Transvascular fluid shift and thoracic duct lymph: analysis of lymph formation in the rat.

To analyze the effect of changes in interstitial fluid on lymph production, intravascular infusions of saline were given to splenectomized rats under pentobarbital anesthesia at 3 different rates (2, 3, and 4 ml/100 g of body weight) over 10 min. Change of blood volume was continuously monitored and, simultaneously, thoracic duct lymph was collected during and after the infusion. Equilibrium was attained approximately 40 min after the infusion; regardless of the infusion rate, 10% of the infused volume was incorporated into the vascular space and 90% was filtered into the extravascular space. Thus, the amount of transvascular fluid shift showed a linear relationship with the infused volume. However, the drainage from thoracic duct lymph amounted to 5.9%, 11.4%, and 17.8% of the infused saline volume when given at the rate of 2, 3, and 4 ml/100 g, respectively. The relation of lymph flow and infused volume could be regarded as a nonlinear system. By means of a simulation study, this relation was found to be attributed to the nonlinearity of conductance for fluid movement from tissue to lymph duct, which was only one sixth of that determined for the capillary membrane. The drainage of lymph protein following the infusion was only about 50% of that expected from the interstitial protein concentration in even distribution of the infused saline. These characteristics of interstitial fluid space play an important role in absorbing water and, hence, buffering changes in circulating blood volume after volume loading.

In vivo 31P-NMR studies on aerobic recovery of frog muscle following tetanus.

Aerobic recovery after a 60-sec tetanus of skeletal muscle of frog was studied using the in vivo 31P-NMR method with a topical magnetic resonance spectrometer (TMR) at 17 degrees C. Creatine phosphate recovered with a time constant of about 25 min-1. Sugar phosphates increased by the Pasteur effect, and saturated at about 4.5 mmol/kg muscle. The high level of the sugar phosphates was sustained during the following 60 min.

Transcapillary protein flux following blood volume modification in dog.

The net fluid and protein movements between intravascular and interstitial space following blood withdrawal and retransfusion of 15% of circulating blood volume were measured in dogs using a continuous monitoring method of circulating blood volume. A significant (p less than 0.01) increase in transvascular fluid shift was observed after the start of blood withdrawal and a new equilibrium state was reached within 15 to 20 min. Associated with the fluid shift, a significant increase in plasma protein of about 1 g was observed. On the other hand, blood retransfusion caused significant (p less than 0.01) increases in the shift of fluid and protein from intravascular space to interstitial space. The magnitudes of the shift of fluid and protein were almost identical in both blood withdrawal and retransfusion. The Kedem-Katchalsky transport equation was applied to the results obtained in the present study in order to analyze the relative role of diffusion and convection and to estimate the reflection coefficient for protein. A significant (p less than 0.01) linear relationship was observed between fluid and protein movement. These results suggest that the convective process is involved in the shift of protein between intravascular and interstitial space observed after blood volume modification.

[Diagnosis and surgical treatment of renal hyperparathyroidism].

Forty-three patients with chronic renal failure and secondary hyperparathyroidism underwent parathyroid surgery. The first 20 patients were submitted to subtotal parathyroidectomy, and the last 23 patients underwent total parathyroidectomy and parathyroid autotransplantation in the forearm. Non-invasive image diagnosis, CT, ultrasonography and scintigraphy are valuable for 1) making a definite diagnosis of secondary hyperparathyroidism, 2) locating the tumor 3) determining the effectiveness of treatment, and 4) differentially diagnosing thyroid tumors. For surgical treatment, we recommend total parathyroidectomy and autotransplantation in the forearm because the second operation for recurrence may be done more safely and easily than after subtotal parathyroidectomy.

A simple method for extended heart-lung preservation by autoperfusion.

A modification of Robicsek's autoperfusing heart-lung preparation preserved normal structure in the canine lung for 8 hrs and in the heart for 12 hrs. Applicable to both heart and heart-lung transplantation, the system would permit a safe extension of preservation time limits.

[Clinical study on six cases of subcortical cavernous angiomas].

Cavernous angioma is a rare but important clinical entity because of its potential curability. We experienced with 6 cases of cavernous angiomas located in the subcortical region and summarized with special respect on clinical symptoms, CT scan, angiographic manifestations and operative findings. In all of these 6 cases, the diagnosis is confirmed as cavernous angioma histologically. The age in these cases ranged from 16 to 45 years and sex distribution was 3 males and 3 females. Initial symptoms of 6 cases were long-standing epilepsy in 4 cases and attack of unconsciousness in 2 cases. Plain craniogram revealed calcification in only one patient. On the other hand, plain CT scan showed high density areas in all cases. Among these, 3 patients showed slight contrast enhancement. Perifocal low density areas were noted in all cases and these low density areas extended to the cortical region. The most important lesion to differentiate from cavernous angioma is low-grade astrocytoma with calcification on CT scan. We misdiagnosed in one patient with cavernous angioma as contusional hemorrhage, because the patient fell unconscious and hit the occiput. Angiographically, most cavernous angiomas are represented as avascular masses with displacement of adjacent vessels. In contrast to this general acceptance, 4 out of the present 6 cases showed abnormal findings such as feeding artery, capillary blush and draining vein. The draining veins were encountered in 4 cases. It must be emphasized that these angiographic manifestations are very small and caution has to be paid not to be overlooked. From the operative view, there are several characteristic findings.(ABSTRACT TRUNCATED AT 250 WORDS)

[Cerebral schistosomiasis with a huge calcification. A case report].

