[Primary malignant lymphoma of the urinary bladder: report of a case].

A case of primary malignant lymphoma of the bladder is reported. A 73-year-old woman was admitted to our clinic with the chief complaint of asymptomatic gross hematuria on January 11, 1982. Preoperative examinations with cystoscopy, DIP, cystogram, CT scan, ultrasonography and transurethral biopsy suggested a non-epithelial tumor of the bladder. Partial cystectomy was performed on March 31, 1982. Histological diagnosis was diffuse lymphoma, medium-sized cell type by the classification of LSG (The lymphoma-leukemia study group of Japan). One month later, she was treated with VEPA therapy, which consisted of vincristine, cyclophosphamide (Endoxan), prednisolone and adriamycin. Postoperative course was uneventful and she has been doing well without any clinical evidence of recurrence during the period of 11 months following the operation.

[A case of female hypospadias].

A 20-year-old female with hypospadias was admitted to our clinic with the complaint of incontinence since her birth. The urethral orifice opened widely at the proximal roof of the vagina and the vesical mucosa was visible through the orifice by colposcopy. Although no anomaly was noticed in the upper urinary tract, there were complications of other congenital anomalies such as vaginal septum, bicolibicornus and ventricular septal defect. Urethroplasty according to Teller 's method gave improvement of the symptoms.

Hepatic gluconeogenesis from alanine following surgery.

To clarify alanine metabolism in the liver with special reference to gluconeogenesis, catheters were placed in hepatic, portal, femoral, and external jugular veins of six male mongrel dogs. A trace amount of 14C-alanine was administered as a single pulse into the external jugular vein on the first postoperative day, and the blood samples were taken from each vein for the subsequent two hours to measure 14C-glucose radioactivity. Cumulative radioactivity after 14C-alanine injection showed that 74 per cent of the radioactivity in whole protein-free serum was that of 14C-glucose. Therefore, it is considered that the metabolic pathway of alanine in the liver after surgery is mainly through gluconeogenesis to glucose. Our in vivo experiment clearly showed that hepatic gluconeogenesis from alanine is one of the important factors related to hyperglycemia after surgery.

Effects of diazoxide and hydrochlorothiazide on water permeability and sodium transport in the frog bladder.

The effects of diazoxide and hydrochlorothiazide on vasopressin-induced increments in osmotic water flow and sodium transport across the frog bladder were studied. Diazoxide enhanced the vasopressin-induced osmotic water flow of the bladder, but did not affect the cyclic AMP- or theophylline-induced water flow. Hydrochlorothiazide did not affect the vasopressin-induced water flow. Our results suggest that diazoxide increased the water flow by inhibiting the activity of phosphodiesterase in bladder epithelial cells, whereas hydrochlorothiazide did not. On the other hand, both drugs suppressed the short-circuit current of the bladder membrane and inhibited the Na,K-dependent ATPase activity of the kidney cells. These results suggest that both drugs decreased sodium transport in the bladder by inhibiting Na,K-dependent ATPase activity.

Cytosine arabinoside bladder instillation therapy for bladder tumors.

Bladder instillation of cytosine arabinoside (CA), 400 mg in 20 ml sterilized distilled water, was performed in 30 cases of bladder tumors. The instillation was done everyday to inpatients or 3 times a week to outpatients, for a total of 20 procedures. The total effective rate was 33%. Although the effectiveness of CA bladder instillation was less than that of mitomycin C and adriamycin, the combination of CA and these drugs might be advisable. Neither general nor local side effect was observed.

Immunochemotherapy for advanced bladder cancer using FT-207, adriamycin and OK-432.

A combination therapy consisting of daily intrarectal administration of 1,500 mg of FT-207 suppository, daily or every three day bladder instillation of 40 mg adriamycin for a total of 20 applications, and once per week intramuscular administration of 5 KE OK-432 was described. The therapy was performed in 30 cases of advanced bladder cancer. The effectiveness evaluated by Karnofsky's criteria was as follows: 8 cases of 0-0, 2 of 0-A, 1 of 0-B, 2 of 0-C, 7 of 1-A and 10 of 1-B, showing an effective rate of 57%. Local pain was observed in 20 cases (67%), pancytopenia in 1 (3%), hepatitis in 4 (13%), stomatitis in 5 (17%) and anorexia in 6 (20%). From this clinical trial, it is presumed that combination therapy administering FT-207 intrarectally, adriamycin intravesically and OK-432 intramuscularly might be used for the treatment of advanced bladder cancer provided sufficient care is taken of general side effects.

[Periodic prophylactic bladder instillation (PPI) of mitomycin C].

