Magnetic resonance imaging in complex partial seizures.

Magnetic resonance imaging (MRI) and computed tomography (CT) were performed on 45 patients with intractable complex partial seizures. MRI was performed with a superconducting whole-body scanner operating at 0.5 tesla (T) and 1.5 T. In patients with temporal lobe epilepsy, 8 of 24 patients had abnormal CT, but 16 of 24 patients showed abnormal MRI. 1.5 T MRI detected more abnormality than 0.5 T MRI when CT was normal. In patients with frontal lobe epilepsy, 5 of 7 patients had normal CT and MRI. In 2 other patients, MRI demonstrated an arachnoid cyst and increased signal intensity area on the T2-weighted images which were not detected by CT. In patients with occipital lobe epilepsy, 5 of 6 patients show abnormal CT and MRI. In patients with tuberous sclerosis, MRI revealed some increased signal intensity areas on the T2-weighted images in the occipital and temporal lobe, which were not detected by CT. Most surface EEG foci corresponded with the side of MRI abnormality. These data indicate that MRI is more informative than CT in complex partial seizures. MRI is the imaging technique of choice in the diagnosis of complex partial seizures.

Impaired susceptibility of human trophoblast to MHC nonrestricted killer cells: implication in the maternal-fetal relationship.

Mammalian pregnancy has frequently been termed "Nature's allograft," and there is developing evidence that the placental trophoblast cells in their key position at the maternal fetal interface are responsible for escape mechanisms to the maternal immune system. In this paper, we show impaired susceptibility of human trophoblastic tumor cells to major histocompatibility complex (MHC) nonrestricted cytotoxicity systems such as natural killer (NK) cells and lymphokine-activated killer (LAK) cells. LAK cells were induced by the culture of peripheral blood mononuclear lymphocytes (PBL) in recombinant interleukin-2 (rIL-2). Among 21 cultured cell lines derived from various tissues and organs tested, five choriocarcinoma-derived cell lines gave decreased levels of LAK lysis. Cold-target inhibition study and trinitrophenyl (TNP) modification experiment clearly indicated that the impaired sensitivity of trophoblast cells to LAK lysis is due to the decrease of a common target molecule recognized by LAK effector cells. It is suggested that the impaired susceptibilty of trophoblast to MHC nonrestricted killer cells should be functional for the survival of the semiallogeneic fetus.

Trophoblast-derived immunoregulatory factor: demonstration of the biological function and the physicochemical characteristics of the factor derived from choriocarcinoma cell lines.

An immunosuppressive factor released by choriocarcinoma cell lines was analyzed in the present study. It inhibited the proliferative responses of human T cells stimulated by lectins or alloantigens. It also blocked the generation of alloreactive cytotoxic T cells. The suppressive activity of the factor was detected in the responses of the T cells costimulated with 1 nM 12-O-tetradecanoyl phorbol 13-acetate and 1 microM A23187, suggesting the possibility that the factor acted on the intracellular signal transduction in T cells rather than interfering with early events such as T cell receptor signal transduction through cell membranes. Moreover, the factor acted directly on T cell proliferation pathways without activation of suppressor cells but did not act on T cell activation pathways. Taken together, all these findings expanded our previous reports on a factor released by normal trophoblasts, indicating the possible identity of the two factors. The physicochemical properties of the choriocarcinoma-derived factor were examined, and the biological significance of the factor during pregnancy was discussed in this paper.

[Case report of long-term treatment of a cleft lip and palate patient].

Cleft lip and palate patients have narrow maxillary arches and mesio-distal disharmony between the jaws caused by cicatricial contracture from cleft lip and palate surgeries. In such cases, early orthodontic treatment is required including improvement of the mesio-distal disharmony of the jaws. Following orthodontic treatment, prosthodontic treatment is required to improve the masticatory function and esthetic appearance in the cleft area. Thus, improving the occlusion of cleft lip and palate patients takes a long time. This case report concerns such a longterm handling of over 17 years from the initial treatment at 5 years 3 months till the final prosthodontic treatment for an under-developed maxilla with partial closure of the fistula to which ilium was later grafted following orthodontic treatment. Considerations are discussed about timing of the initiation of orthodontic treatment, closure of the oronasal fistula and the final prosthodontic restoration.

[Long-term follow-up of absence epilepsy].

This study dealt with 58 patients with absence, of whom 46 patients had started with absence and 12 with generalized tonic-clonic seizures (GTCs). Only those patients followed up for more than five years were included, and 30 patients (51.7%) were over eighteen years of age (up to 33). All patients received the present standard medication with ethosuximide and valproate in this study. In each case the diagnosis was confirmed by clinical observation and the typical EEG pattern. A seizure-free interval of at least 1 year was defined as seizure cessation for absence, and a seizure-free interval of at least 2 years for GTCs. Thirty-eight out of 43 patients (88.4%) with absence at onset (group A) and 18 out of 22 (81.8%) older than 18 years became seizure free. Only 8 of 15 (53.3%) patients with initial GTCs (group B) and 5 of 8 (62.5%) over 18 years became seizure free. In all, about 80% of the patients with absence seizures became seizure free as did those who were followed beyond 18 years of age. Out of 15 patients with initial GTCs, 8 of 10 (80%) patients who developed absence seizures later (group B-1) became seizure free, whereas none of 5 patients who had absence and GTC at the same time (group B-2) did. The social status was mainly favorable, even if seizures were uncontrolled. Adequate predictable factors for the development of GTCs were lacking, but the clinical courses in patients of group B-1 resembled those of group A and were rather benign.

