[A case of idiopathic plasmacytic lymphadenopathy with polyclonal hyperimmunoglobulinemia associated with chronic renal failure].

A rare case of idiopathic plasmacytic lymphadenopathy with polyclonal hyperimmunoglobulinemia (IPL) associated with chronic renal failure was described in this report. A 73-year-old male was admitted and diagnosed as IPL. IPL is proposed by Mori et al. in 1980. Clinical entity of IPL is (1) Polyclonal hyperimmunoglobulinemia (2) Systemic Lymphadenopathy characterized by remarkable mature plasmacytosis without atypism and by no destruction of the structures. (3) All disease with polyclonal hyperimmunoglobulinemia can be excluded. In this patient, physical findings showed enlarged lymph nodes (1-2 cm) in bilateral nuchal, submandibular, axillary and inguinal lesions. Laboratory examinations showed polyclonal hyperimmunoglobulinemia (especially IgG, IgA), anemia and renal dysfunction. Microscopic observation of hematoxylin-eosin staining in the axillary lymph node showed increased mature plasma cells without evidence of malignant growth. Immunoperoxidase staining showed intracytoplasmatic polyclonal immunoglobulins. IPL is known as invading other organs besides lymph node, for example skin, lung or kidney. This patient showed renal dysfunction (Cr clearance 11 ml/min, severe proteinuria). Nine cases of IPL and multicentric plasma cell type Giant Lymphnode Hyperplasia (GLH) concurrent with renal dysfunction were reported. Only in two of them chronic renal failure were reported. Twice a week hemodialysis improved his condition and laboratory findings.

[Surgical treatment of postinfarction left ventricular free wall rupture--experience of 12 cases including 2 successful repairs of acute (blow out) rupture].

Left ventricular free wall rupture (LVFWR) complicating myocardial infarction is still a lethal complication. Although there have been reports of successful repair of LVFWR, most of them were of subacute type in which main symptom was cardiac tamponade and the surgical repair was undertaken several hours after the onset of rupture. Between March, 1984 and June, 1987. We treated 12 cases of LVFWR surgically, thoracotomy and open drainage in 2 cases, thoracotomy and direct closure of rupture in 8 cases, median sternotomy and patch closure of rupture in 2 cases. We used cardiopulmonary bypass (CPB) only in 3 cases, because most of the cases except two developed electromechanical dissociation abruptly, requiring an emergency thoracotomy and there was no time for establishing CPB. In the cases of electromechanical dissociation, cardiopulmonary resuscitation and an emergency thoracotomy were performed simultaneously. There were three early survivors (greater than 30 days) by emergency thoracotomy and direct closure of rupture and one survivor (double rupture case) by patch closure on CPB. We believe that acute type of LVFWR in which initial symptom is electromechanical dissociation without any preceding symptoms can be rescued by emergency thoracotomy and direct closure of rupture with no aid of CPB if rupture is a small tear of anterior or lateral left ventricle. For this purpose, prompt diagnosis is mandatory and this is possible by two dimensional echocardiogram even during cardiopulmonary resuscitation.

[A case of aortitis syndrome with left coronary ostial stenosis and aortic regurgitation treated by transaortic coronary endarterectomy and aortic valve replacement].

A case, 23-year-old female of aortitis syndrome with left coronary ostial stenosis and aortic regurgitation was reported. The coronary angiography showed critical stenosis of the left coronary ostium with intact main stem and its branches. The aortogram revealed aortic regurgitation of grade III, and multiple obstructive or stenotic lesions on the left common carotid artery, the origin of the left renal artery, the inferior mesenteric artery and the abdominal aorta. After improvement of inflammatory findings by steroid therapy during 2 months, transaortic coronary endarterectomy and aortic valve replacement with 21 mm Björk-Shiley valve were performed successfully. Postoperative course was uneventful. Coronary angiography performed at the 57th day after the operation showed complete removal of the left coronary ostial stenosis, and aortography showed no evidence of perivalvular leakage of the aortic valve prosthesis. The indication of transaortic coronary endarterectomy and the technique used to avoid aortic valve detachment which may be caused by recurrence of aortitis were discussed in this paper.

Production and immunohistochemical characterization of a monoclonal antibody raised to proteoglycan purified from a human yolk sac tumour.

