[New approach for diagnosis and postoperative evaluation in herniated lumbosacral disc].

Dermatomally activated somatosensory evoked potentials (DSSEP's) were recorded over the scalp in 50 healthy subjects and 40 patients who later underwent exploration for herniated lumbosacral discs. The control subjects showed three negative peaks, N40, N56 and N78, and N41, N59 and N82 after bilateral stimulation of L5 and S1 dermatomes, respectively. In the patient group, the first two peaks were often abnormal in the presence of clinical sensory deficit, whereas the third peak tended to remain normal. At surgery, herniated disc was confirmed in all but six patients. The findings in myelogram were misleading in eight patients, showing either false positive or false negative results. The DSSEP's were normal in seven patients with documented disc protrusion at the level of L5 or S1 root. The test was also normal in another case with herniation at the level of S2 root. In this series, DSSEP's was extremely accurate in defining the level and the degree of lumbosacral root involvement. The findings on electrophysiology and myelography are not comparable, but are complementary, the former indicating function and the latter structure. Thus, electrophysiologic studies and myelography were complementary in providing an accurate diagnosis for herniated lumbosacral disc.

A potent nucleoside triphosphate hydrolase from the parasitic protozoan Toxoplasma gondii. Purification, some properties, and activation by thiol compounds.

A potent nucleoside triphosphate hydrolase (EC 3.6.1.3) with a number of unusual properties has been found in the parasitic protozoan (Toxoplasma gondii) and has been purified to homogeneity. The enzyme is localized in the cytosol and constitutes 3-4% of the total cytosolic protein. It has a molecular weight of 240,000-260,000 and contains four equivalent subunits of Mr = 63,000. Dithiol compounds such as dithiothreitol, dithioerythritol, or dimercaptopropanol were essential activators of the enzyme. Monothiol compounds had no effect. The specific activity of the purified enzyme was 2,500 mumol/min/mg at 37 degrees C under optimal conditions. Magnesium was the most effective activating metal ion, although manganese and calcium were also active. A higher excess of magnesium over total ATP was essential for maximal activity. Anions were found to inhibit the enzyme activity in an almost chaotropic order. The enzyme demonstrated a wide substrate specificity for both ribo- and deoxyribonucleoside triphosphate and hydrolyzed these nucleotides at almost the same rate. ADP was also a substrate and was hydrolyzed at a rate of 18% of that for ATP. Slight activity was seen with inorganic tri- and tetrapolyphosphates but not with monophosphate compounds. Km values for MgATP2- and MgADP- were 0.12 +/- 0.01 mM and 0.70 +/- 0.06 mM, respectively.

Enzymes of the de novo pyrimidine biosynthetic pathway in Toxoplasma gondii.

All six enzymes of the de novo biosynthetic pathway leading to the biosynthesis of UMP have been characterized in Toxoplasma gondii. The first three enzymes of the pathway, carbamyl phosphate synthetase-II (CPS-II), aspartate transcarbamylase (ATCase) and dihydroorotase (DHOase) could be consistently separated by sucrose gradient centrifugation. Their molecular weights were estimated to be approximately 540 000, 140 000 and 70 000, respectively. The last two enzymes, orotate phosphoribosyltransferase (OPRTase) and orotidylate decarboxylase (ODCase), cosedimented at the same position, corresponding also to a molecular weight of approximately 70 000. The fourth enzyme, dihydroorotate dehydrogenase (DHO-DHase), was associated with the particulate fraction. Apparent Km values for the respective enzymes were: CPS-II, MgATP2- (19.7 1.2 mM), L-glutamine (12.0 +/- 1.7 microM), ammonia (15.5 +/- 2.7 mM); ATCase, carbamyl phosphate (26.2 +/- 3.5 microM), L-aspartate (17.6 +/- 8.5 mM); DHOase (reverse direction) dihydroorotate (1.6 +/- 0.08 microM); ODCase, orotidine 5'-monophosphate (0.41 +/- 0.04 microM). MgUTP2- was found to act as an inhibitor of CPS-II, with an apparent Ki of 0.41 mM. However, 5-phospho-alpha-D-ribosyl-1-diphosphate, dimethyl sulphoxide and glycerol had no effect on the Km value for MgATP2-. The effect of some inhibitors, including pyrimidine and purine nucleotides and analogs and respiratory chain inhibitors, was also determined for the enzymes of the pathway.

Fracture of the hook of the hamate: report of six cases and the suitability of computerized tomography.

