Immune responses in experimental allergic neuritis treated with corticosteroids.

The preventive and suppressive effects of methylprednisolone (MP) were investigated in 22 rabbits immunized with peripheral nerve myelin. Cellular reactivity to bovine P2 protein (P2) and anti-P2 antibody were also examined serially in these animals, using the lymphocyte proliferation test or enzyme-linked immunosorbent assay. Seven animals (preventive group), given 15 mg/day of MP subcutaneously from Days 0-28, showed a significant reduction in maximal severity and a delay in onset in comparison with 8 control animals. The remaining 7 animals (suppressive group), given the same dose of MP from Days 11-28, also showed significantly milder clinical signs than those observed in the controls. However, cellular hypersensitivity to P2 were manifested at Day 14 (just after clinical onset) in the controls, as well as in the preventive or suppressive groups. Furthermore, anti-P2 antibody was detected in each group after Day 14. These results suggested that the interference of inflammatory processes of immune-mediated disease might respond to the preventive and suppressive effects of MP.

[Clinical efficacy of aztreonam in patients with complicated urinary tract infections].

Aztreonam (AZT) (2 g daily) was intravenously administered for 5 to 7 days to 65 patients with complicated urinary tract infections (UTI). Ninety one organisms were isolated from these patients' urine. Among them 66 organisms were eliminated following AZT administration. The elimination rate for Gram negative organisms was 94.7% and that for Gram positive organisms was 33.3%. Thirty one organisms appeared newly by the replacement of organisms. Among them, 12 organisms were E. faecalis. The efficacy rate of AZT in single infection and in mixed infection was 55.8% and 13.7%, respectively. The overall clinical efficacy rate of AZT in the present study was 41.5%. As side effects, skin rash and itching were observed in 3 patients and slight elevations in blood chemistry were observed in 10 patients. The results indicate that AZT is an effective drug for the treatment of patients with complicated UTI, especially those caused by Gram negative organisms.

[Treatment of complicated urinary tract infections by ofloxacin].

To study the treatment effects of ofloxacin (OFLX) on the patients with complicated urinary tract infections (UTI), OFLX at a daily dose of 600 mg divided into either two or three doses per day, was administered for 14 days to 114 patients with complicated UTI. Among the 114 patients, 67 patients satisfied the criteria for clinical evaluation of complicated UTI. One hundred and one organisms were isolated from these patients' urine. Among them, 82 organisms were eliminated following OFLX administration. Twenty organisms appeared newly by the replacement of organisms. Among them, 6 organisms belonged to Candida sp. As to the overall efficacy of OFLX, an excellent response was seen in 37.3% and sum of efficacy rate was 65.7%. Side effects were observed in 3 of the 114 patients. These results indicate that OFLX is both safe and effective enough to be used for the treatment of the patients associated with complicated UTI.

[Postoperative hydrocele testis as a complication of high ligation of internal spermatic vein].

We reviewed 785 patients with varicocele testis who underwent high ligation of internal spermatic vein at Kasukabe City Hospital between 1969 and 1985, and found only 3 patients suffering from postoperative hydrocele testis. Analysis of the hydrocele content in one case revealed a high protein level, which suggested that the etiology of the hydrocele after high ligation is of lymphatic origin, so that it is important to preserve the lymphatics of spermatic cord in order to prevent postoperative hydrocele.

Cell-mediated immunity to bovine P2 protein and neuritogenic synthetic peptide in experimental allergic neuritis.

Cellular reactivity to bovine P2 protein (P2) and its two synthetic peptides, SP66-78 and SP70-78, was serially examined by the lymphocyte proliferation test in animals with experimental allergic neuritis (EAN). SP66-78 and SP70-78 correspond to residues 66-78 and 70-78 of bovine P2. Proliferative response to SP66-78 as well as P2 appeared at day 7 before the onset of EAN and was clearly manifested at day 14 in the active stage, thereafter disappearing in the stable stage, whereas no response to SP70-78 was detected during the course of the disease. These results suggest that cell-mediated immune response to P2 and the specific part residues 66-78 of P2 play an important role in the pathogenesis of EAN.

[Prophylactic intravesical chemotherapy in bladder tumors after surgery of upper tract urothelial carcinoma].