A case of cerebral schistosomiasis with a huge calcification and with a long clinical course was reported. A 39-year-old male developed ataxic gait gradually associated with headache and general fatigability. According to the past history, the patient had suffered from severe meningitis with disturbance of consciousness for 2 weeks when he was 10 years of age. Neurological examination revealed positive Romberg's sign and left homonymous hemianopsia. Plain craniogram showed a large calcification occupying the whole right occipital region. This calcification was lobulated in shape and was 11 X 9 X 9 cm in size. CT scan demonstrated calcification and remarkable hydrocephalus. This calcification had no mass effect. Left vertebral angiogram revealed the right parieto-occipital artery to be stretched and slightly displaced toward midline. Hematological analysis disclosed no abnormality except for eosinophilia (10%). Pre-operative diagnosis was an old calcified granuloma of unknown origin. For the purpose of biopsy, a right occipital craniotomy was performed. On reflecting the dura, fibrous adhesion and vascular network were seen between the inner table of the dura and the arachnoid. Subarachnoid space was enlarged and all cortical vessels were narrow in their diameters. By a corticotomy, a whitish-yellow calcified mass was seen in the depth of 2 mm. The yellowish gelatinous content poured out of the mass. A piece of calcification and a small amount of content were taken as specimen. Microscopic study of the calcified tissue showed many egg shells of schistosoma japonica, necrotic tissues and infiltration of small round cells.

[Preservation of the nipple and areola for breast cancer--histopathological study on cancerous involvement of the nipple and areola].

In 141 mastectomy specimens, performed for invasive or non-invasive carcinomas, histopathologic study was performed to assess the extent of nipple-areola involvement by the tumor. In this study, patients were excluded when 1) the tumor was located beneath the areola, 2) nipple and/or areola abnormalities were clinically present. Tumor involvement of the nipple and/or areola was found in 44 of 141 specimens (31%), with intraductal growth in 36 (82%) of 44, stromal invasion in 3 (7%) and ductal & stromal invasion in 5 (11%). Analysis of nipple-areolar involvement with consideration of the different variables indicates that it occurred in association with tumor size, tumor-areola distance and histological type. Such information provides clinically relevant guide lines in decision making for limited breast surgery.

[Total pelvic exenteration for primary and locally recurrent rectal cancer].

Thirteen patients with advanced carcinoma of the lower colon and no evidence of extrapelvic metastasis were submitted to total pelvic exenteration with urinary diversion. The operative mortality rate was 7.7%. Determinate 5-year survival rate of 40% was achieved. Local recurrence of rectal cancer following abdominoperineal resection is rarely amenable to limited resection. Six patients with deeply invading recurrent lesions had pelvic exenteration combined with sacral resection. This procedure seems a reasonable treatment for palliation and the chance of cure in selected patients. CT examination of the pelvis is very valuable for the early detection and localization of recurrence.

[Stromal sarcoma of the male breast].

A 63-year-old man who had been aware for 5 years of a tumor near his left nipple was admitted complaining of rapid tumor enlargement. He received standard mastectomy under the diagnosis of breast sarcoma. The regional lymph nodes were not involved and the excised tumor was 12.0 cm in diameter it showed a grayish-white cut surface and a sharp border. Histologically we made a diagnosis of stromal sarcoma of the breast showing diffuse proliferation of spindle shaped cells with high cellularity, moderate atypia and scattered mitosis. Postoperatively, he underwent chemotherapy with cyclophosphamide. Two years and five months after the operation, he is alive and well without any sign of recurrence.

[Pelvic exenteration combined with sacral resection for recurrent rectal cancer. The roles of carcinoembryonic antigen and pelvic computed tomography for early detection].

Local recurrence of rectal cancer following abdominoperineal resection is rarely amenable to limited resection. CT study of the pelvis revealed recurrent tumor images in relation to the adjacent pelvic structures. Six patients with deeply invading recurrent lesions were subjected to pelvic exenteration combined with sacral resection. No postoperative deaths were encountered. The postoperative sharp drops of serum CEA levels are useful for judging the adequacy of this procedure. The follow-up status of the 6 patients is as follows, respectively: 16 months dead from pulmonary & hepatic metastasis, 11 months and 9.5 months alive without symptoms but with slightly elevated CEA level, and 5 months, 2 months and 2 weeks alive with no evidence of disease. All patients who were relieved of perineal or sciatic nerve pain are now able to walk without any assistance or trouble. Total en bloc residual pelvic exenteration combined with sacral resection is a reasonable treatment for locally recurrent rectal cancer.
