Twenty-three patients with locally advanced and/or metastatic urothelial tumors were treated by surgery, chemotherapy or radiation. Radical operation with or without radiation resulted in tumor free conditions in 8 patients. Fifteen patients who had a few dimensionally estimable disease parameters, underwent chemotherapy. Objective responses were obtained in 6 of the 15 patients (1 CR and 5 PR). The objective response of low grade or low stage tumors or cases with high Karnofsky performance ratings was moderate. Combination chemotherapy with cisplatin (CDDP), doxorubicin (ADM) and cyclophosphamide or a CDDP-containing combination appeared to be beneficial for patients with advanced urothelial tract tumors with palliative purpose.
The right suprarenal mass was found in a 21-year-old housewife. Her major clinical features were amenorrhea, polydipsia and buffalo hump obesity. Endocrinological and roentgenological studies suggested the presence of Cushing's syndrome due to adrenocortical carcinoma in addition to ipsilateral renal stone. The huge adrenal tumor and renal calculus were successfully removed. The histological diagnosis was adrenocortical carcinoma. Seventeen days after the operation, cis-platinum was administered to prevent the recurrence of tumor development. No recurrence has been observed for approximately 2 years after the surgery. Long follow-up must be pursued to clarify the real efficacy of cis-platinum treatment.
Of 1192 lung resections in patients with bronchogenic carcinoma since 1952, the early post-operative mortality averaged 3.9%, whereas during the last ten years, it was 2.6% of 730 lung resections, including 159 (22%) pneumonectomies, 76 (10%) bilobectomies, 492 (67%) single lobectomies and 3 (0.4%) partial lung resections. In the latter, 352 (48.2%) and 75 (10%) had airway obstructive failure of FEV1.0 less than 70% and 55%, respectively, and, 56 (7.7%) and 9 (1%) represented preoperative hypoxia of PaO2 less than 70 torr and 60 torr, respectively. The ratios of pneumonectomies in these, being similar to those with better lung functions, the perioperative mortality was also similar. Our previously reported indices preoperatively to prove ability to withstand curative lung resections, being supported by these data, we attempted in a certain limited group of patients, extendingly to clear patients with critically poor predicted pulmonary vascular reserve pf 710-930 dyne . cm-5 . sec/m2 for lung resections, resulting in six elevenths of over one year survival. One must extend benefit of lung resection to patients with coexistence of bronchogenic carcinoma and respiratory disorder apparently severe enough to preclude resection surgery by critical assessment of the predicted pulmonary vascular reserve after lung resection.

A 60-year-old man had a history of frontal headaches and chewing-related oscillopsia. Examination disclosed a retro-orbital epidermoid cyst that had eroded through the lateral orbital wall and under the temporalis muscle. The illusion of movement was due to mechanical displacement of the tumor mass and eye by contraction of the temporalis muscle. Removal of the cyst produced complete remission of the oscillopsia.
It has been suggested that recovery from aphasia may depend on the ability of the nondominant hemisphere to subserve language functions. We describe a patient who illustrates a possible "transfer" of language dominance. The patients sustained two infarcts two years apart, one in each hemisphere, involving mirror image territories in the temporoparietal area. The clinical course of each infarct was almost identical, consisting of an initial Wernicke's aphasia which evolved into anomic aphasia during recovery.
An aldosterone-producing adenoma (APA) was associated with chronic renal failure. Following treatment with Trilostane (a 3 beta-hydroxysteroid dehydrogenase inhibitor), furosemide and continuous ambulatory peritoneal dialysis (CAPD), a left adrenal adenoma was successfully removed. This patient, still being treated with CAPD, is a unique example of primary aldosteronism without showing suppressed plasma renin activity.
Acute Wernicke-Korsakoff syndrome is an underdiagnosed cause of reversible coma in the alcoholic patient. Chronic toxic effects of ethanol include nutritional polyneuropathy, cerebellar degeneration, and diffuse cortical damage with resultant alcoholic dementia. The rapid correction of hyponatremia can result in the iatrogenic syndrome of central pontine myelinolysis.
Following 7 days on a low sodium diet, a regular sodium diet or a high sodium diet each, urine samples were collected from 37 subjects in the final days of each sodium treatment. Urinary kallikrein excretion was determined in 9 patients with primary aldosteronism, 15 normal subjects and 13 patients with essential hypertension. Urinary aldosterone excretion, plasma renin activity (PRA), urinary sodium excretion, urinary potassium excretion and p-aminohippuric acid clearance were also determined on the same days. Levels of urinary kallikrein excretion in patients with primary aldosteronism due to aldosterone-producing adenoma (APA) were greater (p less than 0.05 to p less than 0.001) than those in patients with primary aldosteronism due to idiopathic adrenal hyperplasia (IHA) under any sodium diet. Other examined variables were of limited value in differentiating patients with APA from those with IHA. Urinary kallikrein excretion, urinary excretion of electrolyte, urinary aldosterone excretion, PRA and PAH clearance were similar in normal subjects and patients with essential hypertension. It appears reasonable to conclude from these data that urinary kallikrein does not play an important role in the pathogenesis of essential hypertension, and elevated urinary kallikrein excretion in patients with primary aldosteronism due to APA can be used for biochemical differentiation from those with IHA.
