[Clinicopathological study of malignant ovarian germ cell tumor].

In treating malignant germ cell tumors clinicopathologically at Kurume University Hospital, for 18 years, the total number of patients was 112, including 25 yolk sac tumors (YST), 10 mixed form germ cell tumors (MF), 36 dysgerminomas (DYS), 26 immature teratomas (IT), 2 choriocarcinomas (CHO) and 13 dermoid cysts with malignant transformation (DCMT). The average age of patients in each group was 25.9 years for DCMT patients. When DCMT cases were excluded, ages was closely related to the age of menarche in each patient. The 5 year survival rate for each germ cell malignancy after the first operation was 31.6% for YST cases, 20.0% for MF, 76.7% for DYS, 60.0% for IT and 50.0% for DCMT cases. Despite currently used aggressive kinds of chemotherapy, the clinical stage still had a prognostic significance in all malignant germ cell tumors except dysgerminoma. Since histological maturation of the tumor tissue following treatment courses was observed in YST and IT cases, the possibility of a relationship between prognosis and the histogenetic process was shortly discussed.

Tumorigenicity study of sodium erythorbate administered orally to mice.

Sodium erythorbate (SE) was administered at concentrations of 0, 1.25, or 2.5% (maximum tolerated dose, MTD) in the drinking-water to groups of 50 male B6C3F1 mice respectively. Female groups, each consisting of 50 mice, received SE in the drinking-water at concentrations of 0, 2.5 or 5% (MTD). Treatment continued for 96 wks and the experiment was terminated during wk 110. Tumors were observed at various sites including the liver, hematopoietic system, lung and soft tissue. However, at any of the sites, the tumor incidence, the time to death with tumors or the histological distribution of tumors did not differ significantly from those in the untreated control group. Thus, the present study did not demonstrate a tumorigenic effect of SE on B6C3F1 mice by means of oral administration.

Growth and differentiation of a human megakaryoblastic cell line, CMK.

Recently, a human megakaryoblastic cell line, CMK, was established from the peripheral blood of a megakaryoblastic leukemia patient with Down syndrome. Using this cell line, we studied the proliferation and differentiation of megakaryocytic cells in the presence of highly purified human hematopoietic factors and phorbol 12-myristate-13-acetate (PMA). In a methylcellulose culture system, interleukin-3 (IL-3) and granulocyte-macrophage colony-stimulating factor (GM-CSF) facilitated colony formation by CMK cells in a dose-dependent manner. The maximum stimulating doses of these factors were 10 and 200 U/mL, respectively. These concentrations were comparable to those that stimulate activity in normal hematopoietic cells. In contrast, granulocyte-colony stimulating factor (G-CSF), macrophage-colony stimulating factor (M-CSF), and erythropoietin (EPO) had no effects on the colony formation of CMK cells. In a liquid culture system, 20% of the CMK cells expressed glycoprotein IIb/IIIa (GPIIb/IIIa) antigen without hematopoietic factors, whereas 40% of the cells expressed GPIIb/IIIa with the addition of IL-3 and GM-CSF. EPO also slightly enhanced expression of GPIIb/IIIa. On the other hand, PMA inhibited growth of CMK cells and induced most of them to express the GPIIb/IIIa antigen. Furthermore, PMA induced CMK cells to produce growth activity toward new inocula of CMK cells. This growth factor (GF) contained colony-stimulating activity (CSA) in normal bone marrow (BM) cells. The activity was believed to be attributable mainly to GM-CSF, since 64% of this activity was neutralized by anti-GM-CSF antibodies and a transcript of GM-CSF was detected in mRNA from PMA-treated CMK cells by Northern blot analysis. These observations suggest that GM-CSF, as well as IL-3, should play an important role in megakaryocytopoiesis.

Three-dimensional structure of some Sertoli cell organelles in the cotton rat and shiba goat.

The three-dimensional structure of some Sertoli cell organelles in the cotton rat and shiba goat was observed under a scanning electron microscope using the Osmium-Dimethyl sulfoxide-Osmium method. Mitochondria of the Sertoli trunk in the scanning electron microscopic (SEM) observation appeared much longer and more densely distributed than those in the transmission electron microscopic (TEM) observation and had a different shape from oval mitochondria in the basal region of the Sertoli cell. These rod-shaped mitochondria, 2-3 microns in length, accumulated closely among the ER network. Smooth ER in the Sertoli trunk was composed of continuous small bulges like strings of beads and were connected with each other to form a net-arranged network. Although microtubules which were seen in large numbers in the TEM observation were not preserved in the sample of the present study, a quantity of intermediate filaments was recognized three-dimensionally. These intermediate filaments seemed more crowded in the SEM observation than in the TEM observation. They frequently ramified and formed a meshwork. The accurate configurations of some Sertoli cell organelles, especially the smooth ER network, were obtained from the present study. Each intracellular organelle appeared more densely in the SEM observation than in the TEM observation.

