Muscular development of the bladder neck in the human fetus.

We conducted a histologic study of the muscular development of the bladder neck in human fetuses of 5 to 23 cm crown-rump length (CRL) and in neonates in special reference to the middle circular layer of the posterior wall of the bladder neck (posterior circular muscle of the bladder neck) and the deep trigonal muscle. Our attempt was to determine the anatomic relationship between and the origins of the two musculatures. The posterior circular muscle had begun to differentiate in the fetus of 8.3 cm CRL in which the trigonal musculature was indistinguishable. Differentiation of the deep trigonal musculature was evident in the fetus of 17 cm CRL; a complete growth in which the muscle was connected to the posterior circular muscle of the bladder neck was evident only in the largest fetus (23 cm CRL). We conclude that the deep trigonal muscle and the posterior circular muscle of the bladder neck are of different origins.

Experimental model of calcium-containing renal stone formation in a rabbit.

An experimental method of calcium-containing renal stone formation was devised using a rabbit on normal diet. The stone formation was induced unilaterally, massively, and rapidly in an extremely high incidence by temporary obstruction of the upper urinary tract, but not by permanent obstruction. The contralateral kidney was used as a control. The initial stone formation was not solid but muddy and seemed to occur through massive crystal aggregation, resulting in concretion within a few weeks. We strongly suspect that the mechanism of this stone formation is crystal aggregation induced by temporary obstruction of the kidney. This animal model should contribute to the investigation of the process of crystal aggregation which is considered to be the most important factor in idiopathic calcium stone formation in man.

Isolation and characterization of bovine factor XI (plasma thromboplastin antecedent).

Factor XI (plasma thromboplastic antecedent) has been purified approximately 28 000-fold from bovine plasma with an overall yield of about 30%. The isolation procedure involves barium sulfate adsorption of contaminants, ammonium sulfate precipitation, and chromatography on heparin-agarose, CM-Sephadex, and DEAE-Sephadex. The final product was homogeneous when examined by polyacrylamide gel electrophoresis and immunoelectrophoresis. A minimal mol wt of 124 000 was determined by sedimentation equilibrium. Factor XI is composed of two similar or identical polypeptide chain (mol wt of approximately 55 000), and these two chains are held together by a disulfide bond(s). Factor XI is a glycoprotein which contains approximately 11% carbohydrate including 5.4% heose, 4.7% N-acetylhexosamine, and 1.0% N-acetylneuraminic acid. Other properties of this coagulation factor including its amino acid composition and inhibition by antibodies prepared in rabbits are also reported.

Matrix stones.

A scanning electron microscopic examination of matrix stones, which have been obtained from a 27-month-old Japanese boy with congenital hydronephrosis, revealed a rough fibrous net structure with small depositions of calcium phosphate. We believe that fibrous matrix may be the primary constituent and that the matrix stone may be the consequence of calcium deposition to it.

Continuous assessment of left ventricular volume by ultrasonic analogue conversion system.

Applicability of ultrasonic analogue conversion system devised by us for assessments of left ventricular volume was examined in patients with and without heart disease. The conversion system could sample the echoes from the endocardial surfaces of the interventricular septum and the posterior left ventricular wall. The conversion system could also calculate automatically the minor axis and volume of the left ventricle. End-diastolic, end-systolic, and stroke volumes calculated by the conversion system were close to those calculated from biplane ventriculograms and to those calculated from photographic records of B-mode echo-display. Change in left ventricular volume caused by respiration ranged up to 17.5%. Spontaneous variations in left ventricular volume during continuous monitoring was up to 7.5%. End-diastolic, end-systolic, and stroke volumes were reduced by sublingual administration of nitroglycerin. The results indicate applicability of the ultrasonic analogue conversion system for continuous assessments of left ventricular volume in man.

Development of the ureterovesical junction in human fetus: in consideration of the vesicoureteral reflux.

The ureterovesical junctions in human fetuses from 11 to 27 weeks of age were studied histologically. Anatomical relationships and origins of the periureteral sheaths, intravesical ureteral muscles, and trigonal muscles were shown. The superficial periureteral sheath arising from the vesical wall was not the source of the trigonal muscles; the deep periureteral sheath arising from the intravesical ureteral wall formed the deep trigonal muscles. The muscles of the intravesical ureter were differentiated longitudinally and formed the superficial trigonal muscles. The development of the two trigonal muscles--superficial and deep--depends on the development of the deep periureteral sheath and the intravesical ureteral muscles. From the finding that both trigonal muscles are present in fetuses compared with both periureteral sheaths or intravesical ureteral muscles in the latest stage, the ability to maintain the valve mechanism in the ureterovesical junction in the human fetus is based on the periureteral sheaths and intravesical ureter--not on the trigone.

