Parathyroid adenoma found by vertebral vein sampling.

We report on a 33-year-old man with hypercalcemia and recurrent nephrolithiasis, who underwent the first neck exploration for primary hyperparathyroidism on September 27, 1973. No tumor was found and bilateral upper normal parathyroid glands were removed. However, hypercalcemia persisted postoperatively. Selective venous sampling for radioimmunoassay of parathyroid hormone 5 years later revealed a sharp unilateral gradient in the right vertebral vein. On November 2, 1978 the second neck exploration was performed and a right lower parathyroid adenoma was removed from the right lateral wall of the esophagus. Postoperatively, the serum calcium level decreased to 8.8 to 9.3 mg./dl. and convalescence was uneventful. In this case the pathway of parathyroid venous effluent might have been changed by the previous neck exploration and probably drained anomalously through the vertrbral vein. Selective venous sampling for radioimmunoassay of parathyroid hormone should include samplings from the vertebral veins as well as the thyroid veins, especially in cases with 1 or more previous neck explorations.

Dissociation of plasma renin activity and plasma aldosterone level during dexamethasone suppression test in non-salt-losers with congenital adrenal hyperplasia due to 21-hydroxylase deficiency.

We have studied plasma renin activity, the plasma aldosterone level and urinary metabolites of glucocorticoid precursors before and during a dexamethasone suppression test in three non-salt-losers with congenital adrenal hyperplasia due to 21-hydroxylase deficiency, who had never been treated with glucocorticoid. Plasma renin activity, the plasma aldosterone level and urinary pregnanetriol excretion were found to be abnormally elevated before dexamethasone administration. After 7 days' dexamethasone administration, plasma renin activity still remained high above the normal level, while the plasma aldosterone level as well as urinary 17KS and pregnanetriol excretion were lowered to the normal ranges. Several possible mechanisms for this discordant suppression of plasma renin activity and aldosterone level were discussed and the presence of mineralocorticoid resistance, which is not related to ACTH dependent aldosterone antagonists, was suggested in the case of these patients.

Effect of pepleomycin in prostatic cancer. A preliminary report.

Six patients with prostatic carcinoma entered this clinical trial using Pepleomycin, an analogue of Bleomycin, administered intramuscularly in a total dose of 120 mg. Two patients had an objective improvement lasting for three months. Also subjective improvement was observed in one patient. The main side effects were gastrointestinal disturbances, dermatologic symptoms and fever. However, pulmonary fibrosis, a well-known complication of Bleomycin therapy, could not be found.

Incidence of prostatic cancer in Japan and Asia.

The incidence of prostatic cancer has been reported to be low in Japan and Asia, compared with the USA and European countries. However, it is now on the increase in Japan. The possible etiological factors for the origin and development of prostatic cancer were examined on 187 patients with this malignancy who visited the Department of Urology, University of Tokyo during the past 16 years. No evidence that these factors were specifically related to prostatic cancer, was found. The possible etiology and differences in incidence between Japan, the USA, and European countries are discussed.

Increased agglutinability of bladder cells by concanavalin A after administration of carcinogens.

The agglutination by concanavalin A of isolated epithelial cells of the rat bladder was examined after in vivo treatment of rats with various bladder carcinogens for one week. The carcinogens tested were N-butyl-N-(4-hydroxybutyl)nitrosamine, dibutylnitrosamine, N-[4-(5-nitro-2-furyl)-2-thiazolyl]formamide, 2-acetylaminofluorene, 2-napthylamine, benzidine, N-methyl-N-nitrosourea, and cyclophosphamide, and they were given to male Wistar rats p.o., s.c., intravesically, or i.p. As negative controls, the effects of administration of 2-(2-furyl)-3-(5-nitro-2-furyl)acrylamide, dimethylnitrosamine, N-methyl-N'-nitro-N-nitrosoguanidine, and surgical implantation of glass beads in the bladder were also tested. One week after the start of treatment, epithelial cells were isolated from the bladder by sonication, and agglutination of the isolated cells with concanavalin A was assayed. The observed agglutinabilities of isolated cells were found to be closely correlated with the reported bladder carcinogenicities of these chemicals in rats. Thus, concanavalin A agglutination of bladder cells should be a useful rapid in vivo mammalian system for screening bladder carcinogens.

Restoration by levamisole of E-rosette formation and its abrogation by autochthonous serum from patients with bladder cancer.

Incubation in vitro of lymphocytes from bladder cancer patients with levamisole (40 microgram/ml) resulted in a rise of the number of E-rosette forming cells from 39.2 +/- 11.8% to 57.5 +/- 11.3% (p less than 0.005). The same effect was observed when levamisole was administrated 150 mg/day for 3 days to the patients. The stimulatory effect of levamisole was abrogated when the lymphocytes were first incubated with levamisole and afterwards with 50% autochthonous serum from the patient. With more diluted serum concentrations (from 0.5% to 1%), the response decreased, and the response was not observed with allogeneic serum. When lymphocytes from healthy donors were incubated with 50% serum from bladder cancer patients, there were no significant changes in numbers of E-rosette forming cells. It was presumed that the suppression of E-rosette forming cells from the patients with bladder cancer was caused by blocking the receptor sites for sheep red blood cells by autochthonous serum components and that levamisole eliminated such substances from the cryptic sites on the surface of the lymphocytes.

Studies on pituitary-gonadal endocrine function in XYY men.

Serum luteinizing and follicle-stimulating hormones and testosterone levels were studied in 11 patients with 47-XYY chromosomes and a comparison was made to normal men and patients with other testicular diseases, including Klinefelter's syndrome. Serum follicle-stimulating hormone levels in patients with XYY chromosomes were elevated significantly in comparison to those in normal men but lower than those in men with Sertoli cell only syndrome and Klinefelter's syndromes. Serum luteinizing hormone levels were somewhat elevated and serum testosterone levels were somewhat low in comparison to normal men, although the difference was not significant. Results of the short-term human chorionic gonadotropin stimulation test suggested almost normal Leydig cell reserve capacity in patients with XYY chromosomes.