[Low sensitivity of choriocarcinoma cell lines to tumor necrosis factor (TNF)-alpha].

Sensitivity of cultured choriocarcinoma cell lines to tumor necrosis factor (TNF)-alpha and interferon (IFN)-gamma was examined and compared with that of other cultured tumor cell lines. Tumor cells were cultured with 1,000 mu/ml of TNF-alpha and/or 100 mu/ml of IFN-gamma for 24hr. Antitumor effects of the lymphokines were measured by 3H-thymidine uptake and tetrazolium salt (MTT) colorimetric assay. The results revealed that TNF-alpha and/or IFN-gamma had little anti-tumor effect on the choriocarcinoma cell lines tested, while they had cytostatic and cytotoxic effects on other cultured tumor cell lines including Panc-1 (pancreatic carcinoma cell line), Lovo (colon carcinoma cell line) and Ishikawa (uterine endometrial carcinoma cell line). We also examined the effect of TNF-alpha and IFN-gamma on the expression of major histocompatibility complex (MHC) class I antigens in these tumor cell lines by means of cellular binding radioimmunoassay. No enhancing effect was observed on choriocarcinoma cell lines after the treatment with TNF-alpha and/or IFN-gamma. These results suggested the existence of a unique property of choriocarcinoma cells which results in the resistance to host immune attacks.

[Identification of a human sperm antigen to infertile women's serum and follicular fluid with high sperm immobilizing activity].

In our previous paper, we reported a sperm coating antigen with a molecular weight of approximately 12,000 daltons from sperm membrane fraction by using rabbit anti-seminal plasma antiserum. In this paper, we isolated a human sperm antigen with the same sperm preparation by using sera of infertile women with sperm immobilizing antibody. The isolated antigen showed the following characteristics: (1) It is a glycoprotein with an affinity with lentil lectin and its molecular weight is approximately 15,000 daltons. (2) Little activity of the antigen was seen in seminal plasma. The antigen is localized on the equatorial segment of the acrosome. These facts indicate that the antigen is a genuine sperm antigen and is not a sperm coating antigen. (3) The antigen is bound with the sperm immobilizing antibody in follicular fluid of an infertile woman who has a sperm immobilizing antibody in her serum. This indicates that sperm immobilizing antibody in follicular fluid recognizes the same antigen as in the serum.

[Radiological findings of cancerous invasion in colorectal cancer less than 3 cm in diameter].

131 colorectal cancers, less than 3 cm in diameter, experienced at Juntendo University Hospital in Oct. 1977 through Dec. 1988 were studied with respect to their radiological and macroscopic features. Macroscopic features of the cancers were classified into type a (a lesion with a long stalk), type b (a subpedunculated or sessile lesion), type c (a plaque-like lesion) and type d (a lesion with a depression). Type a is early cancer. An early cancer of type b was mostly demonstrated as a lobular pattern of tumor surface, and an advanced cancer as a nodular pattern. Type c less than 1 cm in diameter was seen a sm cancer. A sm cancer of type d was mostly demonstrated as a faint barium fleck of the depression, an advanced cancer as a mild barium fleck. By deformed colonic wall in profile view, an early cancer was mostly demonstrated as a thorn shape deformity, sm or pm cancer as an arch shaped deformity and a cancer with deeper cancerous invasion than pm as a trapezoid deformity.

[Low susceptibility of choriocarcinoma cell lines to lymphokine activated killer (LAK) cells].

We investigated the anti-tumor effect of lymphokine activated killer (LAK) cells on various carcinoma cell lines as it may play an important role in the immunotherapy of gynecologic cancers. Peripheral blood lymphocytes (PBL) were cultured in 10 U/ml of recombinant interleukin-2 (rIL-2). The cytotoxic action of LAK and natural killer (NK) on carcinoma cell lines was measured by a standard 4-hr 51Cr-releasing assay. Daudi and RPMI 1788, NK resistant B cell lines were lysed by rIL-2 activated PBL although they were not killed by human fresh PBL. LAK cells thus induced were markedly cytotoxic to sixteen carcinoma cell lines. However, five choriocarcinoma cell lines were resistant to LAK action because specific action was less than 30% at effector/target ratio of 20. The cold target competitive inhibition experiments showed that the choriocarcinoma cell line GCH-1 induced much lower inhibition than Daudi cells. These results indicate that the low susceptibility of the choriocarcinoma cell lines to LAK cells could be due to the lack of the effector molecules recognized by killer cells.

Clinical evaluation of the enzyme-linked immunosorbent assay (ELISA) kit for antisperm antibodies.

