[Hemodynamic effects of a large dose injection of OK-432 (picibanil)].

Intratumoral administration of a large dose of OK-432 has two antitumor functions; a direct cytotoxic effect and an indirect effect through the host reticuloendotherial functions. It has been pointed out that this method, however, occasionally results in shock. Several investigators have suggested the importance of the use of general anesthesia to avoid such shock. In order to explore the hemodynamic effect of intratumoral injection of OK-432, a simulated experiment (OK-432 or saline intramuscular injection) was carried out utilizing 10 anesthetized and 10 unanesthetized dogs. In addition, hemodynamic alterations during the OK-432 administration were studied in nine patients under neuroleptal anesthesia. In both anesthetized and unanesthetized dogs, OK-432 intramuscular injection resulted in a slight decrease of the mean arterial pressure, left ventricular systolic pressure and cardiac output as compared with those of the saline injected dogs. The hemodynamic alterations, however, were minimal and were thought not to cause severe hemodynamic derangements. Clinical experiences also showed no serious hemodynamic effects by the OK-432 intratumoral injections. It was concluded that the OK-432 intratumoral injection appeared not to induce hemodynamic derangement when applied to euvolemic and hemodynamically stable patients.

Therapeutic efficacy of cytotoxic combination chemotherapy in ovarian "mature" teratoma.

Unintentionally, a mature ovarian teratoma was treated by cytotoxic chemotherapy from an early stage. In spite of the normal appearance of the left ovary at the time of the first operation on the right ovarian immature teratoma, a cystic tumor was found in the left ovary after ten courses of postoperative multi-drug (vincristine, actinomycin D and cyclophosphamide) chemotherapy. This neoplasm was enucleated from the left ovary and was confirmed histologically to be a fully mature teratoma at the time of the second look operation, although no recurrent or implanted lesions of immature tissues were revealed at that time. From these findings, it was supposed that the cytotoxic regimen did not affect the development of the mature teratoma. When considering the therapeutic significance of cytotoxic chemotherapy in the management of teratoma, it is important to realize the intractable nature of the mature elements as a factor resistant to cytotoxic methods.

Radioimmunoassay of group II pepsinogen in human serum.

A new radioimmunoassay (RIA) using a double antibody method for human Group II pepsinogens in serum was developed. (1) Sensitivity of this assay system was of the order of 1 microgram per 1 of serum and optimal assay range was 10 to 50 microgram per 1; (2) no effect of interference of human serum was detected and there was no cross-reaction with human Group I pepsinogens within the optimal assay range; (3) satisfactory results were obtained for both within- and between-assay reproducibility (coefficient of variation was 3.9% and 3.7-15.3%, respectively); (4) the mean (+/- SEM) serum Pg II level in healthy donors was 15.9 +/- 0.7 microgram/l for males and 12.8 +/- 1.3 microgram/l for females; the difference between males and females was statistically significant (p less than 0.05); (5) the mean serum Pg II level in 10 patients with total gastrectomy was 1.2 +/- 0.05 microgram/l. These results suggest that most of Pg II in human serum is derived from the gastroduodenal system.

Potential peptic activity of pepsinogen of human gastroduodenal mucosa determined by fluorescent microassay method using succinyl albumin.

Pepsinogen level, expressed as the potential peptic activity of pepsinogen was determined by a fluorescent microassay using succinyl albumin as substrate, in biopsy specimens from the gastroduodenal mucosa of 95 subjects. The following results were obtained: (1) pepsinogen level in the gastric mucosa becomes progressively higher from pylorus to corpus (p less than 0.001); (2) pepsinogen level in the gastroduodenal mucosa of duodenal ulcer was significantly higher than that of normal mucosa or gastric ulcer (p less than 0.001); (3) the difference in pepsinogen level between the gastric mucosa with and without intestinal metaplasia was statistically significant (p less than 0.01); (4) the correlation between the histology of gastric glands and pepsinogen level was fundic gland greater than intermediate gland greater than pyloric gland (p less than 0.001).

Direct communication of the right pulmonary artery with the left atrium in an infant.

An 8-month-old Japanese girl with cyanosis and evidence of a round shadow connected with the right cardiac border on the chest X-ray film was diagnosed as a case of direct communication of the right pulmonary artery with the left atrium (RPA-LA communication) following cardiac catheterization and angiocardiography. She had a history of transient congestive heart failure in the early neonatal period. A secundum atrial septal defect and a two-lobed right lung were also present. The patient became asymptomatic after a successful ligation of the anomalous connecting vessel.

Nemaline myopathy rod bodies. Structure and composition.

