Cardiac pacing in Japan.

A general update on pacemakers and cardiac pacing in Japan in 1981 is presented, including costs, prepacing studies, follow-up, pacing center activity, average number of first implants per million inhabitants, age of patients at first implantation, clinical indications for pacing, etiology, prepacing electrocardiogram, pacemaker hardware, electrode leads, electrocardiogram indications for pacing, indications for generator or electrode changes, and extended application of pacing. The author's data are compared with those obtained by the survey of the Seventh World Symposium on Cardiac Pacing.

[Studies on an antigen strongly associated with human bronchogenic squamous cell carcinoma (1)--studies by immuno-diffusion method].

Insoluble extracts of human squamous cell carcinoma were solubilized by desoxycholate. The soluble extracts, after partial purification, were used to prepare antisera in guinea pigs. These antisera which were absorbed with insoluble extracts of lung, liver, kidney, spleen, carcinoembryonic antigen and non-specific cross-reacting antigen were commonly detected in all of the 7 squamous cell carcinomas. These antisera failed to reacted with adult organ extract, fetal organ homogenates or extracts from tumors of other histologic types by the methods of immuno-diffusion technique.

[Preoperative treatment with 5-fluorouracil (5-FU) in gastric and colorectal cancer].

Preoperative chemotherapy with 5-FU was carried out on patients with gastric and colorectal cancer. 5-FU was given at a dose of 500 mg daily with a drip infusion for 7 days (Group A), with a one-shot intravenous infusion daily (Group B) and orally for 7 days (Group C). Histological effects were evaluated, using the criteria proposed by Ohboshi. In group A for gastric cancer and in group C for colon cancer, the histological response was stronger at the edge of the cancer than at its surface. In group C for gastric cancer and in group A for colon cancer, the histological response was more remarkable at the surface of the cancer than at its edge. The histological effects on metastatic lymph nodes in gastric cancer were the same as for the cancer nests in groups A and C. 5-FU used as a drip infusion or orally was more effective on metastatic lymph nodes of gastric cancer than on metastatic lymph nodes of colon cancer. 5-FU was detected at a high concentration in tumor tissues of gastric cancer in group A, Grade II response being obtained in 3 of 7 patients. Changes in regional lymph nodes, plasma and other organs in cases of combined resection were not obtained with regard to either the concentration of 5-FU or the histological findings for all three groups.

[Studies on antigen highly associated with human bronchogenic squamous cell carcinoma--enzyme-linked immuno-absorbent assay].

Insoluble extracts of human bronchogenic squamous cell carcinoma were solubilized by desoxycholate. After fractionation, the soluble extracts were used to prepare antisera in guinea pigs. These antisera, after being highly absorbed with homogenates of several normal organ, serum and carcinoembryonic antigen, were reactive only with squamous cell carcinoma. There was no reactivity to fetal lung, normal adult organs or other cancers.

Clinical value of immunotherapy for lung cancer by the streptococcal preparation OK-432.

A streptococcal preparation, OK-432 was employed as the adjuvant immunotherapeutic agent for lung cancer. To evaluate the clinical efficacy of the OK-432, patients admitted between 1975 and 1979 were randomized into two groups: (1) an immunochemotherapy group and (2) a chemotherapy, or control, group. For evaluation of long-term survival, there were 108 cases in the immunochemotherapy group and 103 cases in the chemotherapy group. When comparing the prognosis of the two groups, the survival rate was statistically higher in the immunochemotherapy group than the control group. The resected cases in Stages I and II showed better prognosis with immunochemotherapy than the cases in the same stages treated with chemotherapy alone. Among the resected cases in the more advanced stages, mostly Stage III and a few cases in the Stage IV, the cases treated with immunochemotherapy also showed more favorable prognosis than the cases treated with chemotherapy alone. In terms of the cell type of the lung cancer, the cases with epidermoid carcinoma in Stages I and II showed significantly better prognosis with immunotherapeutics than the control group. The cases with positive reaction to the streptococcal polysaccharide skin test apparently showed better prognosis than those with negative reaction.

Surgical correction of pectus excavatum for adults and adolescents.

As an operative procedure for the correction of funnel chest in adult and adolescents, a strut method using AO-plates was used. All of the concaved parts of the sternum, costae, and cartilages contributing to the formation of the deformity were osteotomized to mobilize and elevate the deformed part. A vertical plate was used for fixation of the transected sternum, and three transverse plates were used as the portions which are curved appropriately to remodel the reconstructing chest wall. These plates were removed twelve to eighteen months after implantation. To date, thirty-one patients have undergone this repair. For the first eight, a single transverse plate was used, however, re-retraction of the region around the transverse plate became evident in the long term follow-up. Therefore, two or three plates were inserted for an improvement. When three transverse plates were used, retraction at the costal arch was relieved. Although a second operation is required to removal of the plates, we hold the view that this is the most effective operative procedure for the correction of funnel chest in adults and adolescents.
