Successful acute heart-lung transplantation after six hours preservation.

The lack of an appropriate and effective technique for prolonged heart-lung preservation has been one of the factors limiting the use of this therapeutic modality. Presently, the donor is transported to the transplant center, which is often an unacceptable burden to the donor's family. A reliable preservation technique that would allow long-distance procurement would markedly increase the use of heart-lung transplantation. Using a dog, a successful heart-lung transplantation was achieved following six hours of preservation of the graft. The experiment was terminated when the animal was able to maintain normal blood gases following extubation.

[Histopathological findings of the pituitary gland in cases of subarachnoid hemorrhage].

Histopathological change of the pituitary gland in cases of the central nervous system disorder, especially in the cases of subarachnoid hemorrhage, was studied. Clinical materials were 23 cases who were died from subarachnoid hemorrhage due to aneurysmal rupture in the acute stage. Histopathological change was examined not only of the pituitary gland but also hypothalamus and the change due to increased intracranial pressure (midbrain hemorrhage and uncal herniation). Main histopathological findings of the pituitary gland are anterior pituitary necrosis and they are found in 8 cases out of 23 subarachnoid hemorrhage cases (34.9%). Histopathological changes in hypothalamus were hemorrhage or infarction, and these changes were present in 16 cases (70%). Eight cases with pituitary necrosis had shown hypothalamic lesions. Three cases out of 23 cases (17.4%) showed midbrain hemorrhage and uncal herniation and 3 cases out of these 4 cases had anterior pituitary necrosis. According to the angiographic findings, vasospasm tended to be severe and widespread in cases of anterior pituitary necrosis, meanwhile vasospasm did not tend to be severe or widespread in cases without anterior pituitary necrosis. It was suspected that the mechanism of the development of pituitary necrosis is related not only to the circulatory disturbance of the portal vessels due to mechanical compression on the pituitary stalk and acutely raised intracranial pressure at the moment of aneurysmal rupture, but also circulatory disturbance of the portal vessels due to vasospasm.

[Effect of preoperative intra-arterial infusion of adriamycin on locally advanced breast cancer].

The effect of adriamycin (ADM) infused intraarterially as a preoperative procedure was analyzed retrospectively in 15 patients with primary advanced breast cancer. A high clinical response rate (CR+ PR) of 73% (11/15) was obtained by the treatment, and also remarkable degenerative changes of tumor cells were histologically noted in 10 out of 15 surgical specimens (67%). A correlation between the dose of ADM infused and the rate of tumor regression was observed. A significantly higher concentration of ADM was detected in metastatic lymph nodes and tumors obtained 3-4 weeks after intra-arterial infusion of ADM than that in normal mammary gland. As for side effects, alopecia, leukocytopenia, anorexia, nausea and vomiting occurred at high frequencies. The side effects due to this treatment were considered to be tolerable. Prognostically, a good survival rate was not observed using preoperative treatment with intra-arterial ADM infusion.

A syndrome with nodular erythema, elongated and thickened fingers, and emaciation.

A 5-year-old boy had a nodular erythema, elongated and thickened fingers, and emaciation. His condition was a rare congenital disease inherited as an autosomal recessive trait. Eleven cases have been previously reported in the Japanese literature. The onset is early in childhood, and nodular erythema is an essential and initial finding. Growth retardation and emaciation progress slowly with age. The characteristic clinical features include large eyes, nose, lips, and ears, disproportionately long and thick fingers, and the loss of adipose tissue from the upper half of the body. Cardiomegaly and hypertrophy of the periosteum of the phalanges have been described in some cases.

[Clinical studies on cefoxitin in the prevention of postoperative infections and the treatment of postoperative pulmonary and urinary tract infections].

This clinical trial was designed to evaluate the efficacy, safety and patient tolerance of cefoxitin (CFS) in 46 patients who were admitted to the hospitals from June 1983 to April 1984. The daily doses of CFX for 34 patients (ages ranged from 6 to 75 years old) were 2 to 8 g to prevent the infections and for 12 patients (ages ranged from 55 to 81 years old) were 2 to 6 g to treat the infections by intravenous drip infusion 1 or 3 times a day in divided doses. The following results were obtained. All of 34 patients with intracranial operation who received CFX for prevention of postoperative infections showed good results. Of 12 patients with postoperative pneumonia, infections of urinary tract and late meningitis, 11 patients showed good results. One patient was discontinued on the 3 days because of the drug eruption which improved 3 days after. The side effect was noted in only 1 patient. This was eruption which improved 3 days after the stop of the administration. The influences to the laboratory data due to CFX were not recognized. The results of this study demonstrated that CFX was an excellent drug for the prevention and treatment of the postoperative infections in the neurosurgical field because of high efficacy rate and safety.

[Current problems in mass screening for breast cancer].

