Effect of the inhibitor of angiotensin I converting enzyme on endocrine function and renal perfusion in haemorrhagic shock.

The present study was undertaken to investigate the effect of inhibition of angiotensin I converting enzyme on endocrine responses and renal perfusion in haemorrhagic shock. Plasma levels of vasopressin (VP), aldosterone, and plasma renin activity (PRA), and renal cortical tissue blood flow were measured both in control dogs and in those treated with an inhibitor of angiotensin I converting enzyme (SQ20881). The drug was administered with an intention of preventing vasoconstriction induced by angiotensin II and to improve tissue perfusion. No significant differences were found in plasma levels of vasopressin, aldosterone and plasma renin activity between control and SQ20881-treated groups. Secondary elevation of blood pressure following haemorrhage was significantly delayed and reduced by the administration of the inhibitor. Decrease in renal cortical tissue blood flow was observed in the SQ20881-treated group. It is suggested that angiotension II appears to play a role in spontaneous recovery of arterial blood pressure following haemorrhage. Furthermore, angiotensin II does not seem to play an important role in the stimulation of secretion of vasopressin in response to haemorrhage. Our data failed to demonstrate any favourable effects of inhibition of angiotension I converting enzyme on renal perfusion during haemorrhagic shock in dogs.

Effects of morphine and ketamine anaesthesia and surgery on plasma concentrations of luteinizing hormone, testosterone and cortisol in man.

Thirty patients received morphine and 27 received ketamine. Morphine anaesthesia did not influence plasma concentrations of luteinizing hormone (LH) significantly, but there was a significant increase during ketamine anaesthesia. Plasma LH increased in the early stage of surgery under both anaesthetics. A significant decrease in the plasma concentration of testosterone was found during and following both techniques of anaesthesia, the lowest concentration being detected on the first day after surgery. Ketamine anaesthesia, but not morphine, stimulated adrenocortical function significantly. Furhter stimulation was detected in the periods during and after operation with both anaesthetic techniques.

[Application of non-invasive methods to assessment of left ventricular function in cardiomyopathy (author's transl)].

Left ventricular function in cardiomyopathy was studied by non-invasive methods. Various indices of left ventricular function were measured in patients with cardiomyopathy by mechanocardiography and echocardiography and were compared with indices in normal subjects and the following conclusions were obtained. 1) Patients with Congestive cardiomyopathy had high PEP/LVET, low mVcf, low EF, and low mPWV, suggesting depressed cardiac function. 2) Patients with Hypertrophic obstructive cardiomyopathy had characteristic findings, such as low DDR, high IVST/PWT, and SAM. 3) Patients with Hypertrophic non-obstructive cardiomyopathy had no characteristic changes in indices, however in some of the findings transition to Hypertrophic obstructive cardiomyopathy was suggested.

Combination chemotherapy for solid tumors using 5-fluorouracil, chromomycin-A, and prednisolone.

Clinical effect of 5-fluorouracil or chromomycin-A3 alone, 5-fluorouracil + chromomycin-A3, and of the first two plus prednisolone on gastrointestinal and other solid tumors was evaluated. Out of 133 cases acceptable for evaluation, the number of responders was as follows: 3 (18.8%) of 6 cases treated with 5-fluorouracil alone, 1 (9.1%) of 11 cases treated with chromomycin-A3 or chromomycin-A3 hemisuccinate, 13 (21.7%) of 60 cases on the two-drug regimen, and 21 (45.7%) of 46 cases on the three-drug regimen. In cases of stomach carcinoma, response rate to the three-drug regimen was 54.2% (13/24), significantly higher than that of other regimens. At least 25% regression in the size of primary tumor was observed in 2 (7.1%) of 28 cases on the two-drug regimen and in 6 (33.3%) of 18 cases on the three-drug regimen. Of 51 cases on the three-drug regimen, steroid diabetes developed in 5 cases, moon face in 4 cases, and gastric ulcer in 1 case. However, toxic effect of these regimens (especially appearance of leucopenia) was less than those of previously tried combined regimens. The duration of response, on an average, was 10.8 weeks in 13 cases on the two-drug regimen and 11.7 weeks in 21 cases on the three-drug regimen. It was concluded from these results that a better response is obtained by the three-drug regimen than other regimens, and that prednisolone in combination, in addition to its favorable effect in improving the general condition of the patients, might enhance the anticancer effect of the drugs used in combination.

Effects of morphine and acetylsalicylic acid on kinin forming enzyme in rat paw.

Coaxial perfusion was carried out in the rat paw; pinching the instep of the foot and electrical stimulation of the sciatic nerve caused an increase in the release of a bradykinin-like substance and kinin forming enzyme into the perfusate. Release of the latter was noted with pinching even independently of the release of the bradykinin-like substance. Acetylsalicylic acid (200 mg/kg i.p.) inhibited the release of kinin forming enzyme due to pinching or to sciatic nerve stimulation, while morphine (5 mg/kg i.m.) inhibited only the release of the enzyme due to sciatic nerve stimulation. Our results suggest that production of localized pain may be largely due to the activity of the released kinin forming enzyme and the bradykinin-like substance. In addition, the mechanism of inhibition by morphine on release of kinin forming enzyme appears to differ from that of acetylsalicylic acid.