Sixteen patients with upper tract urothelial carcinoma underwent intravesical chemotherapy usually at 2 week intervals in the first year and at 4 week intervals in the second year after nephrouretectomy. For bladder instillation 10 mg mitomycin C in 20 ml saline was used on 7 patients, 5 mg carboquone and 100 mg cytosine arabinoside in 40 ml saline on 5 patients and 30 mg adriamycin in 40 ml saline on 4 patients. Two (12.5%) of the 16 patients developed bladder tumors within 2 years after surgery, but 11 (42.3%) of the 26 patients with upper tract urothelial carcinoma who did not receive intravesical chemotherapy suffered from bladder tumor within 2 years after surgery. Prophylactic intravesical chemotherapy reduced significantly (p less than 0.1) the incidence of bladder tumor after the surgery of renal pelvic and ureteral tumors.

[Cytology of percutaneous aspiration biopsy of renal angiomyolipoma: a study of 4 cases].

Percutaneous aspiration biopsy of renal angiomyolipoma (AML) was carried out on four patients to investigate the cytological features of AML. The diagnosis was confirmed histologically after the removal of the tumor. The specimen was fixed in 95% alcohol and stained by the Papanicolaou's method. Cells obtained formed clusters in three of four cases. Cells originating from leiomyomatous element of AML varied in their size, but are usually small with small oval nuclei and rich cytoplasm of lacy appearance. The cytoplasm was fragile and the cell border was not clear. Nuclear chromatin was not increased and was distributed diffusely. Fat cells originating from fatty element of the tumor were mature type and obtained from all of three patients who had much typical lipomatous element in the tumor. In conclusion, cytology of aspiration biopsy is useful to establish preoperative diagnosis of AML in such occasions when there are difficulties in differential diagnoses between AML and renal cell carcinoma even by either computed tomography, angiography or ultrasonography.

[Primary adenocarcinoma of the female urethra: report of two cases].

Two cases of primary adenocarcinoma of the female urethra are reported. Case 1 was a 75-year-old female presenting with the complaint of genital bleeding. Physical examination revealed a pea-sized tumor in the urethral meatus. Tumor resection was performed under the preoperative diagnosis of caruncles, but, histopathological examination revealed adenocarcinoma. She was readmitted because of recurrence of tumor 7 months after the first operation. En bloc resection including the bladder, urethra and uterus was done followed by ileal conduit for urinary diversion. Case 2 was an 85-year-old female presenting with the complaint of macroscopic hematuria. Physical examination revealed a thumb's-head-sized tumor in the urethral meatus and also the left enlarged inguinal lymph nodes. Tumor resection and biopsy of the left inguinal lymph nodes were performed. Histopathological examination revealed adenocarcinoma. Postoperative irradiation to the left inguinal lymph nodes was given to a total dose of 4,000 rads. Ninety-two cases of primary adenocarcinoma of the female urethra in the Japanese literature including our cases are reviewed.

[Bladder instillation of verapamil as a means of enhancing the efficacy of chemotherapy in superficial bladder carcinoma: influence of bladder instillation of verapamil on the cardiovascular system].

Although it is well known that calcium antagonists in vitro possess enhancing effects on anti-tumor drugs, their clinical use is limited on account of side effects on the cardiovascular system. In the present study, the authors attempted the instillation of Verapamil (calcium antagonist) combined with anti-tumor drugs into the bladder for the treatment of superficial bladder carcinoma. The dosage of Verapamil necessary for maintaining an effective concentration for enhancement of the anti-tumor drug was determined and the influences of bladder instillation of Verapamil on the cardiovascular system and on the bladder were studied. The results obtained were as follows: the use of 5 mg of Verapamil was able to maintain an effective concentration (10(-5) M) for enhancement of anti-tumor drugs for at least 2 h. Absorption of Verapamil from the bladder to the vascular system was minimal. No influence on the cardiovascular system or on the bladder was observed. The results of the present study encourage us to pursue the clinical use of Verapamil combined with anti-tumor drugs for the treatment of superficial bladder carcinoma.

Cellular hypersensitivity to nervous antigens in Guillain-Barré syndrome.

Cell-mediated immune responses to various nervous antigens were examined in 12 cases of Guillain-Barré syndrome (GBS), 24 cases of noninflammatory peripheral neuropathy (NIPN), and 18 cases of degenerative disorders of central nervous system (CNSDD), using the lymphocyte-transformation technique. Cellular hypersensitivity to bovine P2 protein (P2) and a synthetic peptide, SP66-78, corresponding to the residues 66-78 of P2, was detected in about two-thirds of GBS cases, especially in the active or improving stages, but not in NIPN and CNSDD. The lymphocytes sensitized to these nervous antigens might play an important role in the pathogenesis of GBS.
