Barium meal study for amyloidosis of the small intestine: measurements on radiograph.

In order to determine barium meal radiographic findings characteristic of amyloidosis, we measured the jejunal diameter, valvular width, and intervalvular distance in 25 patients with small bowel amyloidosis and in 30 control individuals, and compared the two groups with each other. As a result, jejunal diameter demonstrated no difference between amyloidosis cases and controls, while there was a significant difference in valvular width or intervalvular distance between the two groups. Our results indicate that objective estimates of the thickening of the valvulae and the shortening of the intervalvular distance by means of measurements of the given roentgenograms greatly contribute to the clinical diagnosis of this disease.

Ultrasonic and CT findings in penicillin-induced nonpseudomembranous colitis.

The findings of ultrasonography (US) and computed tomography (CT) of the abdomen in two cases of nonpseudomembranous colitis (NPMC) induced by penicillin are described. Both imaging methods revealed diffuse thickening of the colon in patients who were endoscopically diagnosed as suffering from this disease. The clinical and radiographic features of this entity are presented along with a brief review of pertinent literature.

Radiographic demonstration of longitudinal ulcers in patients with ulcerative colitis.

During the past 10 years, 122 patients with ulcerative colitis were diagnosed by double-contrast barium enema and colonofiberscopy with endoscopic biopsy. Among them, five patients (4%) had longitudinal ulcers and eccentric deformities in the colon. Other radiologic findings included thumbprinting (two cases), sacculations (two cases), and inflammatory polyps (four cases). The possibility of the concomitance of ischemic colitis in cases of ulcerative colitis is discussed, due to their radiographic similarities.

Radiation-induced esophageal cancer: a case report and a review of the literature.

A 63 year old woman who had been irradiated for thyroid carcinoma at 36 years of age developed a cervico-esophageal stricture, the biopsy specimens from which revealed squamous cell carcinoma. Total esophagectomy was performed and a small linear ulcer was found in the resected specimen. Histological examination revealed moderately differentiated squamous cell carcinoma with severe fibrosis around the tumor, indicating radiation-induced cervical esophageal cancer. The characteristics of radiation-induced esophageal cancers described in the literature are reviewed in context of the present case.

The effects of hyperthermia on the cell cycle of Ehrlich ascites cancer cells in vivo.

The effects of hyperthermia on the cell cycle of Ehrlich ascites cancer cells were studied, and these effects simultaneously evaluated in terms of prolonging the survival of test mice inoculated with tumor cells from heat-treated mice. DDY mice bearing Ehrlich ascites cancer cells were placed in a water bath at 37 degrees C, 39 degrees C, 41 degrees C, 42 degrees C. The heating of mice at 41 degrees C, 42 degrees C and 43 degrees C induced the accumulation of cancer cells at the G2M phase of the cell cycle with many cells exhibiting polyploidy (16 C). The extent of accumulation increased as the temperature of incubation was raised, however the interrupted cell cycle resumed 120 hours after heating. The retransplantation of cells from the heat-treated mice revealed that the mice which were inoculated with Ehrlich ascites cancer cells from mice heated at 43 degrees C survived longer, while the mice which were inoculated with Ehrlich ascites cancer cells from mice heated at 39 degrees C survived for only a slightly shorter time than those which were inoculated with cells from mice heated at 37 degrees C.

Automated measurement of trypsin inhibitor in urine with a centrifugal analyzer: comparison with other acute phase reactants.

Automated measurement of trypsin inhibitor in urine was performed with good precision using the COBAS FARA. Elevated levels of both trypsin inhibitor in urine and acute phase proteins in serum were shown in most cases of major abdominal surgery. We suggest that the automated assay of urinary trypsin inhibitor might be useful for the clinical diagnosis of acute phase response.

FGF modulates the PDGF-driven pathway of oligodendrocyte development.

PDGF promotes the growth of oligodendrocyte type-2 astrocyte (O-2A) glial progenitor cells and allows their timely differentiation into oligodendrocytes, the CNS myelin-forming cells. We demonstrate that basic FGF is a potent mitogen for brain O-2A progenitor cells, but blocks their differentiation into oligodendrocytes. Treatment with basic FGF also influences the level of expression of PDGF receptors on O-2A progenitor cells. These cells express only the alpha chain PDGF receptor, and the levels of PDGF alpha receptors decrease as the cells differentiate. In contrast, basic FGF maintains a high level of functionally responsive PDGF alpha receptors in O-2A progenitors. Thus basic FGF activates a signaling pathway that can positively regulate PDGF receptors in O-2A progenitor cells. In this way basic FGF or an FGF-like factor may modulate the production of myelin-forming cells in the CNS.