A large proteoglycan with chondroitin sulphate and dermatan sulphate side chains has been isolated and purified from a yolk sac tumour of the left ovary from a 23-year-old female. A monoclonal antibody, designated 2B1, was produced which reacted specifically with the intact molecule of the large proteoglycan and the chondroitinase ABC-treated core molecule. The localization of substances showing cross-reactivity to this antibody was studied in a variety of human tissues by means of indirect immunohistochemistry. The interstitial elements of nearly all tissues of a 5-month-old foetus were intensely reactive with the antibody, but in adult tissues structures that gave positive reactions were limited; only the perivascular and perimuscular fibrous elements were reactive, except for the aorta, which reacted extensively. In contrast, the interstitial elements of the carcinoma tissues tested were intensely reactive. Thus antibody 2B1 can be regarded as a useful tool for studies on the immunohistochemical localization of large proteoglycan in various human tissues.

Lipid peroxidation and the metabolism of glutathione in rat liver and brain following ethylene oxide inhalation.

The effects of repeated exposure to ethylene oxide on lipid peroxidation and glutathione metabolism in both rat liver and brain were examined. Increased levels of malondialdehyde (MDA) in the liver were observed after 6 and 13 weeks of exposure to ethylene oxide. The increased level of MDA observed in the hepatic homogenates of the treated rats reflected that of the microsomal fraction. On the other hand, no change in the level of MDA was detected in the brain of rats either at a 6- or 13-week treatment. Glutathione reductase activity was found to decrease at 6 or 13 weeks in liver and brain of treated rats. Both reduced and oxidized forms of glutathione in homogenates of liver and brain obtained from treated rats were, however, similar to those of the control at 40 h after the last exposure in individual experiments. To elucidate the cause of lipid peroxidation, the time course of glutathione content after exposure with ethylene oxide were studied in more detail. Significant decreases in both GSH and GSSG content in these organs were detected shortly after exposure to ethylene oxide and their levels recovered gradually with time and reached the control values at 40 h in the liver, although the changes were less significant in the brain as compared with those in the liver. These results suggest that enhancement of lipid peroxidation in the microsomal fraction of the liver after repeated exposure to ethylene oxide may possibly arise from repeated depletions of glutathione to certain critical levels and less removal of lipid peroxidation.

Mapping myosin light chains by immunoelectron microscopy. Use of anti-fluorescyl antibodies as structural probes.

The two classes of light chains in vertebrate fast muscle myosin have been selectively labeled with the thiol specific reagent 5-(iodoacetamido) fluorescein to determine their location in the myosin head. The alkali light chains (A1 and A2) were labeled at a single cysteine residue near the COOH terminus, whereas the regulatory light chain (LC2) was reacted at either cysteine 125 or 154. The two cysteines of LC2 appear to be near each other in the tertiary structure as evidenced by the ease of formation of an intramolecular disulfide bond. Besides having favorable spectral properties, fluorescein is a potent haptenic immunogen for raising high affinity antibodies. When anti-fluorescyl antibodies were added to the fluorescein-labeled light chains, the fluorescence was quenched by greater than 90%, thereby providing a simple method for determining an association constant. The interaction with antibody was the same for light chains exchanged into myosin as for free light chains. Complexes of antibody bound to light chain could be visualized in the electron microscope by rotary shadowing with platinum. By this approach we have shown that the COOH-terminal regions of the two classes of light chains are widely separated in myosin: the cysteine residues of LC2 lie close to the head/rod junction, whereas the single cysteine of A1 or A2 is located approximately 90 A distal to the junction. These sites correspond to the positions of the NH2 termini of the light chains mapped in earlier studies (Winkelmann, D. A., and S. Lowey. 1986. J. Mol. Biol. 188:595-612; Tokunaga, M., M. Suzuki, K. Saeki, and T. Wakabayashi. 1987b. J. Mol. Biol. 194:245-255). We conclude that the two classes of light chains do not lie in a simple colinear arrangement, but instead have a more complex organization in distinct regions of the myosin head.

Permissive role of dopamine in renal action of ANP in volume-expanded rats.

A possible role of dopamine in the diuretic and natriuretic action of atrial natriuretic peptide (ANP) was evaluated in the rat. ANP was infused into the left renal artery of anesthetized rats whose kidneys were denervated. ANP both at 12 and 1.2 pmol/h caused immediate ipsilateral increases in urine volume (V), urine Na excretion (UNaV), and fractional excretion of Na (FENa). Ipsilateral glomerular filtration rate (GFR) and renal plasma flow (RPF) increased with 12 pmol/h ANP but not with 1.2 pmol/h ANP. Intravenous infusion of haloperidol, Sch-23390, or carbidopa markedly attenuated the increase in V, UNaV, and FENa with 12 pmol/h ANP and completely abolished the increases in GFR and RPF. Haloperidol, Sch-23390, and carbidopa also completely abolished the renal effects of 1.2 pmol/h ANP. In the presence of carbidopa, a small dose of dopamine infused into the systemic circulation, which by itself has no effects on blood pressure, V, GFR, RPF, and UNaV, restored the diuretic and natriuretic effects of ANP. In addition, an increase in urinary guanosine 3',5'-cyclic monophosphate by ANP was not affected by either haloperidol or carbidopa. These data indicate that dopamine may be necessary, as a permissive agent, for the renal effects of ANP to manifest and that the effects of dopamine may be independent of ANP-stimulated guanylate cyclase activation.

