Characteristics of granulocyte-macrophage colony formation in patients with chronic myelomonocytic leukemia.

3 patients with chronic myelomonocytic leukemia (CMMoL) were investigated using a culture technique in vitro for granulocyte and monocyte/macrophage precursors (GM-CFC). Time course studies revealed that the growth of colonies and clusters was delayed in CMMoL as compared with that in normal control bone marrow. In 1 of 3 cases of CMMoL, the number of GM colonies increased after removal of phagocytes. Chloroacetate esterase and nonspecific esterase activities in the colonies and clusters were examined by means of our improved cytochemical technique. When we classified colonies and clusters into granulocytic, monocyte/macrophagic, myelomonocytic and mixed type, the incidence of "myelomonocytic' colonies and clusters was significantly higher in two cases of CMMoL than in the normal controls. On the basis of these results, the characteristics of abnormal hemopoietic regulatory mechanisms were discussed.

[Treatment of unresectable liver cancer with intrahepatic arterial infusion chemotherapy].

Intra-arterial infusion chemotherapy was performed in 50 patients with cancer of the liver including 12 primary hepatoma and 38 metastatic cancers, at Kurashiki Central Hospital from December 1977 through December 1981. B.D. Fomocath 5 French catheter was inserted through the lateral femoral circumflex artery and advanced retrogradely to the common hepatic or proper hepatic artery using a loop catheter technique and the catheter was connected to the tube of infusion pump. The dose of 5-Fluorouracil and Mitomycin C was 250 mg per day and 4-10 mg once a week, respectively. The patients had average survival of 8.2 months in hepatoma and 4.8 months in metastatic tumor. The results were unfortunately not encouraging but this regimen occasionally provides excellent results and has no contraindication.

[Clinical studies of cefoxitin for the treatment of respiratory tract infections (author's transl)].

A total of 42 patients who were suffering from respiratory tract infections were treated with cefoxitin, and the following results were obtained. 1. Out of 32 patients clinically evaluated, excellent or good responses were observed in 30 patients (94%). 2. Presumed causative organisms were isolated in 14 patients. The organisms were eradicated in 11 patients and the eradication rate was 79% (11/14). The number of the organisms decreased or unchanged in 1 patient each. In other 1 patient the pathogenic agent was replaced with other agents during the course of treatment. 3. As for the side effects, skin eruption was observed in 3 patients. One patient received drugs other than cefoxitin concomitantly that might have caused the eruption. Another patient had an allergic history to many antibiotics. In 4 patients slight elevations of S-GOT and S-GPT were observed but improved soon after the completion of cefoxitin treatment. In 1 patient an elevation of serum creatinine was observed but this was not attributed to the administration of cefoxitin. 4. From the results stated above, cefoxitin is considered to be a safe and effective antibiotic which can be one of the first-choice antibiotics for the treatment of respiratory tract infections.

Detection of functional group on the liposome surface by colloid titration.

Suspensions of phospholipid liposomes (phosphatidylcholine, phosphatidylglycerol, phosphatidylethanoline and phosphatidylserine) were examined by colloid titration with polydiallyldimethylammonium chloride and potassium polyvinylsulfate as polycationic and polycationic and polyanionic titrants, respectively. In addition, the electrophoretic mobilities of the liposomes were measured with a cytopherometer. The following conclusions are drawn from the results of the titration: Phosphate and ammonium groups of phosphatidylcholine in liposomes form intramolecular or intermolecular salt linkages. Phosphate on the surface of phosphatidylglycerol liposomes shows a constant dissociation at above pH 3. Liposomes of phosphatidylethanolamine have no charge at low pH and a maximum negative charge above pH 10.5. On the surface of phosphatidylserine liposomes, the carboxyl group is completely dissociated at pH 6-7 and the phosphate group is released completely by the conversion of the ammonium ion to an amino group above pH 10.5. Stemming from these results, colloid titration can reveal the dissociation or the situation on the surface of liposomes more precisely than electrophoresis. Thus colloid titration is useful for determining the states of functional groups on the surface of phospholipid liposomes.

Carcinoembronic antigen and endocrine response in a patient with Sipple's syndrome following surgery.

Circulating carcinoembryonic antigen (CEA), calcitonin (CT) and adrenocorticotropic hormone (ACTH) and levels in tissue extracts of a patients with Sipple's syndrome were measured simultaneously. Biofocal medullary carcinoma of the thyroid gland (MCT), left adrenal phaeochromocytoma and ectopic para-aortal phaeochromocytoma had been removed in the first operation. Total thyroidectomy was performed thereafter. Evidence has been presented showing that (1) the first operation lowered the elevated serum levels of CEA and CT, and levels of CEA and CT in tissue extracts of MCT were considerably higher than those of tumor-uninvolved thyroid tissue and phaeochromocytomas; (2) markedly elevated levels of plasma ACTH were normalized by the first surgery and, in addition, immunoreactive ACTH and beta-melanocyte stimulating hormone (beta-MSH) were detected in the phaeochromocytomas removed. This is a documented case of the simultaneous production of CT and CEA by MCT associated with ectopic secretion of ACTH and beta-MSH from the phaeochromocytomas without manifesting clinical features of Cushing's syndrome and is based on the findings referred to above.
