[Comparative studies on activities of antimicrobial agents against causative organisms isolated from urinary tract infections (1986). II. Background of patients].

From July to October in 1986, hospital data on 717 isolates of causative microorganism from patients with urinary tract infections and background information on 562 patients were collected. The urinary tract infections were divided into 3 groups; simple infections and complicated infections with or without the use of indwelling catheter. We investigated such factors as the patients' sex, age, and drug treatments, and the isolates obtained from the infections. From 1981 to 1986, about 25% of the patients with simple infections were males and about 60 to 70% among those with complicated infections were males each year. During these 6 years, no noticeable changes occurred in sex and age distributions of patients with any type of the infections, but the species of isolates changed remarkably. In 1982 and 1983, the isolation rates of Enterococcus faecalis (Streptococcus spp. or Streptococcus faecalis) obtained from the patients were about 8% in both of male and female patients. In 1984, the rate in females increased to 17.9%. In 1985, the rates in males and females were 14.9% and 7.6%, respectively. Similar rates were also observed in 1986: 16.1 and 7.4%. The isolation rate of Staphylococcus aureus in 1982 was 0.7%, as was in 1983. In however, increased to 1.7% in 1984 and to 3.2% in 1985. It was 2.5% in 1986. The isolation rate of Gram-positive bacteria was 11.1% in 1982; it increased to 18.9% in 1983 and to over 20% between 1984 to 1986. Among Gram-negative bacteria, the isolation rate of Pseudomonas aeruginosa form patients with complicated urinary tract infections with a catheter was high and over 20% in both 1985 and 1986. When antibacterial agents were administrated for 3 days, changes in number of species of isolates tended to be more pronounced in complicated infections with or without the use of catheter than in simple infections. According to date obtained in 1986, when antibiotic agents were administered for 8 or more days, the changes were definitely more drastic in complicated infections than in simple ones. An upmost care in the selection of antibiotic drugs according to their antibacterial spectra and activities, therefore, appears necessary, and an attention should be paid on future changes in species of bacteria isolated from infections.

Diagnosis and management of massive vitreous hemorrhage caused by retinal tear.

Fourteen cases of massive vitreous hemorrhage caused by a retinal tear, the cause having been confirmed during closed vitrectomy, were studied retrospectively. In the 3 cases with no retinal detachment and in the 6 cases with partial but not extensive retinal detachment at the time of surgery, the electroretinogram (ERG), visual evoked response (VER) and electrically evoked response (EER) were recordable preoperatively. After surgery the retina was attached in all 9 cases, but later redetached in one case. The visual acuity improved in 8 of the 9 cases. In the 5 cases with subtotal or total retinal detachment at the time of surgery, the ERG was nonrecordable preoperatively in all 5 cases, while the VER and EER were recordable in 4 cases. After surgery the retina was reattached in 3 of the 5 cases, but later redetached in one case. Visual acuity improved in only 2 of the 5 cases. When a retinal tear is suspected to be present, surgical intervention should not be delayed in cases of massive vitreous hemorrhage which does not clear after initial bilateral patching and bed rest. Ultrasonographic and electrophysiological tests help establish the diagnosis and prognosis.

[Analysis of systemic lupus erythematosus (SLE) involving the central nervous system by magnetic resonance imaging (MRI)].

Involvement of the central nervous system (CNS) commonly occurs in systemic lupus erythematosus (SLE). But definitive diagnosis remains difficult even with computed tomography (CT). In this study, we used the recently developed technique, magnetic resonance imaging (MRI) for CNS lupus and compared it with CT scans. CT was performed with a General Electric 8800 CT/T scanner. MRI was performed using a Mitsubishi Electric MMI-150 S. Ten patients with CNS lupus were divided into 3 groups. Group I included 4 cases with neurological manifestations alone. All lesions seen on CT were also detected by MRI, with greater clarity and extent. Furthermore, MRI depicted several microinfarcts in white matter without symptoms. Group II included 5 cases with psychiatric features alone. MRI detected a thalamic microinfarct in only one case while CT showed no abnormality in all cases. Group III included 1 case with both neurological and psychiatric symptoms. MRI demonstrated a small infarct of midbrain corresponding with neurological symptoms, more clearly than CT. Therefore MRI demonstrates the degree of brain involvement in SLE more accurately than CT.

