[Urinary NAG and gamma-GTP activities as indicators of CDDP renal toxicity: effect of fosfomycin on CDDP renal toxicity].

Using urinary N-acetyl-beta-D-glucosaminidase (NAG) and gamma-glutamyl transpeptidase (gamma-GTP) activities as the indicator of renal toxicity, the effect of fosfomycin (FOM) on cisdichlorodiamineplatinum (CDDP) renal toxicity was studied in 10 patients with urological malignant tumors. Regimen of chemotherapy was daily 20 mg of CDDP and 800 mg of administered by drip infusion for 5 days and 40 mg of adriamycin injected in one-push on day 1. FOM was not used at 1st cure. At 2nd cure, daily 2 g X 2 of FOM was administered for eleven days from the day before CDDP administration. Urinary NAG and gamma-GTP were measured for eleven days continuously in both 1st and 2nd cure. Effect of FOM on CDDP renal toxicity was discussed by comparison of the urinary NAG and gamma-GTP activities between 1st and 2nd cure. The results showed that urinary NAG and gamma-GTP activities following CDDP administration were deeply influenced by these activities before CDDP administration and the combined therapy of FOM had tendency to reduce CDDP renal toxicity when urinary NAG and gamma-GTP activities before CDDP administration were in the normal range.

[A case of giant hydronephrosis caused by renal fibrosarcoma].

The presented report is fibrosarcoma arising from renal capsule in a 64-year-old woman. The tumor is very rare and is the 25th case in Japan. The patient visited our hospital with the complaint of macroscopic hematuria for several days. Abdominal examination revealed a painless lump from the left lumbar region to para-median abdomen. A diagnosis of hydronephrosis caused by neoplasma or tuberculosis was considered by CT, AG, etc., and transperitoneal nephrectomy was performed on 6-July-1984. Pathology of the tumor was fibrosarcoma arising from the renal capsule. Three months later, the tumor was growing on the peritoneal surface from the left renal region and she died on Nov. 10, 1984.

[Acute change in urinary NAG activity following CDDP administration].

In the present paper, the acute change of urinary NAG activities following CDDP administration was studied in 8 patients with urological malignant tumors. The activities were measured for 11 days from the day before CDDP administration and were expressed as U/day. According to the NAG activity and creatinine clearance (CCr), the patients were divided into 4 types. In type I (normal NAG activity and normal CCr), the increase of the NAG activity following CDDP administration was transient. In type II (normal NAG activity and low CCr), the NAG activity increased and decreased as in type I, but in type II, the NAG activity rebounded a few days after the completion of CDDP administration. In type III (high NAG activity and normal CCr) and in type IV (high NAG activity and low CCr), the increase of the NAG activities continued longer than in type I, II; especially in type IV, the increase and the prolongation of the NAG activity was prominent. The timing of CDDP administration is considered important to avoid renal toxicity of CDDP.

[Encrustation and stone formation in double J ureteral stent: report of two cases].

We recently experienced two cases of encrustation and stone formation on double J ureteral stent which had been indwelt at pyeloplasty and at pyelolithotomy. In these patients, bacteriuria and pyuria had continued. Encrustation and stone formation were recognized by follow-up X-ray fairly soon after operation. Judging from our experience, encrustation and stone formation should be suspected in patients with persistent bacteriuria and/or pyuria and who have episodes of urolithiasis. To avoid encrustation and stone formation, the ureteral stent should be replaced more frequently.

Parameatal urethral cysts.

Two cases of parameatal urethral cysts in boys are reported. Both had been present from early infancy. The etiology of parameatal urethral cysts is discussed.

Enhancement of non-specific resistance to viral infection by muramyldipeptide and its analogs.

Antiviral activity of muramyldipeptide (MDP) and its lipophilic derivatives, B30-MDP and MDP-Lys(L18), was investigated in mice infected with vaccinia virus (VV) and herpes simplex virus type 2 (HSV-2). Mice administered these compounds subcutaneously or orally were protected against VV in tail lesion tests and against HSV-2 in skin lesion tests, respectively. Since in vitro antiviral activity was not demonstrated with these compounds in cultured mammalian cells infected with either VV or HSV-2, host-mediated defense mechanisms may play a role in the activity of the compounds. As for serum interferon (IFN) induction, MDP and its analogs showed no activity in mice, suggesting that IFN does not participate in the antiviral mechanisms against VV and HSV-2. An extrinsic antiviral activity was demonstrated when peritoneal macrophages from the mice administered these compounds were cocultivated with VV-infected 3T3 cells. The results indicate that macrophage activation by MDP and its analogs plays a role in the defense mechanisms against viral infection. This activity was not virus-specific. We also demonstrate that the introduction of lipophilic residue(s) into MDP enhances the antiviral activity of mice against VV and HSV-2.

Identification of benzo[a]pyrene quinone in soybean leaves.

Soybean seeds (Glycine max) were grown in soil containing 10 ppm of benzo[a]pyrene (BaP). After 76 days growth, three kinds of quinones, namely BaP-1,6-, 3,6- and 6,12-quinones, were isolated and identified. However, the formative mechanism of the quinones in leaves has yet to be determined.

Inhibitory effect of saturated fatty acids on the mutagenicity of N-nitrosodimethylamine.

Saturated fatty acids, C5-C12, inhibited the mutagenic activity of N-nitrosodimethylamine (NDMA) in E. coli WP2 uvrA/pKM101. The inhibition by laurate (C12) was due to the suppression of the enzymatic demethylation of NDMA, whereas that by caprate (C10) was simply due to the bactericidal effect of the fatty acid. Caproate (C6) did not affect the NDMA-demethylase, and evidence is presented to show that the inhibition of mutagenesis by caproate was a result of its interference with the uptake of NDMA metabolites into bacterial cells. Possible biological significance of the inhibition is discussed.

Regulation of cyclic GMP phosphodiesterase in the submandibular gland of adult rat.

Cyclic GMP phosphodiesterase was investigated in the submandibular gland of the adult rat to elucidate the regulatory mechanisms of cGMP concentration in this gland. Ca2+ sensitivity was easily demonstrated as in other tissues using EGTA in the buffer for elution from DEAE-cellulose. The presence of inhibitor proteins for basal and calmodulin-activated portions of Ca2+-dependent phosphodiesterase was suggested. The inhibitor for the basal activity of Ca2+-dependent phosphodiesterase was deemed to be a heat-labile protein, which decreased Vmax, but had no effect on the Km value for cGMP. The presence of more than one kind of inhibitor for the calmodulin-activated portion of the Ca2+-dependent phosphodiesterase was also suggested. One of these, which was not absorbed on DEAE-cellulose, was a heat-labile protein which caused an increase of Km for cGMP, but no change of Vmax.
