Laboratory evaluation of cefmenoxime: a new cephalosporin. In vitro and in vivo antibacterial activities and pharmacokinetic properties.

Cefmenoxime is a new syn-methoxyimino cephalosporin antibiotic derived from cefotiam, which has been proved to be a very effective and useful antibiotic for the treatment of respiratory infections. This bacteriological and pharmacokinetic study was therefore performed in order to evaluate the potency of cefmenoxime in the treatment of respiratory infections. The minimum inhibitory concentrations of cefmenoxime against 179 isolates of respiratory pathogens (Streptococcus pneumoniae 53, Haemophilus influenzae 64, Klebsiella pneumoniae 43, Escherichia coli 9, Enterobacter spp. 10) were less than 0.20 micrograms/ml, and 43 (73%) of 60 Pseudomonas aeruginosa were inhibited by 12.5 micrograms/ml. In vitro antibacterial activity of cefmenoxime was superior to 18 other antibiotics, including cefotiam and cefotaxime tested in this study. Pharmacokinetic studies on tissue distribution in rats, serum levels and urinary excretion in 3 healthy volunteers, and penetration into bronchial secretes of 9 patients with respiratory infections, revealed that cefmenoxime has a higher penetration into the lung and bronchial secretes compared with cefotiam and cefotaxime. In 1 patient with chronic bronchiolitis, the concentration of cefmenoxime in the intra-bronchial secrete reached 12.5 micrograms/ml. From these results, it is concluded that cefmenoxime is a highly potent and useful antibiotic, and may be more effective in the treatment of respiratory infections than many other cephalosporins, including cefazolin, cefotiam and cefotaxime.

Urethral response to nerve stimulation measured by strain gauge force transducer.

This paper presents a study of urethral responses to efferent nerve stimulation in dogs. The pelvic, hypogastric and pudendal nerves were stimulated using a programmed instrument under three different in vivo conditions. The preparations consisted of: (A) bladder, urethra, and rectum; (B) urethra and rectum, and (C) urethra only. Stimulation of the pelvic nerve in preparation A resulted in elongation and constriction of the mid and proximal urethra with a resultant rise in pressure. However, this response was reduced in preparations B and C. Similar observations were made with hypogastric nerve stimulation. The results of pudendal nerve stimulation were significantly different, in that the pressure rise occurred in the mild and distal urethra and varied in elongation response.

Thyroid hormones in human milk and their influence on thyroid function of breast-fed babies.

Various assay methods for detection of thyroid hormones in human milk were evaluated in recovery and dilution experiments after which the concentrations of thyroxine (T4) and 3,5,3'-triiodothyronine (T3) were measured and compared with those in serum. The effect of breast feeding on pituitary thyroid function of normal babies also was studied. Competitive protein-binding analysis (CPBA) was found to be unsuitable for measurement of T4 in milk. T4 was not detected in samples of human milk by four radioimmunoassays (RIA), although more than 100% of T4 was recovered in the assays. RIA (double antibody-ANS system) seemed to be reliable for detection of T3 in milk, judging from recovery and dilution experiments. T3 was detectable in all samples obtained 1-4 months postpartum. The T3 concentration in milk was not correlated with protein concentration or daily volume. The concentration of T3 in milk was lower than that in serum and the mean ratio of serum T3 to milk T3 was 2.8 +/- 1.7 (mean +/- S.D.). No correlation was observed between the T3 concentration or daily T3 excretion in milk and the T3 concentration in serum. The total amount of T3 excreted in milk was estimated as only 5-1000 ng/day. The serum levels of thyrotropin, T4, free T4 and T3 were not significantly different between breast-fed and bottle-fed babies. These results indicate that T3 excretion in milk cannot be explained by simple diffusion from the blood into the mother's milk and that breast feeding has no influence on the pituitary thyroid axis of normal babies.

Effect of verapamil on the lower urinary tract.

The effect of verapamil on the in-vivo bladder and urethra was investigated in female dogs. The bladder and urethral pressure responses to electrical stimulation of the pelvic and hypogastric nerves were decreased by verapamil.

