Neurofibroma of the gallbladder seen as a papillary polyp.

A case of neurofibroma of the gallbladder in a 44-year-old woman without von Recklinghausen neurofibromatosis but with right hypochondralgia is reported. The tumor was seen preoperatively as a polypoid lesion and in the resected gallbladder as a papillary polypoid growth over the main intramural tumor mass. Microscopically, the tumor was identified as a neurofibroma with many tactile corpuscle-like structures (Wagner-Meissner bodies), which were found by electron microscopic and immunohistochemical examination to consist of Schwann cells. Twelve cases of non-epithelial neoplasms arising in the gallbladder are also reviewed.

Objective evaluation of binocular function with pattern reversal VER. IV. Effect of spatial and temporal frequency.

The effects of changing spatial and temporal frequencies on the amplitude of pattern reversal monocular and binocular visual evoked response (VER) were investigated. The pattern reversal VER and the degree of binocular summation (binocular VER amplitude/monocular VER amplitude) were lowest at the spatial frequency (check size) of 0.3 cycles per degree (CPD) and highest at the spatial frequency of 4.0 CPD. The largest VER amplitudes were observed at 1.2 CPD under both the binocular and monocular recording conditions. Regarding the effects of changing temporal frequency (alteration or reversal rate) on the pattern reversal VER, the transient condition (1.5 to 3.0 Hz) did not produce significant binocular summation. At the higher temporal frequencies (6 to 12 Hz), significant binocular summation was produced compared with the transient stimulus condition. At very high temporal frequencies, the degree of the binocular summation showed a decrease. From these results, we selected a pattern with an element around 1.2 CPD with a relatively swift temporal frequency for evaluating binocular function with the pattern reversal VER.

Squamous cell carcinoma of bladder diverticulum with initial symptoms produced by metastasis to maxillary sinus.

A rare case of squamous cell carcinoma in the bladder diverticula of a 74-year-old man with initial clinical symptoms caused by metastasis to the maxillary sinus is reported. The histopathological examination at autopsy confirmed the metastatic squamous cell carcinoma in the maxillary sinus from the bladder diverticula. The vertebral vein plexus seems responsible for this type of metastasis.

Analysis of photo-initiators in visible-light-cured dental composite resins.

Seven commercial visible-light-cured (VL) dental composite resins were analytically studied for identification of the photo-initiator consisting of photo-sensitizer and reducing agent. Gas-liquid chromatography (GC) was used for the determination of the dilute components extracted from the composite resin. Mass spectroscopy (GC-MS) was used for confirmation of the qualitative data obtained by GC. The results showed that all composite resins examined included camphorquinone (CQ) as a photo-sensitizer. The concentration of CQ in the resin phase, however, ranged from 0.17 to 1.03% w/w. The composite resin with hybrid-sized filler tended to have a higher concentration of CQ than did the micro-filled composite resin. As for the reducing agent, two out of seven brands contained dimethylaminoethyl methacrylate (DMAEMA), and one included dimethyl-p-toluidine (DMPTI). The mixing ratio between CQ and the amine in these three composite resins also varied. Another four brands did not contain either DMAEMA or DMPTI, and would utilize different reducing agents.

Autocrine regulation of cell proliferation by estradiol and hydroxytamoxifen of transformed mouse Leydig cells in serum-free culture.

We have previously reported that the cloned cell line (B-1-A-2) derived from an estrogen-responsive mouse Leydig cell tumor shows an estrogen-dependent enhancement of cell proliferation in medium supplemented with charcoal-dextran-stripped fetal bovine serum. To avoid the involvement of unknown factors present in the serum in the pathway for estrogen-dependent cell growth, the present study was designed to establish a serum-free culture system to which growth factors could be added. To this end, we subcloned B-1 cells from the parental tumor cell line. The proliferation of B-1 cells was markedly stimulated by the addition of 10(-11)-10(-8) M estradiol into the serum-free medium [Eagle's Minimum Essential Medium-Ham's F-12 (1:1, vol/vol) containing 0.2% (wt/vol) BSA]. Epidermal growth factor (0.1-50 ng/ml) or insulin (0.1-50 micrograms/ml) alone or in combination with 10(-8) M estradiol did not affect the proliferation rate of B-1 cells. In contrast, a greater than 10-fold molar excess of 4-hydroxytamoxifen blocked estradiol-induced cell proliferation, while 4-hydroxytamoxifen alone failed to show a stimulatory effect on cell multiplication. Additionally, the conditioned medium collected from estradiol-stimulated cells was found to contain a growth-promoting factor(s) whose activity was not antagonized by 4-hydroxytamoxifen. Nonstimulated cells secreted a significant but low level of the growth-promoting factor. Finally, B-1 cells were found to be estrogen dependent for cell proliferation in BALB/c mice. Their growth was markedly inhibited by the administration of tamoxifen to the host mice. These results indicate that the serum-free culture system presented here is suitable for studying the autocrine mechanisms of cell growth regulated by estrogens as well as triphenylethylene compounds.

Pharmacokinetics of FK482, a new orally active cephalosporin, in animals.