Establishment and characterization of a clonal line of parathyroid endothelial cells.

We have isolated endothelial cells derived from bovine parathyroid tissue. These cells have been cloned and maintained by serial passage for more than 40 months without showing signs of senescence. Prolonged culture was accomplished by using a medium favoring endothelial cell growth and methods for enriching endothelial cells in primary culture. The cloned parathyroid endothelial cells contained factor VIII-related antigen, took up acetylated low-density lipoproteins and parathyroid hormone, and showed morphological features comparable to other endothelial cells. Bovine parathyroid endothelial cells replicated with a mean doubling time of 65 h. Fibroblast growth factors, platelet-derived growth factor, and calcium acted as mitogens for parathyroid endothelial cells, whereas transforming growth factor beta inhibited proliferation.

Death from asthma in children.

We have recorded 29 cases of death from asthma in children who had visited the Department of Pediatrics of Doai Memorial Hospital between 1965 and 1987. These cases have been analyzed. They were 18 males and 11 females. The mean age at death was 12.4 years and we found that in later childhood and adolescence the age at death had been increasing since 1973. The duration from final attack to death was very short: less than 1 hour: 21% and less than 2 hours: 50%. About half of the cases could not reach the hospital. Ten cases died within one year after cessation of long-term systemic use of steroid. Severe attacks of asthma were particularly likely to occur during the reducing period or following cessation of these drugs. The importance of adequate and vigorous initial therapy in severe asthmatic attacks should be emphasized. The causes of death are as follows: asphyxia: 23 cases (one complicated by aspiration, one withdrawal syndrome of steroid, one subendocardial haemorrhage and two pneumothorax cases), anaphylactic shock to ACTH: 1 case, shock to stimulant: 1 case, following emergency operation: 1 case and unknown: 3 cases. Asthma death in children in Japan is reviewed.

Cytokines and osteoporosis.

Bone remodeling is controlled by systemic factors such as parathyroid hormone (PTH), calcitonin (CT), and 1,25(OH)2 vitamin D and by local factors including cytokines and growth factors such as IL-1, IL-2, TNF alpha, TGF beta, IFN alpha, and IFN gamma. Derangement of such control mechanisms leading to an imbalance between osteoclastic bone resorption and osteoblastic bone formation could cause osteoporosis. Conditions associated with immune dysfunction such as aging, corticosteroid therapy, and rheumatoid arthritis are associated with osteoporosis, which is also more common in females than in males, like most of the autoimmune-collagen diseases. Peripheral lymphocyte subsets CD4/CD8 were higher in patients with senile osteoporosis than in the age-matched controls, and returned to normal after 1 month of 1 alpha(OH)vitamin D3 treatment. On multiple regression analysis of histomorphometric data and lymphocyte subsets, a negative correlation was found between CD4 lymphocytes and bone resorption. High CD4 is thus associated with a low level of osteoclastic bone resorption or low turnover osteoporosis. Plasma interferon reflecting macrophage function decreased with advance in age and increased in response to 1 alpha(OH)D3 treatment. As one of the immunoregulators, vitamin D tends to stimulate the macrophage-natural killer system and suppress the lymphocyte system, stimulating TGF beta and TNF alpha activity. Senile osteoporosis of low turnover thus appears to be associated with vitamin D deficiency, low macrophage function, high CD4 lymphocyte proportion, low IL-1 and high IL-2 activity, low IFN alpha and high IFN gamma activity, and low TGF beta and TNF alpha activity. Treatment with vitamin D derivatives tends to reverse these changes.

The acetylcholine-hypersensitive model of diabetic KK-CA(y) mice analyzed with pulse rate, and modified by repeated cold stress and by adrenalectomy.

The effects of acetylcholine on pulse rate were examined in the conscious state of diabetic KK-CA(y) male mice. By subcutaneously-administered acetylcholine at 3 and 10 mg/kg the pulse rate was increased in the prediabetic state, whereas it was decreased in the diabetic state. The acetylcholine-induced responses in pulse rate were significantly and multiply correlated with both the blood glucose level and the dose of acetylcholine in KK-CA(y) mice (r = 0.47, p less than 0.01). The diabetic state and adrenalectomy produced an acetylcholine-hypersensitive syndrome, respectively. The hypersensitivity to acetylcholine in a whole body of diabetic mice was attenuated by repeated cold stress producing acetylcholine-subsensitivity, and was enhanced by adrenalectomy. The diabetic KK-CA(y) mice are an acetylcholine-hypersensitive animal model.

Increase in the activities of plasma pseudocholinesterase dependent on the blood glucose level and its relation to the hypersensitivity to acetylcholine in striated muscles of KK-CAy mice with diabetes.

