[Endoscopic treatment of early gastric cancer in patients undergoing chronic hemodialysis].

Endoscopic treatment of early gastric cancer (type Ila in all patients) was performed in four patients undergoing chronic hemodialysis. Surgical treatment was contraindicated because of various risk factors in these patients. Endoscopic treatment was performed by concomitant use of YAG laser irradiation, local ethanol injection and mucosal resection. After endoscopic treatment, stomach biopsy revealed no cancer in four patients. In one patient who is alive now, no sign of cancer recurrence has been noted for about two years after endoscopic treatment. This patient has undergone hemodialysis on outpatient department. The remaining three patients died from diseases other than gastric cancer. When one of them was autopsied, no gastric cancer demonstrated. Taking the prognosis related to life in to consideration, endoscopic treatment should be used positively when surgical treatment of early gastric cancer cannot be performed in patients undergoing chronic hemodialysis.

[Report of a case of lung cancer with metastasis to the myocardium which showed electrocardiographic findings similar to acute myocardial infarction and intramyocardial mass on echocardiography].

A 61-year-old man was admitted to our hospital with complaints of cough and left back and chest pain. He had suffered from left tuberculous pleurisy at the age of 20 years. Chest X-ray film and CT revealed atelectasis of the left lung, a left hilar mass and an irregular left atrial wall. Depressed P-Ta segment in the inferior limb and anterior chest leads and an abnormal P wave were found on ECG. Transbronchial lung biopsy showed squamous cell carcinoma. After radiation therapy, the patient complained of chest oppression. ECG revealed a normalized P-Ta segment deviation, markedly elevated ST segment in the inferior limb and lateral chest leads and a depressed ST segment in the anterior chest leads. These findings persisted until his death. An obscure appearance of the pericardium and an echogenic intramyocardial mass in the posteroinferior and lateral wall were evident by echocardiography. The patient died due to heart failure. Postmortem needle biopsy showed scattered intramyocardial tumor cell nests with keratinization. CPK, GOT and LDH were within normal limits throughout the course, but CPK-MB was slightly increased. Cardiac metastasis with an ECG appearance similar to that of acute myocardial infarction has been rarely reported. Our present case showed peculiar feature including 1) ECG findings similar to atrial and ventricular myocardial infarction, and 2) an echogenic intramyocardial mass and an ill-defined pericardium on echocardiography. These findings suggested direct invasion of squamous cell carcinoma of the lung to the ventricular myocardium.(ABSTRACT TRUNCATED AT 250 WORDS)

[Consumption coagulopathy found in mitral stenosis associated with atrial thrombus].

Two cases of consumption coagulopathy found in the patients having mitral stenosis with atrial thrombus were reported. In case 1, although anticoagulant (Warfarin) administration did not correct the hemostatic abnormality, combination of tranexamic acid with anticoagulant was effective. In case 2, atrial thrombus found in the echocardiography prior to the onset of thromboembolism in the lower limbs has been disappeared after embolism. Anticoagulant was effective for the improvement of hemostatic abnormality. We considered mitral stenosis with atrial thrombus as one of causes of the consumption coagulopathy.

Urinary trehalase activity as an indicator of kidney injury due to environmental cadmium exposure.

One hundred and seventy-eight subjects, patients with Itai-itai disease and their family members, aged 12-87 years living in a cadmium (Cd)-polluted area in the Jinzu River basin (Cd-exposed group) and 176 controls (control group) were examined. In the Cd-exposed group urinary trehalase increased with increasing age, urinary beta 2-microglobulin (beta 2-m) and retinol-binding protein. Although urinary cadmium was higher in the Cd-exposed group, no particular correlation was found between urinary trehalase and urinary cadmium. Seventeen men and 11 women showed raised urinary trehalase activities despite normal values of urinary beta 2-m (less than 300 micrograms/g.creatinine), suggesting that urinary trehalase increases earlier than urinary beta 2-m. In 19 patients with Itai-itai disease included in the Cd-exposed group, urinary trehalase decreased with decreasing reciprocal of serum creatinine, suggesting that urinary trehalase decreases in the most advanced cases of chronic cadmium nephropathy due to reduced tubular cell mass.