Application of ultrasonic M-mode scanning and computer evaluation on urodynamics.

The M-mode scanning images of urethra, detrusor pressure and urine flow rate during voiding were recorded together on the line scan recorder, and allowed for plotting and calculation of some valuable urodynamic parameters by computer evaluation. This procedure will permit first application of M-mode scanning and new direction on urodynamics.

Special bacterial polysaccharides and polysaccharases.

Alcaligenes faecalis var. myxogenes 10C3, which we isolated from soil, produces a water-soluble and an insoluble extracellular polysaccharide. The former (succinoglycan) is composed of glucose, galactose, pyruvic acid and succinic acid (molar proportions 7:1:1:1) with (beta 1-3)-, (beta 1-4)- and (beta 1-6)-glucosidic linkages. The latter (curdlan) is composed entirely of (beta 1-3)-linked D-glucose and forms a resilient firm gel when heated in suspension. The organism also produces extracellularly a repeating-unit octasaccharide of succinoglycan and cyclic (beta 1-2)-D-glucan. These polymers or oligomers are also produced by many strains of Agrobacterium and Rhizobium. Spontaneous mutation in ability to produce these polysaccharides or oligosaccharides occurs in these strains. The structures of succinoglycan, and similar polymers containing riburonic acid or galactose as the end residue of the side chain, were elucidated by successive fragmentation with two special enzymes obtained from Cytophaga arvensicola followed by methylation analysis. It is interesting that the unit compound in the biosynthesis of succinoglycan is identical with the unit compound in the enzymic decomposition of the polymer. Studies on curdlan gel by X-ray, 13C n.m.r. and electron-microscopic analysis and other physicochemical methods showed that the molecular structure of curdlan changes from a single helix to a triple stranded helix on heat treatment at high temperature. Curdlan seems to be useful for making new types of jelly products and may also be useful in new procedures for food production. Interestingly, curdlan possesses marked antitumour activity. Extracellular isoamylase (EC 3.2.1.68; glycogen 6-glucanohydrolase) of Pseudomonas amyloderamosa SB 15, which we isolated from soil, is very useful for elucidation of the structure of amylopectin and glycogen, and also for the commercial production of amylose or maltose alone or in combination with beta-amylase. Maltose is useful as a sugar for injection, being better than glucose. Maltitol is easily produced from maltose by chemical reduction and is used as a low-calorie sweetener. The isoamylase is also effective for enhancing the production of glucose from starch by the action of glucoamylase.

[Multiple meningioma].

Meningiomas are the most popular benign intracranial tumors, but they are rarely seen as a multiple growth. The incidence of multiple meningiomas, defined by Cushing and Eisenhardt, is about 1 to 2 per cent of all meningioma cases. Though many cases of the multiple meningiomas were reported previously in the world, some of them were concomitant with von Recklinghausen's disease or acoustic neurinoma. Most of the cases of the multiple meningiomas reported showed multiple lesion at the time of operation or after a few years of the initial operation. We have encountered two patients with multiple meningioma without the stigmata of von Recklinghausen's disease in our clinic. Case 1. A 58-year-old female visited the hospital complaining of headache and occasional nausea on February 6, 1980. Plain and enhanced cT confirmed a large tumor in the right parietal region and three small tumor nodules in the right occipital region. Carotid angiogram detected only two tumors of frontal falx. Apparent two tumor stains were seen on the region, and they were fed by meningeal frontal and parietal region, and they were fed by meningeal arteries through the right ophthalmic artery. A large tumor of parietal and a small tumor of frontal region on the right side of falx were removed. Three nodular tumors of right occipital convexity were extirpated at the same time. Histological examination of the tumors disclosed all extirpated tumors were fibroblastic meningioma. Case 2. A 61-year-old male developed convulsive seizure of the right upper limb and right side of the face was diagnosed as having convexity meningioma in the left parietal region.(ABSTRACT TRUNCATED AT 250 WORDS)
