Experience with non-sterile intermittent self-catheterization.

Although a patient subjected to intermittent self-catheterization must carry a catheter and catheterize himself under a non-sterile technique at regular intervals this method is considered the best one available for patients who lead an active social life. When family cooperation is available intermittent catheterization is a cleaner procedure, causes less complications and is easier to manage for a patient confined to bed than the indwelling catheter, cystostomy or other urinary diversion procedures. The technique is also useful for patients with spinal cord injuries, promoting the early return of bladder activity and a life free of the catheter. It is an ideal method for children with meningomyelocele, after the upper tract has been maintained carefully by an indwelling catheter or cystostomy until the child is able to catheterize himself. In these cases a small capacity bladder may be enlarged using the colon and urinary incontinence may be corrected by other operative procedures. We have treated 26 patients with this technique, including 1 with a 17-year followup. The upper urinary tract has not deteriorated in any case and the urine has remained sterile in 39 per cent of the cases.

[Analgesics and the kallikrein-kinin system].

The mechanism of analgesic actions of aspirin, aminopyrine and morphine was investigated in reference to the kallikrein-kinin system and the results suggested that the analgesic actions of these agents are definitely related with the peripheral kallikrein-kinin activity. The central action of kinin appears to differ from the peripheral action.

Enflurane anaesthesia for removal of aldosterone producing adenoma.

In a 31-year old woman with a six year history of headache and hypertension a diagnosis of primary aldosteronism was made on the basis of urine samples containing 45 mug/day of aldosterone. The preoperative systemic blood pressure was 240 mm Hg systolic and 120 mm Hg diastolic. The serum potassium level was 2.6 mEq/L and other laboratory findings were within normal limits. The patient was to undergo operation. Pre-medication consisted of oral pentobarbitone, intramuscular pethidine and atropine. For induction of anaesthesia, enflurane 2.0-2.5% maximum was given with O2 (21/min) and N2O (61/min); no intravenous agents were used. Suxamethonium chloride 40 mg was administered to facilitate endotracheal intubation. Anaesthesia was maintained with enflurane 1.5-2.0% with 50% N2O and O2. Tubocurarine 27 mg was given for muscle relaxation. When the tumour was manipulated, systemic arterial blood pressure was elevated again to 190 mm Hg systolic and 120 mm Hg diastolic. After removal of the tumour, the arterial pressure and heart rate were stable and recovery from anaesthesia was without circulatory or respiratory complications. Plasma aldosterone levels reached a maximum when the tumour was manipulated and fell to normal levels on the second post-operative day. Cortisol levels were not altered markedly even when the tumour was handled. These data imply that adrenocortical response to enflurane anaesthesia as jadged by plasma aldosterone levels would be different from that as estimated by plasma cortisol levels.

Effects of althesin anaesthesia and surgery on plasma concentrations of luteinizing hormone and testosterone in man.

This study was undertaken to investigate the effects of Althesin and surgery on plasma concentrations of luteinizing hormone (LH) and testosterone in 10 male patients. Plasma LH concentrations increased markedly during Althesin anaesthesia given alone over a period of 45 min. The high concentration lasted for 30 min after the start of surgery and was followed by a gradual decrease, although the concentration remained significantly greater than the control value. Plasma testosterone concentrations decreased during Althesin anaesthesia, and a further decrease was noted during the period following surgery.
