[Clinical evaluation of vesicoureteroneostomy. I. Efficacy of surgery in 65 cases (100 ureters) of primary vesicoureteral reflux].

We have experienced 72 patients with primary vesicoureteral reflux (VUR) during the past 16 years. Sixty-five of them (100 ureters) were treated surgically to prevent reflux. Among the surgical methods employed, the Politano-Leadbetter method was the most frequent, being used in 56 patients (87 ureters). Other methods were the combined method in 2 patients (three ureters), the Glenn-Anderson method in 1 (2 ureters), the Cohen method in 1 (1 ureter) and the Lich-Gregoir method in 5 patients (7 ureters). As postoperative complications in the early stage (within 1 month after surgery), remaining VUR in 1 ureter in 1 of the patients who underwent surgery by the Politano-Leadbetter method and occurrence of VUR on the contralateral side in 3 patients with unilateral VUR were encountered. These conditions disappeared during the follow-up period. On the other hand, in the late postoperative stage, ureteral stenosis requiring further surgery occurred in 1 ureter in the transitional region to the urinary bladder in 1 patient 2 months after surgery by the Lich-Gregoir method. The rate of success was all 87 ureters (100%) for the Politano-Leadbetter method and 6 out of 7 ureters (85.7%) for the Lich-Gregoir method. The success rates for the other procedures were also good, resulting in an overall rate of 99 out of 100 ureters (99.0%). Ninety-nine percent of the patients had been treated at least once preoperatively for pyelonephritis or fever of unknown etiology. Among these patients, those who had had 5 or more such episodes accounted for 39%.(ABSTRACT TRUNCATED AT 250 WORDS)

[Clinical evaluation of vesicoureteroneostomy. II. Efficacy of surgery by Boari's method in eight and the psoas-hitch method in eight cases].

Vesicoureteroneostomy was performed by Boari's method on 8 patients and the psoas-hitch method on 8 patients during the 16 years from 1969 to 1986. Although Boari's method without the formation of a submucosal tunnel was employed for the initial 4 patients, the formation of this type of tunnel was a general principle for all patients thereafter. In choosing the surgical method, the psoas-hitch method was assigned to patients in whom it was possible to extend the urinary bladder to the upper part of the common iliac vessel and in whom the ureter allowed the formation of a submucosal tunnel, and Boari's method was assigned to those in whom it is was not possible to reach the ureter or to form a submucosal tunnel or in whom the ureter would be tensed. The mean length of the resected section of ureter was 75 mm for Boari's method and 43.2 mm for the psoas-hitch method. Follow-up observations revealed the presence of VUR in 2 patients who underwent surgery by the initial form of Boari's method and in 1 patient who underwent surgery by the psoas-hitch method. There was no ureteral stenosis or renal hypofunction in any of the patients.

[A clinical study of xanthogranulomatous pyelonephritis with special emphasis on the differential preoperative diagnosis between xanthogranulomatous pyelonephritis and renal cell carcinoma].

An accurate preoperative diagnosis of xanthogranulomatous pyelonephritis is difficult because of its clinical and radiological similarities to renal cell carcinoma. We report two cases of xanthogranulomatous pyelonephritis. Furthermore, in an attempt to clarify the clinical distinction between this entity and renal cell carcinoma, we summarize the clinical characteristics of 143 cases with xanthogranulomatous pyelonephritis in the literature and 126 cases with renal cell carcinoma experienced in our clinic. According to the clinical reviews, several characteristics of xanthogranulomatous pyelonephritis were revealed. 1) Presence of history of pyelonephritis. 2) gamma-globulinemia in blood chemistry. 3) Non-visualizing kidney on the excretory urogram. 4) Hypovascular or avascular features and dilatation of renal capsular arteries on angiogram. 5) Heterogenous renal mass and thickness of Gerota's fascia on computed tomogram. 6) Positive uptake of renal mass in Ga-scintigram. When some of these features are found in the renal mass, the case could be of xanthogranulomatous pyelonephritis and therefore a kidney preserving operation should be considered.

A possible mechanism of airway hyperresponsiveness induced by prostaglandin F2 alpha and thromboxane A2.

To elucidate the precise mechanisms of airway hyperresponsiveness induced by prostaglandin F2 alpha (PGF2 alpha) and thromboxane A2 (TXA2), we investigated the effects of the subthreshold dose (the highest dose which does not lead to contraction) of PGF2 alpha and stable TXA2 analogue, STA2 (epithiomethano-TXA2), on the smooth muscle contraction evoked by acetylcholine (ACh) and electrical field stimulation (EFS) in the canine trachea. These prostanoids produced no changes in the contractile response to both these stimuli. Thus, neither PGF2 alpha nor TXA2 affect the smooth muscle, as the site of action for ACh is the muscarinic receptor located on smooth muscle. Neither compound affect the postganglionic vagal efferent nerve, because the EFS contracts smooth muscle via activation of the postganglionic vagal efferent neuron. This study, together with our previous observation, suggests that PGF2 alpha and TXA2 induce airway hyperresponsiveness by stimulating vagal sensory endings and by activating the reflex pathway.

