Detection of amino acids as possible promoters of bladder cancer in rats by measuring their enhancement of agglutination of bladder cells by concanavalin A.

The effects of amino acids on the enhanced agglutinability of bladder cells with concanavalin A induced by subcarcinogenic treatment with N-butyl-N-(4-hydroxybutyl)nitrosamine were examined. The amino acids examined were L-alanine, L-arginine, L-asparagine, L-aspartic acid, L-cysteine, L-glutamic acid, L-glutamine, L-glycine, DL- and L-histidine, L-hydroxyproline, L-isoleucine, D- and L-leucine, L-lysine, L-methionine, DL- and L-phenylalanine, L-proline, L-serine, L-threonine, DL-, D- and L-tryptophan, L-tyrosine and D- and L-valine. They were added to powdered diet at a concentration of 2.0%. L-Leucine, L-isoleucine, L-valine, DL- and D-tryptophan prolonged the period during which the bladder cells showed enhanced agglutinability with concanavalin A. Leupeptin, a protease inhibitor, and L-leucyl-L-leucine were also examined at a concentration of 0.1% because of their similar chemical structures, and were found to have the same effect. The tumor-promoting effects of DL-tryptophan and leupeptin have already been established by in vivo carcinogenesis experiments. The effects of L-leucine, L-isoleucine, L-valine, D-tryptophan and L-leucyl-L-leucine, detected by this short term assay, suggest that these compounds may also be promoters of bladder cancer in rats.

[Clinical effect of long-term administration of PSK in a combination in patients with malignant urologic cancer].

Long-term administration of PSK was made in 40 patients with urological cancer. Mean duration of the administration was 201, 358 and 243 days for the patients with bladder cancer, renal cancer and other urological cancers, respectively. The clinical evaluations were performed by the standard of Japan Society for Cancer Therapy and historical control study. In some cases, it seemed that the immunological therapy by PSK was a benefit to some degree. However, more accurate evaluation should be made by the matched control study.

The effect of estramustine phosphate on prostatic cancer estimated by transrectal ultrasonotomography.

Transrectal ultrasonotomography was performed in 44 patients with prostatic cancer before, during, and after estramustine phosphate (Estracyt) administration. In 75.7% of 37 previously untreated patients, the deformity of the horizontal section of the prostate with prostatic cancer was corrected considerably, while in 89.2%, prostatic weight was remarkably reduced. In 57.1% of seven previously treated patients, appreciable changes were also observed in the shape and weight of the prostate. We concluded that estramustine phosphate was effective not only for untreated prostatic cancer, but also, at least in some degree, for relapsed cases.

Transurethral ultrasonotomography of the prostate.

Newly developed transurethral ultrasonic scanners were used to study 20 patients with histologically proven prostatic cancer. Fifty-two patients with or without other prostatic disease were also evaluated. The characteristic findings of prostatic cancer were asymmetry of the shape (85%), ill-defined contour (75%), protruded areas from the boundary (70%), elongation of anteroposterior diameter (65%), and enlargement of the prostate (65%). In addition, transurethral ultrasonography delineated a clearer contour of the prostate compared with the transrectal procedure.

Chemotherapy of advanced prostatic cancer with peplomycin.

Peplomycin, a new anticancer agent, was produced as a side-chain derivative of bleomycin to reduce the incidence of pulmonary complications. Intramuscular injections of 5-10 mg of peplomycin were given 3 times a week to 35 patients with stages C and D disease. Patients were evaluated at the end of 12 weeks using the National Prostatic Cancer Project criteria. A response rate of 26% was achieved when peplomycin was used as the primary treatment. Prostatic cancer patients resistant to standard endocrine therapy had a 17% response rate to peplomycin therapy. Bone metastases were not influenced by this agent. The main side effects of peplomycin are fever and respiratory disturbances, but the incidence of these conditions is lower than that experience with bleomycin.

Suppression of spermatogenesis in patients with Behçet's disease treated with cyclophosphamide and colchicine.

Reproductive function was studied in 31 adult male patients with Behçet's disease during treatment with cyclophosphamide and/or colchicine for 1 to 64 months. Semen was obtained from 27 patients. Azoospermia or severe oligospermia was found in 13 of 17 patients receiving cyclophosphamide with or without colchicine, whereas the sperm count was almost normal in six patients treated with colchicine alone and in four patients receiving neither drug (control patients). Blood samples were available for 31 patients. The mean follicle-stimulating hormone level among 12 cyclophosphamide-treated patients was significantly higher than that of 6 colchicine-treated patients and 6 control patients. These results indicate that cyclophosphamide impairs spermatogenesis in adult men, whereas colchicine does not.

