Noninvasive carcinoma of the gallbladder arising in localized type adenomyomatosis.

We report a case of noninvasive carcinoma of the gallbladder arising in the surface mucosa of localized type adenomyomatosis. It was first detected as a polypoid lesion during routine examination with ultrasonography. The gross appearance of the resected specimen showed localized type adenomyomatosis with multiple cysts containing mucin at the fundus of the gallbladder. Microscopic study revealed noninvasive carcinoma in its surface mucosa associated with papillary adenoma in the cyst wall. The occurrence of carcinoma in adenomyomatosis is very rare, and only a few cases have been reported in the literature. Although it has been generally accepted that there is no fear of malignant transformation of adenomyomatosis, the present case suggested that it might be a possible candidate for carcinoma.

Effects of endotoxin (lipopolysaccharides) on experimental aspergillosis in leukemic mice.

In an attempt to evaluate effects of bacterial endotoxin on systemic fungal infection, experimental systemic Aspergillus infection was induced in leukemic mice (AKR strain) with (Group B) or without (Group A) preceding intraperitoneal administration of Pseudomonas aeruginosa derived lipopolysaccharides. All mice in group A died within 4 days after intravenous inoculation of Aspergillus fumigatus spores and developed extensive and disseminated fungal lesions without inflammatory reactions. In contrast, 5 of 10 mice in group B were alive at day 4 and 1 mouse in group B was alive when the experiment was terminated on the 14th day. These mice showed less extensive fungal lesions with definite, albeit minimal, inflammatory reactions which were composed of macrophages and neutrophils. In addition, serum iron levels and iron saturation rates were significantly lower in mice in group B than in group A. These results indicate that P. aeruginosa endotoxin has a deterring effect on systemic Aspergillus infection in leukemic mice.

Elevation of urinary trehalase activity in patients of itai-itai disease.

The elevation of urinary trehalase activity in patients of itai-itai disease was examined. Urinary trehalase was correlated with tubular reabsorption of phosphorus (%TRP): the lower the trehalase activity, the worse was %TRP. Furthermore, this activity was inversely correlated with urinary glucose and urinary total protein. In itai-itai disease, the excretion of beta 2-microglobulin seems to be maximal, and urinary trehalase activity was low in the latter stages of the disease. It is inferred that itai-itai disease produces extremely severe tubular damage as well as glomerular dysfunction.

Effects of propafenone on sinus nodal and ventricular automaticity: in vitro and in vivo correlation.

The electrophysiologic effects of the new antiarrhythmic drug, propafenone, were evaluated in anesthetized closed-chest dogs and on isolated cardiac tissues with the microelectrode technique. Propafenone (2 to 4 mg/kg intravenously) had no effect on sinus rate or on sinus nodal recovery time, but caused a dose-dependent significant decrease in the rate of idioventricular rhythm and increased the duration of ventricular overdrive suppression in dogs (n = 8) with complete atrioventricular block. On isolated canine Purkinje fibers (n = 8) manifesting automaticity with resting membrane potential less negative than -70 mV, propafenone reduced the slope of phase 4 depolarization and reduced the rate of automatic impulse initiation in a concentration-dependent manner (10(-6) M-4.10(-5) M). At these concentrations, propafenone had no effect on rabbit sinus nodal automaticity (n = 8) or on sinoatrial conduction. However, significant depression of sinus nodal automaticity occurred with propafenone concentrations above 5.10(-6) M in the presence of cholinergic or complete autonomic blockade with atropine (10(-6) M) and propranolol (5.10(-5) M). Propafenone caused a concentration-dependent decrease in the disparity of Purkinje fiber-ventricular muscle action potential duration (APD), mainly by shortening Purkinje fiber APD. We conclude: that propafenone suppresses idioventricular rhythm in the intact dog, most likely by depressing Purkinje fiber automaticity; the depressant effect of propafenone on sinus nodal automaticity is evident only during cholinergic receptor blockade; and the antiarrhythmic properties of propafenone may include removal of APD disparity by selective shortening of Purkinje fiber and not of ventricular muscle APD.

Subparticles of Anabaena phycobilisomes. II. Molecular assembly of allophycocyanin cores in reference to "anchor" protein.