Rheumatoid adherent synovial cells produce B cell differentiation factor activity neutralizable by antibody to B cell stimulatory factor-2/interleukin 6.

B cell differentiation factor (BCDF) activity assayed on SKW6-CL4 cells was found in adherent synovial cell (ASC) conditioned medium. ASC of patients with RA (n = 8) produced significantly larger amounts of BCDF than those of patients with osteoarthritis (n = 5) (219.0 +/- 212.1 vs 25.5 +/- 12.4 units/microgram.DNA, p less than 0.01). ASC clones, established by the limiting dilution technique, also produced this factor. Experiments with several neutralizing antibodies revealed that this BCDF is inhibited by anti-B cell stimulatory factor-2/interleukin 6 antibody (anti-BSF-2/IL-6 ab) up to 90%, but not by antiinterleukin 1, antiinterleukin 2 or antiinterferon-r antibodies. Our data suggest that ASC could participate in the B cell differentiation process in joint space by producing a molecule which has a similar active site to BSF-2/IL-6.

[Correlations between masticatory performance in chewing gum method and occlusion as well as dento-facial morphology].

In order to examine the characteristics of the chewing gum masticatory test, the amount of sugar elution, the chewing time and the amount of sugar elution per second were measured in 109 young adults (93 males and 16 females), after 70 strokes of mastication. Their sex differences, differences among various occlusions, correlations with occlusal areas of the buccal teeth and correlations with roentgen cephalometrics were analyzed. 1. There was no sex difference in the amount of sugar elution. However, chewing time was shorter and the amount of sugar elution per second was higher in males than in females. 2. Compared with the normal occlusion group, the amount of sugar elution was significantly lower in the crowding and anterior cross bite groups (Inoue's classification), and in the Class III malocclusion group (Angle's classification) in males. The amount of sugar elution per second was also significantly lower in the crowding and anterior cross bite groups. However, significant differences could not be found for chewing time. 3. The amount of sugar elution per second was significantly correlated with occlusal areas in males and total samples. 4. The amount of sugar elution was significantly correlated with less than ANB and less than L 1 to MP in males, with less than L 1 to MP in total samples. The chewing time was significantly correlated with S-N, N-Me, N-ANS, S'-Ptm' in females. The amount of sugar elution per second was significantly correlated with less than SNA and less than L 1 to MP in males, and with A'-Ptm', Cd-Go, Go-Pog', Cd-Gn, less than SNA, less than L 1 to MP, less than MP-SN and gonial angle in total samples. 5. It is concluded that the chewing gum masticatory test is highly correlated with occlusion and dento-facial morphology. In particular, amount of sugar elution per second was thought to be a useful measure for masticatory performance in clinical orthodontics.

Effect of number of elements and size of stimulus field on recordability of pattern reversal visual evoked response.

The interrelationships among amplitude of the pattern reversal visual evoked response (VER), check size (spatial frequency) of the stimulus pattern elements, size of the stimulus field, and number of elements in the stimulus field were investigated. When the stimulus field was smaller than 4.0 degrees X 4.0 degrees, the VER amplitude-check size curve had a high-pass filter shape. When the stimulus field was larger than 5.0 degrees X 5.0 degrees, the curve showed the inverted-U shape. At the largest stimulus field, 12.0 degrees X 12.0 degrees, the curve retained the inverted-U shape, but its peak moved to a larger check size. With smaller check sizes (10 and 20 min), the VER amplitude increased up to a certain stimulus field size and then showed either saturation or a decrease. When the number of elements in the stimulus field was less than 40, the VER amplitude did not increase significantly in all check sizes. Above this number of elements in the field, the amplitude increased linearly until saturation was observed at an element number of 300-400. We concluded that the amplitude of the pattern reversal VER depended strongly on the interrelationship among check size, stimulus field size, and number of elements in the field.

[A clinical study of genital herpes and the clinical efficacy of acyclovir tablets].