[Results of abdominal surgery in patients over 90 years old].

During a year period (1978-1987), 105 cases of abdominal surgery were performed at Sasebo Chuo Hospital on patients over 80 years of age. This study discusses 4 patients who were over 90 years old at time of their surgery. Emergency surgery had been required for 2 patients (a 91-year-old male with a sigmoid colon ca., and a 91-year-old female with a descending colon ca.), and elective surgery had been performed on the other two patients (a 95-year-old male with a caecum ca., and a 97-year-old female with a leiomyosarcoma of stomach). The patients who underwent elective surgery had a uniformly smoother hospital course and an overall better prognosis then the patients who underwent emergency surgery, whose prognosis was poor. We have concluded that early diagnosis and elective surgery is the necessary procedure in the elderly patient.

[Fecal fat excretion rate in patients with Crohn's disease--comparison of a diet containing 50 g/day of fat with elemental diet with an addition of 50 g/day butter fat].

Fecal fat excretion rate was determined by the method of Van de Kamer in 31 patients with Crohn's disease and six healthy controls, when fed by the diet containing 50 g/day of fat. Patients with jejuno-ileitis and those with partial and extensive resection showed a significantly greater excretion rate compared with patients with ileitis and healthy controls, however, there was no significant difference between patients with ileitis and healthy controls. In this study, the fecal excretion rate was less than 10 g/day in non-operated cases. Our results seems to be smaller than those in previous studies from the western countries. Fecal fat excretion rate was determined in 12 patients with non-operated Crohn's disease and six healthy controls, when fed by the additional 50 g/day butter fat during the elemental diet. With ingestion of the additional 50 g/day butter fat during elemental diet, fecal fat excretion rate was significantly increased in the patients with non-operated Crohn's disease than the results of the diet containing 50 g/day fat, but not significantly changed in the healthy controls. This result suggest that determination of fecal fat excretion rate with fed by the additional 50 g/day butter fat during elemental diet is a useful test for detecting a mild fat malabsorption in non-operated Crohn's disease.

Differential regulation of fibronectin synthesis in three different types of corneal cells.

The production of fibronectin (FN) and its response to serum or epidermal growth factor (EGF) were investigated in three different types of rabbit corneal cells cultured in vitro. The corneal epithelial cells, stromal fibroblasts (keratocytes) and endothelial cells were separately cultured in different media: basic medium containing minimal serum (0.5%), basic medium with supplementary serum at a final concentration of 10% and basic medium with 100 ng/ml EGF, respectively. FN production by each type of cell was examined either by the immunofluorescent staining method or by the metabolic labeling method followed by immunoprecipitation of FN in the culture medium. Each type of corneal cell produced and secreted FN. FN secretion into the culture medium by keratocytes and by endothelial cells was enhanced by the addition of EGF. However, FN secretion by epithelial cells was lowered by the additional serum or EGF. Furthermore, when the epithelial cells were cultured in the basic medium, DNA synthesis was low but FN secretion was high. These results suggest that the control mechanism of FN production differs between epithelial cells and keratocytes or endothelial cells.

[Surgical experience with tracheoplasty].

We have had experience with 31 tracheoplastic procedures. Underlying diseases consisted of primary tumor, cancer invasion from carcinoma of the thyroid gland and the esophagus, traumatic rupture and post-tracheostomy and tuberculous stenoses. During operation, respiratory support has been satisfactorily established by using the intubation method via the operative field and/or high frequency ventilation instrument. Furthermore, development of absorbable suture material like Dexon and Vicryl benefited by minimizing granulation stenosis at anastomosis due to less foreign body reaction. The prognosis for carcinomatous involvement from surrounding organs was much worse according to advanced cancer stages. However, surgical outcome of this procedure was satisfactory in terms of relieving respiratory distress.

The transcription of the interleukin 1 beta gene is induced with PMA and inhibited with dexamethasone in U937 cells.

Interleukin 1 beta mRNA was induced with phorbol ester, not LPS, in the human histiocytic lymphoma cell line U937, but interleukin 1 alpha mRNA was not induced. Nuclear run-on analysis showed that phorbol ester induced the transcription of interleukin 1 beta but did not induce it in the presence of cycloheximide. This indicates that the induction of the transcription with PMA requires de novo protein synthesis. Dexamethasone, an immunosuppressive and anti-inflammatory agent, decreased the level of interleukin 1 beta mRNA induced with phorbol ester. The transcriptional analysis showed that dexamethasone inhibits the transcription of the interleukin 1 beta gene.

Dexamethasone inhibition of phagocytosis by corneal keratocytes in culture.

