[Radioresistant elements in adenocarcinoma of the uterus].

Radioresistant elements in adenocarcinoma of uterine cancer were studied using SKG-3a and HEC-59 cell lines as spheroids and monolayer culture. Dose-survival curves for monolayer cultured SKG-3a and HEC-59 cells were almost similar at acute exposure. On the other hand, as spheroids grown to 300 microns in diameter, dose-survival curves for SKG-3a cells were biphasic and those for HEC-59 were monophasic. The increased shoulder in survival curves proved the contract effect of both cells. Conventionally fractionated irradiation promoted shrinkage and disintegration of spheroids as the dose was increased. All SKG-3a spheroids were destroyed over 60 Gy, but some of HEC 59 spheroids survived over 68 Gy at the total dose. The total dose at disintegration-50% was 40 Gy in SKG-3a and 48 Gy in HEC-59 spheroids. In a comparison of single with 5-fractionated irradiation, fractionation induced an increase in radioresistance of adenocarcinoma cells. The maximum recovery-dose of HEC-59 cell as spheroids was 0.4 Gy and that of SKG-3a was 0.28 Gy. The magnitude of PLD-repair in HEC-59 cells was greater than that in SKG-3a. In conclusion, the PLD-repair and contact effect could be more important factors than the hypoxic element in the acquisition of radioresistance in adenocarcinoma cells as a solid tumor under fractionated irradiation.

Ultrasonographic detection of early and curable carcinoma of the gallbladder.

Five patients with early carcinoma of the gallbladder detected by ultrasonography were studied. Three complained of non-specific upper abdominal symptoms or were asymptomatic and two had severe biliary colic. Gallstones were present in the two patients without biliary symptoms. Early carcinoma of the gallbladder was demonstrated as a polypoid tumour by ultrasound in four patients and as thickening of the gallbladder wall in one. The tumour was over 2 cm in diameter in all but one patient in whom the tumour enlarged rapidly from 5 to 10 mm. Size of the tumour and extent of spread were closely related. All but one patient underwent curative resection. Ultrasonography enhances the detection of early carcinoma of the gallbladder especially in old patients with non-specific abdominal symptoms and the operative cure rate is thereby improved.

Peroneal island flap for skin defects in the lower extremity.

Two types of peroneal island-flap transfer were used in fourteen patients with a large defect in the soft tissues of the lower extremity. The peroneal artery and vein and their cutaneous branches form the pedicle, and an extensive transfer of skin is possible either from above the knee or from the lateral side of the leg to as far distal as the foot. Two methods were used: in the first we severed the peroneal artery and vein distally, with an island of skin attached, and elevated them proximally, and in the second we severed the proximal portions of the peroneal vessels, with an island of skin attached, and elevated them distally. The largest flap that we used in this series measured fourteen by 16.5 centimeters and the smallest, 2.5 by five centimeters. All fourteen transfers healed, with no necrosis of the flap.

A new method for monitoring circulation of grafted bone by use of electrochemically generated hydrogen.

The patency of the anastomosed blood vessels in a free vascularized bone graft is difficult to ascertain during the early postoperative stage. For this purpose, the local blood flow was measured by means of electrochemically generated hydrogen. Although only three cases have been tested thus far, the method proves to be a simple and useful monitor of the blood flow in a free vascularized bone graft.

Primary carcinoma of the gallbladder.

Sixty-five Japanese patients with primary carcinoma of the gallbladder treated in the Department of Surgery I at Kyushu University from 1965 through 1982 were reviewed. Forty-one patients had histologically proven primary carcinoma and the other 24 patients were diagnosed by laparotomy and/or combination of several diagnostic procedures. The female to male ratio was 1.5 to 1. The peak incidence was at the ages of 60-69 years for both sexes. Gallstones were found in 53 per cent of resected patients and were demonstrated in 58 per cent of unresected patients by ultrasonography. Laparotomy was performed on 50 patients and the tumor could be removed in 15 patients. Resection could be done in only one of 18 patients correctly diagnosed. Fourteen of 15 patients who underwent resection had been diagnosed as cases of benign lesions. Curative resections were feasible in six of 15 patients. In curative resection cases, the longest survival time was 42 months; and in cases of non-curative resection, 26 months. Most of the nonresected patients died within 6 months with an operative mortality of 16.6 per cent. This report reinforces the difficulty in diagnosis and the poor prognosis for patients with primary carcinoma of the gallbladder. However, ultrasonography seems to be the best tool at present for detecting early cancer, especially cancer of the polypoid type. We hold the view that more radical approaches for relatively early cancer will enhance the operative results.

Determination of sisomicin, netilmicin, astromicin and micronomicin in serum by high-performance liquid chromatography.

A high-performance liquid chromatographic method for monitoring serum levels of the aminoglycoside antibiotics sisomicin, netilmicin, astromicin, and micronomicin, using an amino acid analysis system is described. The procedure involves sample preparation with a CM-Sephadex column, and quantitation using internal standards chosen from each of the other aminoglycosides. The aminoglycosides were separated by reversed-phase ion-pair chromatography on Zorbax C8 and ODS columns, and detected by the post-column derivatization technique. The calibration curves of serum concentration for sisomicin, netilmicin, astromicin, and micronomicin were linear over the ranges 0.32-22.8 micrograms/ml, 0.17-11.6 micrograms/ml, 0.1-6.3 micrograms/ml, and 1.0-30 micrograms/ml, respectively. The coefficients of variation were 2.5, 2.8, 3.1 and 1.9%, at the serum concentrations of 1.3, 1.45, 1.58 and 2.5 micrograms/ml, respectively (n = 6). Determination by the internal standard method using another aminoglycoside gives accurate and reproducible results. This method is applicable also to other aminoglycoside antibiotics.

The possible applications of a composite skin and subcutaneous vein graft in the replantation of amputated digits.

A composite graft of skin and attached vein was taken from the dorsal aspect of the foot and used in the replantation of 6 amputated digits in 5 patients which were complicated by loss of skin and blood vessels and with exposed bone and tendon. The 5 replanted digits survived in 4 patients. Complete survival of the skin graft was achieved in two digits and partial necrosis occurred in three digits. The technique is put forward as a possible method of vascular reconstruction in which skin loss is complicated by exposure of the deeper structures i.e., bone, tendon and/or joint.

Peroneal flap for reconstruction in the extremity: preliminary report.

The peroneal flap has many advantages, such as thin subcutaneous tissue, availability of a large-diameter artery and vein for anastomosis, a long pedicle, and freely designed size and shape. This technique is introduced because it has many outstanding features that allow it to be used to meet a variety of specific needs, such as a free peroneal flap, a peroneal island flap, a free vascularized fibular graft with skin, and a monitoring flap in free vascularized fibular grafts. We have found the peroneal flap to be especially useful.