The effect of preoperative bladder instillation (POI) and periodic prophylactic bladder instillation (PPI) of anticancerous drugs was evaluated in connection with the prevention of recurrent bladder tumors after surgery. A total of 191 patients with pTa or pT1 tumors including patients submitted to TURbt and partial cystectomy from January, 1967, to December, 1980, was chosen for the study. They were divided into the following 4 groups: Group A (49 cases) was treated with PPI and POI, Group B (11 cases) with PPI but not POI, Group C (46 cases) with POI but not PPI and Group D (85 cases) with neither PPI nor POI. POI was performed three times a week for a total of 20 applications of anticancerous drugs from two months before surgery. PPI was performed twice a month from one month after TURbt or parital cystectomy with a combination of 20mg of Mitomycin C and 1,000mg of 5-FU. The non-recurrence rate in these 4 groups was estimated by the actuarial method. The 3 year non-recurrence rates in Groups A,B,C and D were 95.4%, 90.9%, 44.0%, and 45.6%, respectively. The 5 year non-recurrence rates in Groups A,B,C and D were 82.4%, 81.0%, 32.0% and 35.1%, respectively. It is presumed from our study that PPI was effective in preventing the recurrence of bladder tumors. In comparison, POI showed a very limited effect and only in the first two years after surgery. No carcinogenic action on the bladder epithelium was observed from the topical use of Mitomycin C.

[Nuclear magnetic resonance (NMR) imaging of brain edema from sliced rat brain: preliminary report].

A series of the sliced rat brain were imaged by proton nuclear magnetic resonance (1H-NMR) using a Carr-Purcell-Meiboom-Gill sequence. The slices were obtained from the adult male Wistar rats, normal or suffering from vasogenic brain edema at 2, 6, 24Hr, 2Wk and 2Mo period after the application of cryo-injury. The imaging time was arranged 17 to 22 minutes dependently upon signal to noise ratio. The slice thickness was 1.2 mm, and pixel dimensions were 0.2 X 0.2 mm. Whereas a voxel size in our images is mere 1/1,200 compared to that reported about the prototype human NMR imaging devices, high resolution has been realized. The spatial resolution is very fine, as evidenced by the appearance of major internal structures of rat brain, and the object contrast is so high that cerebral white-gray contrast is excellent, although the difference in water concentration between them is only 7%. It has been impossible to measure serially in vivo change in water concentration of small organs as rat brain, NMR has the capability to generate images in response to tissue state of hydration, therefore we can easily recognize the extent and intensity of the brain edema, which has been defined as an expansion of the brain volume resulting from an increase in its fluid content. And in this study, using the prototype mini-NMR-CT of excellent spatial and contrast resolution, we have been able to show this advantage of NMR as a new tool for research in the field of neurological surgery.

Glucocorticoid receptors in rat kidney cortical tubules enriched in proximal and distal segments.

Cytoplasmic and nuclear binding of [3H]triamcinolone acetonide was assessed in isolated rat kidney cortical tubules, enriched in distal (fraction A) or in proximal segments (fraction B). The concentration dependence of specific [3H]triamcinolone acetonide binding in cytoplasm was determined (range = 4.4 X 10(-10) to 2.1 X 10(-7) M) and analyzed by a least-squares curve-fitting method. A single, high-affinity binding class with a dissociation constant of 1 X 10(-8) M (25 degrees C) was obtained in both fractions A and B. Based on competition for the [3H]triamcinolone acetonide sites, the following sequence of affinities was obtained: triamcinolone acetonide = dexamethasone > progesterone = corticosterone > d-aldosterone > 17 beta-estradiol. These specificities imply that these sites are glucocorticoid receptors. Fraction B contained 1.6 times more cytosol sites for [3H]triamcinolone acetonide than fraction A (5.0 +/- 0.5 X 10(-13) vs. 3.0 +/- 0.5 X 10(-13) mol/mg protein). In the presence of a onefold excess of d-aldosterone specific cytoplasmic binding of [3H]triamcinolone acetonide was 1.4-fold greater in fraction B than in fraction A, and specific nuclear binding was 1.3-fold greater in fraction B than in fraction A (5.1 +/- 0.6 X 10(-13) vs 4.0 +/- 0.5 X 10(-13) mol/mg DNA). These results and the measured lengths of proximal and distal tubules yielded estimates of a higher proximal content (three- to sixfold) compared to distal content of glucocorticoid receptors.

Levels of carbenicillin in serum and prostatic tissue after intravenous administration in patients with benign prostatic hyperplasia.

Carbenicillin (CB-PC) was administered intravenously (5 gm over one-half hour) to 25 patients suffering form benign prostatic hyperplasia. Prostatic biopsies and sera were obtained one, two, three, four, and five hours later for histology and biochemical analyses of the serum and prostatic tissue levels of CB-PC. The serum and prostatic tissue levels of carbenicillin attained were sufficient to eradicate gram-negative bacteria within the five time intervals studied. The ratio of prostatic tissue level of CB-PC to the serum level of this agent increased in proportion to the grading of round cell infiltration of the prostatic tissue. The results of this study point to the clinical usefulness of CB-PC in the treatment of chronic and acute prostatitis.
