Simultaneous measurement of intestinal blood flow and plasma levels of vasoactive substances in a case of early dumping syndrome. Case report.

Increase in intestinal blood flow was observed in a patient with postprandial hypotension. Simultaneous measurement of hemodynamic parameters and plasma levels of gut hormones revealed that the release of various vasoactive substances and resulting vasodilation of abdominal organs were responsible for the vasomotor changes of early dumping syndrome.

[Ectopic salivary amylase-producing IgA- lambda-type multiple myeloma with expression of MDR-1/P-glycoprotein].

We report a case of ectopic salivary amylase-producing IgA- lambda-type multiple myeloma. A 70-year-old man was admitted because of anemia and renal failure. After chemotherapy for eight months, the serum amylase markedly increased. Amylase activity in the supernatant of cultured myeloma cells, which were obtained from the bone marrow, also increased. The myeloma cells expressed MDR-1/P-glycoprotein). The case implies the association of drug resistance and the ectopic amylase production in a case of multiple myeloma.

[Effects of single ethanol administration on liver regeneration in rats].

The effects of a single ethanol administration on liver weight gain, RNA synthesis, DNA synthesis, and polyamine metabolism as parameters of liver regeneration in rat liver after partial hepatectomy were studied. Ethanol, given 1 hr before partial hepatectomy at the dose of 3 g/kg, inhibited RNA synthesis 4 hr after partial hepatectomy and DNA synthesis 24 hr after partial hepatectomy in the liver, but did not affect liver weight gain. Further, we studied the effects of a single ethanol administration on polyamine metabolism in order to elucidate the mechanism of the inhibition of liver regeneration by ethanol. Ethanol inhibited the increase in the putrescine level in the liver 4 and 8 hr after surgery, which was a metabolic product of ornithine decarboxylase. But spermidine and spermidine levels in the liver were not affected by ethanol administration. Hepatic spermidine N1-acetyl transferase activity increased by ethanol administration 6 hr after partial hepatectomy in compensation for the suppression of putrescine. Judging from these results, a single dose of ethanol inhibited liver regeneration polyamine metabolism after partial hepatectomy in rats.

[Resin adhesion on the young permanent ground enamel (Report 3)].

The purpose of this study was to investigate the influence of thermal cycling on the adhesion of the resin on the young permanent ground enamel which was etched with different etching times. Labial surfaces of 50 extracted and frozen bovine mandibular young permanent incisors were used. The etchant used in this study was 40% phospholic acid gel and the etching times were 0, 10, 20, 30 and 60 seconds. All of the specimens were washed with an air water spray after etching. The bonding agent and composite resin used in this study were Photo Bond and Photo Clearfil A (Kuraray Co.). After thermal cycling tests of 10,000 temperature cycles between 60 degrees C and 4 degrees C water baths, shear bond strengths on the ground enamel were measured. After the shear bond strength tests, all the test surfaces of the enamel and resin specimens were observed using the SEM. The results of this study were compared with the previous results which were not thermal-cycled. The following conclusions were obtained. 1) The etching time which showed the highest bond strength was 30 seconds (45.21 +/- 8.49 Mpa). 2) When the enamel was etched with all of the etching times, the bond strength were significantly higher than that of the enamel without etching. 3) In the etched groups, the bond strength with 20 seconds of etching time was significantly lower than those with 10, 30 and 60 seconds of etching times. 4) In the 10, 30 and 60 seconds etching times, the bond strengths with thermal-cycled groups were significantly higher than those without thermal-cycled groups. 5) On the enamel specimens after the shear bond strength tests, the frequency of the clear prism structures observed on the enamel was higher in the groups which showed higher bond strengths. However, the differences were not significant.

Endoscopic removal of inflammatory fibroid polyps of the stomach.

Three cases of gastric inflammatory fibroid polyp removed by endoscopic polypectomy are reported. All lesions were located in the gastric antrum or prepyloric region, and their depth was restricted to the submucosa. The correct diagnosis was obtained, and the digestive symptoms disappeared after polypectomy. In contrast to observations in previous reports, we believe these lesions are suitable for removal by endoscopic means, provided they are small in size and strictly submucosal in location.

[Ultrastructural abnormalities of spermatozoa in infertile male patients and their implications in fertilizing capacity in vitro].

The purpose of this study was to clarify the relation between ultrastructural abnormalities of spermatozoa and the fertilizing capacity of the spermatozoa in vitro. The infertile male patients who underwent in vitro fertilization were divided into two groups: the no fertilization group (NFG, n = 20) and the fertilization group (FG, n = 18). Morphological features of the spermatozoa in the two groups were studied by transmission electron microscopy (TEM). 1) The percentage of normal heads in the FG was significantly higher than that in the NFG (27.9 +/- 15.4% vs 9.6 +/- 13.3%, p less than 0.001). The percentage of abnormal acrosomes (62.3 +/- 16.4% vs 78.9 +/- 17.6%, p less than 0.01) and nuclear defect of spermatozoa (14.5 +/- 7.2% vs 24.9 +/- 13.3%, p less than 0.01) in the FG was significantly lower than that in the NFG. 2) The percentage of normal tails in the FG was significantly higher than that in the NFG (64.9 +/- 20.2% vs 47.7 +/- 18.9%, p less than 0.05). The percentage of deleted peripheral microtubules was significantly lower in the FG (9.3 +/- 5.9% vs 15.8 +/- 9.4%, p less than 0.02). 3) Three effective variables, normal heads, deleted peripheral microtubules, and nuclear defects, were selected by the forward selection method to discriminate the two groups. The discriminant analysis by means of these three variables showed that apparent accuracy was 84%. The present study demonstrated that the ultrastructural analysis by TEM makes it possible to show more precisely the abnormalities of acrosomes and tails of spermatozoa. This may be useful in evaluating the fertilizing capacity of spermatozoa in male infertility.