[Soluble interleukin-2 receptor levels in patients with pulmonary tuberculosis].

Utilizing an enzyme-linked immunosorbent assay, we detected markedly elevated serum levels of soluble interleukin-2 (IL-2) receptors in patients with untreated pulmonary tuberculosis. In these patients, we also found that serum levels of soluble IL-2 receptors were closely correlated with serum adenosine deaminase levels (r = 0.869, p less than 0.001). Therefore, serum soluble IL-2 receptors appear to reflect the existence of active cell-mediated immunity in pulmonary tuberculosis and may prove to be a useful immunological marker for pulmonary tuberculosis.

Heterogeneity of B cell responsiveness to interleukin 4, interleukin 6 and low molecular weight B cell growth factor in discrete stages of B cell activation in patients with systemic lupus erythematosus.

To investigate the differential stages of B cell activation in patients with systemic lupus erythematosus (SLE), the effects of recombinant interleukin-4 (rIL-4) interleukin-6 (rIL-6), and low mol. wt BCGF (1-BCGF) on B cell proliferation and differentiation were evaluated. In a co-stimulatory assay with anti-IgM, proliferative response to rIL-4 and 1-BCGF showed no significant differences between SLE patients and healthy controls. In a restimulation assay, after pre-activation with anti-IgM for 3 days, proliferative response to rIL-4 and 1-BCGF was significantly decreased in SLE patients compared with normal healthy controls (P less than 0.01). With regard to rIL-6 induced B cell differentiation, SAC-pre-activated low density, large B cells from both SLE patients and healthy controls differentiated well, whereas high density B cells did not differentiate at all. Of particular interest, low density B cells from active patients directly differentiated into Ig secreting cells in response to rIL-6 without SAC activation. These data indicate that heterogeneity of B cell responsiveness to B cell-tropic interleukins resulted from the discrete stages of B cell activation in SLE.

[Vasoactive intestinal peptide (VIP) protects against acid-induced acute lung injury in isolated perfused rat lungs].

Acute, diffuse lung injury, the principal lesion in ARDS, is often refractory to treatment. Recently, pretreatment with several pulmonary vasodilators that increase cAMP levels: isoproterenol, terbutaline, theophylline, and prostacyclin, was found to reduce the severity of lung injury in animal models. We have investigated the possible modulation of HCl-induced pulmonary edema in rats by VIP, a lung neuropeptide with potent vasodilator and cAMP-producing properties. The lungs of rats were perfused in situ at 10 ml/min with Krebs-4% albumin solution, and ventilated at constant tidal volume (6.5 ml/kg). Peak airway pressure (PAW), mean pulmonary arterial pressure (PPA) were measured throughout the experiment, and wet to dry lung weight ratio (W/D), afterwards. All animals were observed for one hour. In 6 rats receiving HCl only, 0.2 N-HCl was instilled intratracheally at 2 ml/kg. Four rats received 2 ml/kg of physiological saline intratracheally as control. In 6 other animals, VIP was infused into the pulmonary artery at 1 micrograms/kg/min, beginning 10 minutes before HCl and for the rest of the experiment. Another 6 rats were pretreated with atrial natriuretic peptide (ANP, atriopeptin II) just like the VIP group. Lungs of saline control animals showed little or no chage in PAW or PPA. With HCl alone, PAW increased immediately and continued to rise for the rest of the hour, reaching 500% of basal value at 30 minutes. PPA increased by 68% and W/D by 74% compared to saline-instilled lungs. In the VIP + HCl group, all abnormalities were significantly reduced relative to the HCl group. The rise in PAW was attenuated by 79% (p less than 0.01).(ABSTRACT TRUNCATED AT 250 WORDS)

A review of 324 cases of idiopathic premacular gliosis.