A commercially available enzyme-linked immunosorbent assay (ELISA) kit for the detection of antisperm antibodies in serum was compared with standard sperm immobilization test (SIT) with the use of sera from 83 infertile women and 29 control individuals. For the ELISA, 24% of the infertile patients and 10% of the controls showed positive results, whereas 15% of the patients and none of the control were positive in the SIT. Parallel tests carried out on the same sera indicated that these methods detect a different, though often overlapping, spectrum of antibody activity. The presence and number of motile sperm in cervical mucus during postcoital tests were found to be related to the results of the SIT. On the other hand, the ELISA did not appear to be related to the quality of postcoital tests. These data indicate that care must be employed to interpret the results of this ELISA kit for the detection of antisperm antibodies.

Developmental changes of pattern reversal visual evoked potentials.

Analysis of developmental changes in pattern reversal visual evoked potentials (PVEPs) was performed on 141 normal children ranging in age from 1 month to 19 years 4 months. The stimulus was a black and white checkerboard pattern on a television screen, and the check edge subtended 50 min of retinal arc. The major positive peak (P2) was observed in all subjects, and the incidence of other peaks tended to increase with age. The P2 latency decreased rapidly during the first six months of life and reached a constant value after the age of 2 years. The P2 latency with monocular stimulation was significantly longer than that with binocular stimulation in most age groups. Developmental changes of the N1-P2 or P2-N2 amplitudes were unclear, and the standard deviation of the amplitude in each age was too large for clinical application. The P2 latency with binocular stimulation was shorter for females than for males only at the age of 8-11 years.

Identification and characterization of a human sperm antigen corresponding to sperm-immobilizing antibodies.

A sperm antigen has been isolated from radiolabeled human sperm cell membrane by detergent solubilization, lectin affinity chromatography, gel filtration, and indirect immune precipitation using sperm-immobilizing antisera from patients with unexplained infertility. Isolated material was characterized by sodium dodecyl sulfate-polyacrylamide gel electrophoresis (SDS-PAGE). Among 20 infertile women's sera with sperm-immobilizing antibodies, two were found to react predominantly with a sperm membrane polypeptide having the approximate molecular weight of 15,000 daltons. No significant binding to this molecule was observed in any sera from pregnant women, unmarried women, and normal men. By the absorption with spermatozoa, the antisera lost their binding activity to the molecule, while the sera absorbed with seminal plasma did not lose the activity. The results indicated that the molecule is a genuine sperm antigen and not a sperm-coating seminal plasma antigen. By the indirect immunofluorescence of washed ejaculated spermatozoa with the antisera, strong fluorescence was localized only in an equatorial segment of the acrosome, while no specific staining was observed in the controls. The antigen is relatively unstable against acid, alkali, and heat treatment. Treatment with proteolytic enzymes such as pronase and trypsin inactivated the antigen activity, indicating that the antigen epitope could be a peptide portion of the glycoprotein.

Determination of L-6-keto-piperidine-2-carbonyl-L-leucyl-L-proline amide (RGH-2202), a new analog of thyrotropin-releasing hormone, in plasma by radioimmunoassay.

A sensitive and specific radioimmunoassay has been developed for the determination of L-6-keto-piperidine-2-carbonyl-L-leucyl-L-proline amide (RGH-2202), a new analog of thyrotropin-releasing hormone (TRH), in plasma. An antiserum was produced in guinea pigs immunized with RGH-2202 which was conjugated to bovine serum albumin by an N-succinimidyl ester method. Iodine-125 labeled L-6-keto-piperidine-2-carbonyl-L-leucyl-L-prolyltyramine [( 125I]RGH-2202) was used as a tracer. Polyethylene glycol was used to separate bound and free [125I]RGH-2202 in the reaction mixture. The assay of RGH-2202 in plasma was possible over a concentration range from 0.1 to 6.4 ng/ml, using 0.1 ml of plasma without the need for an extraction procedure. The antiserum used for the assay was highly specific for RGH-2202, and did not cross-react with TRH and the hydrolyzed derivatives of RGH-2202 such as L-6-keto-piperidine-2-carbonyl-L-leucyl-L-proline, L-6-keto-piperidine-2-carbonyl-L-leucine and L-leucyl-L-proline amide, which were assumed to be present in plasma as metabolites. The coefficients of variation were 4.6-6.7% for within-assay and 6.0-8.8% for between-assay. Plasma levels of RGH-2202 in rats were determined after an intravenous administration of RGH-2202 at a pharmacologically effective dose (0.625 mg/kg).

[An autopsy case of gastric metastasis simulating linitis plastica carcinoma from primary linitis plastica carcinoma of the rectum].

Cases of a metastatic involvement of the stomach from a colorectal cancer are extremely rare, and once such case, confirmed by autopsy, is reported. The patient, an 86-year-old woman, had a metastatic lesion of the stomach from a rectal cancer. The rectal cancer was a combined linitis plastica type and signet ring cell carcinoma, and the appearance of metastatic lesion of the stomach had simulated the linitis plastica type cancer. A metastatic involvement of the stomach presenting a linitis plastica type cancer from a breast cancer is well documented, but a linitis plastica type from a colorectal cancer has not been reported. This case is the first report concerning a gastric metastasis presenting linitis plastica type lesion from a colorectal cancer.
