From 1968 through 1984, 110 cases (113 eyes) with indirect traumatic optic neuropathy were seen at the Department of Ophthalmology and Otorhinolaryngology of the Kobe University Hospital; 43 eyes were treated nonsurgically with steroid and 70 eyes were operated on by the endonasal-transethmoidal method of optic canal decompression. By comparison of the visual improvements in the two groups, the effectiveness of the surgical treatment was evaluated. In the nonsurgical group, 19 eyes showed a visual improvement, giving an overall improvement rate of 44.2%. In 9 eyes with complete visual loss at the initial visit no visual improvement could be seen. In 34 eyes with the vision better than light perception at the initial visit, vision improved in 19 eyes, ie, the improvement rate was 55.9%: the recovery occurred relatively rapidly within 3-4 weeks, and thereafter the condition remained unchanged. In 30 eyes treated within 3 weeks after trauma, 17 eyes (57%) showed visual improvement, but in 13 eyes where treatment started after 3 weeks, improvement was seen only in 15%. In the surgical group, 34 eyes showed a visual improvement; the overall rate of improvement was 47.7%. In 28 eyes where complete visual loss was seen at the first visit, 7 eyes showed visual improvement. In 38 eyes treated within 3 weeks after trauma, 18 eyes (45%) showed visual improvement, and in 32 eyes where the surgery was performed after 3 weeks, 16 eyes (50%) showed improvement. The latter improvement rate was significantly higher (P less than 0.05) than the improvement rate of 15% found in eyes of the nonsurgical group, where the treatment started 3 weeks after trauma. In cases with indirect traumatic optic neuropathy, surgical decompression of the optic canal can give a visual improvement even when complete visual loss is found in the early period. The surgery is effective in cases where visual recovery by conservative treatment is not satisfactory after 3 weeks. On this basis the criteria for surgical treatment were determined: when the vision is better than light perception in the early period, conservative treatment must first be given, but surgery is indicated when the vision does not improve to 0.5 or better within 3 weeks. In cases where complete visual loss is found soon after injury, earliest possible surgical intervention is recommended.
A chronological analysis of gastric cancer originating from fundic gland mucosa was made by measuring the tumor size with reference to the serosal invasion and peritoneal dissemination. As for serosal invasion by cancer cells there were significant differences in the group with tumors exceeding 4 cm in diameter, and peritoneal dissemination was liable to be seen in the group with those exceeding 5 cm. This suggests that gastric cancer originating from fundic gastric mucosa must be detected before it reaches 4 cm in diameter and that the intraperitoneal administration chemotherapeutic agents is recommendable, considering residual microcancer dissemination.
The age of the patient and the laterality of the lesion are said to play an important role in determining surgical indications and prognosis of any neurosurgical disease. These two factors have been evaluated in 355 cases of putaminal haematoma, which were seen during the last 10 years after the introduction of the CT scan. Patients have been divided in three age groups; less than 59 years, 60-69 years and greater than 70 years, consisting of 235, 80 and 40 cases in each group respectively. Long-term outcome of three groups has been compared in both surgically and conservatively treated patients. The hematoma was situated on left side in 182 and on right side in 173 cases. 56.6% of left sided and 48.7% of right sided hematoma cases were operated. The long-term outcome of the two sides has also been compared in both surgically and conservatively treated patients. One has to be careful while considering patients of greater than 70 years of age as there is always some natural selection among these patients before they are referred to the hospital or are operated upon thereafter. One tends to be less aggressive for so old patients. Only in moderate type and operated cases the younger patients (less than 59 years) showed a better long-term outcome (ADL) than other surgically treated patients. Otherwise age did not effect the long-term outcome (ADL) in any type of putaminal haematoma. No effect of laterality was found in mild and very severe types of hematoma for the long-term outcome.(ABSTRACT TRUNCATED AT 250 WORDS)

We report a patient with spontaneous rupture of the common bile duct. This is an extremely rare condition which produces free leakage of bile into the peritoneal cavity. There has been no previous report concerning the formation of a large retroperitoneal encapsulation of bile. The preoperative diagnosis in our patient was very difficult and endoscopic retrograde pancreatocholangiography and cystography by ultrasound guidance were helpful.

Postprandial elevation of 5-phosphoribosyl 1-diphosphate (PPRibP) concentration in the mouse liver (Lalanne, M. and Henderson, J.F. (1975) Can. J. Biochem. 53,394-399) was further studied regarding the effects of protein intake and the underlying mechanisms. The extent and duration of the increase depended on the quantity and quality of proteins ingested. The order of effectiveness of various diets was as follows: 60% casein greater than 20% egg albumin greater than 20% casein greater than 20% gelatin = 20% gluten greater than 20% zein greater than 0% casein. Hepatic purine and pyrimidine biosyntheses de novo, as measured by labelled tracer incorporation, increased with increasing protein intake. Nicotinic acid incorporation into NAD increased equally, whether casein-containing or casein-free diets were given. Therefore, the increase of PPRibP level may be brought about by increase in its synthesis. Administration of glucagon or epinephrine similarly elevated the hepatic level of PPRibP. Somatostatin, known to inhibit secretion of pancreatic hormones, suppressed the casein-diet-dependent PPRibP level increase. Colchicine markedly inhibited the casein-diet- and glucagon-dependent responses, but not the epinephrine effect. It is likely that glucagon is a major factor in mediation of the protein-diet-dependent PPRibP level increase and that the cytoskeleton is involved in the glucagon-mediated response.
We saw three cases of optic nerve disturbance caused by frontal and fronto-ethmoidal mucopyoceles. A causative factor of optic nerve disturbance due to pyoceles might be not only related to compression but also to inflammatory invasion and vascular disturbance. The importance of axial and coronal computed tomography scans is emphasized for evaluating the extension of mucopyoceles and the relationship between the lesion and the adjacent structures.

The ultrasonic studies were performed on 203 normal fetuses ranging from 16 weeks to 42 weeks gestational age. Structures of fetal heart and aorta were visualized by real time imaging. Fetal UCG were recorded by TM mode simultaneously under the guidance of an ultrasonic electro-scanner. Aortic root diameter, descending aortic diameter, left atrial dimensions, mitral valve diastolic excursion, left ventricular dimensions and right ventricular dimensions were measured in fetal UCG. The following correlations were obtained.
