Morphology of the developing articular disc of the human temporomandibular joint.

The morphology of the developing articular disc of the temporomandibular joint in eight human fetuses, ranging in age from 13 weeks to 17.5 weeks, was studied. The shape of the articular disc at the time of initial delineation resembled the shape of the adult articular disc, suggesting a predominantly genetic determination. Elastin fibers were not demonstrated. The vascularity of the fetal articular disc was confined to the anterior and posterior thickened peripheral zones, with no evidence of capillaries in the thinner intermediate zone. Lateral pterygoid muscle fibers inserted into the medial aspects of the developing articular disc.

Effects of exercise on riboflavin requirements: biological validation in weight reducing women.

The present study was designed to evaluate our previous estimates for riboflavin requirement, 0.96 mg/1000 kcal during nonexercise and 1.16 mg/1000 kcal during exercise in overweight women. Two groups of 6 weight reducing women consumed either 1.16 mg riboflavin/1000 kcal (HR) or 0.96 mg/1000 kcal (MR). The study was two, 3 period by 2 treatment (exercise or nonexercise) crossover designs, one design at each level of riboflavin. Erythrocyte glutathione reductase activity coefficients (AC) significantly increased in both groups from 1.16 +/- .02 to 1.20 +/- .03 in group HR and from 1.31 +/- .04 to 1.36 +/- .02 in the MR group during nonexercise and exercise, respectively. ACs increased in the HR group due to an increase in total enzyme activity while ACs increased in the MR group due to a decrease in basal enzyme activity reflecting decreased flavin availability. There were no differences in aerobic capacity, weight loss, nor change in lean body mass between the two groups. Thus, 0.96 mg/1000 kcal was not adequate during either nonexercise or exercise periods while the 1.16 mg/1000 kcal was adequate.

Eminectomy and meniscorhaphy for internal derangements of the temporomandibular joint. Rationale and operative technique.

To date, most reports of surgical procedures to correct anterior meniscal displacement have focused on excision and repair of redundant posterior attachment tissue in conjunction with some form of arthroplasty. A procedure involving direct plication of the fibrous disk to the articular capsule (meniscorhaphy) and excision of the articular eminence (eminectomy) has yielded good results in most of the thirty-three patients with internal TMJ derangements on whom this operation has been performed within the past 44 months. The procedure is uncomplicated and physiologically sound, and it satisfies the concept of intracapsular decompression which is developed and described in this article. When surgical intervention is indicated for patients with internal TMJ derangements, eminectomy and meniscorhaphy should be given strong consideration.

Effects of aerobic exercise and weight loss on riboflavin requirements of moderately obese, marginally deficient young women.

In a previous study, exercise was shown to increase riboflavin requirements of active, normal weight young women. The present study examined the effect of exercise and weight loss on riboflavin status of moderately overweight women. The experiment was designed as a two-period cross-over with an initial base-line period and two 5-wk metabolic periods. The basic diet contained 1200 kcal with a riboflavin concentration of 0.8 mg/1000 kcal. Exercise consisted of a program of dance exercise. Riboflavin depletion, as measured by increased erythrocyte glutathione reductase activity coefficients and decreased urinary excretion of riboflavin, occurred during both nonexercise and exercise periods. Erythrocyte glutathione reductase activity coefficients increased from a base-line mean of 1.28 +/- 0.11 to 1.40 +/- 0.12 during nonexercise and to 1.49 +/- 0.16 during exercise. Urinary excretion of riboflavin fell from 48 +/- 12% of intake during base-line to 30 +/- 13% during nonexercise and to 19 +/- 6% during exercise. Riboflavin depletion was not related to the rate or composition of weight loss or to change in aerobic capacity.

Prevalence of hepatitis B markers in the anesthesia staff of a large inner-city hospital.

Thirty-four of seventy anesthesia staff members from an inner-city hospital evidenced past infection with hepatitis B. This is more than twice the prevalence previously reported for university-affiliated hospitals. Our findings suggest that screening before vaccinating is likely to be cost-effective for senior or foreign-born urban health care personnel in high-risk specialties, and vaccinating without screening is likely to be cost-effective for all persons newly entering high-risk specialties, including anesthesiology.

Retropharyngeal tendinitis.

We report a case of retropharyngeal tendinitis. A brief review of the literature, as well as extensive differential diagnosis, is presented. Retropharyngeal tendinitis should be a prime diagnostic consideration in patients who present with acute cervical pain, retropharyngeal calcium deposition, and prevertebral soft tissue swelling.

Fistulas and sinus tracts: radiographic evaluation, management, and outcome.

A retrospective review of 50 sinus tracts and 87 fistulas is presented. The etiologies, methods of radiographic evaluation including computed tomography and ultrasound, means of radiographic intervention, and disposition of sinus tracts and fistulas are discussed. Despite the newer imaging modalities, a sinogram or a fistulogram is still the best means of evaluating a sinus tract or fistula when an external communication is present. Computed tomography is helpful if exact spatial delineation of the tract is necessary or an associated abscess is suspected. Ultrasound examination is generally not useful, being limited by bowel gas and surgical incisions.