Experimental candidiasis associated with liver injury. Role of transferrin.

In an attempt to perform a further investigation on the proposal that an increasing susceptibility to Candida infection in liver injury may be related to unsaturated transferrin level (UIBC) and/or to a total amount of transferrin represented by TIBC, we conducted experimental candidiasis using mice with galactosamine-induced liver injury and investigated the effect of preadministration of transferrin prior to inoculation of Candida albicans. Final mortality was 10% in the mice without liver injury and without transferrin (Group 1) and ones with liver injury and with transferrin (Group 3). By contrast a 50% mortality was given in one only with liver injury (Group 2). The TIBCs in Groups 1 and 3 were significantly higher than that in Group 2. The UIBCs in Groups 2 and 3, although there was no significant difference between them, were significantly lower than that in Group 1. This study confirmed that transferrin (TIBC) may have a direct deterring effect on systemic Candida infection and the decreased TIBC in the liver injury enhances the growth of C. albicans.

Application of the "imaging plate" to TEM image recording.

The "imaging plate" is a highly sensitive image recording plate for X-ray radiography, which is coated with photo-stimulable phosphor. The imaging plate is exposed to electrons in a transmission electron microscope. Its fundamental properties (sensitivity, dynamic range and sharpness) have been estimated in detail. Also, the image quality of the imaging plate for some specimens in a transmission electron microscope has been estimated. As a result, it has been ascertained that the imaging plate has superior properties and high practicability as an image recording material in a transmission electron microscope.

Interaction of alkali light chain 1 with the isolated 20-kilodalton fragment of myosin subfragment-1 heavy chain and F-actin.

The binding of one of the alkali light chains of myosin, A1, with the isolated renatured 20-kDa fragment of myosin subfragment-1 heavy chain was demonstrated by means of difference UV absorption spectroscopy. The difference spectrum with either rabbit or chicken A1 showed two characteristic peaks at 279 and 287 nm indicating a perturbation of tyrosyl chromophores by the association with the 20-kDa fragment. The delta epsilon at 287 nm increased with an increase in the molar ratio of A1/20-kDa fragment and reached a maximum value at around equimolar ratio. The maximum delta epsilon value was approximately three times larger with rabbit A1 than with chicken A1. Based on the positions of Tyr residues in the amino acid sequences, the contact surface of A1 with myosin heavy chain was concluded to be spread over a large area of A1. The binding of 20-kDa fragment with F-actin was measured by following the increase in turbidity. The affinity appeared to increase several times in the presence of A1. A1 may possibly control the affinity of myosin for actin.

Purification and some properties of cytotoxin produced by Clostridium difficile.

The cytotoxin produced by Clostridium difficile was highly purified by using ammonium sulfate fractionation and successive column chromatographies of DEAE-Sephadex A-25, hydroxyapatite, Bio-Gel A-0.5m, Phenyl-Sepharose CL-4B, and Mono Q. The purified cytotoxin gave a single band on conventional and sodium dodecyl sulfate-polyacrylamide gel electrophoresis in the presence of dithiothreitol. Its molecular weight was estimated to be 260,000 and 50,000 by gel filtration and sodium dodecyl sulfate-polyacrylamide slab gel electrophoresis in the presence of dithiothreitol, respectively. Thus it was supposed that the toxin consists of 5 subunits having molecular weight of approximately 50,000. It had an isoelectric point of 6.6. The toxin was heat-labile (60 C for 10 min) and inactivated by treatment with trypsin and pronase, or at pH below 4 or over 10. The minimum cytotoxic dose of the cytotoxin against Chinese hamster ovary cells was 3 ng. It was also demonstrated that the toxin is antigenically different from enterotoxin of C. difficile.