As the trend in the incidences of morbidity and mortality from breast cancer in Japan has been going up in recent years, mass screening for breast cancer is being undertaken on a nationwide scale. In Tokushima prefecture, 111,571 women (49,319 in actual number) were subjected to mass screening with physical examination from 1970-1984. In 116 including 26 cases of so-called "interval cancer", breast cancer was detected. Those with cancer detected by screening were in the earlier stage of the disease as compared to those diagnosed through regular medical care at our out-patient clinic during the same period. In the breast cancers detected by screening, the classifications of tumor size and lymph node involvement were not significantly different among the initial screening cases, subsequent screening cases and interval cases. The detection rate was 0.08% of total examinees, having 0.13% in the initial screening and 0.04% in the subsequent screening. The cancer was detectable mostly in patients aged over 60 and there were few cases in the younger generation. Repeat examinees accounted for 55.8% with an annual increase, but the proportion of initial examinees remained unchanged. Women in their 30s and 40s accounted for about 64.2% of all examinees. The results suggest that procedures such as inspection and palpation are useful for early detection of breast cancer among Japanese women. It is important to emphasize the following items for further improvement in our program, older women should be encouraged to actively participate in the screening, and percentage of initial examinees should be increased. Furthermore, the current objectives of mass screening for breast cancer are: to determine an appropriate screening survey method, to organize an adequate follow-up system for women with abnormal findings, and to educate the examinees in breast self-examination.

Near-total gastrectomy.

Total gastrectomy has more frequent and serious postoperative complications than subtotal gastrectomy. An esophago-gastric stoma with a side-to-side anastomosis permits a near-total gastrectomy to be undertaken with virtually the same results as a total gastrectomy. This procedure is used for early gastric cancers that have widely spreading, single or multiple lesions, or lesions near the esophago-gastric junction, and for the cure or palliative resection of selected patients with advanced gastric cancer.

Surgical treatment of cavernous hemangioma of the liver.

This report describes our experiences with 12 cases of cavernous hemangioma of the liver. Eight patients were submitted to surgical procedures. Five out of eight underwent liver resections that ranged from simple excision to hepatic lobectomy. Three patients underwent hepatic artery ligation. There were no surgical deaths or any late deaths. Four more asymptomatic patients have been observed for up to 11 years (mean 4.0 years). None of the patients in this group has evidenced intensification of symptoms or any remarkable change of the size of lesions.

A He-Ne laser position-detector for recording jaw movements: principle of operation and application in animal experiments.

A new equipment for recording jaw movements in animals by use of a He-Ne laser and a phototransistor (PhT) array is described. The principle of operation of this equipment is that the beam angle of the He-Ne laser is continuously controlled to irradiate the center of the PhT array by a feedback mechanism composed of a microcomputer (CPU) and XY scanners. The driving currents sent from the CPU to the XY scanners are utilized for monitoring two-dimensional jaw movements. The linearity of this recording system is preserved both vertically and horizontally over a range of 11 mm from the center of the scanning field of the laser beam within less than 1.5%, which is satisfactory for monitoring rabbit jaw movements. When this equipment was applied to the rabbit, there was no sign of disturbance to the animal during chewing carrots.

Subtotal versus total parathyroidectomy with forearm autograft for secondary hyperparathyroidism in chronic renal failure.

Forty-three patients with chronic renal failure and secondary hyperparathyroidism underwent parathyroidectomy; 20 of the 43 underwent subtotal parathyroidectomy (Group A) and 23 patients underwent total parathyroidectomy and parathyroid autotransplant in the forearm (Group B). Postoperative clinical improvement was similar in both groups. In the immediate postoperative period eight patients in Group A who had severe bone changes and 21 patients in Group B needed supplemental calcium administration. The grafted tissues in all cases functioned well; reimplantation of the cryopreserved parathyroid tissues was unnecessary. One case in each group showed a recurrence. One patient in Group A was submitted to reexploration of the neck with a lateral approach. The other patient in Group B underwent excisions of the transplanted parathyroid tissues on three separate occasions under local anesthesia. The second operation was definitely easier and safer to manage after a total parathyroidectomy with autotransplantation to the forearm.

Frequency-dependent modulation of rhythmic human jaw movements.

Modulation of jaw movement patterns by alteration of the chewing rhythm was analyzed in nine subjects. Jaw movements were monitored by an electronic transducer system. The subjects were asked to chew gum either at voluntarily determined rates or at rates guided by a light flash that varied from 2 to 7 Hz at 1-Hz intervals. The results can be summarized as follows: Chewing was generally conducted at rates below 3 Hz when the rhythm was voluntarily determined. When the rhythm was guided, however, the maximum chewing rate could occasionally be raised up to 6 Hz; the duration of the chewing cycle was more closely related to changes in the duration of the opening and occlusion phases than to that of the closing phase; at chewing rates below 3 Hz, the maximum gape did not appreciably change with the chewing rhythm, while it decreased linearly at rates above 3 Hz; and at chewing rates below 3 Hz, the jaw opening velocity increased with speed of chewing, whereas it decreased or sometimes fluctuated over a wide range at rates above 3 Hz. It is concluded that, during normal mastication at rates below 3 Hz, chewing rhythm is altered by changing both the velocity of jaw opening and the duration of the occlusion phase.

Responses of the vascular-interstitial-lymph system to saline loading in the rat.

The responses of the vascular-interstitial-lymph system to saline loading were examined in rats by continuous monitoring of circulating blood volume and simultaneous collection of thoracic duct lymph. About 90% of the infused volume shifted out of the intravascular space, of which 72% was imbibed in the interstitial space, 21% was drained through the thoracic duct, and 7% was excreted in the urine by the 50th min after the infusion amounting to 4% of body weight. The drainage of lymph protein following the infusion was about 50% of that expected from the interstitial protein concentration and even distribution of the infused saline. This result can be explained partly by the increase of lymph from skin and muscle after the infusion.