Sub-chronic effects of styrene and styrene oxide on lipid peroxidation and the metabolism of glutathione in rat liver and brain.

Sub-chronic effects of styrene and styrene oxide on lipid peroxidation, glutathione contents and glutathione reductase activities in the liver and brain were examined after intraperitoneal administration to rats 3 times a week for 7 weeks. Styrene (300, 400 and 500 mg/kg) and styrene oxide (200 and 300 mg/kg) increased lipid peroxidation in the liver after 7 weeks of treatment. Hepatic lipid peroxidation in the rats treated with a higher dose of styrene oxide (400 mg/kg) was significantly enhanced even after 2 weeks of treatment. On the other hand, no change in lipid peroxidation was observed in the brain under the above conditions. Neither glutathione contents nor glutathione reductase activities in the liver and brain were altered at 40 h after the last of these sub-chronic treatments. To elucidate the cause of lipid peroxidation, the time courses of glutathione content after treatment with either styrene or styrene oxide (300 mg/kg) were studied in more detail. Significant decreases in both the GSH and GSSG contents were detected shortly after these treatments and the levels recovered to the control values at 40 h in these organs, although the changes were less significant in the brain of rats treated with styrene. These results suggest that enhancement of lipid peroxidation in the liver after treatment with styrene or styrene oxide was a consequence of repeated depletions of glutathione to certain critical levels and delayed recovery of lipid peroxides.

Immunohistochemical techniques for detection of dermatan sulfate proteoglycan in tissue sections.

Dermatan sulfate proteoglycan chains were detected in tissue sections treated with chondroitin B-lyase (0.01 units/ml) in 20 mM Tris-HCl (pH 8.0) for 1 hr, followed by staining with antibody 9A2 specific for unsaturated uronic acid coupled to N-acetylgalactosamine-4 sulfate. In contrast, after treatment with chondroitin B-lyase, no positive staining was observed with antibodies 3B3 and 1B5 which react to the unsaturated uronic acid coupled to N-acetylgalactosamine 6-sulfate and unsaturated uronic acid coupled to N-acetylgalactosamine, respectively. The distribution of dermatan sulfate thus revealed was confirmed by comparison with that found by monoclonal antibody 6B6 which reacts with small proteoglycans carrying dermatan sulfate side chains. The localization of positive staining in fibrous connective tissues was almost identical with these two procedures.

Validity of MR imaging for monitoring effects of percutaneous ethanol injection for HCC.

Magnetic resonance (MR) images of 44 hepato-cellular carcinoma (HCC) patients who had undergone percutaneous ethanol injection therapy (PEIT) were analyzed during and after the treatment. Using a long TE/TR spin-echo technique, the initial high intensity of the tumor decreased during treatment in all patients. Decrease in tumor intensity corresponded well with decrease of serum alpha-fetoprotein level and histopathological changes of the necrosis. During the long-term follow-up of three patients, the reappearance of a highly intense area was readily detected by MR imaging, and hence the recurrence of the tumor, as determined clinically or histopathologically, was confirmed. The long TE/TR spin-echo technique proves most helpful in monitoring the effectiveness of PEIT for HCC, as well as for its recurrence.

[Raghib's syndrome associated with cor triatriatum--a rare surgical case report].

A 27-year-old male with a history of congenital heart disease was admitted for pre-operative evaluation of a cardiac malformation. Echocardiography and cardiac catheterization revealed an incomplete endocardial cushion defect with a persistent left superior vena cava which drained into the left atrium but echo-cardiographic evidence of an abnormal intra-atrial septum was not found. The patient was placed on cardiopulmonary bypass and prepared for the surgical correction of his primary cardiac lesion. Intra-cardiac examination during reconstruction of Raghib's syndrome also revealed the presence of cor triatriatum. Reghib's syndrome is characterized by the combination of abnormal drainage from the left superior vena cava into the left atrium, the presence of an atrial septal defect and the absence of a coronary sinus. To prevent secondary complications such as a brain abscess, we redirected blood flow from the left superior vena cava to the right atrium utilizing a trimmed woven dacron vascular graft that was placed in the intra-atrial position. Mitral valvoplasty, excision of the obstructing diaphragm and atrial septation were also performed successfully. Although the literature has described the surgical repair of Raghib's syndrome, its correction in combination with cor triatriatum is considered to be extremely rare.
