Sixteen cases with ovarian immature teratoma received our managements have been reviewed. After exhaustive tissue examination, those neoplasms were graded histologically according to the criteria proposed by Thurlbeck and Scully, in addition, to exclude the mixed germ cell tumors, alfa fetoprotein stainings by PAP method were performed. Ten of all patients are alive without any signs of recurrence and none of grade 3 patients could survive. In our small series, the histologic grade seemed to correlate with the prognosis. Now-a-days the prognostic significance of the grading systems, however, still remains controversial, and it is discussed in this paper. Of particular interest is that the immature teratoma containing glioblastoma element took most fulminant clinical course. It was suggested that special attention should be paid to the histologic components of glioblastoma in the neoplasm. Serum alfa feto-protein level, elevated in two cases, was not associated with the clinical situation and AFP value appears to have questionable diagnostic importance as the tumor marker. Although we had no standard chemotherapeutic programs for immature teratomas, it is believed that the more aggressive surgery followed by good combination chemotherapy brings better prognosis.
The prophylactic and chemotherapeutic efficacy of PMPC against infections after TUR-P has been investigated. Bacteriological evaluation: PMPC , 200-300 mg/day for 2-12 weeks, was administered to 49 patients, who had over 10(3)CFU/ml of microorganisms after CET or CEC treatment for 3-7 days. The eradication rate of microorganisms was 40.8% after 2 weeks, 52.2% after 4 weeks, 64.1% after 6 weeks, 65.0% after 8 weeks and 70.6% after 12 weeks. Effectiveness on pyuria : The improvement rate of pyuria against 59 patients who had over 10(5)/hpf of pyuria , was 15.3% after 2 weeks, 16.4% after 4 weeks, 25.4% after 6 weeks, 58.5% after 8 weeks, 72.7% after 10 weeks and 75.0% after 12 weeks. Overall clinical efficacy on PMPC was examined in 26 patients. The results of efficacy were 27.3% after 2 weeks, 48.0% after 4 weeks, 50.0% after 6 weeks, 69.2% after 8 weeks, 75.0% after 10 weeks and 77.0% after 12 weeks. The clinical response was evaluated according to a criterion for clinical evaluation of antimicrobial agent on chronic complicated UTI proposed by UTI committee in Japan. No severe adverse effect including allergic reaction was found. Following administration of PMPC , three patients experienced adverse gastric reactions, and drug administration was discontinued at week 6 or 8. PMPC was effective as a prophylactic chemotherapeutic drug against infections after TUR-P and prostatectomy.
The sequence of the appearance of fibronectin in the rabbit cornea after thermal burn was investigated by indirect immunofluorescent microscopy. One day after thermal burn, the keratocytes disappeared completely from the burned area, and faint fibronectin-specific fluorescence was detected beneath the epithelium and on the stromal side of the Descemet's membrane. On day 3, activated keratocytes began to migrate to the burned lesion. Fibronectin was detected in the burned area as well as in the surrounding uninjured area, and on both sides of the Descemet's membrane. One week after the burn, the keratocytes had increased in the lesion and, concomitantly, fibronectin-specific fluorescence could be observed at the locations corresponding to the keratocytes. Fibronectin in the surrounding area gradually diminished. After 4 weeks, activated keratocytes had accumulated in the lesion, and fibronectin was detected only in the central lesion and at the level of the Descemet's membrane. Thus the appearance of fibronectin was concordant with the appearance of the keratocytes, and this suggests that fibronectin may be involved in the wound-healing of thermally burned rabbit corneas.
Unintentionally, a mature ovarian teratoma was treated by cytotoxic chemotherapy from an early stage. In spite of the normal appearance of the left ovary at the time of the first operation on the right ovarian immature teratoma, a cystic tumor was found in the left ovary after ten courses of postoperative multi-drug (vincristine, actinomycin D and cyclophosphamide) chemotherapy. This neoplasm was enucleated from the left ovary and was confirmed histologically to be a fully mature teratoma at the time of the second look operation, although no recurrent or implanted lesions of immature tissues were revealed at that time. From these findings, it was supposed that the cytotoxic regimen did not affect the development of the mature teratoma. When considering the therapeutic significance of cytotoxic chemotherapy in the management of teratoma, it is important to realize the intractable nature of the mature elements as a factor resistant to cytotoxic methods.