[Clinical and laboratory studies on S-6437 (a new longacting granule of cephalexin) in pediatric field (author's transl)].

Clinical and laboratory studies on S-6437 were made, and the following results were obtained. 1. Thirty pediatric patients with various types of infections such as tonsillitis, bronchitis, pneumonia, cystitis, pyelitis, lymphadenitis colli and pyodermia were treated with S-6437 at the daily dosage of 50 mg/kg orally, the clinical effectiveness was 89.3%. 2. The peak blood level of cephalexin after a single oral administration (25 mg/kg) was observed after 4 hours and the average peak blood level of 10 cases was 7.4 microgram/ml. 3. The average urinary excretion rate of 5 cases was 71.2%. 4. Mild side effects were noticed in a few cases.

Mutual potentiation by magnesium and calcium of growth in animal cells.

The effects on DNA synthesis of various combinations of Mg2+ and Ca2+ in cultures of chick embryo cells have been studied. When [Mg2+] larger than or equal to 0.24 mM, reduction of Ca2+ from the standard concentration of 1.72 mM to 0.01 mM had no effect on the incorporation of [3H]thymidine ([3H]dThd) into DNA over a 16-hr period. When Mg2+ was reduced to 0.04 mM, [3H]dThd incorporation into DNA decreased directly with [Ca2+] below 1.72 mM and increased slightly up to [Ca2+] = 5.02 mM, where cell damage began to occur. The change in [Ca2+] necessary to maintain a half-maximal rate of [3H]dThd incorporation was found to depend inversely on the fourth power of the change in [Mg2+]. Chelation of Ca2+ with approximately equimolar ethylene glycol-bis(beta-aminoethyl ether)-N,N'-tetraacetic acid (EGTA) in the presence of [Mg2+] larger than or equal to 0.24 mM reduced [3H]dThd incorporation about 10-fold, and large excesses of EGTA did not further reduce it. The amount of EGTA required to produce a detectable inhibition of [3H]dThd incorporation was independent of [Mg2+] larger than or equal to 0.24 mM, as was the level of residual incorporation in excess EGTA. When [Mg2+] was reduced to 0.04 mM, however, [3H]dThd incorporation declined even when [EGTA] less than [Ca2+], and vanished when EGTA was in large excess. The results are discussed within the framework of a model for the regulation of cell metabolism and growth in which the availability of free Mg2+ is the central coordinating factor. The metabolic effects of Mg2+ depend on its distribution between elements such as ATP and binding sites on membranes. We propose that the major metabolic effects of varying [Ca2+] are produced indirectly through its competition with Mg2+ for membrane sites, thereby making more or less Mg2+ available for rate-limiting transphosphorylation reactions.

Transplantation of human tumors in nude mice.

Ninety-one human tumors, including various common carcinomas, low-grade malignant tumors, and benign tumors, were transplanted into athymic nude mice. Tumor take was confirmed histologically for 22 neoplasms at the initial transplantation, and 14 serially transplantable tumors were established, including some hitherto unestablished or unreported, such as lung and hepatic cell carcinomas. Among the 91 tumors were 21, 14, and 13 carcinomas of the lung, stomach, and breast, respectively. Transplantability was highest in lung carcinomas (10/21), followed by gastric carcinomas (2/14) and breast carcinomas (1/13). Morphology of original tumors was retained well in most transplanted tumors, but desmoplastic or scirrhous tumors, such as gastric and breast carcinomas, tended to become medullary with a decrease in amount of tumor stroma. The ability to produce mucin in gastric carcinomas or melanin in malignant melanoma was maintained in serially transplantable tumors. In addition, ectopic production of adrenocorticotropin and beta melanocyte-stimulating hormone continued in a transplanted small cell carcinoma of the lung. Preliminary results were obtained on hormone dependency of the transplantable breast carcinoma and on alpha1-fetoprotein in the transplantable hepatic cell carcinoma.

Hemodynamic studies on hypertrophied hearts in man.

A total of 31 patients were studied during cardiac catheterization. Hemodynamics in 9 cases with cardiac hypertrophy secondary to chronic volume load (MI, AI) and in 9 cases with cardiomyopathy were compared to those in 5 normal cases, 5 cases with MS and 5 cases with near normal left ventricle. A hypertrophied left ventricle had an enlarged cavity, an increased wall mass and an elevated systolic midwall tension. Peak systolic stress in left ventricular wall was normal in compensated patients with volume load, but below normal in compensated patients with cardiomyopathy. Peak systolic stress was elevated in decompensated patients either with regurgitation or with cardiomyopathy. The significance of this stress difference between volume over load and cardiomyopathy was discussed. Depressed pump function was observed in hypertrophied left ventricle especially in cardiomyopathy.