During the last 3 years, six patients with a fracture of the hook of the hamate were seen. All fractures were recognized by the carpal tunnel radiogram, but three were much more clearly demonstrated by computerized tomography of the carpal bones. Immobilization in plaster cast was beneficial in one patient and the other five patients required removal of the fractured fragment. All six patients were able to return to their previous occupations and sport activities.

Cerebral blood flow and CO2 reactivity in transient ischemic attacks: comparison between TIAs due to the ICA occlusion and ICA mild stenosis.

Hemispheric mean cerebral blood flow (CBF), together with its CO2 reactivity in response to hyperventilation, was investigated in 18 patients with transient ischemic attacks (TIAs) by intraarterial 133Xe injection method in a subacute-chronic stage of the clinical course. In 8 patients, the lesion responsible for symptoms was regarded as unilateral internal carotid artery (ICA) occlusion, and in 10 patients, it was regarded as unilateral ICA mild stenosis (less than 50% stenosis in diameter). Resting flow values were significantly (P less than 0.05) decreased in the affected hemisphere of TIA due to the ICA occlusion as compared with the unaffected hemisphere of the same patient, regarded as the relative control. It was not decreased in the affected hemisphere of TIA due to the ICA mild stenosis as compared with the control. With respect to the responsiveness of CBF to changes in PaCO2, it was preserved in both TIAs, due to the ICA occlusion and ICA mild stenosis. Vasoparalysis was not observed in either types of TIAs in the subacute-chronic stage. However, in the relationship of blood pressure and CO2 reactivity, expressed as delta CBF(%)/delta PaCO2, pressure-dependent CO2 reactivity as a group was observed with significance (P less than 0.05) in 8 cases of TIA due to the ICA occlusion, while no such relationship was noted in 10 cases of TIA due to the ICA mild stenosis. Moreover, clinical features were different between TIAs due to the ICA occlusion and ICA mild stenosis, i.e., more typical, repeatable TIA (6.3 +/- 3.7 times) with shorter duration (less than 30 minutes) was observed in TIAs due to the ICA mild stenosis, while more prolonged, less repeatable TIA (2.4 +/- 1.4 times) was observed in TIAs due to fixed obstruction of the ICA. From these observations, two different possible mechanisms as to the pathogenesis of TIA might be expected, e.g., TIA of microembolic origin due to the ICA mild stenosis, and TIA of hemodynamic origin due to fixed obstruction of the ICA, for whom the bypass surgery might be beneficial, i.e., all TIAs are not based on the same mechanism.

Hemodynamics in Shy-Drager syndrome and treatment with indomethacin.

Hemodynamic studies were performed in a case of Shy-Drager syndrome with severe orthostatic hypotension. Marked depression of blood pressure was recognized immediately after the tilt-up, wherein decrease in cardiac output was detected (65 leads to 35 ml; stroke volume) during measurements by echocardiography. In association with the depression of blood pressure and decrease in cardiac output, Doppler sonograms showed the overall blood flow reduction in the brain-supplying arteries, suggesting some breakdown of autoregulation of the cerebral blood flow. Medication with indomethacin obviously limited the depression of blood pressure during the standing exercise. The pressor mechanism of indomethacin might be regarded as a result of increased vasoconstrictivity by inhibiting the synthesis of prostaglandins.

Chordoma.

Four cases of intracranial chordomas are reported. Intracranial chordomas initially tend to occur with sixth nerve paresis and subsequently with visual disturbance and optic disc changes. Computed tomography demonstrated an isodense mass in one case, a low-density mass in another case, and both an isodense and a partial high-density in two cases. Histopathologically, isodense areas were coincident with regions of compact tumor cells, low-density areas coincided with abundant mucinous intercellular matrices, and high-density areas were coincident with cartilaginous matrices or calcifications.

Stimulation by 6-azauridine of carbamoyl phosphate synthesis for pyrimidine biosynthesis in mouse spleen slices.