Production and characterization of monoclonal antibody to dermatan sulfate proteoglycan.

We purified dermatan sulfate proteoglycan (PG) from the capsule of human ovarian fibroma for use as an immunogen. A monoclonal antibody, designated 6B6, was produced which reacts to the intact molecule of dermatan sulfate PG and the chondroitinase AC-treated core molecule on Western-blotted nitrocellulose membrane. Localization of materials showing crossreactivity to this antibody was studied in human tissues by indirect immunohistochemistry. The interstitial elements of almost all tissues examined were positive for the antibody: dermis, submucosal layer of digestive tract, perichondral layer, perivascular connective tissue, perineurium, adventitia of aorta, vessel wall of vein, pleura, and fibrous capsule of kidney and liver. Positive staining was also observed in fibrous elements at post-necrotic foci of cardiac muscle and pancreas, and at atherosclerotic lesions of aorta. The distribution of the antigen, core protein of the dermatan sulfate PG, revealed with 6B6 was compared to that of the dermatan sulfate side chain, which was demonstrated with antibody 9A-2 (Couchman et al.: Nature 307:650, 1984) after treatment with chondroitin sulfate B-lyase. The distribution of both antigens, core protein, and dermatan sulfate side chains showed the same pattern, with minor exceptions. The antibody 6B6 will be a useful tool to study the immunohistochemical localization of dermatan sulfate PG.

Antisecretory effect of a PGE2 derivative, enprostil (TA/RS-84135): inhibitory action of amogastrin-stimulated secretion.

A newly developed prostaglandin E2 derivative, enprostil (TA/RS-84135), was administered to five healthy volunteers in the stomach and duodenum through a teflon tube to examine its effect on amogastrin-stimulated gastric acid secretion. The acid output 1-3 h after administration of enprostil (35 micrograms) decreased remarkably as compared with that in the same period after placebo administration. However, no significant difference in antisecretory effect was observed between the two routes of administration. A decrease in pepsin output occurred in parallel with the decrease in acid output. These results suggest that the antisecretory effect of enprostil is at least partially due to systemic absorption and the predominantly topical action seen in animals does not seem to occur in man. No side-effects attributable to enprostil were observed during the test period. In conclusion, enprostil seems to be clinically useful as an antiulcer agent.

Effects of chronic inhalation of ethylene oxide on lipid peroxidation and glutathione redox cycle in rat liver.

The chronic effects of ethylene oxide (EO) on hepatic metabolism were investigated. Both reduced and oxidized forms of glutathione in liver were not changed during chronic exposure to EO for 13 weeks. On the other hand, increased levels of malondialdehyde were detected at 6 weeks and at the end of exposure periods, as compared with those of control livers. The activity of glutathione reductase was decreased at 6 and 13 weeks and therefore in inverse proportion to the lipid peroxide, although the reduced form of glutathione did not decrease probably due to the increase of de novo synthesis. These results indicate that EO treatment enhances lipid peroxidation and also affects glutathione redox cycle, but glutathione did not play a causative role in the induction of lipid peroxidation under the present conditions.

[Experience of HLA-matched platelet transfusion in patients who are refractory to random donor platelets].

Platelets collected from random and multiple donors were as effective on transfusion as those obtained from single donor by platelet pheresis if patients were not alloimmunized. In alloimmunized patients, effects of platelet transfusion were correlated with the HLA-matching grade; 100% effective in A-match, 92% in B-match and 65% in C-match. This correlation was not observed if the patient had infection. The transfusion of A- or B-matched platelets resulted in a lowering or disappearance of HLA antibody activities in most patients. However, the transfusion of C-matched platelets caused increasing or appearance of HLA antibody activities in some patients. The ABO-incompatibility did not show a significant effect on platelet transfusion when anti-A or anti-B antibody activities in donors and antiglobulin tests on patients' red cells were negligible. Non HLA, platelet specific antibodies were produced in some patients and presumed to be participated in the destruction of transfused HLA-matched platelets.