Ca2+-activated protease (CAF) digestion of glycerinated nemaline myopathy muscle removed the electron-dense material covering rods and Z-lines and exposed longitudinal backbone filaments, 6-7 nm wide, which span the lengths of the original rods. Decoration of the exposed filaments (which are responsible for the periodicity parallel to the long axis of intact nemaline rods) with heavy meromyosin (HMM) proved they are actin filaments. After CAF treatment, cross-striated periodical patterns in longitudinal sections and Z-filament-like proteins connecting actin filaments seen in cross-section disappeared. This suggests that alpha-actinin may be involved in formation of this pattern because of the specificity of CAF toward alpha-actinin. Gel electrophoresis of CAF-treated nemaline muscle showed that most alpha-actinin is released into the supernatant, whereas the residue is mainly actin and myosin. Electron microscope examination of longitudinal sections of intact rods shows an oblique filament pattern, thin (7 nm) lines, thick (11 nm) lines, and an amorphous-appearance previously observed in normal Z-lines, patterns observed depend on sectioning angle and section thickness. In cross-section, rods show small square net (SS) and basket-weave (BW) forms. The SS form predominates and coexistence of the 2 forms, which also occur in normal Z-lines, is observed. Results support the idea that rods are lateral polymers of Z-line units. We think that the length of rods, as well as the width of Z-lines, is determined by the amount of overlap of actin filaments of opposite polarity. Initiation of rod formation may be due to deregulation of actin filament length.

Upper gastrointestinal mass survey with panendoscopy in a workplace.

We performed a mass survey using a thin forward-viewing gastroscope (Olympus GIF-P2) as a secondary examination of 449 persons from 3457 employees who had undergone the first step of the screening with photofluorography. Comparing these results with those obtained in the mass survey of 1976 and 1977, in which the lateral-viewing gastrocamera, GT-PA2, was employed for the second step of the screening in 372 of 3548 persons examined, we arrived at the following conclusions: (1) the thin forward-viewing gastroscope for mass survey permits accurate diagnosis not only of gastric lesions but also of bulbar lesions; (2) there was no significant difference between the two mass surveys in the rate of discovery of gastric lesions, but gastric ulcer scars were detected at higher rate when using the gastrocamera; and (3) few lesions were noted in cases with an incompletely filled bulb; gastric erosions, especially prepyloric, were noted with bulbar deformity.

Abortifacient effect and uterine cervix-dilating action of 16, 16-dimethyl trans delta 2 PGE1 methyl ester (ONO 802) in the form of a vaginal suppository (a randomized, double-blind, controlled study in the second trimester of pregnancy).

A newly synthesized 16, 16-dimethyl trans delta 2 PGE1 methyl ester (ONO 802) was clinically applied in the form of a vaginal suppository for therapeutic abortion of second trimester pregnancies. Its effects were studied in a double-blind test comparing it with an inactive placebo suppository. The study was conducted at 12 Japanese university hospitals. The number of patients was 125 in total, i.e. 63 receiving ONO 802 vaginal suppository (containing 1.0 mg ONO 802) and 62 receiving inactive placebo suppository. ONO 802 was more effective than placebo with a success rate of 87% (complete (71%) and incomplete (16%) abortions). The onset of uterine contractions was observed in 154.3+/-18.1 min. and the onset of uterine bleeding in 323.6+/-41.0 min. The expulsion of the fetus and placenta was observed in 955.4+/-97.0 and 961.6+/-97.0 min., respectively. The cervix dilating effect of ONO 802 was observed in 63.5% of the patients at 3 hours after the start of administration. Nausea, vomiting, abdominal cramps, diarrhea and pyrexia were noticed. However, all these side effects were transient and mild, requiring no treatment. No abnormality was observed in the puerperal course, duration of uterine bleeding or onset of subsequent menstruation following the therapeutic abortion. Therefore, the present study demonstrated that ONO 802 vaginal suppository was an effective and valuable drug for therapeutic abortion of second trimester pregnancies.

Significance of anti-deoxyribonuclease-B (ADN-B) determination in clinical practice.

The determination of anti-deoxyribonuclease-B (ADN-B) is very important for the diagnosis of antecedent streptococcal infection because almost all of group A streptococci have this antigen and a strong elevation of the antibody is observed in patients with acute rheumatic fever, acute glomerulonephritis and other streptococcal infections. Moreover, ADN-B titers are elevated in cases with streptococcal skin infection, whereas anti-streptolysin O (ASO) titers are rarely elevated in these cases. So the determination of ADN-B is recommended as the second streptococcal antibody test to ASO detection (manual of WHO). In our study, the upper limits of ADN-B in normal subjects were 1:60 in preschool age, 1:480 in school age and 1:340 in adult age groups. In acute rheumatic fever, ADN-B titers were elevated in 87% of the cases and also 87% of the patients with acute glomerulonephritis had high ADN-B titers. In inactive rheumatic fever, 72% of the patients had high ADN-B titers, while, ASO titers were elevated in only 22% of the cases. In streptococcal carrier states, 39% of the children had high ADN-B titers but ADN-B was positive only 8% in non-carrier children.