We reviewed the records of 324 patients (395 eyes) seen between 1973 and 1987 with a diagnosis of idiopathic premacular gliosis. Mean age of onset was 64.6 years, 189 (58.3%) were women, and all but six patients were white. Initial visual acuity was 20/40 or better in 214 eyes (54.2%), 20/50 to 20/100 in 136 eyes (34.4%), and poorer than 20/100 in 45 eyes (11.4%). Follow-up examinations were made in 214 eyes. After a mean follow-up period of 33.6 months, 106 (49.5%) of the 214 eyes maintained a visual acuity within one line of initial visual acuity, 28 (13.1%) were more than one line better, and 80 (37.4%) were poorer. Partial or complete posterior vitreous detachment was present in 303 (84.9%) of the 357 eyes undergoing vitreous study; 43 (14.2%) of these 303 eyes had partial vitreous detachment with vitreous adhesion to the macula. Cystoid macular edema was present on angiography in 77 (20.6%) of 373 eyes undergoing fluorescein angiography; 20 (26.0%) of these 77 eyes had partial posterior vitreous detachment with vitreous adhesion to the macula, whereas only 23 (7.8%) of the 296 eyes without cystoid macular edema had such vitreous adhesion (P less than .001). Other findings were myopia in 116 of 367 eyes (31.6%) (103, or 88.8%, of the myopic eyes had posterior vitreous detachment), and increased intraocular pressure in 56 of 324 eyes (17.3%).

Analysis of the complete nucleotide sequence of the group IV RNA coliphage SP.

We report the nucleotide sequence of the Group IV RNA bacteriophage SP. The entire sequence is 4276 nucleotides long. Four cistrons have been identified by comparison with the related Group III phage Q beta. The maturation protein contains 449 amino acids, the coat protein contains 131 amino acids, the read-through protein contains 330 amino acids and the replicase beta-subunit contains 575 amino acids. SP is 59 nucleotides longer than Q beta. We have analyzed both sequence and structural conservation between SP and Q beta and shown that the sequences for the coat and central region of the replicase are strongly conserved between the two genomes. We also show that the S and M replicase binding sites of Q beta are strongly conserved in SP. Interestingly, the base composition of SP and Q beta differ significantly from one another, and most of the differences can be accounted for by a strong preponderance of U in the third position of each codon of Q beta relative to SP. We also compare conserved hairpins associated with potential coat protein and replicase binding sites.

Production of B cell stimulatory factor-2/interleukin-6 activity by human endothelial cells.

The effect of culture supernatants of endothelial cell (EC) lines on the immunoglobulin-M(IgM) synthesis by human B cell line, SKW6-CL4 cells, was investigated. Supernatants of human EC stimulated IgM synthesis, as high as 6-fold, but supernatants of bovine EC did not. This enhancing activity was completely blocked by addition of anti-human B cell stimulatory factor-2/interleukin-6 (BSF-2/IL-6) antibody. These data suggest that human EC might participate in the human antibody production system by producing soluble factor, BSF-2/IL-6.

Effect of stimulus field size and localization on the binocular pattern reversal visual evoked response.

The role of the central and peripheral stimulus fields on monocular and binocular amplitude and binocular summation of the pattern reversal visual evoked response were investigated. When the central stimulus field size was smaller than 2.4 degrees, there was no significant difference between the amplitude of the monocular and the binocular responses, but when it was equal to or larger than 3.2 degrees x 3.2 degrees, the binocular amplitude was significantly larger than the monocular. The value of binocular summation was highest at the central stimulus field of 4.0 degrees x 4.0 degrees; at larger sizes, there were no significant changes in the value. Use of a central stimulus field size larger than 3.2 degrees x 3.2 degrees was therefore considered a prerequisite for the effective assessment of visual function, especially binocular function, by means of the pattern reversal visual evoked response. With regard to the role of peripheral stimulus field on pattern reversal response, both the monocular and binocular responses, but particularly the latter, were found to be sensitive to a scotoma produced by covering the center of a full-field stimulus. The value of the binocular summation showed a significant reduction with a small central scotoma. We concluded that the pattern reversal visual evoked response is very sensitive to a central scotoma and that binocular function is mediated mainly through the central stimulus field.

Cat scratch disease. An epidemiological and ultrastructural study of lymphadenitis caused by Warthin-Starry positive bacteria.