Transurethral real-time scanner.

Transurethral intravesical scanning is useful in staging bladder tumors. A new ultrasonic scanner was developed to obtain a real-time image. Diameter of the probe sheath was reduced to 24F. The resolution of the tomogram was improved by using 7 MHz, ultrasound, which clearly displayed the ureteral penetration of the bladder wall. It was demonstrated that the real-time scanner was effective not only in staging bladder cancer but also in the followup of tumor volume, in monitoring transurethral resection of the tumor and in the observation of tumor movement effected by the finger.

Inhibition by sugars of opsonization of Staphylococcus aureus in fetal calf serum.

Phagocytosis of Staphylococcus aureus by human peripheral polymorphonuclear leucocytes in vitro in the presence of fetal calf serum was inhibited by D-galactose, N-acetyl-D-glucosamine and asialofetuin but not by fetuin. Absorption of opsonin in the serum with the bacteria, inhibition of the absorption by sugars and the effect of glycosidase treatment of the bacteria on phagocytosis showed that terminal structures of polysaccharides on the surface of bacteria play an important role in binding of opsonin to the target.

Maintenance by saccharin of membrane alterations of rat bladder cells induced by subcarcinogenic treatment with bladder carcinogens.

Saccharin is known to have a tumor-promoting effect on bladder cancer in rats, but its mechanism of action is unknown. We demonstrated that the increased agglutinability of isolated epithelial cells of the bladder in the presence of concanavalin A caused by a subcarcinogenic dose of bladder carcinogens disappeared shortly after the end of their administration. However, saccharin maintained the increased agglutinability when given continuously after administration of carcinogen. Moreover, the agglutinability of bladder cells previously exposed to a subcarcinogenic dose of bladder carcinogens increased again when saccharin was given after the agglutinability had disappeared completely.

Restoration of E-rosette formation by bestatin in patients with bladder cancer.

Incubation of peripheral blood lymphocytes from patients with bladder cancer in bestatin (specific inhibitor of aminopeptidase B and leucine aminopeptidase) solution resulted in a significant increase in the proportion of E-rosette-forming lymphocytes. The increase in the proportion of E-rosette-forming lymphocytes was abolished after reincubation with 100% autochthonous serum. The fraction of E-rosette-forming lymphocytes from patients with bladder cancer which had restored receptors for sheep erythrocytes (SRBC) by incubation with bestatin also expressed receptors for the Fc portion of IgG and IgM. The proportion of EAC-forming lymphocytes remained unchanged after the incubation with bestatin. When 600 mg of bestatin was administered for 1 day to patients with bladder cancer, restorative effect on the proportion of E-rosette forming lymphocytes was also demonstrated. The proportion of EAC-forming lymphocytes, however, did not change significantly.

Staging of bladder cancer by ultrasonography: a new technique by transurethral intravesical scanning.

A new ultrasonic scanning device has been developed for preoperative staging of bladder cancer. The device uses a transurethrally intubated probe with a head that can be angled 90 degrees against its long axis and intravesical scanning. The propagation of the ultrasound is ideal and the tomograms provide good resolution. The ultrasonic images in each stage of bladder cancer were demonstrated. Comparison of ultrasonic and pathological stages showed concordance in 19 of 20 tumors removed by total or partial cystectomy. One tumor diagnosed by ultrasonography as T3 was identified pathologically as T1. Calcification on the tumor surface obstructed ultrasonic propagation, resulting in overstaging. Transurethral intravesical scanning is valuable clinically because of its accuracy in staging bladder cancer.

Surface markers of lymphocytes infiltrating seminoma tissue.

Infiltrating lymphocytes were separated from human seminoma tissue obtained surgically. The relative distributions of B and T lymphocytes, and a subset of T lymphocytes with IgG-Fc receptors were measured by the rosette-forming method in peripheral blood and in seminoma tissue from 7 patients with pure seminoma of the testis. The proportions of T and TG cells were increased in seminoma tissue compared to those of peripheral blood from the same patients, whereas the proportion of B cells did not change. This result might correlate with the favorable prognosis of seminoma usually accompanied with lymphocytic infiltration.