Removal of metastatic lung tumor was performed for 27 patients in a period from January, 1972 to December, 1981. They included 22 carcinomas and 5 sarcomas. Diagnosis as having lung metastasis was made after removal of the primary tumor in 18 patients and at the time of the first detection of the primary tumor in 4 patients. In 5 patients the operation for the metastatic tumor preceded that for the primary malignancy. Metastatic tumors were localized in one lobe in 13 patients. Thirteen of the 27 patients underwent lobectomy and 14 received other types of operation, with or without hilar and/or mediastinal lymphnode dissection. Four patients underwent bilateral thoracotomy. Survival rates at 3 and 5 postoperative years of the group of patients who underwent the operation for primary malignancy first were both 49 percent. The number of lesions removed or the type of operation did not seem to have correlations with the survival rate. Patients whose tumor-free interval was more than 40 months tended to survive longer than those who had an interval less than 40 months. The group with a tumor doubling time of more than 50 days showed a higher survival rate than the group with a doubling time of less than 50 days. Of the 4 patients thoracotomized bilaterally 2 are alive for 20 and 34 months postoperatively, while the other 2 died at 20 and 65 months after the operation. In the selected patients with metastatic lung tumors, surgical treatment can be conducted with a reasonably good result, even though the metastatic malignancy is multiple or bilateral.
Seventy-seven adult mongrel dogs underwent left lung transplantation to study the immunological detection of posttransplant rejection response using the lymphocyte blastogenesis assay. Experimental animals were divied into 3 groups; control autotransplantation (4 dogs), control allotransplantation (32 dogs) and immunosuppressed allotransplantation (41 dogs). The lymphocyte blastogenesis assay included mixed lymphocyte reaction (MLR), lymphocyte reactivity to lectins (PHA, ConA and PWM) and spontaneous blastogenesis in peripheral lymphocyte culture methods. The latter two tests were performed in serial blood samples up to the 38th posttransplant day. In the control autotransplant group there were no particular changes in posttransplant PHA reactivities. In the animals with lung allograft, results of preoperative MLR between donor and recipient showed no particular relationships to posttransplant rejection response. The recipient, however, showed a decreased response to PHA and an increased spontaneous blastogenesis at the period undergoing rejection which were confirmed by blood samples taken within 2 days before sacrifice of animals whose transplant specimens showed apparent histologic findings of rejection. In the immunosuppressed allotransplantation group, those findings of PHA response and spontaneous blastogenesis had often preceded the rejection episode detected on chest x-ray films, whereas there were no particular changes in both parameters of dogs bearing enhanced allografts. The serial detection of spontaneous blastogenesis and PHA reactivity in peripheral lymphocyte may be one of the useful methods for early prediction of rejection episode in lung transplantation.

A case of the XYY syndrome is reported. A 29-year-old man visited our hospital because of infertility and right undescended testicle. He was tall (181 cm) and his intelligence level was below average but his character was not violent. His right testis was touched in his right inguinal region and it was atrophic. Analysis of the semen showed no sperm. The biopsied specimen of his left descended testis as well as right undescended testicle showed spermatogenic arrest. Consequently we speculated that the spermatogenesis was injured by the surplus Y chromosome. On endocrinologic examination, plasma LH and FSH were high and testosterone revealed to be low. These data were slightly abnormal, but similar to those of most previous investigators.
Sustained upgaze is a rare phenomenon that is observed primarily in those patients with coma secondary to cardiac arrest or prolonged hypotension. Because of "selective vulnerability" of the brain, cardiac arrest or prolonged hypotension may produce both cerebral and cerebellar cortical damage sparing the brain stem vestibulo-ocular reflex (VOR) pathways. This selective damage may be responsible for the generation of sustained upgaze. The VORs receive their selective inhibition from the cerebellar flocculus. This inhibition is exerted on only one of the two VOR pathways converging on each extra-ocular muscle, so that total disinhibition of the VOR pathway activities will result in a tonic upward bias. The underlying neuronal mechanism of sustained upgaze appears to be due to total floccular disinhibition resulting in upward drift produced by the biased VOR and the lack of correcting saccades due to cerebral dysfunction releasing the brain stem ocular motor systems from their control.
