Coronary artery steal via a large anastomosis between the coronary and bronchial arteries successfully treated by surgical division.

A 56-year-old woman is presented with a large coronary-artery-to-bronchial-artery anastomosis associated with aortitis syndrome. Her angina, which was the result of a coronary artery steal, was relieved by surgical division of the anastomotic vessel. Pulmonary artery obstruction, as a pulmonary lesion of aortitis syndrome, might increase the collateral flow from the coronary artery through the bronchial artery to the pulmonary circulation.

Noninvasive evaluation of aortocoronary bypass graft patency by contrast-enhanced computed tomography: incrementation mode and dynamic mode.

Fourty-three coronary bypass graft patients were examined with contrast-enhanced computed tomography (CT) to evaluate bypass graft patency. Six 4.8-second sequential scans with 1.4-second interscan intervals were obtained during the injection of a bolus of a contrast medium into a peripheral vein. The sequential dynamic mode and the rapid incrementation mode were used for scanning with contrast enhancement. CT diagnosis correlated with angiographic assessment of graft patency in 36 out of 36 grafts to the left anterior descending artery, in 4 out of 4 grafts to the diagonal branch, in 19 out of 20 grafts to the left circumflex artery, and 6 out of 7 grafts to the right coronary artery. Overall, CT correctly identified 62 out of the 64 patent graft (sensitivity 96.9%), and 3 out of the 3 occluded grafts (specificity 100%). The sensitivity of the incrementation mode alone to detect the bypass graft was 93.0% and that of the dynamic mode was 88.6%. The sensitivity of CT with both modes was 100%. Although CT does not have sufficient resolution to define graft stenosis, it is a noninvasive and clinically useful technique to evaluate bypass graft patency with high accuracy.

[Studies on the new long-acting anti-cancer preparation, 5-fluorouracil-polyglycolic acid composite].

Polymer polyglycolic acid is biocompatible in the body. A needle-shaped long-acting anti-cancer preparation (F-PGA needle) was prepared by compounding polyglycolic acid and the anti-cancer drug 5-Fluorouracil (5-FU). The release of 5-FU from the needle was maintained for about 10 days, and the needle disappeared after one year. A clinical study with the F-PGA needle was made on patients with terminal carcinomas. Shrinkage of tumors and tumor necrosis were observed in two patients with metastatic liver carcinomas. The F-PGA needle supplies high-dose 5-FU locally for a long time with fewer side effects by embedding it into the tumor tissue. Therefore the F-PGA needle can be said to be promising in the treatment of unresectable cancer as a topical application of chemotherapy.

[A successfully treated case of Wolff-Parkinson-White syndrome with Ebstein's anomaly complicated with protein-losing gastroenteropathy].

A 17-year-old male underwent the division of the double accessory pathways under the diagnosis of Wolff-Parkinson-White (WPW) syndrome. At this time, we perceived that Ebstein's anomaly was combined with WPW syndrome, but we didn't treat Ebstein's anomaly, because it was slight. After this operation he relieved from the tachycardia attack completely. Ten months later, he was re-admitted to our hospital because of cardiomegaly and systemic edema. Hypoproteinemia was pointed out. On the basis of the intracardiac catheterization and 131I-RISA test, it was made to be sure that hypoproteinemia was caused by the protein-losing gastroenteropathy due to the tricuspid valve regurgitation of Ebstein's anomaly. After the tricuspid valve replacement with Hancock valve, protein-losing into the gastrointestinal tract disappeared. The aggravation of the tricuspid regurgitation was unrelated to the division of the accessory pathways, and was caused by the natural course of Ebstein's anomaly.

[Simultaneous bilateral operations for bullous emphysema by median sternotomy].

Eighteen patients with bilateral bullae underwent simultaneous operations via median sternotomy. Each patient had giant bullae occupying at least one third of the each hemithorax and also had various degree of exertional dyspnea. All patients showed subjective as well as objective improvement and there was no mortality or morbidity. Late follow-up study for objective evaluation of this operative procedure clarified that the forced expiratory volume for one second (FEV1.0) and arterial oxygen tension (PaO2) were significantly improved after the operation, whereas the vital capacity and arterial carbon dioxide tension (PaCO2) showed no apparent changes. Serial study of cardiopulmonary hemodynamics during the bilateral thoracotomy was performed with Swan-Ganz catheter and hemodilution method. By abrupt decompression of the cyst, decreases in pulmonary arterial pressure, cardiac output, or systemic arterial pressure were noted during the bilateral thoracotomy. These hemodynamic changes were much more evident in the cases ventilated by single lumen tube than the cases operated upon using double lumen tube with separate ventilation. This operative procedure was applied on the cases with bilateral spontaneous pneumothorax and metastatic lung tumor. The clinical advantages of this bilateral operation via median sternotomy were discussed.

[Gastric lymphosarcoma associated with early carcinoma (type IIa)--a case report].

We report a case of gastric lymphosarcoma associated with early carcinoma (type IIa). Lymphosarcoma was also seen in the small intestine, but there were no metastatic lesions in the lymph nodes , liver, and peritoneum. This suggest that lymphosarcoma was the primary cancer in both the stomach and small intestine. Microscopically, the early carcinoma was a moderately differentiated tubular adenocarcinoma . Only two earlier cases of coexistent gastric lymphosarcoma and early gastric carcinoma have been reported in the literature.

Successful surgical ablation of reentrant ventricular tachycardia caused by myocardial fibroma.

We successfully removed a fibroma of the left ventricle in a 13-year-old boy. The tumor had caused recurrent ventricular tachycardia, which was resistant to antiarrhythmic drugs. Preoperative and intraoperative electrophysiological studies led to a diagnosis of reentrant ventricular tachycardia. After surgical excision of the tumor, the ventricular tachycardia stopped completely and until now, 1 year after the operation, no antiarrhythmic drugs have been required.

Phase analysis in the Wolff-Parkinson-White syndrome with surgically proven accessory conduction pathways: concise communication.

Twenty-one patients with the Wolff-Parkinson-White (WPW) syndrome who underwent surgical division of the accessory conduction pathway (ACP) were studied by gated blood-pool scintigraphy. In each case, a functional image of the phase was generated, based on the fundamental frequency of the Fourier transform. The location of the ACP was confirmed by electrophysiologic study, epicardial mapping, and surgery. Phase analysis identified the side of preexcitation correctly in 16 out of 20 patients with WPW syndrome with a delta wave. All patients with right-cardiac type (N = 9) had initial contraction in the right ventricle (RV). In patients with left-cardiac type (N = 10), six had initial movement in the left ventricle (LV); but in the other four the ACPs in the anterior or lateral wall of the left ventricle (LV) could not be detected. In patients with multiple ACPs (N = 2), one right-cardiac type had initial contraction in the RV, while in the other (with an intermittent WPW syndrome) the ACP was not detected. These observations indicate that abnormal wall motion is associated with the conduction anomalies of the WPW syndrome. We conclude that phase analysis can correctly identify the side of initial contraction in the WPW syndrome before and after surgery. However, as a method of pre-operative study, it seems difficult to determine the precise site of the ACP by phase analysis alone.