1. Slices of spleen from anaemic mice were incubated with [14C]bicarbonate in the presence and absence of 6-azauridine and the amounts of 14C that entered the de novo pyrimidine biosynthetic pathway were assessed and compared. Compounds analyzed included carbamoylaspartate, dihydroorotate, orotate plus its derivatives, acid-soluble uracil and cytosine 5'-nucleotides, nucleic acid pyrimidines, free pyrimidine bases and nucleosides. As the intracellular levels of carbamoyl phosphate and acid-soluble deoxyribonucleotides are known to be relatively low, the radioactivities of these compounds were not measured. Degradation of labelled uridine was limited in this tissues, therefore the radioactivity of degradative products of pyrimidines was not considered. 2. When the slices were incubated with 0.5 mM 6-azauridine for 10 min and then with [14C]bicarbonate for an additional 10 min and 30 min, the sum of radioactivity found in the above compounds, which represents the total amount of 14C that entered the pyrimidine pathway, was 2.1 and 2.3 times greater than when the tissue slices were incubated in the absence of the analogue. 3. When the 14C distribution among the carbon atoms of the molecules of labelled carbamoylaspartate and uracil was investigated, we found that more than 90% of the total 14C in these compounds derived directly from carbamoyl phosphate and the remaining portion was from aspartate, either in the presence or absence of 6-azauridine. 4. There was no indication that 6-azauridine altered [14C]bicarbonate permeation through the cell membrane or its intracellular metabolism. 5. These results, along with the pattern of early intermediate accumulation seen in the presence of 6-azauridine, indicate that 6-azauridine stimulates the production of carbamoyl phosphate for the pyrimidine biosynthetic pathway in the mouse spleen. 6. Of the radioactive early intermediates which accumulated, only orotate, its derivatives (orotidine and orotidine 5'-monophosphate) or both appeared in the medium, presumably the result of leakage through the cell membranes. 7. Stimulation of the pyrimidine pathway was not observed in the case of Ehrlich ascites tumour cells incubated under similar conditions with 6-azauridine.

Enhancement of intracellular 5-phosphoribosyl 1-pyrophosphate levels as a major factor in the 6-azauridine-induced stimulation of carbamoyl phosphate synthesis in mouse spleen slices.

Brief exposure to 6-azauridine stimulates the production of carbamoyl phosphate for de novo pyrimidine biosynthesis in vitro in slices of haematopoietic spleen from anaemic mice (preceding paper). In studies of the underlying mechanism for this response we turned our attention to changes in the level of substrates and effectors for carbamoyl-phosphate synthetase II. Intermediates of the orotic acid pathway and 6-azauridine had little effect on the synthetase activity in vitro. 6-Azauridine 5'-monophosphate (6-AzaUMP) stimulated synthetase II, possibly in an allosteric manner. However, in view of the potency as an activator and the tissue levels, 6-azaUMP may be only partially responsible for the stimulation. Adenine nucleotide levels in the tissue showed only minor changes after brief exposure (15 min) to 6-azauridine. The level of UTP and UDP, potent inhibitors for synthetase II, showed no significant change. The level of 5-phosphoribosyl 1-pyrophosphate (PPRibP), a potent positive effector for the synthetase II, showed a more than 1.5-fold increase after 15 min. The relative importance of these factors was evaluated by assay of the synthetase, partially purified from mouse spleen, under simulated conditions in vitro. The results indicated that the enhanced level of PPRibP played a major role in increasing the production of carbamoyl phosphate. In Ehrlich ascites cells in vitro, where 6-azauridine did not increase carbamoyl phosphate production, the basal PPRibP level was high (range over 0.1 mM) and the changes in the level, brought about by the analogue, were relatively small.

Regional blood flows measured in Mongolian gerbil by a modified microsphere method.

Regional blood flow to major organs and regional cerebral blood flow were determined in seven anesthetized male gerbils by a modified microsphere method. Carbonized microspheres, 15 micrometers in diameter and labeled with 85Sr or 141Ce, were injected into the left ventricle by cardiac puncture through the closed thorax, and reference samples of known volume were withdrawn from tail-artery cannula. No significant hemodynamic alterations were observed during microsphere administration, and extraction of 15-micrometers microspheres by the pulmonary or systemic capillary beds was nearly 100%. The adequacy of mixing of microspheres in the left ventricle was also validated. The absolute regional blood flow to various organs and regional cerebral blood flow were expressed in terms of milliliters per minute per gram tissue weight, and the values mostly agreed with those reported previously in rats. The results indicate that the reference sample method can be applied to the gerbil. This method should allow the gerbil, which is an animal widely used in stroke research, to be conveniently used for hemodynamic studies when organ blood flow and regional cerebral blood flow are necessary.

The throat culture in Kawasaki disease.

Recently, Kawasaki disease has been attracting more attention as a new acquired heart disease in pediatrics, but the etiology of this disease has still not been established. In this study, the authors performed throat cultures not only on the patients themselves but also on their parents in order to investigate its etiology. No abnormal and specific bacterium was found in the present study. However, there have been many reports which may suggest bacterial or viral involvement at the onset of this disease. Therefore, it may be necessary to continue bacteriological analysis more systematically.