Endoscopic findings in carcinoid tumor of the duodenum.

The endoscopic findings in six patients with carcinoid tumor of the duodenum were analyzed. Radiographic and endoscopic examinations revealed a smooth and round elevation (measuring 5-20 mm in diameter) in the duodenal bulb. In three patients, the lesion was accompanied by an irregularly shaped erythematous depression, a characteristic endoscopic finding of this tumor. Because the endoscopic forceps biopsy technique led to an accurate diagnosis, we wish to stress the clinical value of the endoscopic modalities.

Endoscopic removal of gastric lipoma: diagnostic value of endoscopic ultrasonography.

A 47-yr-old male with gastric lipoma is presented. X-ray and endoscopic examinations revealed a submucosal tumor on the posterior wall of the gastric antrum. Endoscopic ultrasonography demonstrated the hyperechoic mass in the submucosa without continuity to the muscularis propria, by which the lesion was diagnosed as lipoma. The lesion was successfully removed by endoscopic polypectomy without complications. The cross-sections of the removed specimens were quite consistent with the ultrasonographic findings. Endoscopic ultrasonography is valuable in assessing the exact extent of the tumor and in determining whether or not endoscopic polypectomy is called for.

Chimeric alpha- and beta-platelet-derived growth factor (PDGF) receptors define three immunoglobulin-like domains of the alpha-PDGF receptor that determine PDGF-AA binding specificity.

Binding of platelet-derived growth factor (PDGF) to its cell surface receptors stimulates a variety of biochemical and biological responses. Two receptor gene products (designated alpha and beta) have been identified, and the different binding affinities of various PDGF isoforms for these receptors are prime determinants of the spectrum of responses observed. The beta receptor binds PDGF-BB, but not PDGF-AA, while the alpha receptor binds PDGF-AA and PDGF-BB. We utilized these different ligand binding specificities to investigate the PDGF-AA binding site in the human alpha-PDGF receptor by constructing chimeric molecules between the human alpha- and beta-PDGF receptors. Our results demonstrate that amino acids 1-340 of the alpha-PDGF receptor comprise the region that confers PDGF-AA binding specificity. This region corresponds to immunoglobulin-like sub-domains 1, 2, and 3 of its external domain.

Effects of ebselen (PZ51) on ischaemic brain oedema after focal ischaemia in cats.

Using a transorbital middle cerebral artery (MCA) occlusion model in cats, we evaluated the anti-oedema effects of a new anti-inflammatory agent, ebselen (PZ51), on ischaemic cortical oedema caused by prolonged ischaemia and recirculation. Local cerebral blood flow was measured by the hydrogen clearance method in the MCA territory and the corresponding cortical specific gravity was assessed by a microgravimetric technique. Ebselen had no significant effect on normal and ischaemic lCBF, while it significantly ameliorated post-ischaemic hypoperfusion following recirculation. In the severely ischaemic regions, microgravimetry showed the beneficial effects on the ischaemic oedema caused by prolonged ischaemia and recirculation as well. Although the exact site of action is undetermined in this study, the observed effects of ebselen may be ascribed to this agent's broad-spectrum of anti-inflammatory activities.

Effects of succinylated concanavalin A on infectivity and syncytial formation of human immunodeficiency virus.

The effects of various lectins on the infectivity of human immunodeficiency virus (HIV) type 1 was investigated. Among the 25 lectins investigated, 2 types of concanavalin A (Con A) and 3 types of phytohemagglutinin were found to inhibit HIV infection. Succinylated Con A (S-Con A) efficiently blocked HIV-induced formation of syncytia in a coculture of MOLT-4 cells and blocked cell-free infection by HIV of MT-4 cells. The HIV-binding study revealed that S-Con A only partially inhibited viral binding to cells, although the control Leu-3a monoclonal antibody strongly inhibited it. When S-Con A was added to cultures after the initiation of viral adsorption, the number of HIV antigen-positive cells that developed depended on the time interval before addition of the compound. S-Con A inhibited HIV infection even after viral binding to cells at 0 degrees C and further incubation at 37 degrees C for 1 day. These data suggest that S-Con A inhibited mainly the fusion process rather than viral binding to cells in exerting its anti-HIV activity.

The value of double-contrast study of the small intestine in immunoproliferative small intestinal disease.

Radiological findings of 2 patients with immunoproliferative small intestinal disease were studied. One of the cases was associated with alpha-chain disease. Both patients complained of severe diarrhea, and diagnosis was made by endoscopic biopsy of the duodenum and jejunum. Double-contrast studies of the small intestine and hypotonic duodenographies in these patients revealed thickened folds and innumerable fine granular elevations without ulcerations or luminal narrowings. Double-contrast studies are one of the most important examinations in the diagnosis of this disorder.