A clinical study for genital herpes was conducted on 154 patients and the efficacy of treatment with oral acyclovir was investigated in 51 of these patients. The diagnosis was confirmed by direct immunofluorescence or viral isolation from the lesion. This disease has increased in both males and females in recent years and was found in 2.3-2.9% of the out-patients examined in 1986. Seventy percent of the patients were between 20 and 30 years old. About 70% of the male patients had phimosis. In patients with the first infection, bilateral eruption (62%) and lymphadenopathy (54%) were more common than unilateral lesions. However, in those with recurrent infection, unilateral eruption (72%) and lymphadenopathy (52%) were more common. Sixty two percent of those with the first infection had scattered eruption on external genitalia, but 71% with recurrent infection, had lesions concentrated in several areas. Local symptoms such as pain in the external genitalia (male: female, 16%: 85%), pain in the lower extremities (26%: 45%), discomfort in the lower extremities (20%: 41%) and systemic symptoms such as malaise (22%: 48%) and anorexia (4%: 35%) were seen more frequently in females than in males. In addition, systemic symptoms such as fever (first episode: recurrent episode, 36%: 4%), malaise (34%: 9%) and anorexia (18%: 2%) were seen more frequently in patients with the first episode than in those with recurrence. HSV type 1 infections were found in 16% of males and 28% of females with the first episode, but were less common in the recurrent episode, 0% and 13%, respectively. Direct immunofluorescence was positive in 75 (59%) of 128 samples diagnosed by viral isolation. Treatment with oral acyclovir tablets, 200 mg five times daily, was very effective in 26 of 30 patients (87%). No side effects were observed. In this study, acyclovir tablet has been shown to be a very effective and well-tolerated treatment for genital herpes infections.

Pattern reversal VER as a tool for evaluating unbalanced visual inputs between the two eyes.

We studied the effects of unequal or unbalanced visual inputs on the binocular system of normal subjects by measuring the amplitudes of the binocularly and the monocularly recorded VER and determining the value of binocular summation. The stimuli used were patterns that were either fused or not fused, of unequal luminosity and of unequal image size. When identical patterns were delivered to each eye and the patterns were fused, the binocular VER demonstrated a larger amplitude than the monocular VER, resulting in a binocular summation that was prominent in the low-contrast stimulus pattern. With stimulus patterns of higher contrast, the amplitudes of the binocular and the monocular VER did not differ greatly, and the value of binocular summation was significantly decreased. When fusion was disturbed with a base-in prism, the binocular VER displayed smaller amplitudes than the monocular VER, indicating binocular interaction or inhibition. With a small interocular luminosity difference, the binocular VER exhibited a larger amplitude than the monocular VER (binocular summation), but as the difference exceeded a certain level (0.6 to 0.8 log units), the binocular VER amplitudes were smaller than those of the monocular VER (binocular inhibition). With very large luminosity differences, the binocular VER amplitudes were almost similar to those of the monocular VER (suppression). When the perceived image size of the retina was altered with an aniseikonic lens, the binocular VER displayed larger amplitudes than those of the monocular VER (binocular summation) in aniseikonia equal to or less than 3.0%. At 5.0% aniseikonia, the binocular VER amplitudes were almost equal to the monocular VER (zero summation). At aniseikonia equal to or larger than 8.0%, the binocular VER amplitudes were significantly smaller than those of the monocular VER (binocular inhibition or interaction).

Primary structure of human poly(ADP-ribose) synthetase as deduced from cDNA sequence.

Human poly(ADP-ribose) synthetase consists of three proteolytically separable domains, the first for binding of DNA, the second for automodification, and the third for binding of the substrate, NAD (Ushiro, H., Yokoyama, Y., and Shizuta, Y. (1987) J. Biol. Chem. 262, 2352-2357). We have isolated and sequenced cDNA clones for the enzyme using synthesized oligodeoxyribonucleotide probes based on the partial amino acid sequence of the protein. The open reading frame determined encodes a protein of 1,013 amino acid residues with a molecular weight of 113,203. The deduced amino acid sequence is consistent with the partial amino acid sequences of tryptic or alpha-chymotryptic peptides and the total amino acid composition of the purified enzyme. The native enzyme is relatively hydrophilic as judged from the hydrophilicity profile of the total amino acid sequence. The net charge of the NAD binding domain is neutral but the DNA binding domain and the automodification domain are considerably rich in lysine residue and quite basic. The DNA binding domain involves a homologous repeat in the sequence and exhibits a sequence homology with localized regions of transforming proteins such as c-fos and v-fos. Furthermore, this domain contains a unique sequence element which resembles the essential peptide sequences for nuclear location of SV40 and polyoma virus large T antigens. These facts suggest the possibility that the physiological function of poly(ADP-ribose) synthetase lies in its ability to bind to DNA and to control transformation of living eukaryotic cells like the cases of those oncogene products.

Inhibitory effect of human recombinant interleukin-1 alpha and beta on growth of human vascular endothelial cells.

Endothelial cell growth factor (ECGF) is a potent polypeptide mitogen which stimulates the growth of endothelial cells. The mitogenic effect of ECGF was inhibited by addition of recombinant interleukin-1 (rIL-1) alpha or beta in a concentration dependent manner. The morphological change was not observed distinctly. In the condition without ECGF, both types of rIL-1 enhanced [3H]-thymidine uptake slightly, but failed to increase cell numbers. These data suggest the possibility that the effect of rIL-1 on EC is modulated by the presence of ECGF.