To investigate the effect of steroid hormone on phagocytosis by keratocytes, we investigated the phagocytic activities for latex beads by rabbit corneal keratocytes cultured in the presence and absence of dexamethasone. Culturing the corneal keratocytes with dexamethasone inhibited the phagocytic activity for latex beads in a dose-dependent fashion. The inhibitory effects depended on the duration of the incubation period of the cells with dexamethasone.

Interactions of extracellular collagen and corneal fibroblasts: morphologic and biochemical changes of rabbit corneal cells cultured in a collagen matrix.

Corneal fibroblasts, also known as keratocytes are surrounded by an extracellular matrix of collagen in vivo. To understand the physiology and pathology of these corneal fibroblasts, it is important to study their interactions with this extracellular matrix. We cultured rabbit corneal fibroblasts on tissue culture plastic dishes or in a hydrated collagen gel and compared the changes in morphology and mitotic activity. Corneal fibroblasts on plastic dishes were flattened and widely spread, whereas those in collagen gel became spindle-shaped with long processes. Examination with an electron microscope revealed that the corneal fibroblasts in collagen gel formed gap junctions with neighboring cells. Gap junctions were hardly ever observed between corneal fibroblasts cultured on plastic dishes. Corneal fibroblasts cultured in a collagen matrix showed much less incorporation of [3H]thymidine than did corneal fibroblasts cultured on plastic, and this incorporation decreased with increasing concentration of collagen. Our present results suggest that the morphologic and biochemical characteristics of corneal fibroblasts cultured in collagen gel are different from those cultured on plastic.

Childhood myelodysplastic syndromes with 11p15 translocation.

Two cases of childhood myelodysplastic syndrome with chromosome abnormalities involving band 11p15 are described. The first case, with inv(11)(p15q23), had a complex clinical course; the initial diagnosis was aplastic anemia, then refractory anemia with excess of blasts in transformation (RAEB-t), and finally, before death, chronic myelomonocytic leukemia with hematologic features similar to those of chronic myelogenous leukemia (CML). The second case, with t(4;11)(p13;p15), progressed from RAEB to acute myelogenous leukemia (M2). In the literature, we found 12 patients with nonlymphocytic leukemia and chromosome abnormalities involving band 11p15, including seven cases with t(7;11)(p13-p15;p15); four cases (including the present case 1) showed CML-like hematologic features. It is suggested that translocations involving 11p15 are a nonrandom chromosome abnormality in nonlymphocytic leukemia.

Hepatic accumulation of pyrophosphate during acetate metabolism.

Accumulation of pyrophosphate induced by acetate administration was investigated in rat liver in situ and in perfused rat liver. Intraperitoneal injection of acetate into rats increased the pyrophosphate concentration in the liver to about 2 mumol/g liver, which was 200 times that in control liver. Perfusion of liver with acetate alone did not result in accumulation of pyrophosphate. However, the further addition of a Ca2+-mobilizing hormone, such as noradrenaline or angiotensin II, together with glucagon to the perfusion medium containing 1 mM acetate caused accumulation of pyrophosphate to a similar level to that observed in vivo. Acetate, glucagon and a Ca2+-mobilizing hormone were all required for accumulation of pyrophosphate in perfused liver. Omission of Ca2+ from the perfusion medium or addition of a Ca2+-antagonist reduced the accumulation significantly. The two kinds of hormones, glucagon and an alpha-agonist, either singly or in combination, did not affect the rate of acetate utilization. These results show that liver cells accumulate a large amount of pyrophosphate during acetate metabolism at high intracellular levels of Ca2+ that can be realized by the synergistic actions of the two kinds of hormones.

Cloning of the fic-1 gene involved in cell filamentation induced by cyclic AMP and construction of a delta fic Escherichia coli strain.

PA3092 is an Escherichia coli mutant that forms filaments at 43 degrees C in the presence of cyclic AMP (cAMP). The mutation responsible for this phenotype is called fic-1. We cloned fic-1 from PA3092 by selection for the neighboring argD gene. The fic-1 gene product had a relative molecular mass of 21 kilodaltons by the maxicell method. A strain with the fic gene completely deleted was constructed by replacing fic with a kanamycin resistance gene. In one of the fic-deleted strains derived from PA3092, cAMP did not induce cell filamentation at 43 degrees C, but it did in the same strain harboring a plasmid containing the fic-1 gene. These results indicate that the fic-1 gene product is necessary for the induction of cell filamentation by cAMP but is dispensable to the cell. We also found that high levels of NaCl suppressed the cell filamentation induced by cAMP.

Use of the 'H' reflex recovery curve in selective posterior rhizotomy.

Selective posterior rhizotomy is a safe and effective treatment in carefully selected patients. It produces objective and lasting changes in muscle tone and function without serious sensory complications. The 'H' reflex recovery curve technique allows objective evaluation of sensory rootlets and a physiologic rationale for the selection of abnormal rootlets. In 95 patients this technique has provided good results without significant sensory compromise.