The pharmacokinetic profile of FK482 was studied in mice, rats, rabbits and dogs after oral dosing and compared with that of cefixime, cefaclor and cephalexin. Considerable differences in oral absorption of FK482 were observed among the animal species. Absolute bioavailabilities of FK482 were 12.6% in mice, 15.3% in rats, 32.3% in rabbits and 72.3% in dogs. In mice and rats, the absorption of FK482 was poor, and was the lowest of the reference antibiotics. FK482 was moderately well absorbed, with higher plasma levels than cefixime in rabbits and, like cefixime, gave higher plasma levels and a longer half-life than cefaclor or cephalexin in dogs. The increase in the area under the serum concentration time curve (AUC) of FK482 was strictly proportional to the increase in dose in the range of 2.5 to 40 mg/kg in rats and dogs, and 2.5 to 20 mg/kg in rabbits and the urinary recovery rates were almost constant. All tissue concentrations of FK482 in rats and rabbits were lower than those of the reference antibiotics and reflected its lower plasma concentrations in these animals. The urinary recovery rates of FK482 were 9.8% for mice, 15.5% for rats, 45.8% for rabbits and 47.1% for dogs. The biliary recovery rate of FK482 was low; 1.4% in rats and less than 0.1% in rabbits and dogs. No active metabolites were detected in the plasma, urine or bile samples from rats, rabbits or dogs. FK482 was mainly absorbed in the jejunum, and was inactivated in the large intestine. The serum-protein binding of FK482 was almost the same as that of cefixime: 60-77% for mouse, rabbit and human serum, and 90-93% for rat and dog serum.

Nonsurgical correction of cleft lip nasal deformity in the early neonate.

Auricular cartilage is soft and plastic, and congenital auricular deformities are easily corrected nonsurgically in the early neonatal period. Alar cartilage is the same kind of elastic cartilage as auricular cartilage. Therefore, alar cartilage is also malleable in the early neonate. When cleft lip repair is performed around the age of three months, it is difficult to obtain good nasal shape without surgical correction. In the early neonate, there is a fair possibility for success in the nonsurgical correction of cleft lip nasal deformity. We performed cleft lip repair accompanied with nonsurgical correction of nasal deformity in 48 neonates aged two to seven days. A special retainer was placed in the affected nostril during the postoperative period for three months. In 10 cases, preoperative nonsurgical correction with a special retainer began in the early neonatal period and the lip repair was done at about three months of age. Following observation of 40 infants for 12 months or longer, their nasal shapes and symmetry were considered superior to those conventionally operated on at about three months of age without nonsurgical correction of their nasal deformity during the early neonatal period. Except for one case of nasal infection, there were no other complications.

A method of bilateral cleft lip nose repair--a preliminary report.

By redefining the surgical anatomy of the nose, we were able to develop a new technique for the repair of the bilateral cleft lip nose. The lower one-third of the nose was divided into an outer layer, composed of skin, and an inner composed of fascia, muscle, cartilage and the skin of the vestible. From this viewpoint, the bilateral cleft lip nose deformity was reinterpreted as a combination of dislocation and deformity of the inner layer, and rotational displacement of the inner layer with respect to the outer layer. Using the double alar wing flap incision, we were afforded good visualization and anatomic reconstruction without tension by rotating the inner and outer layers in opposite directions. The length of the columella, the alar facial angle, the width of the nose and the size of alae have been satisfactory in 6 patients.

[Efficacy of ofloxacin in sexually transmitted male urethritis and cervicitis].

Efficacy and safety of ofloxacin (OFLX), a new quinolone antibacterial agent, were investigated in cases of sexually transmitted male urethritis and cervicitis. Chlamydial infections occupied a large part of the patients enrolled. This study was conducted in 29 institutions. A total of 1,126 patients with gonococcal, chlamydial or non-gonococcal non-chlamydial infections were enrolled in the study. The general dosing regimen of oral OFLX was 100 mg t.i.d. or 200 mg t.i.d. for 14 consecutive days. Along with the clinical efficacy evaluated by doctors in charge, therapeutic and clinical efficacies were determined according to standardized evaluation criteria. The evaluation of therapeutic efficacy was based on bacterial eradication. On the other hand, clinical efficacy was determined upon the improvement in causative bacteria, WBC counts in smear (or initial urine) specimens and urethra/cervical discharges. Clinical efficacy was determined on 7 and 14 days after starting the medication, except for gonococcal infections for which the efficacy was determined on Day 3 and Day 7. The results obtained are summarized as follows. 1. Efficacy evaluation by standardized criteria Therapeutic efficacy rates in gonococcal infections were 97.9% (93/95) on Day 3 and 100% (119/119) on Day 7. In chlamydial infections, the rates were 77.7% (363/467) on Day 7 and 93.6% (392/419) on Day 14. Chlamydial urethritis rated 94.8% (239/252) and cervicitis 91.6% (153/167) on Day 14. Clinical efficacy rates in gonococcal infections were 63.2% (60/95) on Day 3 and 95.8% (114/119) on Day 7. In chlamydial infections, they were 70.0% (327/467) on Day 7 and 89.7% (376/419) on Day 14. Chlamydial urethritis rated 88.5% (223/252) and cervicitis 91.6% (153/167) on Day 14. Clinical efficacy rates in non-gonococcal non-chlamydial infections were 93.3% (97/104) for male urethritis and 65.9% (27/41) for cervicitis on Day 14. 2. Clinical efficacy determined by doctors in charge A total of 1,028 cases, 752 urethritis and 276 cervicitis cases, was evaluated by doctors in charge. Clinical efficacy rates were 90.3% for urethritis and 92.4% for cervicitis. 3. Safety Safety evaluations was conducted in 1,087 patients. Side effects were observed in 23 patients among them (2.12%). None of them were deemed serious. General laboratory examinations were conducted for 169 patients. Abnormal findings were seen in 11 patients (6.51%) or 13 cases, all of which were minor. The results of this multi-center clinical trial demonstrated that OFLX is effective and well tolerated in the treatment of sexually transmitted disease (STD), both gonococcal and chlamydial infections.
