).biliverdin IX alpha occurred only in the pentacoordinated apomyoglobin.verdoheme Fe(II) complex.

A case of small cell lung cancer successfully treated with VM26.

A 79-year-old man was admitted to our hospital for a close examination of an abnormal shadow on a routine chest roentgenogram. A large tumorous shadow in the right S1 segment, and hilar and mediastinal node swelling were observed. Transbronchial biopsy revealed that the histologic type of the tumor was oat cell carcinoma. The clinical stage was considered to be limited disease. Because of his age, 79 years, he was treated with VM26 as a single agent. VM26 was administered by intravenous drip infusion of 60 mg/m2 daily for five consecutive days every three to four weeks. After the third course of treatment with VM26, he achieved partial response, and then radiation therapy to the primary site and the hilar and mediastinal nodes was given at a total dose of 6,000 rad. There have been few phase II studies of VM26 in patients with small cell lung cancer (SCLC). Further phase II studies should be conducted for confirmation of the reproducibility of the phase II study of VM26 in SCLC, and the efficacy of combination chemotherapy including VM26 against SCLC should also be tested.

Phase II study of UFT in patients with advanced non-small cell lung cancer.

A phase II study of UFT (a mixture of uracil and tegafur; molar ratio of uracil to tegafur = 4) was undertaken in 21 patients with advanced non-small cell lung cancer (NSCLC). UFT was administered orally at a dose of 400 mg/m2 every day, for more than four weeks. Of 16 adequately treated patients, one (6.3%) showed a partial response. Toxic effects included minimal myelosuppression, anorexia, nausea, vomiting and epigastralgia. Gastrointestinal toxicity was well tolerated. Considering the poor response and mild toxicity, a further phase II study of higher-dose UFT is necessary for patients without prior therapy.

Phase II study of mitoxantrone in patients with non-small cell lung cancer.

A phase II study of mitoxantrone was performed in 24 patients with non-small cell lung cancer (NSCLC). Mitoxantrone was administered by intravenous drip infusion of 12 mg/m2 every three weeks. There were no responders among the 21 evaluable patients including five patients without prior therapy. The major hematological toxic effect was leukocytopenia. Thrombocytopenia and decrease in hemoglobin were slight. A change in the electrocardiogram was observed in one patient and one patient experienced cardiogenic shock. Mitoxantrone is not acceptable for the treatment of NSCLC because of its low antitumor activity, and careful observation is needed for administration of this agent to patients with pre-existing risk factors, such as prior anthracycline exposure, mediastinal radiation or underlying cardiovascular disease.

The protective effect of 2-mercapto-ethane sulfonate (MESNA) on hemorrhagic cystitis induced by high-dose ifosfamide treatment tested by a randomized crossover trial.

High-dose ifosfamide (one or two courses of 6 g/m2) with or without mesna was administered to 13 patients with advanced non-small cell lung cancer. The protective effect of 2-mercapto-ethane sulfonate (mesna) against the urotoxic side effects induced by ifosfamide was examined by a randomized crossover trial. A significant reduction in the incidence of hematuria was observed in the patients receiving mesna. Macroscopic hematuria was observed in only one patient who received treatment with mesna versus seven patients treated with ifosfamide alone. Other symptoms, such as frequency and dysuria, tended to be diminished in the patients receiving mesna, although the difference was not statistically significant. Our results suggest that mesna is effective in preventing or diminishing ifosfamide-induced hemorrhagic cystitis. Concomitant use of mesna should allow the administration of a high dose of ifosfamide although more extensive studies are needed to define the optimal dose and schedule of administration of mesna to prevent or attenuate the hemorrhagic cystitis.

A case report of a three-year survivor with advanced non-small cell lung cancer producing alpha-fetoprotein.

An 80-year-old patient with poorly differentiated adenocarcinoma of the left lung with metastasis to both lungs and supraclavicular lymph nodes, stage III M1 (T2N2M1), was treated with cisplatin (cis-diamminedichloroplatinum, CDDP) at a dose of 80 mg/m2 intravenously. He achieved a partial response, however, he could not continue therapy with CDDP because of its renal toxicity. He was then given etoposide and vindesine as a single chemotherapeutic agent, but no response was observed. Therefore, CDDP was administered again in a fractionated regimen, the first course of treatment achieved a minor response, but the second course resulted in stable disease. He therefore received combination chemotherapy consisting of mitomycin, vindesine and CDDP, and tumor regression of more than 30% was observed after one course of this combination chemotherapy. After three years from the initiation of chemotherapy, he had no symptoms except for hoarseness, and has been followed up on an ambulatory basis. In addition, the tumor was producing alpha-fetoprotein (AFP), which was shown by immunohistochemical staining with polyclonal antibody against AFP. The changes in serum AFP level correlated well with the disease status.

Ultrastructural study on an appendiceal carcinoid tumor showing intra-nerve fiber growth.

Ultrastructural and immunohistochemical studies on an appendiceal carcinoid tumor in a 53-year-old man disclosed an intimate association of nerves and tumor cells. Electron microscopy revealed that the tumor nests were composed of neurosecretory cells containing neurosecretory granules and peripherally-located Schwann cells encasing axons. Some tumor nests were continuous with nerve fibers invested in the common basal lamina. These findings suggest that at least some carcinoid tumors of the appendix show intra-nerve fiber growth, and that the neurosecretory cells might be present in the nerve fibers from the beginning or infiltrated into the nerve fibers at a very early stage of neoplastic transformation.