Entire phycobilisomes (PBS) and two derived particles, whole allophycocyanin (APC) cores and the far-red-emitting fragment of APC cores (14.5 S APC), all containing the 115-kDa polypeptide ("anchor protein"), were compared for the readiness with which the 115-kDa protein could be modified chemically, be degraded by chymotrypsin, and react with the anti-115-kDa serum. The 115 kDa in PBS and the whole APC cores were digested slightly by chymotrypsin and did not react with anti-115-kDa IgG. In contrast, the 115 kDa in 14.5 S APC was digested to 42 kDa and showed a positive reaction with anti-115-kDa IgG. Reconstitution of APC cores from 14.5 S APC and APC trimers was inhibited by the anti-115-kDa IgG, and APC particles with partly digested 115-kDa cannot reconstitute APC cores. These results imply that 115 kDa is embedded mostly inside PBS and is involved more in the maintenance of the molecular assembly of APC cores than in the "anchoring" of PBS to thylakoid. The PBS from Anabaena variabilis (M3) have an APC core larger than those of other PBS and show atypical morphology consisting of five APC discs. They have a polypeptide (115 kDa) that is significantly longer than the corresponding polypeptides (around 95 kDa) of other blue-green algae. This can be interpreted by assuming a relationship between the size of APC cores and the length of the polypeptide.

Severity of tubular brush border damage in cadmium-polluted area (Jinzu river basin): clinical role of urinary trehalase.

Urinary trehalase activity and leucine aminopeptidase activity were parabolically correlated with urinary beta 2-microglobulin, and these enzymes were observed to be biphasic in relation to urinary beta 2-microglobulin when the study populations included patients of Itai-itai disease and inhabitants of a cadmium-polluted area. Furthermore, urinary trehalase activity was parabolically correlated with urinary total protein and urinary glucose. From these results, it is inferred that by measuring both urinary trehalase and urinary beta 2-microglobulin, one can elucidate the degree of tubular damage.

Suppurative osteomyelitis of the mandible in a child.

The rare case of osteomyelitis of the mandible in a 4-year-old boy is reported. The literature of osteomyelitis in children is reviewed and discussed. Osteomyelitis in children is mainly affected in the mandible. In childhood, mandibular structure is so immature that the inflammation of the mandible extends easily and rapidly to a wide area. It is very difficult to perform adequate treatment. The therapy for osteomyelitis in childhood must be carried out after due consideration of the stage and anatomical features.

Is curative endoscopic treatment of early gastric cancer possible?

Fifty-four lesions found in 51 cases of early gastric cancer were treated with the Nd:YAG laser, N2 dye laser, local injection of ethanol, polypectomy and a combination of these methods between November, 1980 and July, 1986. Among these lesions, 34 were followed for more than one year with the endoscope. In 86% of the lesions treated, no recurrence of cancer was detected during the observation period, nor were there any deaths from gastric cancer. An analysis of the results showed that local cure of early gastric cancer in the mucosal layer could be accomplished with many kinds of endoscopic devices and that the difficulties for curative treatment depended on tumor localization and size. Further, we were able to assess local cure after endoscopic treatment on the basis of the endoscopic features of the tumor; local cure presents as a scarred stage with a smooth surface and with no abnormal granules or irregular redness. At present, endoscopic treatment should however, be limited to inoperable cases, since we cannot definitively diagnose lymph node metastasis or depth of cancer invasion.

The minimum alveolar concentration (MAC) of sevoflurane in humans.

Forty surgical patients were divided into two groups and anesthetized with either sevoflurane and oxygen or sevoflurane, oxygen, and nitrous oxide. The minimum alveolar concentration (MAC) for sevoflurane required to prevent movement in response to surgical incision in healthy patients was 1.71 +/- 0.07% (SE). The AD95 (anesthetic ED95) that prevented 95% of patients from moving was 2.07%. The addition of 63.5% end-tidal nitrous oxide allowed a reduction in the alveolar sevoflurane concentration to 0.66 +/- 0.06% (SE). The reduction in sevoflurane MAC was 61.4%. The AD95 for sevoflurane with 63.5% end-tidal nitrous oxide was 0.94%.

A case of secondary syphilis with a solitary pulmonary lesion.

This study describes a case of secondary syphilis with a papular eruption and penile ulceration. A chest roentgenogram showed a solitary lesion in the left middle lung field. Histological examination revealed epithelioid cell granuloma in both the penile ulcer and the lung lesion. After antisyphilitic treatment, the lung lesion shrunk remarkably and at the same time the skin eruption and the systemic symptoms also improved.

[Bronchioloalveolar carcinoma diagnosed by bronchoalveolar lavage].

A case of bronchioloalveolar carcinoma of the lung is reported. A 63-year-old man identified during a chest mass-survey program, came to our hospital for a precise examination. He had no subjective complaints though his plain chest X-ray film showed a diffuse alveolar pattern in the bilateral hilar regions mimicking a butterfly shadow. Further, cytological examinations of both the sputa and specimens from brushings obtained on bronchoscopy failed to demonstrate malignant cells, and only a cytological examination of the fluid obtained by bronchoalveolar lavage (BAL) revealed the presence of a malignancy. Thus, BAL would seem to be a simple and reasonable method for the detection of suspected malignant cells in bronchioloalveolar carcinoma.