In a study by the Radiation Effects Research Foundation, Hiroshima and Nagasaki, of parathyroid tumors in autopsy cases, 18 cases of parathyroid adenoma were detected among 6, 102 cases that were autopsied between 1961-77. Two of these were giant adenomas 5 cm in diameter, complicated with hyperthyroidism. Both were atomic bomb survivors of Hiroshima. One was exposed to 55 rad at the age of 51 and died at age 71. The other was exposed to 28 rad at age 44 and died at age 71. These two cases are reported with a review of the literature on parathyroid tumors that developed following irradiation of the head and neck.
A non-radioactive tracer, deuterium, incorporated in chenodeoxycholic acid, was used to study bile acid absorption kinetics employing gas chromatography-mass fragmentography for its analysis. Eight patients with Crohn's disease and four control volunteers were administered chenodeoxycholic-11, 12-d2 acid. In order to obtain constant dietary conditions, they were fed by an elemental diet during the study. There were significant reductions of the biological half-life of chenodeoxycholic-11, 12-d2 acid, the pool size of chenodeoxycholic acid, and the total bile acid pool size in patients with Crohn's disease as compared with those in normal subjects. There was a marked increase in the relative concentration of ursodeoxycholic acid with a concomitant decrease of deoxycholic acid in patients with Crohn's disease, although such differences did not reach statistical significance. The present study confirms that there are some impairments of enterohepatic circulation of bile acids in patients with Crohn's disease.

We examined the effects of cyclic AMP (cAMP) on the growth and differentiation of RAO 188 cells, a cultured cell line derived from a retinoblastoma-like tumor induced in an inbred rat by intravitreous inoculation with human adenovirus serotype 12. After adding cAMP analogs (dibutyryl cAMP and 8-bromo cAMP) and phosphodiesterase inhibitors (theophylline, amino-phylline, and 1-methyl-3-isobutyl xanthine) to the RAO 188 cell culture medium, we measured changes in cell incorporation of the DNA and RNA precursors 14C-thymidine and 3H-uridine, and we observed the morphologic alterations of RAO 188 by phase-contrast and transmission and scanning electron microscopy. Incorporation of the labeled precursors decreased with increased concentrations of cAMP analogs and phosphodiesterase inhibitors. Incorporation of the labeled precursors was inhibited shortly after the addition of dibutyryl cAMP to the culture medium. The effect was maximal at 8 hr and was sustained for up to 48 hr. Reversibility of cAMP effects on incorporation gradually decreased for 10 days; at 10 days these effects were essentially irreversible. Neuritelike processes developed shortly after cAMP analog treatment and formed a network after 24 hr. Transmission electron microscopy disclosed changes in the cell membrane and cytoplasm of cells treated with 8-bromo cAMP and theophylline: perturbation of the cell membrane and the appearance of intercellular junctional devices and microfilaments. The activity of glutamate decarboxylase, which is involved in the biosynthesis of gamma-aminobutyric acid, was increased in treated cells. These results show that cAMP decreases DNA and RNA synthesis and cell proliferation and facilitates morphologic and biochemical differentiation of RAO 188 cells.

Primary non-teratomatous ovarian squamous cell carcinomas are very rare. A case of rat ovarian squamous cell carcinoma has been found in which the tumor arose from a squamous element in the ovary. Squamous epithelial aggregates were found in the wall of a cyst and on serial section, the cyst was found to be unaccompanied by any teratomatous components and showed a thin squamous epithelial lining with keratinizing debris. On the basis of these findings, it was suggested that the squamous element was a rat ovarian epidermoid cyst from which a squamous cell carcinoma arose, presumably by a direct carcinogenic effect of 7,12-dimethylbenz[a]anthracene.