Acetylcholinesterase activity and pseudocholinesterase activity were examined in plasma and in striated muscles (whole heart and diaphragm muscles) of diabetic KK-CAy mice. Both activities of acetylcholinesterase in heart muscle and pseudocholinesterase in plasma were significantly increased in diabetic KK-CAy mice compared to pre-diabetic KK-CAy mice. Both acetylcholinesterase and pseudocholinesterase activities in skeletal muscle were not changed by the diabetic state. The increases in activity of plasma pseudocholinesterase was significantly correlated to the increase in blood glucose level in alloxan-, streptozotocin (STZ)-diabetic ddY mice and diabetic KK-CAy mice. The increase was not correlated to the body weight in non-diabetic female-KK-CAy mice. Furthermore, the activity of heart acetylcholinesterase was significantly correlated with the activity of plasma pseudocholinesterase (r = 0.79, P less than 0.01). The activities of acetylcholinesterases in heart muscles from STZ- and alloxan-diabetic ddY mice also tended to increase. The hypersensitivity of the pulse rate to a low dose (1 mg/kg) of acetylcholine was correlated to the activity of plasma pseudocholinesterase (r = -0.51, P less than 0.05). These results demonstrate that the activities of plasma pseudocholinesterase were increased by the diabetic state being associated with the increasing alteration of cardiac sensitivity to acetylcholine in the whole body.

Replacement of the lumbar vertebrae of sheep with ceramic prostheses.

We prepared a prosthesis for the replacement of the lumbar vertebrae of sheep, using apatite- and wollastonite-containing glass-ceramic. The material is stronger than human cortical bone and has the special feature of chemical bonding to bone. Ten sheep underwent replacement of L3 and L4 vertebrae, without bone grafting. The animals were killed at intervals from three months to 27 months after operation, and the interface between the prosthesis and bone was examined radiologically, histologically and crystallographically. Bone bonding with the prosthesis had occurred in half the implants. It took at least one year for bonding to be complete, but an apatite layer on the surface of the prosthesis was observed as early as three months after the operation, suggesting the possibility of much earlier bone bonding if more rigid fixation of the prosthesis had been provided.

[Effects of cisapride on gastric emptying].

To evaluate the effects of cisapride on gastric emptying, gastric emptying was measured in six healthy volunteers using radioisotopic techniques which were established three areas of interest, i.e. the whole stomach, the proximal stomach and the antrum. Following results were obtained. 1. At the half emptying times, cisapride induced a more rapid gastric emptying than did placebo in 5 of 6 volunteers. 2. By analyzing gastric emptying curves obtained from three areas of interest, it was suggested that cisapride enhanced the transfer of gastric contents from the proximal stomach to the antrum together with the antral emptying, so that gastric emptying was accelerated.

[Effect of AS-4370 on gastric emptying--double-blind clinical pharmacological study].

The effect of AS-4370 given in single doses of 2.5, 5 and 10 mg on gastric emptying was investigated using 99mTc-DTPA in healthy volunteers in the placebocontrolled double-blind crossover design. There was no difference in percent activity throughout the period of measurement between AS-4370 2.5 mg and placebo. The percent activity decreased after administration of 5 and 10 mg of AS-4370 in comparison with placebo: the decrease was found during the period from 10 to 18 minutes after start of recording at 5 mg and from 8 to 18 minutes after start of recording at 10 mg. These results demonstrated that AS-4370 at these dose levels apparently accelerated gastric emptying. The analysis of the half emptying time by the crossover method showed that only the time effect was found at 2.5 mg, whereas the half emptying time was obviously shortened at 5 and 10 mg. There was no difference between 5 mg and 10 mg. There was no finding that might warrant a caution in terms of safety. In conclusion, AS-4370 did not affect gastric emptying at 2.5 mg but apparently accelerated gastric emptying at 5 and 10 mg in healthy volunteers.

Improvement of semen characteristics after surgical repair of bilateral testicular varicocele as compared to unilateral varicocele patients.

The operative treatment for surgical repair of bilateral testicular varicoceles was evaluated in terms of the improvement achieved in the sperm density and percentage progressive motile sperm. These parameters were compared for 27 cases of bilateral varicocele diagnosed by palpation and subjected to bilateral high ligation of the internal spermatic vein and for 40 cases of left unilateral testicular varicocele. There were no significant differences in preoperative values (such as sperm density; progressive motile sperm; serum levels of luteinizing hormone, follicle-stimulating hormone, and testosterone; and Johnson's score count) between the bilateral and unilateral testicular varicocele groups. The improvement in postoperative sperm density in the cases with bilateral testicular varicocele was apparently less satisfactory than in the cases with testicular varicocele only on the left side.
