An epidemiological survey of prostatic cancer from the Annual of the Pathological Autopsy Cases in Japan.

One thousand two hundred and ten cases of prostatic cancer from the Annual of the Pathological Autopsy Cases in Japan (1958--1975) were collected and subjected to statistical studies. The number of deaths caused by prostatic cancer increased yearly. The age distribution showed a peak in 70--74 years. Neither particular diseases nor cancers were observed to be associated with prostatic cancer. The incidence of liver cirrhosis was significantly lower in prostatic cancer cases than in all male autopsy cases in the Annual. The lymphnodes, the bones and the lungs were the most frequent metastatic regions for prostatic cancer. Histologically, 85% of the cases were adenocarcinoma.

Adriamycin instillation therapy for bladder tumors.

Topical instillation of Adriamycin, 40 mg in 20 ml sterilized distilled water, was performed in 20 cases of bladder tumors every day for two weeks. In 8 cases (40%) the tumors disappeared completely, while in 5 cases (25%) tumors were reduced in size, but in 7 cases (35%) there was no effect. Therefore, the rate of effectiveness was 65%. The local bladder reaction and urethral pain were noted in all cases, as a result of which the therapy was interrupted in 2 cases. Abnormal values of red blood cells, white blood cells and platelets were not observed. In 3 cases the serum level of Adriamycin was only trace 1 or 2 hr after instillation. In conclusion, Mitomycin C was thought to be superior to Adriamycin as far as the effectiveness and irritability on the normal bladder epithelium were concerned.

Absorption of 5-fluorouracil through gastric epithelium.

We examined the amount of 5-Fu absorbed through the gastric epithelium after oral administration in both dogs and patients with bladder tumors. The concentrations of 5-Fu in the serum, bladder wall, tumor mass and regional lymphnodes were sufficiently high to suppress the growth of tumor cells.

Staging of bladder tumors by means of transrectal ultrasonography.

Transrectal ultrasonography is a specially devised technique to obtain clear tomograms of the intrapelvic organs, such as the prostate, urinary bladder and seminal vesicles. We used this technique to evaluate 64 patients with bladder tumors. The results were compared with histological findings after exploratory surgery. Ultrasonograms of these tumors were estimated on the basis of bladder wall irregularity, bladder deformity, bladder capacity and extravesical spread of tumor. Each tumor stage showed characteristic ultrasonic features. Extravesical spread of tumor, which was observed in 67 percent of stage D tumors, proved to be an important finding to distinguish stage D tumours from other tumor stages. We suggest these features as diagnostic criteria for ultrasonic staging of bladder tumors.

Mass screening program for prostatic diseases with transrectal ultrasonotomography.

A special chair-type apparatus for transrectal ultrasonotomography was developed in our laboratory 3 years ago. We did a mass screening program for prostatic disease, using this equipment as the primary study in the system. The 132 apparently normal men evaluated ranged in age from 40 to 76 years (mean 55.0). The 48 patients suspected of having prostatic disease underwent secondary studies, consisting of the usual urological examinations. Prostatic disease was detected in 24 cases, including 18 cases of benign prostatic hyperplasia. Thus, the model program revealed an extraordinarily high prevalence of latent prostatic disease among apparently normal older men.

Ultrasonic diagnosis of intrascrotal contents.

Our brief experience of the use of ultrasonography and ultrasonic Doppler method for the diagnosis of intrascrotal contents were described. Five cases of testicular tumor, two cases of hydrocele and one case of epididymal tuberculosis were successfully diagnosed by ultrasonography. Only one case of chronic inflammation was misdiagnosed as a testicular tumor. Two cases of testicular torsion and two cases of acute epididymitis were easily differentiated by the ultrasonic Doppler method. We suggest that this is the most reliable technique for differentiation between these two diseases.

The inhibitory effect of reserpine on the active sodium transport across the frog bladder.

1. The effects of reserpine and harman derivatives on the sodium transport across the frog bladder were examined using a short-circuit current method. The effects of harman derivatives on the Na,K-ATPase activity of the frog kidney were also investigated. 2. Reserpine and harman derivatives inhibited active sodium transport of the frog bladder and their inhibitory effect decreased as reserpine greater than harmine greater than harmaline = harman greater than harmalol. 3. Harman derivatives inhibited Na,K-ATPase activity of the frog kidney. 4. These results suggest that reserpine and harman derivatives inhibit active sodium transport by suppressing the Na,K-ATPase activity of the frog bladder.