The aetiological agent of cat scratch disease (CSD) has been unknown for more than 30 years. Recently, a micro-organism clearly shown with Warthin-Starry silver (W-S) stain was found and thought to be a possible cause of the disease. In this study, 32 cases of regional lymphadenopathy histologically compatible with CSD and 20 contrasting cases of lymphadenopathy were examined retrospectively with W-S stain. W-S positive pleomorphic organisms were clearly demonstrated in 20 of the 32 suspected cases of CSD, but in none of the other cases. The onset of disease in these 20 cases with W-S positive organisms occurred between July and January. This seasonal variation in the onset of disease was highly significant (P less than 0.005) and was not due to a single epidemic. Moreover, some characteristic morphological features of the organism were found by electron microscopic observations. Ultrastructurally, the organism was a bacterium showing a chain-like arrangement, septal formation, branching and clubbed ends.

Stimulatory effect of CD5 antibody on B cells from patients with rheumatoid arthritis.

In order to clarify the role of CD5 antigen on B cell in autoimmunity, we examined B cells from patients with rheumatoid arthritis (RA). The percentages of CD5 positive B cells were increased in peripheral blood from RA compared with normal. Normal and RA B cells were stimulated with two kinds of monoclonal antibodies to CD5 (Leu-1, SL-1) which recognize different epitopes. RA B cells proliferated and secreted IgM by CD5 antibody stimulation in combination with IL-1. Our observations imply that CD5 positive B cells in RA are in their differentiation stage and that CD5 antigen might be one of the triggers to activate CD5 positive B cells in vivo to produce autoantibody.

Establishment of a monoclonal antibody recognizing ultraviolet light-induced (6-4) photoproducts.

We obtained a monoclonal antibody directed against UV-induced DNA damage. Analysis of the antigenic determinant in UV-irradiated DNA recognized by this antibody, 64M-1, revealed that it bound UV-irradiated oligo- or poly-nucleotides containing thymine-thymine or thymine-cytosine sequences. The antibody failed to bind DNA irradiated with 313 nm UV in the presence of acetophenone, which contained predominantly thymine dimers as DNA damage. The binding activity of this antibody to 254-nm UV-irradiated DNA decreased with 313-nm UV irradiation, and the decrease of this binding activity correlated with the decrease of fluorescence corresponding to (6-4) photoproducts. These results suggest that the antigenic determinant recognized by this monoclonal antibody is a (6-4) photoproduct. Using autoradiography with 3H-antibody, we could detect the formation of the (6-4) photoproduct in individual human cells irradiated with 254-nm UV doses as low as 20 J/m2.

Binocular function in unilateral aphakia. Correlation with aniseikonia and stereoacuity.

Aniseikonia and stereoacuity were measured in patients with unilateral aphakia, most of whom were postoperative senile cataract cases. The New Aniseikonia test was used to evaluate aniseikonia and the Titmus Stereotest to measure stereoacuity. Ninety cases were studied, 57 (63.3%) of which had intraocular lens (IOL) implants; 27 (30%) had extended-wear soft contact lenses; and six (6.7%) had spectacle lenses. In the IOL group, aniseikonia averaged 2.8%, and 39 patients (68.4%) had good stereoacuity. In the contact lens group, aniseikonia averaged 4.6%, and 11 (40.7%) had good stereoacuity. In the spectacle lens group, aniseikonia averaged 17.8%; none of the patients had good stereoacuity. The authors concluded that in cases with unilateral aphakia, correction with an IOL implant is superior to the other alternatives in achieving good binocular function.

Chest-wall reconstruction by contralateral latissimus dorsi musculocutaneous flap.

Reconstruction of chest wall and axilla are performed in 11 patients using a contralateral latissimus dorsi musculocutaneous flap. The entire lattisimus dorsi muscle, including the fascial portion, safely carried an island of skin from the area of the lumbodorsal fascia to the contralateral axilla. The flap was transposed to the defect through a tunnel between the pectoralis major and minor muscles. Most patients who needed reconstruction of the chest wall and axilla had compromised ipsilateral vasculature that prohibited its use in a pedicled flap but had an intact contralateral chest wall, axilla, and thoracodorsal vessels. Therefore, this procedure was performed easily in comparison with a free flap or pedicled omental flap. This is a new, valuable application for the versatile latissimus dorsi musculocutaneous flap.
